SWEET SYNDROME INITIALLY MANIFESTING ODONTOGENIC BUCCAL CELLULITIS; REPORT OF A CASE (치성 협부 봉와직염의 증상으로 발현된 Sweet 증후군; 증례 보고)

  • Sweet syndrome is characterized by acute onset of fever. neutrophilic leukocytosis, painful erythematous plaque on the face and extremities, infiltration of mature neutrophils in the dermis. Cutaneous lesion and clinical symptoms rapidly improve after treatment with systemic corticosteroids. The cause of sweet syndrome is unknown but the associations with hypersensitivity to bacteria, virus, or tumor antigen have been reported. Sweet syndrome itself can be a premonitory manifestation of malignancy, so diagnostic work up for other internal malignancy is recommended. Because of fever and leukocytosis, cutaneous infections are important differentials. Sweet syndrome can be divided into 4 categories according to associated disease and symptom. (Idiopathic Sweet syndrome, Parainflammatory Sweet syndrome, Paraneoplastic Sweet syndrome, Pregnacy associated Sweet syndrome.) Sweet syndrome is relatively rare disease and the association with myelodisplastic syndrome has been reported. We report a case of Sweet syndrome associated with myelodisplastic syndrome which has initial manifestation of odontogenic buccal cellulites.

Sweet's Syndrome Associated with Graves' Disease

  • Sweet's syndrome, or acute febrile neutrophilic dermatosis, occurs in association with autoimmune diseases such as Hashimoto's thyroiditis but is rare in Graves' disease, in which all cases are induced by propylthiouracil (PTU). We report a case of Sweet's syndrome in a patient with Graves' disease treated with methimazole (MMI) during three weeks. A 34-year-old man presented with the acute onset of high fever, skin rashes on the whole body, arthralgia, and acroparesthesia. Laboratory results showed leukocytosis and elevated C-reactive protein. MMI first stopped and antibiotics and antihistamine therapy started, but his symptoms dramatically improved after oral prednisolone. Graves' disease has again been treated by MMI because of his aggravated ophthalmopathy. After one year of retreatment with MMI, there has been no recurrence of Sweet's syndrome, supporting that Sweet's syndrome in this case was not related to MMI exposure. To our knowledge, this is the first report of Sweet's syndrome associated with Graves' disease per se but not PTU or MMI use.

  • Sweet's syndrome is characterized by clinical symptoms, physical features, and pathologic findings which include fever, neutrophilia, tender erythematous skin lesions, and a diffuse infiltrate of mature neutrophils. This is a report of our experience of Sweet's syndrome with parotitis. A 57-year-old man initially presented with tender swelling on the right cheek similar to parotitis. His symptoms relapsed despite the use of an oral antibiotic agent for 3 weeks. He additionally presented with erythematous papules and plaques on the periocular area and dorsum of both hands. Histiopathologic findings on punch biopsy of the right dorsum of the hand showed superficial perivenular histiocytic infiltration without vasculitis. We confirmed this as histiocytoid Sweet's syndrome and used systemic corticosteroid. After initiation of treatment with systemic corticosteroids, there was a prompt recovery from both the dermatosis-releated symptoms and skin lesions. Sweet's syndrome should be considered in patients with therapy-refractory parotitis and unclear infiltrated nodules. We present a confusing case who initially appeared to have parotitis but turned out to have histiocytoid Sweet's syndrome.

  • Sweet's syndrome also known as acute neutrophilic dermatosis is a multisystem inflammatory disorder characterized by fever, malaise, leukocytosis, and skin lesions. Sweet's syndrome affects multiple organs though only rarely does it affect the central nervous system (CNS) when it does it is called Neuro-Sweet disease (NSD). We report on a case study of a biopsy-proven NSD in a 50 year old man. Serial magnetic resonance imaging (MRI) showed repeated CNS involvement of Sweet's syndrome after a respiratory tract infection preceded it. On the MRI, T2 hyperintense lesions occurred at multiple sites and disappeared after steroid therapy.

A Case Report of Carpal Tunnel Syndrome with Raynaud's Phenomenon Treated by Bee Venom and Carthami Flos Pharmacopuncture (봉약침과 홍화약침으로 치료한 레이노 현상을 동반한 수근관증후군 환자 1례)

  • Objectives : This study is to report the effect of Pharmacopuncture therapy on a patient suffering from the pain and cold intolerance of hand caused by Carpal tunnel syndrome with Raynaud's phenomenon. Methods : We had treated the patient diagnosed as Carpal tunnel syndrome with Raynaud's phenomenon by Sweet BV and CF pharmacopuncture. We injected Sweet BV and CF into acupuncture points on both hands - Sweet BV into Baxie (EX-UE9), CF into Naegwan ($PC_6$) and Daereung ($PC_7$). And then we evaluated her symptoms by VAS (Visual Analog Scale). Results : Clinical symptoms about Carpal tunnel syndrome with Raynaud's phenomenon were remarkably improved by Sweet BV and CF Pharmacopuncture. Conclusion : Therefore, we concluded that pharmacopuncture therapy - Sweet BV, CF etc. - may be useful to treat Carpal tunnel syndrome with Raynaud's phenomenon.

  • Objectives : The purpose of this study is to report the improvement after Sweet BV herbal acupuncture therapy of shoulder impingement syndrome with Subacromial bursitis. Methods : We treated a patient having shoulder pain due to shoulder impingement syndrome with Sweet BV herbal acupuncture therapy. We checked visual analogue scale (VAS) score and range of movement (ROM). Results : We treated shoulder pain. Thereafter ROM improved and VAS score dropped to the level 3 points from 10 points at onset. Conclusions : This report shows Sweet BV herbal acupuncture therapy has effectiveness on shoulder impingement syndrome with Subacromial bursitis.

  • Recently, according to increasing population to enjoy sports, Shoulder Impingement Syndrome occurred frequently. In this report, we introduce a case suffered from chronic shoulder pain, ROM restriction because of Shoulder Impingement Syndrome treated by Sweet BV acupuncture. Sweet BV acupuncture maintains the BV acupuncture effects and restricts allergic reaction at the same time. In this report, Sweet BV acupuncture is a new therapy that have a safety and efficiency simultaneously.

Clinical Analysis about Treatment of Myofascial Pain Syndrome(MPS) with Sweet Bee Venom on Hand Paresthesia based on Thoracic Outlet Syndrome (흉곽출구증후군으로 손 저림을 호소하는 환자들에 대한 Sweet BV의 아시혈적 치료 효능 관찰)

  • Objectives: The objective of this study was to compare the effects of Sweet Bee Venom(Sweet BV) Therapy between the hand paresthesia patients with Osteoporosis and without Osteoporosis. Methods: This study was carried out to established the clinical criteria of hand parethesia. The patients who had past history of diabeics, neuropathy induced by alcohol or drug and was positive on Myofacial Pain Syndrome Theory were excluded. 32 patients who had hand paresthesia related with unknown-reason was selected by the interview process. And the effects of treatment were analyzed using VAS score before treatment, after treatment, after 1 month and after 3 months. Results and conclusion: After treatment, While Osteoporosis group decrease from $64.81{\pm}7.81$ to $27.21{\pm}7.32$, Non-Osteoporosis group decrease from $58.76{\pm}1.43$ to $24.74{\pm}3.81$ by VAS scores. and After 3 months, While Osteoporosis group increase from $27.21{\pm}7.32$ to $54.96{\pm}9.40$, Non Osteoporosis group increase from $24.74{\pm}3.81$ to $32.43{\pm}5.57$. Non-Osteoporosis group was accordingly more effective than Osteoporosis group after 3 months. So Sweet BV therapy for hand numbness patients without Osteoporosis was e effective than patients with Osteoporosis.

The Pathologic study on 『Wenbingtiaobian』 (『온병조변』의 병리학적 고찰)

  • This study on "Wenbingtiaobian" covers identifying pattern of prescription, understanding system of multiple syndrome differentiations, characteristics of treatment and medicinal substances. The source books are "Korean translation of Wenbingtiaobian", "Modern Shanghanlun", "Jinkuiyaolueyishi", "Chinese Medicine Formulas". "Wenbingtiaobian" has system of multiple patterns including three energizer syndrome differentiation, classification of disease, six meridian syndrome differentiation and wei-qi-ying-xue syndrome differentiation. That describes cause, location, nature, power and transmutation of disease. Wei-qi-ying-xue pattern is meaningful to warm-heat disease and three energizer pattern is relevant to dampness-heat disease. The warm disease shows mostly yang brightness bowel syndrome and patterns of three yin viscera. In aspect of the heat disease, qi aspect pattern makes up the largest number of syndrome differentiation and have sometimes with bowel excess or fluid deficiency. And treatment for wei aspect pattern is primarily 'outthrust the pathogen with pungent-cool'. Deficiency cold pattern and cold pattern with dampness occupy most of cold patterns. And many dampness patterns are dampness-heat pattern in middle energizer and 'inhibited lung qi transforming' is major mechanism. Patterns with fluid deficiency in qi aspect syndrome appear mostly in upper or middle energizer and in xue aspect syndrome appear mostly in lower energizer and they form 20% of all syndrome differentiations. The treatment of clearing heat uses pungent-cool(cold) for upper energizer, sweet-cold for middle energizer, sweet(salty)-cold for lower energizer. The treatment of tonifying yin uses mostly salty-cold for middle or lower energizer. The treatment of outthrusting pathogen is applied to all the wei-qi-ying-xue aspect combined with other treatments by using pungent-cool(cold) and light herbs. Understanding diseases in the respect of syndrome differentiation can enhance understanding of modern diseases from a perspective of Korean Traditional Medicinal(KTM) and can make clinical application of KTM treatments easy. Data from this study are expected to be basic for standardization and systemization of KTM.

Comparison of the Effects between Sweet Bee Venom Pharmacopuncture and Scolopendrid Pharmacopuncture on Carpal Tunnel Syndrome (Randomized, Controlled Clinical Trial) (수근관증후군에 대한 Sweet봉약침과 오공약침의 효과 비교)

  • Objectives : The purpose of this study is to compare the effects of Sweet Bee Venom Pharmacopuncture and Scolopendrid Pharmacopuncture on Carpal Tunnel Syndrome. Methods : From February to September 2010, the number of patients with Carpal Tunnel Syndrome who volunteered for this clinical study was 16 and 7 out of 16 patients complained both hands. Total 23 cases of hands were randomly divided by 2 groups. We injected Sweet Bee Venom Pharmacopuncture on PC7(Daereung) twice a week for 4weeks for experimental group(n=11), and Scolopendrid Pharmacopuncture with the same methods for control group(n=12). One case was dropped out due to itchiness of allergic response in the experimental group. Improvement of the symptoms was evaluated by Visual Analogue Scale, Pain Rating Scale, Tinel's sign, Phalen's sign and Nerve Conduction Velocity. Nerve Conduction Velocity was checked at baseline and the end of the trial and others were checked at baseline, after 2 and 4 weeks. Results : Both groups showed significant improvement in Visual Analogue Scale, Pain Rating Scale, but no significant difference between two groups. Only the control group showed significant reduction of the 'poitive response' in the Tinel's sign and Phalen's sign. However, no groups improved in Nerve Conduction Velocity. Conclusions : These results showed that Sweet Bee Venom Pharmacopuncture and Scolopendrid Pharmacopuncture could decrease the symptoms of Carpal Tunnel Syndrome. Further studies will be required to examine more cases for the long period and use more various concentration and amount pharmacopuncture for the effect on Carpal Tunnel Syndrome.
