Solid dispersion (SD) system of everolimus (EVR) with Gelucire 50/13 (Stearoyl polyoxyl-32 glycerides) was prepared using melt granulation technique with the aim of improving the physicochemical properties and dissolution rate. The solid state characterization using scanning electron microscopy and X-ray powder diffraction, indicated that the drug was homogeneously distributed in the surfactant carrier in a stable amorphous form. The dissolution rate of EVR from the optimized SD composed of the drug, Gelucire 50/13 and microcrystalline cellulose in a weight ratio of 1:5:10, was markedly rapid and higher than that from the drug powder and the market product (Afinitor$^{(R)}$, Novartis Pharmaceuticals) in all dissolution mediums tested from pH 3.0 to pH 6.8. The results of this study suggest that formulation of SD with Gelucire 50/13 using melt granulation procedure may be a simple and promising approach for improving the dissolution rate and oral absorption of the anti-cancer agent without the need for using an organic solvent.
The aim of this study is to describe the physical processes taking place in the solar photosphere. Based on 3D hydrodynamic simulations including a detailed radiation transfer scheme, we investigate thermodynamic structures and radiation fields in solar surface convection. As a starting model, the initial stratification in the outer envelope calculated using the solar calibrations in the context of the standard stellar theory. When the numerical fluid becomes thermally relaxed, the thermodynamic structure of the steady-state turbulent flow was explicitly collected. Particularly, a non-grey radiative transfer incorporating the opacity distribution function was considered in our calculations. In addition, we evaluate the classical approximations that are usually adopted in the onedimensional stellar structure models. We numerically reconfirm that radiation fields are well represented by the asymptotic characteristics of the Eddington approximation (the diffusion limit and the streaming limit). However, this classical approximation underestimates radiation energy in the shallow layers near the surface, which implies that a reliable treatment of the non-grey line opacities is crucial for the accurate description of the photospheric convection phenomenon.
Open calcaneal fracture with more than lateral half of bone loss and soft tissue defect occurred in 17 year-old male patient due to motor vehicle accident. Soft tissue defect included heel pad, peroneal tendon. Bone loss involved mainly most part of inferior tuberosity but not subtalar joint. Open dressing and debridement were done daily in operating room and antibiotics administration was started. After granulation tissue formed, femoral head allograft was performed and fixed with 6.0 mm screws to replace bone defect. Soft tissue defect was covered with latissimus dorsi musculocutaneous free flap. No sign of infection nor major osteolysis was observed in 15 months follow up period. Aspirin or acetylsalicylic acid, a member of the salicylate family, is frequently used as an analgesic, antipyretic, anti-inflammatory and antiplatelet drug. Because aspirin is chemically unstable in water and heat for tablet formulation, additives including lubricants are used in preparing aspirin tablets, using a dry-granulation process. Aspirin tablets are produced by a number of manufacturers which usually use their own unique combination of additives during the manufacturing process. In this study, we employed an NMR based metabolomics technique to identify the manufacturers of various aspirin tablets. Aspirin tablets from six different companies were analyzed by 1H 400 MHz NMR. The acquired data was then integrated and processed by principal component analysis (PCA). Based on the NMR data, we were able to identify peaks corresponding to acetylsalicylic acid in all of the six samples, whereas different NMR patterns were found in the aromatic and aliphatic regions depending on the unique additive used. These observations led to the conclusion that the differences in the NMR patterns among the different aspirin tablets were due to the presence of additives.
Gouty ulcer can be caused by the accumulation of clumps of uric acid in body tissues that lead to acute or chronic inflammation at sites of accumulation. Furthermore, tophi-inhibiting granulation tissue may form a canal that channels microbial infection from the underlying involved joint space, and thus, presents the risk of osteomyelitis development. Accordingly, gouty ulcer must be treated appropriately. In this case, refractory wounds on gouty ulcers at the left shin and left radial ankle were treated by surgical debridement. Negative-pressure wound therapy was used successfully to prevent post-operative delayed wound healing.
본 연구는 몽골 도르릭나르스 유적 2호, 3호 및 4호분에서 출토된 청동제 및 금제 유물에 대한 과학적 분석 결과로부터 각 유적 출토품의 특징을 알아보는데 그 목적이 있다. 몽골의 흉노무덤인 도르릭나르스 유적 2호, 3호 및 4호분은 비교적 소형이고, 도굴 된 상태였음에도 불구하고 많은 유물이 다양하게 출토되었다. 특히 출토 유물에는 중국계 유물과 북방계 유물이 공존하고 있어 매우 중요한 유적으로 평가되고 있다. 먼저 청동 용기류의 경우 중국제인 한경이나 일산살 꼭지의 성분 조성과는 달리 주석에 비하여 납의 함유량이 높은 특징을 보였는데, 특히 2호의 청동쟁반, 청동등잔 그리고 4호의 청동솥의 경우는 주석이 1wt% 정도로 적게 함유되어 있어 Cu-Pb의 2원계 합금 조성을 보였다. 금제품의 경우는 2호분 장식품의 제작에 비교적 높은 순도의 금이 사용된 것으로 조사되었으며, 금제 누금 장식의 금 알갱이의 접합에는 금과 구리를 합금한 금땜이 사용되었거나 확산접합(말라카이트나 산화구리 화합물 가루를 사용함)에 의하여 접합했을 가능성이 높았다. 향후 몽골지역 뿐만 아니라 다른 지역의 흉노 무덤 출토품에 대한 과학적 조사 및 분석이 더 많이 진행된다면 흉노 문화에 대한 더 많은 문제들을 해결할 수 있는 실마리를 제공하게 될 것이다.
본 연구에서는 영양과 건강, 그리고 편의성까지 고려한 즉석식품 형태의 쌀미음을 제조하였다. 제조된 쌀미음 분말의 영양성분 구성비를 일반성분 분석을 통하여 분석 하였고, 과립공정에 따른 재수화능의 향상을 연구하였다. 과립분말은 혼합분말보다 지방함량과 단백질 함량이 각각 0.9, 1.9% 높게 나타난 반면, 탄수화물 함량은 상대적으로 3.2% 낮게 나타났다. 과립분말의 단백질 조성은 71.1-118.5 g으로 1일 권장 단백질 섭취량 45-55 g보다 많았는데, 이는 일반적인 환자식이 지니는 공통적인 특성이다. 분말 입자의 침강 속도를 보여주는 분산성의 경우 과립분말(93.7%)은 혼합분말(77.0%) 보다 높게 나타났고, 재수화에 걸리는 시간도 과립분말(122.3 초)이 혼합분말(305.3 초)보다 빨랐으며 이들 결과는 과립공정이 분말의 재수화능을 향상시켜 주었음을 보여준다. 호화쌀가루를 이용한 쌀미음은 기존의 우유 단백질 위주의 환자식을 곡류로 대체하며 영양강화 쌀 가공제품의 개발에 의미가 있다.
Inter-granular Bright Points (igBPs) are small-scale objects in the Solar photosphere which can be seen within dark inter-granular lanes. We present a new algorithm to automatically detect and extract igBPs. Laplacian and Morphological Dilation (LMD) technique is employed by the algorithm. It involves three basic processing steps: (1) obtaining candidate "seed" regions by Laplacian; (2) determining the boundary and size of igBPs by morphological dilation; (3) discarding brighter granules by a probability criterion. For validating our algorithm, we used the observed samples of the Dutch Open Telescope (DOT), collected on April 12, 2007. They contain 180 high-resolution images, and each has a $85{\times}68\;arcsec^2$ field of view (FOV). Two important results are obtained: first, the identified rate of igBPs reaches 95% and is higher than previous results; second, the diameter distribution is $220{\pm}25km$, which is fully consistent with previously published data. We conclude that the presented algorithm can detect and extract igBPs automatically and effectively.
Since the insertion of self expandable metalic stent[SEMS has became popular method for hollow organ stenosis, many attempts for further apply the stent to airway stenosis as an simple procedure has been made, but intrabronchial migration of stent or occurrence of inflammatory granuloma around stent develop occasionally and sometimes it worsen bronchial stenosis further more. This report describes 2 case of surgically treated bronchial restenosis in whom intrabronchial stent were applied for release of bronchial stenosis. Our surgical option was pneumonectomy and bronchoplasty with sleeve right middle and upper lobectomy respectively. During the operation we found the SEMSs were tightly impacted in restenotic bronchial lumen with overgrowth of granulation tissues. The bronchial obstructions occupied more than 90% of lumens in both cases, and needed much complicated procedure to be relieved. Therefore, even though the insertion of SEMS remains as a prcedure determined by the physician`s preference, it has to be considered prudently that the use of SEMS can cause severe restenosis and the surgeon has more difficulties in performing segmental resection of restenotic bronchus in patient with SEMS previously inserted. Throughout these experiences we can conclude that the insertion of SEMS must be performed only in very selected cases of bronchial stenosis.
Background and Objectives : The treatment of supraglottic stenosis remains a challenging problem in the field of otolaryngology due to its association with dyspnea, dysphagia, and frequent recurrence. Any satisfactory treatment is not yet known. The author experienced six cases of supraglottic stenosis and report the successful treatment of five cases by repeated endoscopic laryngeal excision with KTP-532 laser under suspension layngoscopy. Materials and Methods : Six adults who were treated for supraglottic stenosis between March 1994 and December 1998 at the Department of Otoloaryngology-Head and Neck Surgery, Korea University Medical Center were studied retrospectively. The patients were placed under general anesthesia followed by endoscopic laryngeal excision with KTP-532 laser under supension laryngoscopy. The scar tissue and granulation tissue were visualized with an operating microscope, and then removed using KTP-532 laser (15watts, continuous mode). Intraoperative local steroid(Triamcinolone ) was injected in all cases after the stenotic portions were removed. Results : Endoscopic excision was performed in five cases ; among the five cases, cricoid cartilage was concomitantly removed in two cases, and epiglottis was removed in one case. Satisfactory swallowing and airway respiration were possible in all five patients who underwent endoscopic widening. Conclusion : The treatment of supraglottic stenosis is different from that of tracheal or glottic stenosis in that supraglottic stenosis is mainly developed in membraneous form. Repeated laser excision and local steroid injection under suspension laryngoscopy is an effective and recommend able method for the treatment of supraglottic stenosis.
