자조그룹에 대한 개념 분석 (Concept Analysis of Self-help Groups)

  • Purpose: The purpose of the study was to identify the attributes of self-help groups, their antecedents and consequences relating to self-help groups. Methods: We used the Walker and Avant (2010) method using the key word "self-help groups" the Korea Education and Research Information Service (www.riss4u.net), Pubmed, CINAHL and ProQuest for articles on this topic published between January 2000 and March 2013 were searched. Ultimately, 64 domestic and 21 foreign papers were selected for in-depth analysis. Results: The attributes of self-help groups are as follows: 1) members share common experiences and are supportive of each other; 2) members set goals for individual change; 3) groups are self-monitoring; 4) groups learn problem-solving processes through voluntary and active participation; and 5) groups are small and meet regularly. The antecedents of self-help groups are as follows: 1) an intervention by an expert; 2) a diagnosis of their illness; 3) motivation to change individuals' state; and 4) educational desire. The consequences of self-help groups are the relief of symptoms, the improvement of physiological parameters and quality of life, the decrease in depression, stress, and anxiety, the improvement of illness-related knowledge and self-help activity, and a change in beliefs. Conclusion: Self-help groups can be used as an intervention strategy to help people with chronic illness manage their own problems.

Effect of pectoralis major myofascial release massage for breastfeeding mothers on breast pain, engorgement, and newborns' breast milk intake and sleeping patterns in Korea: a randomized controlled trial

  • Purpose: Supportive interventions to improve breastfeeding practice are needed in nursing. This study investigated the effects of pectoralis major myofascial release massage (MRM) on breast pain and engorgement among breastfeeding mothers and on breast milk intake and sleep patterns among newborns. Methods: Breastfeeding mothers who had delivered between 37 and 43 weeks and had 7-to 14-day-old newborns were recruited from a postpartum care center in Gunpo, Korea. Participants were randomized to the MRM or control group. The outcome variables were breast pain and breast engorgement among breastfeeding mothers and breast milk intake and sleep time among newborns. The experimental treatment involved applying MRM to separate the pectoralis major muscle and the underlying breast tissue in the chest. After delivery, the first MRM session (MRM I) was provided by a breast specialist nurse, and the second (MRM II) was administered 48 hours after MRM I. Results: Following MRM, breast pain (MRM I: t=-5.38, p<.001; MRM II: t=-10.05, p<.001), breast engorgement (MRM I: right, t=-1.68, p =.100; left, t=-2.13, p=.037 and MRM II: right, t=-4.50, p<.001; left, t=-3.74, p<.001), and newborn breast milk intake (MRM I: t=3.10, p=.003; MRM II: t=3.09, p=.003) differed significantly between the groups. Conclusion: MRM effectively reduced breast engorgement and breast pain in breastfeeding mothers, reducing the need for formula supplementation, and increasing newborns' breast milk intake. Therefore, MRM can be utilized as an effective nursing intervention to alleviate discomfort during breastfeeding and to improve the rate of breastfeeding practice (clinical trial number: KCT0002436).

치매환자 돌봄 전략에 관한 일 고찰 (Caring for older adults with dementia -focused on therapeutic environment-)

  • Even though enormous governmental expenses and scientists' efforts to find out definite causes and treatment methods of senile dementia have been investigated, little has been known in this area. Along with knowledge development of the etiology and treatment of the dementia, researchers have started to focus on improving the quality of life of the older adults with dementia through psychosocial intervention. This study was designed to propose a theoretical framework for establishing therapeutic environment for the older adults with dementia and for developing principles and strategies of caring. The results of this study were expected to help family members of the older adults with dementia to understand behavioral problems of the demented persons. The results can be utilized for health professionals to provide nursing interventions to reduce family caregivers' burden and to improve the quality of life of the older adults with dementia and their family. Caring principles developed from this study were as follows: 1. To minimize the stressors that can stimulate older adults with dementia. 2. To assess demented person's needs for safety and provide intervention based on the assessment. 3. To provide therapeutic environment for older adults with dementia to reduce confusion and to improve orientation. 4. To organize simple regular daily activities that older adults can anticipate. 5. To enhance demented person's self-esteem and self-confidence by providing supportive care. 6. To promote social interaction of the older adults with dementia by utilizing adequate activity programs.

NICU에 입원한 미숙아 어머니의 스트레스 (Study on the Perceived Stress Level of Mothers in Neonatal Intensive Care Unit)

  • With advances in neonatal medicine, smaller and sicker infants are surviving premature birth. As these tiny patients battle for their lives, their parents also struggle with psychological consequences of an unexpected role. The birth of a sick infants has long been documented as stressful events for the parents. High stress levels may alter parents' interaction with their baby and impair their communication with health professionals, which can in turn affect parents' participation in care planning and decision making. Nursing interventions aimed at reducing the parental stress and anxiety levels during this crisis may have a positive impact on their ability to form an attatchment to their baby and make prudent decisions about his care. The planning of such interventions would be inhanced if the contributions of various factors to increased parental stress levels were identified. This descriptive study was conducted to understand the contents and degree of parental stress in the NICU during their premature's hospitalization, and to give a baseline data in developing nursing intervention program. Subjects were the 60 mother of hospitalized newborn in NICU of 2 University Hospital in Taejon City from April 1st, 1999 to June 30th, 1999, who agreed to take part in this study. The instrument used in this study were Parental Stressor Scales:NICU(PSS:NICU) developed by Miles et al.. and validated by 3 NICU practitioners and 3 child health nursing faculties. The questionnaire has 4 dimensions and 45 items; sight and sounds of NICU(5 items), babies' appearance and behavior nursing intervention(19 items), parental role alteration and relationship with their baby(10 items), health team communication(11 items). The questionnaire asks parents to rate each item on a five-point Likert type scale that ranges from "not stressful" (1) to "extremely stressfu1"(5). Total scores representing overall stress from the NICU environment are calculated by summing response to each item. A high score indicates high stress. A subscale score is calculated by summing the responses to each item in the subscale. Cronbach's ${\alpha}$ coefficients were .93. The data was analyzed as average, Frequency, Standard deviation, t-test, ANOVA, Pearson correlation coefficient by use of SPSS/PC+. The results of this study is summarized as follows ; 1. The total perceived stress level score of mothers of premature baby was slightly high($3.66{\pm}1.1$). The highest scored dimension was 'relationship with their baby and parental role change'($4.21{\pm}0.9$), and next were 'appearance and behavior of the baby'($3.92{\pm}1.1$), 'communication with health team'($3.32{\pm}1.3$), 'sight and sounds of NICU'($3.30{\pm}1.1$). 2. There were statistically significant correlation noted ; mother's perceived severity of the baby's condition(F=8.0012, P= .0125) and baby's gestational age(r = -.4724, P = .0500). In summary, information about physical environment of NICU, the mother's perceived severity of premature baby's state, maternal role change related variables and the knowledge of characteristics of premature baby must be included in nursing intervention program of mother's of premature baby in reducing the parental stress and anxiety level. It is suggested that each NICU needs to develop a nurse managed supportive care program for parents of premature baby. Also, it is suggested that there need to investigate the coping mechanism of mother of premature baby.

대사증후군 위험군의 가족기능과 건강행위의 관계에서 질병인식의 매개효과 (Mediating Effect of Illness Perception on the Relationship between Family Function and Health Behavior in Patients with Risk Factors for Metabolic Syndrome)

  • Purpose: This study aimed to examine the mediating effect of illness perception on the relationship between family function and health behavior of patients with risk factors for metabolic syndrome. Methods: This is a cross-sectional correlational study. Data were collected from 160 patients using self-administered questionnaires including the Illness Perception Questionnaire-Revised, Family APGAR, and the Health Behavior Scale. The data were analyzed using Pearson's correlation coefficients, independent t-test and multiple linear regression analysis by the SPSS 23.0 program. Results: Health behavior was significantly correlated with family function (r=.30, p<.001) and illness perception of controllability by treatment (r=-.21, p=.007). Family function was a significant predictor of health behavior and illness perception, and the influence of family function on health behavior was partially mediated by illness perception of controllability by treatment. Conclusion: The findings of this study indicate that family function is a critical factor affecting health behavior and illness perception, and illness perception of controllability by treatment mediates the influence of family function on health behavior. It is necessary to develop a psycho-cognitive intervention program for enhancing supportive family function and for modifying negative illness perceptions to improve health behavior in patients with risk factors for metabolic syndrome.

동거유형에 따른 노인의 인지기능, 영양상태, 우울의 비교 (Comparison of Cognitive Levels, Nutritional Status, Depression in the Elderly according to Living Situations)

  • Purpose: The purpose of this study was to compare the degree of cognitive level, nutritional status and depression in elderly according to living situations. Method: The subjects consisted of 173 elderly classifying three groups(living alone, living with spouse, living with children). Data was collected from March to June, 2003 by a structured questionnaire that included general characteristics, MMSE-K, nutritional status and depression scale. The collected data was analyzed by the SPSS program including descriptive statistics, $\chi$$^2$-test, ANCOVA, Scheffe test and Pearson Correlation Coefficient. Result: In MMSE-K, the living alone group showed suspicious dementia while the other groups were normal. The living alone group showed a high nutritional risk and all three groups showed depression. In MMSE-K, the nutritional status and depression were statistically significant by the living situation. In each group except living with spouse, MMSE-K indicated a significantly negative correlation to depression and nutritional status, while nutritional status showed a significantly positive correlation to depression. Conclusion: It is necessary to develop supportive programs for decreasing the risk of bad health in the elderly and an individual approach according to their living situation. Especially, more concern and intervention is necessary for the solitary elderly.

Lived Experience among Patients Newly Diagnosed with Lung Adenocarcinoma Stage IV within One Year

  • Background: lung cancer (LC) is the fifth of the 10 leading causes of death in the world. LC is in first place for cancer-related mortality for both males and females in Taiwan. It is one of the most difficult cancers to treat and is often diagnosed at a late stage. Patients with stage IV are often unprepared for the diagnosis. Materials and Methods: To explore lived experience among patients newly diagnosed with lung adenocarcinoma stage IV within one year. Results: Twelve participants were recruited in this study. Content analysis of the interviews revealed four themes: (1) emotional roller coaster, (2) trying to find out causes, (3) adjusting my lifestyle, and (4) cancer fighter. Conclusions: This study provides new insight into the experiences of lung cancer patients y with newly diagnosed lung adenocarcinoma stage 4. These results will inform future supportive care service development and intervention research for patients with advanced stage cancer.

일부 응급구조과 학생과 간호과 학생의 안락사에 대한 태도조사 연구 (Altitudes or EMT and Nursing students toward Euthanasia)

  • This descriptive study was conducted to identify the attitude toward euthanasia of EMT and nursing students and to compare their attitude before and after clinical practice. The convenience sample was comprised of 40 first grade and 40 second grade EMT students attended at G college in G-city, and 40 first grade and 40 third grade nursing students attended at C college in C-province. The variable was tested with an euthanasia attitude scale developed by Kim Ae Kyoung(2001). The valid responses were obtained and analyzed by using SPSS PC+ from November 25 to December 6, 2002. The results showed that most of the respondents agreed to have positive attitude toward euthanasia in terms of client's right and client's quality of life. Also, they thought that euthanasia should be legalized and the right to die with dignity should be guaranteed. The EMT respondents having a clinical experience showed significantly higher score than respondents having no experience in the subarea of medical ethics, and nursing respondents who had a clinical practice showed significantly higher score than others no experience in the subarea of quality of life. But, the other sub-areas had no significant difference between the respondents before and after clinical practice. The attitude toward euthanasia composed of sub-areas such as being after the quality of life, having a high regard for a life and recognizing client's right was not affected by the single factor of clinical practice. Therefore, much supportive intervention need to be done for the students about the significant affecting factors found in this study like a religion, values, education and observing experience of someone's death.

일부 대학생의 사회적지지, 외로움, 음주 및 건강지각에 관한 연구 (Social Support, Loneliness, Alcohol Use and Perceived Health Status in College Students)

  • The purpose of this study was to investigate the level of social support, loneliness, alcohol use and perceived health status in college students and to explore the relationships between the variables. The convenience sample consisted of 473 students attending a college located at Chonnam province. Data were collected by a structured questionnaire which included the PRO85-partⅡ Social Support Scale, revised UCLA Loneliness Scale, Alcohol frequency, Perceived Health Status Scale from June 10 to 25, 2001. And data were analyzed by SPSS/PC+program including descriptive statistics, t-test, ANOVA, Pearson's Correlation Coefficients. The results are summarized as follows; 1. The mean scores of social support, loneliness and perceived health status were 3.01($SD={\pm}0.31$), 2.08($SD={\pm}0.34$), 1.64($SD={\pm}0.65$) respectively. 2. Students in use of alcohol mostly reported that they had started a drinking in the period of high school(35.7%), motivated with friendship(32.6%) and drank with their friends(56.9%) in drinking frequency of 2-3times per a month(49.6%). 3. The students who have friend of the opposite sex were significantly more likely to have higher than the students having no friend of the opposite sex in scores of social support. 4. The scores of loneliness were significantly lower in female students than male students, in students with friend of the opposite sex than with no friend of the opposite sex. 5. There were significant negative correlations between social support and loneliness(r=-5.25, p<.000), and between loneliness and perceived health status(r=-0.93, p<.05), while there was a significant positive correlation between social support and perceived health status(r=1.01, p<.05). The findings suggested that supportive social support, especially lowering loneliness, would be a powerful nursing intervention in maintaining good health of college students. And, more variables affecting health status in college students will be identified with further research.

보조생식술 시술 여성의 난임치료 지속 의도 관련 요인: 횡단적 연구 (Factors Influencing the Intention for Continual Fertility Treatments by the Women Undergoing Assisted Reproductive Technology Procedures: A Cross-Sectional Study)

  • Purpose: This cross-sectional study aimed to identify factors influencing the intention for continual fertility treatments among women undergoing assisted reproductive technology (ART). Methods: A total of 197 women were recruited through convenience sample from fertility hospitals in Gyeonggi-do and Busan, South Korea. Data were collected using a self-report questionnaire incorporating measures of uncertainty; Depression Anxiety Stress Scales; Fatigue Severity Scale; Coping Scale for Infertility-Women; spousal support; treatment environment; and intention for continual fertility treatment. Descriptive statistics, chi-square tests, t-tests, and logistic regression analysis were conducted using IBM SPSS 26.0. Results: As many as 70.6% of the participants expressed an intention for continual fertility treatments. Logistic regression analysis revealed that factors such as uncertainty (odds ratio [OR] = 0.44, 95% confidence interval [CI] 0.20~0.95), active coping (OR = 4.04, 95% CI 1.11~14.71), treatment environment (OR = 2.77, 95% CI 1.26~6.07), and the duration of marriage (OR = 2.61, 95% CI 1.24~5.49) were significantly related with this intention. Conclusion: These findings underscore the significance of uncertainty management, having proactive coping strategies, having supportive treatment environments, and considering the duration of marriage concerning women's intention for continual fertility treatment in the context of ART. The implications of these results extend to the development of nursing intervention programs aimed at providing crucial support for women undergoing ART and seeking to continue their infertility treatment.
