Management of hydronephrosis: a comprehensive review in pediatric urology perspective

  • Hydronephrosis, characterized by dilation of the renal pelvis and calyces due to urine flow obstruction, poses a significant clinical challenge. Although often asymptomatic and capable of spontaneous resolution, surgical intervention is necessary for specific scenarios such as febrile urinary tract infections, deteriorating hydronephrosis, or declining renal function. The efficacy of continuous antibiotic prophylaxis in preventing urinary tract infections remains controversial. Yet, it may benefit high-risk patients, emphasizing the importance of individualized patient selection, as surgical treatment methods for patients with hydronephrosis have become less invasive than in previous decades. However, long-term follow-up outcomes are lacking, necessitating further clarification. This review presents an overview of the etiology, natural progression, and modern management of hydronephrosis, encompassing advancements in minimally invasive procedures.

  • Purpose: The purpose of this study was to construct, develop, and apply a nursing information system (NIS) using NANDA-NOC-NIC linkage in medical-surgical nursing units. Methods: This study consisted of three phases which were the construction of the database, development of the NIS, and application of the NIS. To construct the database, a questionnaire and nursing record review by an expert group were used. Collected data were analyzed by the SPSS/WIN 13.0 program. Results: In first phase, the database was made up of 50 nursing diagnoses, 127 nursing outcomes and 300 nursing interventions. In the second phase, NIS was developed according to its flow diagram and then tested. In the third phase, the developed NIS was applied to 130 inpatients. Nursing diagnoses frequently used were acute pain, delayed surgical recovery, and deficient knowledge (specify). Nursing outcomes for a nursing diagnosis of 'acute pain' were identified as pain control, pain level and comfort level. Nursing interventions for the nursing outcome 'pain control' were pain management, patient controlled analgesia assistance and medication management. Conclusion: The results of this study will facilitate the use of the newly proposed NIS in nursing practice and provide a guideline for evidence-based nursing.

농흉의 임상적 고찰: 60례 보고 (Empyema Thoracis: 60 Cases Analysis)

  • There appears to be significant problems remained in the treatment of thoracic empyema inspire of the more potent broad spectrum antimicrobial agents available and improved surgical managements. Clinical analysis of 60 patients of thoracic empyema was done who received major and/or minor surgical intervention at the Dept. of Thoracic and Cardiovascular Surgery, Korea University Hospital in the period of 3 years from May 1973 to July 1976. Following was the results: 1. Male was predominent to female with the ratio of 2.16 to 1, and adult to infant was 2.75: 1. 2. Most frequent predisposing factors of thoracic empyema in infancy and childhood was pneumonia [38.3%], and in adult was pulmonary tuberculosis [36.7%]. 3. Cardinal symptoms were dyspnea, fever, chest pain and productive cough etc. 4. Positive result of bacteriological culture study was reported in 26. 7%, and among them, Staph. aureus [10%], Diplo. pneumonia [3.3%], Streptococcal group [1.7%], Pseu. aeruginosa [1.7%], Paracolon bacilli [1.7%] and others [8.3%] in respectively. No growth of pathologic organism was reported in 60.0%, and not requested in 13.3%. Among the negative group, tuberculosis was confirmed on pathological tissue slide in half and other half was not determined bacteriologically and pathologically. 5. Surgical treatment was performed to the almost all of the patients with closed thoracostomy [48.3%], empyemectomy [28.3%], empyemectomy with pulmonary resection [11.7%] and others [I1.6%] in respectively with favorable results. 6. Mortality rate was 8.3% [5 cases], each one with lung cancer, diabetes mellitus, respiratory insufficiency, sepsis and another, two months old infant with asphyxia.

하악치은에 발생한 평활근육종의 치험례 (LEIOMYOSARCOMA OF THE MANDIBULAR GINGIVA: A CASE REPORT)

  • Leiomyosarcoma(LMS) is a malignant tumor from smooth muscle origin that arises most commonly in the gastrointestinal tract and uterus, but rarely in the oral and maxillofacial area which lacks smooth muscles. 63 cases of oral LMS have been reported, especially LMS that occurred in the mandibular gingiva is only 2 cases. Major symptom of oral LMS is painful or painless swelling. Besides it, oral LMS has no specific characteristics. So it is difficult to diagnose the case as LMS of the oral cavity. Certain cases report that LMS of the oral cavity was misdiagnosed as periodontitis and the patients were treated with unnecessary procedures. Conventional treatment of LMS is the radical surgical excision. LMS that is infiltrative and very malignant has poor prognosis despite of well-circumscribed boundary. LMS of the oral cavity is often recurred, has high rate of distant metastasis and 5-year-survival rate is as low as 23%. This article reports LMS of the mandibular gingiva that treated with surgical intervention, had local recurrence and metastasis to the lymph node after 16-month's follow-up examination.

Complex sternal and rib reconstruction with allogeneic material

  • Sternal malunion, or loss, developed after a median sternotomy cannot only be difficult to manage and treat, but also may diminish one's quality-of-life drastically. The technique presented here represents a multispecialty approach in one stage for the reconstruction of an unstable thoracic cage. The procedure utilized a donated sternum and ribs. The sternum with ribs harvested from a single donor included adipose derived stromal vascular fraction (ADSVF) cells with marrow also from the same donor. Autologous muscle flaps, stabilized with acellular dermal matrix were utilized to provide a robust blood supply to the ADSVF cells and bone grafts. Acellular dermal matrix was used to construct the ribs and stabilize the plugs of stem cells and bone. These procedures, in the hands of multispecialty physicians, have led to several successful reconstructions involving complex chest wall deformities. This surgical intervention was performed in a one stage operation. This represents the first successful complete sternal transplant in a patient with return to normal activities and increased quality-of-life.

구강 악안면 근막간극 감염에 관한 임상통계학적 분석 (A CLINICO-STATISTICAL ANALYSIS ON THE FASCIAL SPACE INFECTIONS OF ORAL AND MAXILLOFACIAL REGION)

  • We have conducted a retrospective study of 224 patients with the diagnosis of oral and maxillofacial infection who had been treated between 1988 and 1999 at Gyeong-Sang National University Hospital. This study was aimed to furnish the data of oral and maxillofacial infection and to aid diagnosis and treatment. The most common fascial space involved, as determined by clinical, radiologic, and operative findings, were the submandibular space(39.4%). The most frequent cause of oral and maxillofacial infection was odontogenic 68.8%. In the odontogenic cause, dental caries was the most common cause. Two-hundred three patients required surgical drainage of the abscess. Seventeen patients needed tracheostomy for airway control. The overall mortality was 0.9% despite aggressive anti-microbial therapy and early surgical intervention. All other patients had an uneventful recovery without major complication except osteomyelitis case(6.0%). The combination of early radiologic diagnosis, effective antimicrobial therapy, and intensive surgical management contributed to the good prognosis.

Importance of immediate surgical intervention and antifungal treatment for rhinocerebral mucormycosis: a case report

  • Rhinocerebral mucormycosis (RCM) is an opportunistic, potentially life-threatening fungal disease. This infective disease invades not only the facial sinuses, but also the maxilla, zygoma, and rhino-cerebral structures with a massive destruction of the facial skeletons and soft tissue. This disease progresses within various underlying diseases, such as diabetes mellitus, hematologic malignancy, renal failure, and systemic immunodepression. The relationship between mucormycosis and these underlying conditions have been discussed extensively in the literature. The authors studied 6 cases of RCM diagnosed by a tissue biopsy and treated at the department of oral and maxillofacial surgery, from 1997 to 2012. Patients were treated with several kinds of surgical interventions and antifungal agents, and their clinical & radiological signs, underlying conditions, surgical methods, and outcomes were analyzed.

소아의 위장관 중복증 (Gastrointestinal Duplications in Childhood)

  • Gastrointestinal duplications are rare congenital malformation that may require surgical intervention in the neonate, infant, and occasionally the older child. Symptoms produced by duplications vary according to their location, size, type and histology. We report the clinical characteristics and the surgical results of 9 cases of the gastrointestinal duplications treated at at Asan Medical Center between 1989 and 2000. Five patients were boys and four were girls; age of patients ranged from 5 days to 10 years. Eight duplications were cystic and one was tubular. One involved the stomach; five were in the ileum, and two in the cecum. The most common presentation was intestinal obstruction. There was associated anomaly in one patient, pulmonary sequestration and double ureter. Ectopic gastric mucosa was found in two. All patients underwent surgical resection. There was no perioperative mortality or morbidity. Although gastrointestinal duplication is a rare entity. consideration of associated anomalies and being familiar with the anatomy and clinical features are required for adequate management. In cystic form. complete excision is recommended but planned surgery is required for long segment tubular lesion.

수술환자를 위한 동영상과 소책자를 활용한 자가통증조절장치 교육의 효과 (The Effects of Preoperative PCA Education with Multimedia and Brochure on Pain Management in Surgical Patients)

  • Purpose: The purpose of this study was to evaluate the effects of preoperative PCA (Patient-Controlled Analgesia) education on pain, patient attitude, and patient satisfaction in surgical patients. Methods: The study was a quasi-experimental research design. The participants were 54 patients who were admitted for surgery at I hospital in G city, Korea. Of the 54 patients, 26 were assigned to the experimental group and the rest to the control group. The PCA education was provided in the nurses' station, individually to patients in the experimental group the day before their operation. Multimedia and brochure, and a real PCA model were used. The control group received only verbal education about PCA. Results: The postoperative pain scores were significantly different for lapse of time in the experimental group compare to the control group. Patient attitude toward using pain medicine was significantly more positive in the experimental group than in the control group. The postoperative patient satisfaction with pain management was significantly higher in the experimental group than in the control group. Conclusion: The study findings indicate that preoperative PCA education could be an effective nursing intervention for pain management of surgical patients.

향기흡입법이 복부수술 환자의 수술 전 불안 감소에 미치는 효과 (The Effect of Aroma Inhalation Method on the Preoperative Anxiety of Abdominal Surgical Patients)

  • Purpose: The study was done to analyze the effects of aroma inhalation method on preoperative anxiety of abdominal surgical patients. Methods: The research design was a nonequivalent control group non-synchronized design. The data were collected from July 25 to October 21, 2005 at C Medical Center in Seoul. The patients were divided into two group of 24 subjects each. In order for measuring the all patient's anxiety before operation and aroma inhalation, Spidlberger(1975) trait anxiety, VAS(visual analogue scale) state anxiety, blood pressure, pulse rate were taken. After experimental group was taken aroma inhalation, VAS state anxiety, blood pressure, pulse rate were measured for two group. Results: After aroma inhalation, VAS state anxiety level, systolic blood pressure, diastolic blood pressure, pulse rate of the experimental group were decreased significantly than those of control group(p = .000, p = .000, p = .030. p = .000). Conclusion: The aroma inhalation method can be considered an effective nursing intervention that relieves the preoperative anxiety of abdominal surgical patients and stabilizes vital signs.
