Study on the Factors that Have Correlation with Improvement of Cerebrovascular Accidents (중풍 환자의 호전도와 연관성이 있는 인자들에 대한 연구 - 제천 세명대 부속 한방병원 입원 환자를 중심으로 -)

  • The purpose of this study is to evaluate factors that have correlation with improvement of Cerebrovascular accidents(C.V.A) and to decide ranking of influence about improvement of C.V.A. This observation was made on 153 subjects of C.V.A. that were diagnosed through brain MRI or brain CT. They were hospitalized in the Semyung University Oriental Medicine Hospital from the January 1st 2006 to December 31th 2007. The subjects of this study are divided into two groups. The one group has slight motor disturbance, and the other group has severer motor disturbance. Based on medical treatment chart, we analyze differences of many factors like past history, family history, drinking, smoking, several symptoms with C.V.A., etc between two groups. As a result, The past history of cerebrovascular disease and past history of hypertension are the most influencing factors in improvement of C.V.A.

  • Use of the Pipeline embolization device (PED) has increased based on studies about its safety and effectiveness, and new reports that describe perior postprocedural complications are now emerging. We report a rare periprocedural device-related complication that occurred during endovascular treatment with the pipeline embolization device for a dissecting aneurysm on the vertebral artery. A 55-year old woman was admitted due to left medullary infarction, and angiography showed a fusiform dilatation in the left vertebral artery that was suspicious for dissecting aneurysm. Endovascular treatment with PED was planned. Under general anesthesia, the procedure was performed without significant problems and a PED was deployed in an appropriate position. However, in the final step of the procedure, the distal tip of the PED delivery wire became engaged within a small branch of the posterior cerebral artery and fractured. Fortunately, imaging studies after the procedure revealed neither hemorrhagic nor ischemic stroke, and the patient recovered without neurological morbidities except initial symptoms.

One Case Treated Semi-coma by Scalp Acupuncture with Electric Stimulation (두침전기자극을 시술한 반혼수 환자 1례에 관한 보고)

  • Objectives : To report one case of semi-coma patient was treated by scalp acupuncture with electric stimulation. Methods : Patient (state of semi-coma) was treated by scalp acupuncture with electric stimulation, herbal medication and physical therapy. The improvement of patient was measured every other week by Glasgow Coma Scale(GCS), Modified Bathel Index(MBI), National Institutes of Health Stroke Scale(NIHSS). Results : We observed the patient decrease in NIHSS score and increase in GCS and MBI. Symptoms of patient such as semi-coma were improved following the treatment. Conclusions : It is suggested that oriental medical treatment; scalp acupuncture with electric stimulation, herbal medication and physical therapy were effective in semi-coma caused by traumatic brain injury.

  • Background : Use of Ma Huang is controversial. Multiple reports of adverse effects led the U.S. Federal Drug Administration (FDA) to announce about prohibiting dietary supplements containing ephedrine alkaloids/Ma Huang. Objective : The objective of the study was to review the evidence from literature and rigorous clinical trials on the dosage, effectiveness, and adverse effects. Methods : Literature searches were conducted in U.S. FDA database after 2003 and digital Uibujeonok(醫部全錄). Hand searches of medical journal and bibliographies of identified articles were conducted. Results : Ma Huang has been used in Korean Oriental Medicine to treatment asthma and other ailments. Recently, Ephedrine/Ma Huang are used for weight loss or to enhance athletic performance in Europe and America, Korea. Ephedrine/Ma Huang are associated with increased risk of psychiatric, autonomic, or gastrointestinal symptoms, and heart palpitations. In addition to that, a risk of serious adverse events, including heart attack, stroke, epilepsy and psychiatric problem are also have been reported. Therefore, max dose of total alkaloids of ephedrine is studied in need of a specialist's part. Conclusion : In most reports, Ma Huang effectively promoted short-term weight loss and enhancement of athletic performance, But, safety with long-term use requires further investigation.

Case Report of Recovered Stroke Patient with Pneumonia Complication with Korean Traditional Medical Therapy (중풍의 폐렴합병증에 대한 한방 치료 1례)

  • A 72 year-old male patient with Pneumonia and Cerebral infarction was treated. He was suffering from cough, sputum, fever, decreased mentality and dyspepsia. His symtoms were diagnosed as damp and phlegm of the Spleen and the Stomach, and he administered Hwajung-Tang(和中湯). After 23 days, clinical symptoms of Pneumonia had improved. Results of Chest PA and CBC profile also showed improvement. These results support a role for oriental medicine treatment in the treatment of elderly patients with Pneumonia.

  • Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) syndrome is a maternally inherited mitochondrial disorder of which m.3243A>G is the most commonly associated mutation, resulting in an inability to meet the energy requirements of various organs. MELAS poses a diagnostic challenge owing to its multiple organ involvement and great clinical variability due to its heteroplasmic nature. We report three cases from a family who were initially misdiagnosed with myasthenia gravis or undiagnosed. Although there is no optimal consensus treatment approach for patients with MELAS because of the disease's heterogeneity, our 21-year-long therapy regimen of ${\text\tiny{L}}-arginine$, ${\text\tiny{L}}-carnitine$, and coenzyme Q10 supplementation combined with dietary management appeared to provide noticeable protection from the symptoms and complications. Prompt early diagnosis is important, as optimal multidisciplinary management and early intervention may improve outcomes.

  • Anticonvulsant hypersensitivity syndrome includes fever, skin eruptions, lymphadenopathy, hematologic abnormality and hepatitis, but its mechanism remains unknown. Anticonvulsants including phenytoin, carbamazepine can cause hypersensitivity reaction. We treated a patient who had severe itching sensation and insomnia: he had undergone an operation for cerebral hemorrhage and was administered anti-convulsant agents to prevent convulsions. We administered the anti-convulsant, Dangkwieumja(Dangguiyinzi). After the treatment, clinical symptoms caused by hypersensitivity were improved.

A Case Report of Korean Medicine Treatment for Acute Cerebral Infarction with Cerebral Hemorrhage (출혈을 동반한 급성기 허혈성 뇌중풍 환자에 대한 한방치료 증례 보고 1례)

  • Objectives: The purpose of this study was to report the improvement of symptoms by Korean medicine in acute hemorrhagic infarction. Method: The patient was diagnosed with a cerebral infarction of the right temporal lobe accompanied by cerebral hemorrhage of the left basal ganglia. He did not receive intravenous thrombolytic treatment. Sunghyangjungi-san-gamibang was initially administered, and Gami-daebo-tang was administered during the recovery phase, together with Uhwangchungsim-won, Simjeok-hwan, and acupuncture. The prognostic observation was conducted using the manual muscle test (MMT), the Korean version of the modified Bathel index (K-MBI), and subjective assessment. Results: After Korean medicine treatment, the K-MBI score was improved from 52 to 93. The MMT score and subjective assessment also showed improvement. Conclusions: For patients who cannot be treated with intravenous thrombolytic treatment, Korean medicine treatment is effective during the early and recovery stages of stroke.

  • Paroxysmal Kinesigenic Dyskinesia ( PKD ) is an unusual dyskinesia often precipitated by voluntary movements and characterized by brief episodes of choreic/ dystonic movements. Little is known of the pathophysiology of PKD, although secondary etiologies such as multiple sclerosis, ischemic stroke, birth injury , head trauma, drug abuse, etc. are known to induce the disease. We report a case of a male patient suffering from both legs dystonia for 10 years. He had a past history of drug abuse, and was diagnosed as PKD by a neurologist. We treated him with acupucture and herbal medicine and was able to observe the improvement in certain symtoms and general condition.

  • Objective: To evaluate the effectiveness of traditional Korean medicine treatment (TKM) on a patient with ACA infarction and hemorrhagic transformation with hemiplegia and urinary disturbance. Methods: A patient diagnosed with left anterior cerebral artery (ACA) infarction and hemorrhagic transformation was treated with electroacupuncture, moxibustion, and herbal medicine. Results: Improvements in the Manual Muscle Test (MMT), modified Barthel Index (MBI), National Institutes of Health Stroke Scale (NIHSS), modified Rankin scale (mRS), and K-MMSE were observed following the TKM treatment [MMT Gr. (3/0)→Gr. (4+/4+), MBI 20→89, NIHSS 8→2, mRS 5→1, MMSE-K 21→28]. Conclusion: TKM treatment could help improve the symptoms of patients with ACA infarction and hemorrhagic transformation.
