관절원판 유착을 동반한 악관절 내장증 환자의 수술후 증상의 변화 (Changes of the symptoms following surgical treatment of temporomandibular joint internal derangement with disc adhesion)

  • Purpose: The aim of this study was to find the clinical characteristics of the patients who had temporomandibular joint internal derangement(ID) with disc adhesion(adhesion group) compared to only disc displacement without disc adhesion, perforation, hyperemia, and so on(ID group). Materials and methods: Thirty seven joints were included in adhesion group and 54 joints in ID group of all 174 patients(174 joints) treated surgically and had been checked periodically over 12 months at TMJ clinic of Yongdong Severance Hospital, Yonsei University, between 1992 and 1997. Mouth opening range, pain during mouth opening and biting, headache, neck/shoulder pain and TMJ sound were checked his/her every visit before and after surgery. Results: The maximum mouth opening was improved significantly after postoperative 3 months in two groups(p<0.01), but adhesion group was less improved. Pain during mouth opening was improved significantly over 3 months after surgery in adhesion group(p<0.01), but in ID group 1 month after surgery. Biting pain was improved and maintained it after surgery and not significant difference between two groups. Headache and neck/shoulder pain were much improved after surgery(p<0.01), but slight relapse was found in adhesion group after 12 months. TMJ sound was more found in adhesion group after 1 month(p<0.05), but after 3 months, no significant difference was found between two groups. Conclusions: The postoperative results of adhesion group were worse than ID group. Therefore, it is considered more carefully to diagnose and treat in cases of internal derangement with adhesion.

Cranioplasty Results after the Use of a Polyester Urethane Dural Substitute (Neuro-Patch®) as an Adhesion Prevention Material in Traumatic Decompressive Craniectomy

  • Purpose: This study was conducted to investigate the usefulness of a polyester urethane dural substitute (Neuro-Patch®, B. Braun, Boulogne, France) as an anti-adhesion agent in subsequent cranioplasty by analyzing the use of Neuro-Patch® during decompressive craniectomy in traumatic brain injury patients. Methods: We retrospectively analyzed patients with traumatic brain injury who underwent decompressive craniectomy followed by cranioplasty from January 2015 to December 2018. Patients were analyzed according to whether they received treatment with Neuro-Patch® or not (Neuro-Patch® group, n=71; control group, n=55). Patients' baseline characteristics were analyzed to identify factors that could affect cranioplasty results, including age, sex, hypertension, diabetes mellitus, use of antiplatelet agents or anticoagulant medication, the interval between craniectomy and cranioplasty, and the type of bone used in cranioplasty. The cranioplasty results were analyzed according to the following factors: operation time, blood loss, postoperative hospitalization period, surgical site infection, and revision surgery due to extra-axial hematoma. Results: No significant difference was found between the two groups regarding patients' baseline characteristics. For the cranioplasty procedures, the operation time (155 vs. 190 minutes, p=0.003), intraoperative blood loss (350 vs. 450 mL, p=0.012), and number of surgical site infections (4 vs. 11 cases, p=0.024) were significantly lower in the Neuro-Patch® group than in the control group. Conclusions: The use of Neuro-Patch® was associated with a shorter operation time, less blood loss, and a lower number of surgical site infections in subsequent cranioplasties. These results may provide a rationale for prospective studies investigating the efficacy of Neuro-Patch®.

  • Acute Achilles tendon rupture is a frequent injury during sports and recreational activities. Treatments for Achilles tendon rupture have been controversial in recent decades. Traditionally the surgical treatment had benefit over nonsurgical treatment in terms of low rerupture rate and early functional restoration. Recently, nonsurgical treatment was found to show no statistically significant inferiority in re-rupture rate, functional outcome, and calf strength. Whereas, surgical treatment had some complications including adhesion, nerve injury, and infection. Nonsurgical treatment has been increasing due to functional rehabilitation with early weight bearing and restricted early motion. It focuses more attention on the course of caring for patients with deep discussion. There are open repair and minimally invasive repair in terms of surgical treatment. There are various techniques for minimally invasive repair of Achilles tendon, which has some advantages over the open repair. However, the optimal technique for minimally invasive repair has not been established. The number of suture strands is important regardless of suture technique.

Foreign Body Granulomas after the Use of Dermal Fillers: Pathophysiology, Clinical Appearance, Histologic Features, and Treatment

  • A foreign body granuloma is a non-allergic chronic inflammatory reaction that is mainly composed of multinucleated giant cells. Foreign body granulomas may occur after the administration of any dermal filler. Factors such as the volume of the injection, impurities present in the fillers, and the physical properties of fillers affect granuloma formation. The formation of granulomas involves five phases: protein adsorption, macrophage adhesion, macrophage fusion, and crosstalk. The clinical and pathologic features of granulomas vary depending on the type of filler that causes them. Foreign body granulomas can be treated effectively with intralesional corticosteroid injections. Surgical excisions of granulomas tend to be incomplete because granulomas have ill-defined borders and moreover, surgical excisions may leave scars and deformities.

Congenital syngnathia: a case report

  • Congenital syngnathia refers to the fusion of bony tissues, a rare disorder with only 41 cases reported in the international literature from 1936 to 2009. The occurrence of syngnathia without any other associated systemic disease or congenital anomaly is extremely rare. This report presents a case of congenital syngnathia with unilateral maxillomandibular bony adhesion without any other oral or maxillofacial anomaly. No recommended protocol for surgery exists due to the rarity of the disorder. There is a very low survival rate for the few patients who have forgone surgical management. This case describes a 74-year-old female patient who was suffering from limitation of mouth opening and was subsequently diagnosed with congenital syngnathia. The surgical staff performed separation surgery and reconstructed the malformed oral vestibule and cheek using the radial forearm free flap operation.

  • Surgical correction of patent ductus arteriosus is,under most circumstances,highly successful and carries a low mortality. But infected PDA is yet potentially dangerous due to its frequent recurrence and resistant organisms to antibiotics. And,in surgical correction,surgeon may face the possibility of tearing of ductus arteriosus arterial end due to friability and adhesion of its surrounding tissue.This report demonstrats another problem in treatment of infected patent ductus arteriosus.This thirteen years old female patient received susceptible combined antibiotics intravenously from the day of admission and remitted from 4th.week of therapy.This remission state continued for 12days without relapse.But the pulmonary artery ruptured in this remission period.In autopsy,bacteria was not found in ductal vegetation.Also,there was no pulmonary artery aneurysm,Our experience show that in infected PDA,pulmonary artery can rupture spontaneously during remission period without aneurysmal formation.

Gastropexy Using MIC-KEY Tube in a Dog with Esophageal-Gastric Leiomyoma

  • An 8-year-old neutralized male Dachshund was presented with severe vomiting, anorexia, and weight loss for two weeks. The patient had to feed in a standing position due to problem of vomiting immediately after meal. Serum chemistry and CBC results were all within reference limits. CT imaging revealed a mass distributed widely in the caudal part of the esophagus including the sphincter and the gastric cardia. Complete surgical removal was difficult because of adhesion to the caudal vena cava. A partial surgical excision and gastropexy with MIC-key feeding tube were performed under the owner's permission. Histologically, the mass was interpreted as leiomyoma. This patient is doing well at present time, six years after the tube installation in 2015.

  • Lumbar foraminal pathology causing entrapment of neurovascular contents and radicular symptoms are commonly associated with foraminal stenosis. Foraminal neuropathy can also be derived from inflammation of the neighboring lateral recess or extraforaminal spaces. Conservative and interventional therapies have been used for the treatment of foraminal inflammation, fibrotic adhesion, and pain. This update reviews the anatomy, pathophysiology, clinical presentation, diagnosis, and current treatment options of foraminal neuropathy.

개에서 복강내 잔존한 거즈 이물의 진단영상 (Diagnostic Imaging Features of Abdominal Foreign Body in Dogs; Retained Surgical Gauze)

  • This study was performed to describe the radiographic and ultrasonographic features of retained surgical gauze known as gossypiboma in 9 dogs. Female dogs (n = 8) were at higher risk and seven out of the eight cases had a history of ovariohysterectomy. Seven dogs were symptomatic and the most common clinical signs were vomiting, anorexia, and inertia. A palpable abdominal mass was detected in six dogs. Radiographic signs included a localized abdominal mass with soft tissue density (n = 7) or a mass containing speckled gas (n = 1). Ultrasonography showed a hypoechoic mass with a hyperechoic center (n = 4), or a homogeneous hypoechoic mass (n = 3). The remaining dogs (n = 2) showed an intestinal wall surrounding a hyperechoic center. Regardless of the characteristics of a mass, an acoustic shadowing was accompanied from the center of a mass in all dogs. Ultrasonography also revealed complications such as adhesion between a mass and adjacent organs, and peritonitis and intestinal obstruction around a mass. The gossypiboma can be considered when a hypoechoic mass accompanying a hyperechoic center with acoustic shadowing is observed on ultrasound examination.

한의과에서 시행된 지방종(Lipoma)의 수술적 치료 증례 (A Case of Surgical Treatment of Lipoma Performed at the Korean Medicine Department)

  • Objectives : The purpose of this study is to report a case of lipoma removed by surgical method. Methods : A 69-year-old female was diagnosed with lipoma on the left upper shoulder. Bufonis venenum pharmacopuncture was used for local anesthesia. The 11th blade and the CO2 laser was used for incision. After removal of lipoma, irrigation with soyeom pharmacopuncture solution and simple interrupted suture were carried out. For 7 days, Yeonkyopaedok-san was given and the suture was removed after 14 days. At last, saengkigo was applied. Results : Local anesthesia was maintained. Lipoma sized 3.0×1.0×1.0cm was clearly removed. Adverse effects were not reported until the clear skin adhesion. Conclusions : Lipoma were surgically removed without any complications by using pharmacopuncture and oral herbal medicines that have anti-inflammatory effects. Korean medicine has the potential for surgery and needs to extend the range of surgical treatment.
