Utility Evaluation of Supportive Devices for Interventional Lower Extremity Angiography (인터벤션 하지 혈관조영검사를 위한 보조기구의 유용성 평가)

  • The purpose of this study is to evaluate the effectiveness of supportive devices which are for minimizing the patient's movement during lower extremity angiography and to verify image quality of phantom by analyzing of Mask image, DSA image and Roadmap image into SNR and CNR. As a result of comparing SNR with CNR of mask image obtained by DSA technique using the phantom alone and phantom placed on the supportive devices, there was no significant difference between about 0~0.06 for SNR and about 0~0.003 for CNR. The study showed about 0.11~0.35 for SNR and 0.016~0.031 for CNR of DSA imaging by DSA technique about only water phantom of the blood vessel model and the water phantom placed on the device. Analyzing SNR and CNR of Roadmap technique about water phantom on the auxiliary device (hardboard paper, pomax, polycarbonate, acrylic) and water phantom alone, there was no significant difference between 0.02~0.05 for SNR and 0.002~0.004 for CNR. In conclusion, there was no significant difference on image quality by using supportive devices made by hardboard paper, pomax, polycarbonate or acryl regardless of whether using supportive devices or not. Supportive devices to minimize of the patient's movement may reduce the total amount of contrast, exam-time, radiation exposure and eliminate risk factors during angiogram. Supportive devices made by hardboard paper can be applied easily during angiogram due to advantages of reasonable price and simple processing. It is considered that will be useful to consider cost efficiency and types of materials and their properties in accordance with purpose and method of the study when the operator makes and uses supportive devices.

  • Purpose: The objective is to extract insights that can contribute to the formulation of harmonized international policies and support measures for innovative medical devices and management systems. This study aims to propose effective strategies for future medical device innovation and healthcare delivery. Results: It investigates technological advancements, regulatory approval systems, insurance policies, and successful commercialization cases in South Korea, the United States, and the European Union. In 2018, the FDA implemented insurance coverage for Software as a Medical Device (SaMD) and recognized insurance coverage for Digital Therapeutics (DTx). Germany is a country that ensures permanent reimbursement for healthcare applications since 2020, making it the first country to provide legal health insurance coverage for fostering a digital ecosystem. Conclusion: The findings of this research highlight the importance of cultivating a supportive regulatory and environmental framework to facilitate the adoption of innovative medical devices. Continuous support for research and development (R&D) efforts by companies, along with the validation of clinical effectiveness, is crucial.

The Effect of Early Childhood Education and Care Institution's Professional Learning Environment on Teachers' Intention to Accept AI Technology: Focusing on the Mediating Effect of Science Teaching Attitude Modified by Experience of Using Smart·Digital Device (유아보육·교육기관의 교사 전문성 지원 환경이 유아교사의 인공지능 기술수용의도에 미치는 영향: 스마트·디지털 기기 활용 경험에 의해 조절된 과학교수태도의 매개효과를 중심으로)

  • Objective: This study aims to investigate whether science teaching attitude of early childhood teachers mediates the relationship between the professional learning environment of institutions and their intention to accept artificial intelligence (AI) technology, and whether the experience of using smart and digital devices moderates the effect of science teaching attitude. Methods: An online survey was conducted targeting 118 teachers with more than 1 year of experience in kindergarten and day care center settings. Descriptive statistical analysis, correlation analysis, and The Process macro model 4, 14 were performed using SPSS 27.0 and The Process macro 3.5. Results: First, the science teaching attitude of early childhood teachers served as a mediator between the professional learning environment of institutions and teachers' intention to accept AI technology. Second, the experience of using smart and digital devices was found to moderate the effect of teachers' science teaching attitude on their intention to accept AI technology. Conclusion/Implications: This results showed that an institutional environment that supports teachers' professionalism development and provides rich experience is crucial for promoting teachers' active acceptance of AI technology. The findings highlight the importance of creating a supportive institutional envionment for teacher's professional growth, enhancing science teaching attitudes, and facilitating the use of various devices.

Application of Bioabsorbable Plates in Orthognathic Surgery

  • Backgrounds: Though they are considered as reliable devices, titanium plates and screws have limitations due to some potential problems. To overcome these problems, researches on bioabsorbable materials for internal fixation have been continuing. Recently, there are many clinical trials to apply biodegradable internal fixation devices in oral and maxillofacial surgery. The purpose of this study was to evaluate the clinical results of application of bioabsorbable plates and screws in orthognathic surgery. Methods: Fifty-four patients with dentofacial deformity (17 male patients and 37 female patients) were selected in this study. The patients were treated by internal fixation using bioabsorbable plates and 2.4-mm, 2.0-mm pretapped screws (Biosorb FX, Bionix Implants, Inc., Finland). The patients were evaluated for complications during the follow-up period. Results: Five patients (9.3%) experienced complications. All complications in these cases were infection. No other complications related with physical or mechanical properties of bioabsorbable plates were found such as malunion or nonunion, fractures of plates and loosening of screws. All complications were minor and adequately managed with drainage and supportive care with antibiotics coverage. Conclusions: From the results, the use of these fixation systems in orthognathic surgery will provide a promising alternative titanium fixation in appropriate cases.

Game Transformation from Non-Augmented Reality to Augmented Reality

  • The evolution of technology has lead gaming playable in augmented reality and more games are developed only for augmented reality. There are mainly no supportive reasons for game designed in non-augmented reality and transforms to augmented reality. Genre of game played in augmented reality is always one of the defined game genres. In this paper, transformation process and effects of mobile games to augmented reality is discussed. Augmented reality game is recommended for mobile devices due to its portability. Modification for mobile games transformation including way of game rendering, game controlling, camera view controlling and game environment design are discussed. User experience is affected after the game transformed to augmented reality because of the differences in game controlling. The presence of game in real world is increased due to the usage of actual view in game playing.

On forced and free vibrations of cutout squared beams

  • Perforation and cutouts of structures are compulsory in some modern applications such as in heat exchangers, nuclear power plants, filtration and microeletromicanical system (MEMS). This perforation complicates dynamic analyses of these structures. Thus, this work tends to introduce semi-analytical model capable of investigating the dynamic performance of perforated beam structure under free and forced conditions, for the first time. Closed forms for the equivalent geometrical and material characteristics of the regular square perforated beam regular square, are presented. The governing dynamical equation of motion is derived based on Euler-Bernoulli kinematic displacement. Closed forms for resonant frequencies, corresponding Eigen-mode functions and forced vibration time responses are derived. The proposed analytical procedure is proved and compared with both analytical and numerical analyses and good agreement is noticed. Parametric studies are conducted to illustrate effects of filling ratio and the number of holes on the free vibration characteristic, and forced vibration response of perforated beams. The obtained results are supportive in mechanical design of large devices and small systems (MEMS) based on perforated structure.

Evaluating the Efficacy of Anti-wrinkle Products in USA

  • Introducing to the market place, safe and effective product is an important responsibility of clinical investigators as well as regulatory agencies in all developed countries. Products claiming to improve skin wrinkles are no exceptions. To date, Renova(R) (all-trans retinoic acid), Avage(R) (tazarotene), and Botox(R) (botulinum toxin) are the only agents FDA approved to ameliorate wrinkles associated with photoaged skin in the USA. For all three, clinical evaluation of wrinkle severity was the primary endpoint required for the approval process. No sophisticated instrument measurements of wrinkles were required, nor used in the pivotal studies. The Division of Dermatologic & Dental Products of the US FDA (Director, Jonathan Wilkin, MD) is not against the use of mechanical instruments in assessing wrinkle severity. Its position on this issue however, remains that any such device must be grounded in patients' or product users' perspective, which means that the evaluation instrument must be clinically relevant and clinically perceptible. Sophisticated devices that can detect minimal improvement, but imperceptible to the users are considered useless in the eyes of the US FDA. Two instruments that have been tried in some antiwrinkle studies in the USA are silicone replicas and Primos. Despite their sophistications, they have clear limitations; thus have never replaced clinical evaluations in these studies. At most, they have served as secondary measures to provide corroborative data on the clinical efficacy of antiwrinkle products. For the foreseeable future, at least in the USA, careful clinical assessment of wrinkles will continue to serve as the critical benchmark to determine whether an antiwrinkle product has enough efficacy to benefit its users. We must not lose sight of the fact that sophisticated devices are only to serve in generating supportive evidence, and not the primary evidence, in any clinical studies.

Needle Stick Injuries and their Related Safety Measures among Nurses in a University Hospital, Shiraz, Iran

  • Background: This study aimed to determine the prevalence and factors related to needle stick injuries (NSIs) and to assess related safety measures among a sample of Iranian nurses. Methods: In this cross-sectional study, a random sample of 168 registered active nurses was selected from different wards of one of the hospitals of Shiraz University of Medical Sciences (SUMS). Data were collected by an anonymous questionnaire and a checklist based observational method among the 168 registered active nurses. Results: The prevalence of NSIs in the total of work experience and the last year was 76% and 54%, respectively. Hollow-bore needles were the most common devices involved in the injuries (85.5%). The majority of NSIs occurred in the morning shift (57.8%) and the most common activity leading to NSIs was recapping needles (41.4%). The rate of underreporting NSIs was 60.2% and the major reasons for not reporting the NSIs were heavy clinical schedule (46.7%) and perception of low risk of infection (37.7%). A statistically significant relationship was found between the occurrence of NSIs and sex, hours worked/week, and frequency of shifts/month. Conclusion: The study showed a high prevalence of NSIs among nurses. Supportive measures such as improving injection practices, modification of working schedule, planning training programs targeted at using personal protective equipment, and providing an adequate number of safety facilities such as puncture resistant disposal containers and engineered safe devices are essential for the effective prevention of NSI incidents among the studied nurses.

Contents Analysis of Media on Long-term Care Insurance (노인장기요양보험 관련 미디어 내용 분석)

  • Objectives : This study confirmed the limitation of long-term care insurance by analyzing media contents. Methods : Articles and reviews were searched with the article searching system (KINDS) from July 2008 to December, 2015. Results : Among the 155 articles examined, 61.1% highlighted the faults of suppliers, and 25.2% indicated the responsibility of the insurer. As for their purpose, 56.8% reported on accidents, and 32.3% provided information. Furthermore, 74.2% reported on negative contents and only 25.8% on neutral contents. The negative contents consisted of requesting false insurance benefits, amending the range and price indicating the very low salaries of the care givers, limitations on the care grade assessment, and problems related with assistive devices. The majority of neutral articles is for providing information. Conclusions : There were many problems starting from the early stage of the insurance. We must pay attention to these problems. Moreover, we should try to handle and prevent these problems with supportive responses from authorities.

Deep Neural Network-Based Critical Packet Inspection for Improving Traffic Steering in Software-Defined IoT

  • With the rapid growth of intelligent devices and communication technologies, 5G network environment has become more heterogeneous and complex in terms of service management and orchestration. 5G architecture requires supportive technologies to handle the existing challenges for improving the Quality of Service (QoS) and the Quality of Experience (QoE) performances. Among many challenges, traffic steering is one of the key elements which requires critically developing an optimal solution for smart guidance, control, and reliable system. Mobile edge computing (MEC), software-defined networking (SDN), network functions virtualization (NFV), and deep learning (DL) play essential roles to complementary develop a flexible computation and extensible flow rules management in this potential aspect. In this proposed system, an accurate flow recommendation, a centralized control, and a reliable distributed connectivity based on the inspection of packet condition are provided. With the system deployment, the packet is classified separately and recommended to request from the optimal destination with matched preferences and conditions. To evaluate the proposed scheme outperformance, a network simulator software was used to conduct and capture the end-to-end QoS performance metrics. SDN flow rules installation was experimented to illustrate the post control function corresponding to DL-based output. The intelligent steering for network communication traffic is cooperatively configured in SDN controller and NFV-orchestrator to lead a variety of beneficial factors for improving massive real-time Internet of Things (IoT) performance.
