Study on Comparison and Mutual Complementary Development between Sasang Constitutional Medicine and Syndrome Identification and Treatment Medicine (사상체질의학(四象體質醫學)과 증치의학(證治醫學)의 비교(比較) 및 상호(相互) 보완(補完) 방안(方案) 연구(硏究))

  • Objectives : Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about the theory of diagnosis, the theory of herbal medicine, the theory of prescription, the theory of acupuncture and constitutional medicine to put forward a complementary perfect solution which has good clinical utility. Methods : We compared and sorted out the textbook of Korean Sasang constitutional medicine which named Sasang medicine, and also the textbook of Chinese TCM which was including Basic theory of TCM, Diagnostics of TCM, Science of Chinese materia medica, Pharmacology of traditional Chinese medical formulae, Science of acupuncture and moxibustion, Constitution of TCM, and also related papers. Results : (1) Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about diagnosis, herbal medicine, prescription and acupuncture, Sasang constitutional medicine is a medical system which gives priority to balance of organs, when Syndrome identification and treatment medicine is a medical system that gives priority to improve symptom. (2) The prescriptions of Sasang constitutional medicine are used for the treatment of basic pathogenesis in the chronic stage of the disease, which can recover vital qi, when Syndrome identification and treatment medicine are used for the treatment of stage pathogenesis in the acute stage of the disease, which can get rid of pathogenic factors. (3) There are two kind of complementary perfect solution between Sasang constitutional medicine and Syndrome identification and treatment medicine. One perfect complementary medicine is that Sasang constitutional medicine plays a major role when Syndrome identification and treatment medicine plays a supporting role. The other one is that Syndrome identification and treatment medicine plays a major role when Sasang constitutional medicine plays a supporting role. It determined by the usual symptoms and the symptoms now, vital qi and pathogenic factors, symptom and the root cause, acute stage and chronic stage, and the state of pathogenesis. Conclusion : A new perfect complementary medicine can be created which is based on the setting that Sasang constitutional medicine primarily treat the usual symptoms, vital qi, the root cause, chronic stage and basic pathogenesis when Syndrome identification and treatment medicine primarily treat the symptoms now, pathogenic factors, symptom, acute stage and stage pathogenesis.

A Patient with Liver Fire Flaming Upward and Metabolic Syndrome Improved by Korean Medicine: A Case Report (대사증후군이 동반된 간화상염 변증 환자에 대한 한의정신요법 병행 한의치료 증례보고)

  • The purpose of this study is to report a case of a patient with pattern identification of liver fire flaming upward and metabolic syndrome improved by Korean medicine. The patient felt completely lost. She was treated for headache, insomnia, and metabolic syndrome with Korean medicine and psychotherapy. Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Pattern Identification Tool for Insomnia (PIT-Insomnia), Minnesota multiphasic personality inventory-2 (MMPI-2), and bioelectrical impedance analysis (BIA) were used for assessment. During treatment for six months, the patient stopped taking headache pills. Her blood pressure was stabilized. Her body weight, body fat, body mass index (BMI), and visceral fat decreased significantly. The sum of ISI, PSQI, PIT-insomnia also decreased significantly. Her Ego strength increased in the MMPI-2 supplementary scale. We assume that Korean medicine treatment through acupuncture, herbal medicine, and psychotherapy is effective in treating pattern identification of liver fire flaming upward and metabolic syndrome.

Development of Standardized Pattern Identification for Dizziness by Delphi Method (현훈(어지럼증) 한의표준변증안 개발을 위한 전문가 델파이 조사)

  • Objectives : The goal of this study is developing standardized pattern identification of dizziness using delphi method. Methods : The pattern identification of dizziness which derived through literature review is studied by delphi method. A group of 9 experts of korean medicine participated in Delphi examination. Experts carried out evaluating and correcting the pattern identification and symptoms by e-mail. Results : Through 3 delphi examinations, final standardized pattern identification of dizziness was suggested. It consisted of 2 items of excess syndrome, 2 items of excess-deficiency combination syndrome, and 3 items of deficiency syndrome. Conclusions : By the delphi examinations among experts, a standardized pattern identification of dizziness was suggested. These pattern identification will contribute to research and treatment of korean medicine. Further study is necessary for modification of pattern identification by practical clinical use.

Literature Review of Clinical Studies for the Relationship between Ultrasonographic Examination and Syndrome Differentiation Classification in Chinese Medicine (초음파영상검사와 한의변증분류와의 관계와 관련된 중의학 임상연구에 대한 문헌고찰)

  • This study was to investigate the relationship between ultrasonographic examination and pattern identification classification on cinical studies in chinese medicine. We searched clinical studies related correlation between ultrasonographic examination and pattern identification classification in chinese medicine, that published from 2013 to 2016 in China National Knowledge Infrastructure (CNKI) databases by keywords, 'ultrasound(超?)', 'chinese medicine(中?)', 'syndrome differentiation (辨?)'. Seventeen studies were found. There were 7 studies of gynecological diseases including polycystic ovary syndrome and uterine myoma, 5 studies of fatty liver, 3 studies of arthritis, and 1 studie of thyroid nodule and lymphadenopathy respectively. As a result, ii is thought that there was a certain degree of correlation between the change of the ultrasonographic image and the pathological types according to traditional chinese medicine (TCM) syndrome differentiation and ultrasonographic examination could be used as secondary means for the TCM syndrome differentiation classification. In conclusion, by using ultrasonograph device in a medicinal way of TCM and traditional korean medicine (TKM), it is thought that more detailed and accurate diagnosis and treatment are possible and the evidence for reasonableness of syndrome differentiation in TCM and TKM its validity can be secured.

Traditional Chinese Medicine Treatment for Childhood Cancer - Review of Case Studies (소아암의 한의치료 임상 실제 - 중의 증례연구를 중심으로)

  • Objective : To investigate specific prescriptions, herbal medicines and treatment strategy, this study systematically reviewed case studies on traditional Chinese medicine (TCM) treatment for childhood cancer. Method : Literature searches were performed on database of China National Knowledge Infrastructure (CNKI). Studies published before December 2020 were categorized and analyzed according to the treatment strategy and we also investigated Chinese herbs used in the prescriptions. Results : Twenty-five studies including 17 case series and 8 single case reports were included in our review. Of the total 439 patients, 379 (85.7%) had leukemia and other myeloplastic disease. Conventional western medicine (CWM) treatment was also used with TCM based on the syndrome identification. Most studies reported favorable effect of combination treatment with CWM and TCM. Among 266 herbs in the studies, reinforcing herbs were more frequently used than anti-cancer herbs. Conclusion : Traditional herbal treatment for childhood cancer should be performed considering the syndrome identification as well as stage of CWM. Reinforcing herbs should be more focused because of the fragility and growth of the childhood.

Study on the Data Analysis of CaPSPI for clinical application, a Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification (갱년기 변증 진단 도구 CaPSPI(Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification) 임상적용 결과 분석 연구)

  • Objectives: It is a statistical study to examine the data of CaPSPI (Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification), developed for diagnosis of menopause disorders and to record the status of treatment of it. Methods: From November 1, 2020 to June 19, 2021, 36 cases of data of 33 respondents of the CaPSPI were analyzed. For the use of the basic data of the clinical menopausal disorder, we investigated frequency of menopausal symptoms and the difference between them depending on the period of menopause, and the presentation of usage prescriptions. And the diagnostic results for three kinds' diagnosis [for examination (D1), for treatment (D2), by doctors (D3)] were compared. The diagnostic consistency of D1 and D3 and the statistical significance between DT and disease elements (證素) was investigated. Results: 1. Hot flush was the highest in the symptom survey of the menopause that the subjects complained of, followed by insomnia. There was no significant difference in symptom expression according to menopausal period. 2. The diagnostic consistency of D1 and D2 showed significant diagnostic consistency only in liver depression, and the diagnostic consistency of D1 and D3 showed significant consistency in liver depression and Dual Deficiency of Heart and Spleen. 3. D3' diagnosis and disease elements had statistical significance for cases of P<0.1 was found to be related to the theory of oriental medicine. Conclusions: It is needed to continue to accumulate diagnosis and treatment results through CaPSPI in the future to strengthen the basis for patten identification and treatment of menopause disorders.

Potential application of herbal medicine treatment based on pattern identification for canine cognitive dysfunctional syndrome: a comparative analysis of Korea medicine therapy for patients with dementia (반려견 인지기능장애증후군에 대한 한의 진단 및 한약치료 적용 가능성 고찰: 치매환자 국내한의치료기술과 비교 분석)

  • Canine cognitive dysfunction syndrome (CDS) is a neurodegenerative disease that causes cognitive and behavioral disorders and reduces the quality of life in dogs and their guardians. This study reviewed the complementary and alternative medicine (CAM) for CDS and compared the diagnosis and therapy of CAM between CDS in canines and dementia in humans. The evaluation tools for the diagnosis of CDS and dementia were similar in the neurological and neuropsychiatric examinations, daily life activity, cognitive tests, and neuroimaging, but the evaluation for dementia was further subdivided. In CAM, pattern identification is a diagnostic method for accurate, personalized treatment, such as herbal medicine. For herbal medicine treatment of cognitive impairment in canines and humans, a similar pattern identification classified as deficiency (Qi, blood, and Yin) and Excess (phlegm, Qi stagnation, and blood stasis) is being used. However, the veterinary clinical basis for verifying the efficacy and safety of CAM therapies for CDS is limited. Therefore, based on CAM evidence in dementia, it is necessary to establish CDS-targeted CAM diagnostic methods and therapeutic techniques considering the anatomical, physiological, and pathological characteristics of dogs.

The Principle of Acupoint Selection Based on Branch and Root Treatment (표치와 본치의 측면에서 경혈 선혈의 원리)

  • Objectives : Since there are complex associations between diseases/symptoms and acupoints, one-to-one correspondence may not be the proper approach. Pattern identification has been being used as a clinical framework to make treatment decisions by extracting and synthesizing clinical data including patients' signs and symptoms. In this article, we propose two different models explaining the relationships between diseases and acupoints based on the branch treatment [Zhibiaofa] and the root treatment [Zhibenfa]. Methods : We explained the relationships between diseases/symptoms and acupoints from the example data from our previous study on traditional acupuncture point selection patterns for pain control. Diseases include low back pain, migraine, irritable bowel syndrome, osteoarthritis, ankle sprain, carpal tunnel syndrome, and dysmenorrhea, and acupoints included LI4, BL23, BL25, SP6, BL60, TE5, and CV4. Results : The relationships between diseases/symptoms and acupoints can be explained directly based on the branch treatment, and also can be explained indirectly through pattern identification based on the root treatment. Pattern identifications included both meridian-based pattern identification based on the spatial information of diseases and visceral organ-based pattern identification based on the characteristics of diseases. Conclusions : In the East Asian traditional medicine, Korean medicine doctors choose the most appropriate acupoints based either on the diseases/symptoms (i.e., branch treatment) or on the results of pattern identifications (i.e., root treatment). It is necessary to understand the two different approaches to choose specific acupoints for the targeted diseases.

  • Severe sepsis or septic shock is characterized by an excessive inflammatory response to infectious pathogens. Acute respiratory distress syndrome (ARDS) is a devastating complication of severe sepsis, from which patients have high mortality. Advances in treatment modalities including lung protective ventilation, prone positioning, use of neuromuscular blockade, and extracorporeal membrane oxygenation, have improved the outcome over recent decades, nevertheless, the mortality rate still remains high. Timely treatment of underlying sepsis and early identification of patients at risk of ARDS can help to decrease its development. In addition, further studies are needed regarding pathogenesis and novel therapies in order to show promising future treatments of sepsis-induced ARDS.

A Review on Patterns and Classification Criteria of Psoriasis by analyzing Chinese Theses (중국 논문에 나타난 건선의 변증 분석 및 변증체계에 대한 고찰)

  • Objectives : The aim of this study is to explore the types of pattern identification (PI, 辨證) and the differential points of PI used for the treatment of psoriasis in Traditional Chinese Medicine (TCM) based on the Chinese references and to provide the evidences applying PI for the treatment of psoriasis in clinical practice. Methods : This study extracted patterns of psoriasis through database CNKI (China National Knowledge Infrastructure) and analysis the patterns and classification criteria of the patterns. Those examined in the study are dermal symptoms, general symptoms, formula and herbs which are different depending on the patterns. Results : Total 60 studies were selected and 44 pattern types were extracted from them. We categorized the main pattern types on psoriasis used in TCM as 'blood-heat syndrome(BHS, 血熱證)', blood-stasis syndrome(BSS, 血瘀證), and 'blood-dryness syndrome(BDS, 血燥證)', 'dampness-heat syndrome(DHS, 濕熱證)' and 'yang-deficiency syndrome(YDS, 陽虛證)'. Among these patterns, BHS was the most common. In TCM, the pattern of BHS tended to have skin symptoms and signs related to inflammatory erythema and heat. Both BSS and BDS were characterized by long disease duration and poor healing. In addition, DHS tended to have the skin symptoms and signs such as oozing and severe itching. The symptoms and signs related to coldness mainly showed in YDS. For PI criteria, 'qi-blood-essence criteria(氣血津液辨證)' and 'eight-doctrine criteria(八鋼辨證) are commonly used. Conclusions : Our findings show that each PI on psoriasis in TCM has different characteristics related to dermal and general symptoms or signs. Further studies are needed to develop the diagnostic tool of PI on psoriasis reflecting on clinical practices in Korean Medicine by referring to the findings of this study about PI on psoriasis in TCM.
