청소년기에서 발생한 슬개골 상단부 소매형 골절 (Sleeve Fracture of the Superior Pole of Patella in an Adolescent)

  • A patella sleeve fracture is a type of avulsion fracture in which a small osseous fragment gets, along with a sleeve of periosteum and cartilage, is pulled off from the patella. The avulsed sleeve of cartilage and periosteum is not apparent on plain radiographs, which can result in a missed diagnosis or an underestimate of the extent of injury. An avulsion or sleeve fracture of the patella in a child can occur at the superior or the inferior pole of the patella. However, most reported cases of sleeve fractures involve the inferior patellar pole, with fractures involving the superior patellar pole being very rare. The authors report a case of a sleeve fracture of the superior pole of the patella in an adolescent; the fracture was diagnosed with magnetic resonance imaging (MRI) and was treated with surgery after early diagnosis. The course was uneventful, and the outcome was excellent.

  • 소아에서의 슬개골 골절은 성인을 포함한 전체 슬개골 골절의 약 2% 미만에서 발생하고, 모든 소아 골절의 약 1% 정도로 매우 드물게 발생한다. 성장중인 슬개골은 성인에서의 슬개골 골절과는 달리 골연골 골절 또는 견열 골절의 형태로 흔히 발생하며, 그 형태는 주로 소매형 골절로 나타난다. 소아 슬개골의 견열 골절 또는 소매형 골절은 슬개골의 상단부 혹은 하단부에서 발생하나, 대부분 슬개골 하단부가 주를 이루며, 상단부에 생긴 경우는 드물다. 저자들은 16세 소아에서 슬개골 상단부의 소매형 골절 1례를 경험하여 이를 문헌고찰과 함께 보고하는 바이다.

  • The FRP pole had great advantages over other material poles. Light weight, easy installing and transporting were good characteristics. The material's superior properties represented the good durability for sea weather and air pollution, good insulation for electric, and changeable colors. In those properties, usages were like a area affected by sea, downtown, the area among the mountains and a special area for the outstanding views. It was studied that pole manufacturing method, structure analysis of pole by FEM in this study. Filament winding method was selected for a new pole manufacturing method. It produced the tapered poles and mechanically strong properties.

2차계 Pole-Zero IIR필터를 사용한 Hybrid필터의 설계 (The Design of Hybrid Filter using 2-nd order Pole-Zero IIR Filters)

  • 전단의 IIR필터를 2차계의 pole-zero형으로 취하고 후단의 FIR필터의 필터 길이를 최소화시키는 hybrid필터의 설계를 제시한다. 시뮬레이션 결과 FIR필터의 경우보다 필터 길이가 현저히 줄어 들었고 또한 IIF필터를 all-pole형으로 취했을 때보다도 다소 줄어들었다. Hybrid 필터가 정확한 선형위상(linear phase)특성을 갖지는 못하나 통과대역과 서지대역에서 IIR필터보다는 비교적 정확한 선형특성을 갖게 됨을 알 수 있다.

Associated Factors and Prevention of Upper Pole Rippling in Prepectoral Direct-to-Implant Breast Reconstruction

  • Background Despite its many advantages, prepectoral breast reconstruction also carries the risk of implant rippling. The recent introduction of partial superior implant coverage using a pectoralis muscle slip in prepectoral direct-to-implant (DTI) breast reconstruction has shown the potential to minimize upper pole rippling. The purpose of this study was to identify factors associated with rippling and the effectiveness of our surgical technique. Methods In total, 156 patients (186 breasts) who underwent prepectoral DTI breast reconstruction between August 2019 and March 2021 were identified retrospectively. Patient data were analyzed from medical records. Univariable and multivariable logistic analyses were performed to contextualize the risks associated with rippling deformity relative to demographic characteristics and other clinical factors. Retrospective propensity-matched analysis was performed to identify the relationship between rippling deformity and the reconstruction method. Results Patients with body mass index (BMI; odds ratio [OR], 0.736; p < 0.001), those with a postoperative chemotherapy history (OR, 0.324; p = 0.027) and those who received breast reconstruction via the superior coverage technique (OR, 0.2; p = 0.004), were less likely to develop rippling deformity. The median follow-up period was 64.9 weeks, and there were no significant differences between patients in types of mastectomy, implant, or acellular dermal matrix. Patients who underwent superior coverage technique-based reconstruction showed significantly reduced rippling (OR, 0.083; p = 0.017) Conclusion Patients with higher BMI and prior postoperative chemotherapy were less likely to develop rippling deformity. The superior coverage technique can be effective in minimizing upper pole rippling.

Delayed versus Delayed-Immediate Autologous Breast Reconstruction: A Blinded Evaluation of Aesthetic Outcomes

  • Background The technique of delayed-immediate breast reconstruction includes immediate insertion of a tissue expander, post-mastectomy radiation, followed by reconstruction. The aesthetic benefits of delayed-immediate reconstruction compared to delayed reconstruction are postulated but remain unproven. The purpose of this study was to compare aesthetic outcomes in patients following delayed and delayed-immediate autologous breast reconstruction. Methods A retrospective analysis was performed of all patients who underwent delayed or delayed-immediate autologous breast reconstruction by the senior author from 2005 to 2011. Postoperative photographs were used to evaluate aesthetic outcomes: skin quality, scar formation, superior pole contour, inferior pole contour, and overall aesthetic outcome. Ten non-biased reviewers assessed outcomes using a 5-point Likert scale. Fisher's Exact and Wilcoxon-Mann-Whitney tests were used for comparative analysis. Results Patient age and body mass index were similar between delayed (n=20) and delayed-immediate (n=20) cohorts (P>0.05). Skin and scar quality was rated significantly higher in the delayed-immediate cohort (3.74 vs. 3.05, P<0.001 and 3.41 vs. 2.79, P<0.001; respectively). Assessment of contour-related parameters, superior pole and inferior pole, found significantly improved outcomes in the delayed-immediate cohort (3.67 vs. 2.96, P<0.001 and 3.84 vs. 3.06, P<0.001; respectively). Delayed-immediate breast reconstruction had a significantly higher overall score compared to delayed breast reconstructions (3.84 vs. 2.94, P<0.001). Smoking and the time interval from radiation to reconstruction were found to affect aesthetic outcomes (P<0.05). Conclusions Preservation of native mastectomy skin may allow for improved skin/scar quality, breast contour, and overall aesthetic outcomes following a delayed-immediate reconstructive algorithm as compared to delayed breast reconstruction.

세탁기용 외전형 BLDC 전동기의 코깅 토오크 저감에 관한 연구 (A Study on the Reduction of Cogging Torque of Outer-Rotor Type BLDC Motor for Washing Machines)

  • This paper deals with the reduction of cogging torque of a outer-rotor type BLDC motor mainly used for washing machines. The motor comprises permanent magnet outer-rotor and stator with coils and core. This structure inherently produces vibration and cogging torque because of uneven reluctance according to rotation of the rotor. Up to now, adopted a type of 24 magnet pole and 36 slot-stator. This generates high main torque but accompanies comparatively large cogging torque. This paper proposes a 32-pole 36-slot type motor which reduces cogging torque remarkably. The influence of cogging torque is varied according to pole-slot combinations. The characteristic of the motor was obtained by a two-dimensional finite element method coupled with a drive circuit. The performance of the proposed model is superior to that of the existing model because of the reduction of torque ripple and the improvement of back ernf wave form.

  • 본 논문에서는 세계최초로 내철형 주상변압기 코일의 권선을 일반적인 권선방법에서 Zig-Zag(가칭) 권선방법으로 설계 제작하는 방법을 제안하였으며, 또한 바니시 함침공정을 생략하고 단락강도 및 절연내력을 향상시킬 수 있는 내철형 변압기용 권선의 신프레임 구조개발 내용을 제안하였다. 기존의 주상변압기 권선방법에 비하여 전기절연 측면에서 층간 절연지의 사용매수 및 두께를 획기적으로 절감할 수 있는 장점이 있다. 또한 원가절감과 손실저감 및 단락기계력 대처능력이 우수함을 검증하였으며 국내전력회사 배전계통 주상변압기에 유용하게 활용할 수 있다.

Generalized optimal active control algorithm with weighting matrix configuration, stability and time-delay

  • The paper presents a generalized optimal active control algorithm for earthquake-resistant structures. The study included the weighting matrix configuration, stability, and time-delays for achieving control effectiveness and optimum solution. The sensitivity of various time-delays in the optimal solution is investigated for which the stability regions are determined. A simplified method for reducing the influence of time-delay on dynamic response is proposed. Numerical examples illustrate that the proposed optimal control algorithm is advantageous over others currently in vogue. Its feedback control law is independent of the time increment, and its weighting matrix can be flexibly selected and adjusted at any time during the operation of the control system. The examples also show that the weighting matrix based on pole placement approach is superior to other weighting matrix configurations for its self-adjustable control effectiveness. Using the time-delay correction method can significantly reduce the influence of time-delays on both structural response and required control force.

  • The PM(Permanent Magnet) stepping Motor has been used widely to a variety of industries because of the open-loop control characteristic, a relatively large frequency range of velocity control and non-accumulated error of the step angle. Moreover, It has been gradually applied to office automation, machine intelligence, digital appliance, and automobile parts. This paper presents the optimal design results by use of the experimental method. The design variables of the PM stepping motor are shape of the claw-pole, material of core, and air-gap. As a result, a superior claw-pole PM stepping motor for OA machinery was developed.
