Risk Assessment Technique for Gas Fuel Supply System of Combined Cycle Power Plants (II) : Based on Piping System Stress Analysis (복합화력발전의 가스연료 공급계통에 대한 위험도 평가 기법 연구 (II) : 배관 시스템 응력 해석을 이용한 위험도 평가)

  • The combined cycle power plant has a cycle of operating the gas turbine with fuel, such as natural gas, and then producing steam using residual heat. The fuel gas is supplied to the gas turbine at a level of 4 to 5 MPa, $200^{\circ}C$ through a compressor and a heat exchanger. In this study, the risk assessment method considering the piping system stress was carried out for safe operation and soundness of the gas fuel supply piping system. The API 580/581 RBI code, which is well known for its risk assessment techniques, is limited to reflect the effect of piping stress on risk. Therefore, the systematic stress of the pipeline is analyzed by using the piping analysis. For the study, the piping system stress analysis was performed using design data of a gas fuel supply piping of a combined cycle power plant. The result of probability of failure evaluated by the API code is compared to the result of stress ratio by piping analysis.

FMEA for rotorcraft landing system using Dempster-Shafer evidence theory (Dempster-Shafer 증거 이론을 이용한 회전익 항공기 착륙장치의 FMEA)

  • The quality assurance activities can detect the factors that affect the quality based on risk identification in the course of mass production. Risk identification is conducted with risk analysis, and the risk analysis method for the rotorcraft landing system is selected by failure mode effects analysis (FMEA). FMEA is a method that detects the factors that can affect the product quality by combining severity, occurrence, and detectability. The results of FMEA were prioritized using the risk priority number. On the other hand, these methods have certain shortcomings because the severity, occurrence, detectability are weighted equally. Dempster-Shafer evidence theory can conduct uncertainty analysis for the opinions with personal reflections and subjectivity. Based on the theory, the belief function and the plausibility function can be formed. Moreover, the functions can be utilized to evaluate the belief rate and credibility. The system is exposed to impact during take-off and landing. Therefore, experts should manage failure modes in the course of mass production. In this paper, FMEA based on the Dempster-Shafer evidence theory is discussed to perform risk analysis regarding the failure mode of the rotorcraft landing system. The failure priority was evaluated depending on the factor values. The results were derived using belief and plausibility function graphs.

  • Energy industry facilities can cause fatal damage for internal industry employee as well as external general people because handling various kinds of gas and harmful substance might be spread to large scale severe accident by fire, explosion, and toxic gas leakage. In order to prevent these accidents, quantification by damage effect on structure and human is tried by using quantitative risk assessment, but it is difficult to process instantly exceptional cases and requires knowledge of expert. This paper aims to minimize exceptional cases and knowledge of expert, and present risk with human perceptible. So, we designed and developed zone-base risk analysis system that can compute risk of zone in real time at that point using database and incremental model.

Quantitative risk analysis of industial incidents occurring in trap boats (통발어선에서 발생하는 산업재해에 대한 정량적 위험성 분석)

  • This study employs Bayesian network analysis to quantitatively evaluate the risk of incidents in trap boats, utilizing accident compensation approval data spanning from 2018 to 2022. With a dataset comprising 1,635 incidents, the analysis reveals a mortality risk of approximately 0.011 across the entire trap boat. The study significantly identifies variations in incident risks contingent upon fishing area and fishing processes. Specifically, incidents are approximately 1.22 times more likely to occur in coastal compared to offshore, and the risk during fishing processes outweighs that during maintenance operations by a factor of approximately 23.20. Furthermore, a detailed examination of incident types reveals varying incidence rates. Trip/slip incidents, for instance, are approximately 1.36 times more prevalent than bump/hit incidents, 1.58 times more than stuck incidents, and a substantial 5.17 times more than fall incidents. The study concludes by providing inferred mortality risks for 16 distinct scenarios, incorporating fishing areas, processes, and incident types. This foundational data offers a tailored approach to risk mitigation, enabling proactive measures suited to specific circumstances and occurrence types in the trap boat industry.

  • Due to the increasing complexity of the information systems environment, modern information systems are facing more difficult and various security risks than ever, there by calling for a higher level of security safeguard. In this paper, an information technology security risk management model, which modified by adopting the concept of business processes, is applied to client/server distributed systems. The results demonstrate a high level of risk-detecting performance of the model, by detecting various kinds of security risks. In addition, a practical and efficient security control safeguard to cope with the identified security risks are suggested. Namely, using the proposed model, the risks on the assets in both of the I/O stage(on client side) and the request/processing stage(on server side), which can cause serious problems on business processes, are identified and the levels of the risks are analyzed. The analysis results show that maintenance of management and access control to application systems are critical in the I/O stage, while managerial security activities including training are critical in the request/processing stage.

Economical selection of optimum pressurized hollow fiber membrane modules in water purification system using RbLCC

  • A water treatment utility in South Korea operates a large system of pressurized hollow fiber membrane (PHFM) modules. The optimal selection of membrane module for the full scale plant was critical issue and carried out using Risk-based Life Cycle Cost (RbLCC) analysis based on the historical data of operation and maintenance. The RbLCC analysis was used in the process of decision-making for replacing aged modules. The initial purchasing cost and the value at risk during operation were considered together. The failure of modules occurs stochastically depending on the physical deterioration with usage over time. The life span of module was used as a factor for the failure of Poisson's probability model, which was used to obtain the probability of failure during the operation. The RbLCC was calculated by combining the initial cost and the value at risk without its warranty term. Additionally, the properties of membrane were considered to select the optimum product. Results showed that the module's life span in the system was ten years (120 month) with safety factor. The optimum product was selected from six candidates membrane for a full scale water treatment facility. This method could be used to make the optimum and rational decision for the operation of membrane water purification facility.

  • As a form of industrial control systems (ICS), nuclear instrumentation and control (I&C) systems have been digitalized increasingly. This has raised in turn cyber security concerns. Cyber security for ICS is important because cyber-attacks against ICS can cause not only equipment damage and loss of production but also personal and public safety hazards unlike in general IT environments. Numerous risk analyses have been carried out to enhance the safety of ICS and recently, many studies related to the cyber security of ICS are being conducted. Many existing risk analyses and cyber security studies have considered safety and cyber security separately. However, both safety and cyber security perspectives should be considered when analyzing risks for complex and critical ICS facilities such as nuclear power plants (NPPs). In this paper, the STPA-SafeSec methodology is selected to consider both safety and security perspectives when performing a risk analysis for NPPs in order to assess impacts on the safety by cyber-attacks against the digital I&C systems. The STPA-SafeSec methodology was applied to a test-bed system that simulates a condensate water (CD) system in an NPP. The process of the application up to the development of mitigation strategies is described in detail.

  • To ensure the reliability of the safety system so that handing large quantities of hazardous materials in chemical plant is considered basic information in chemical process design. However, the reliability of the production system may be reduced when the reliability of the safety system emphasized in order to ensure the safety of chemical process. It is necessary to balance the reliability of the production system and reliability of the safety. In this study, a quantitative risk assessment was performed by selecting the furnace process, which is widely used in the chemical plant in order to suggest a way to ensure the safety and productivity of chemical process, based on the quantitative data. Quantitative risk assessment methodology have been used directed graph analysis methodology. It is possible to evaluate the reliability of the safety system and the production system. In this study, the optimum system design requirement to improve the safety and the productivity of the furnace is two-out-of-three logic for TT and PT.

  • The rubber type AGT system is operated by driverless that signaling is CBTC type. The system function is acomplished exactly to obtain the safety and reliability of system and the system is designed to minimize risk. In order to design the system, the functional analysis is performed. Recently functional analysis is performed by RDD-100 which is used and verified by aerospace, military, etc. Therefore, The design tool RDD-100 is used to perform functional analysis in AGT system development project.

  • Hazard analysis identifies probability to hazard occurrence and its potential impact on business processes operated in organizations. This paper illustrates a quantitative approach of hazard analysis of information systems by measuring the degree of hazard to information systems using probabilistic risk analysis and activity based costing technique. Specifically the research model projects probability of occurrence by PRA and economic loss by ABC under each identified hazard. To verify the model, each computerized subsystem which is called a business process and hazards occurred on information systems are gathered through one private organization. The loss impact of a hazard occurrence is produced by multiplying probability by the economic loss.
