Objectives: Among the Korean elderly (those 65 years of age and older), the suicide rate is 80.3/100 000 people, which is ten times higher than the Organization for Economic Cooperation and Development average. Because South Korea is rapidly becoming an aging population, this high elderly suicidal rate will only get worse. Although the size of the elderly suicide problem is quite large, previous research in South Korea has surveyed restricted areas and not the entire country. Even though the factors that affect elderly suicide are complicated, there has been little research into these influencing factors. Thus, this research uses the national survey data (Community Health Survey) that was obtained in 2009. Additionally, we analyze factors affecting elderly suicidal ideation and attempts as well as the paths of these effects. Methods: Community Health Survey data obtained by the Korea Centers for Disease Control and Prevention in 2009 was used for this study. We additionally examined the factors that affect suicide with chi-squared tests, t-tests, Pearson's correlation test, and path analysis. Results: Depressive symptoms and suicidal ideation are the only factors that directly affect suicidal attempts. Demographic, behavioral, and physical activity factors have indirect effects on suicidal attempts. Conclusions: Depression has the strongest influence on suicidal ideation and attempts. Demographic, behavioral, and physical activity factors affect suicidal attempts mostly through depressive symptoms. In addition, there is a path that suggests that demographic, behavioral, and physical activity factors affect suicidal attempts not through depression symptoms but only through suicidal ideation. This means that the elderly who do not have depression symptoms attempt suicide according to their own situations and characteristics.
Objectives: The study aimed to analyze the effects of household financial difficulties caused by COVID-19 (2019 coronavirus disease) on suicidal tendencies of adolescents. Methods: We selected 54,948 middle and high school students who were surveyed based on the Korean Youth Risk Behavior Web-based Survey 2020. To analyze the data, we used the STATA 16.0 program to conduct propensity score matching (PSM). Results: After controlling for selection effects by using PSM, the household financial difficulties caused by COVID-19 maintained a significant predictive effect on increasing suicidal ideation, suicide attempts, and decreasing tendency in hospital-treated after suicide attempts. However, depressive symptoms and suicide plan did not show a significant correlation with household financial difficulties associated with COVID-19. Conclusions: It was found that the rate of suicidal ideation and suicide attempts among adolescents who experienced a household financial difficulties due to COVID-19. Therefore, It can provide empirical evidence for estimating the impact of COVID-19 on adolescent suicide rates.
Previous studies on the vast increase in suicide mortality in Southeast Asia have indicated that suicide rates increase in parallel with a rise in unemployment or during periods of economic recession. This paper examines the effects of economic recession on suicidal rates amongst agriculture, fisheries, and forestry workers in Korea. Monthly time-series gross domestic product (GDP) data were linked with suicidal rates gathered from the cause of death records between1993-2008. Data were analyzed using generalized additive models to analyze trends, while a polynomial lag model was used to assess the unconstrained time lag effects of changes in GDP on suicidal rate. We found that there were significant inverse correlations between changes in GDP and suicide for a time lag of one to four months after the occurrence of economic event. Furthermore, it was evident that the overall relative risks of suicide were high enough to bring about social concern.
Purpose: Various intervention strategies are needed to reduce the suicide rate among late adolescents. Therefore, we aimed to investigate the factors that can help overcome suicidal tendencies in late adolescents from the perspective of case managers. Methods: This qualitative study explored the reinforcing factors that can help overcome suicidal behaviors in late adolescents, by conducting focus group interviews with case managers from the local community. Interviews with 10 case managers were recorded and transcribed, and the data were analyzed using the content analysis method. Results: The main theme of this study was "a step in a new direction". Three categories were derived: "inner changes", "another possibility opened up by the interest of significant others", and "connecting with community resources to help overcome". Each category had subcategories that influenced the intention and course of action of behaviors to overcome the suicide crises. Conclusion: Our findings, from the exploration of the experiences of case managers, provide an in-depth understanding of the reinforcing factors for overcoming suicide among late adolescents. They provide useful preliminary data for the development of effective suicide prevention programs for the adolescents.
본 연구는 도시 농촌지역 고등학생들의 자살에 대한 인식 및 태도를 알아보고, 그에 관련된 요인을 파악하기 위하여 우리나라 중부권에 소재한 K시의 도시지역 학생 1,114명, 농촌지역 학생 968명을 대상으로 2009년 7월 1일부터 7월 31일까지 구조화된 자기기입식 설문지를 이용하여 설문조사하였다. 연구결과 최근 1년간 자살생각을 갖은 고등학생은 38.8%였으며 도시와 농촌 간에 유의한 차이가 없었다. 자살에 대한 인식 및 태도는 농촌지역학생이 도시지역학생보다 높았으며, 자살 인식도에 영향을 미치는 관련요인으로 도시지역학생은 부모와 동거유무, 학교계열, 최근1년 간 자살생각유무, 자살태도가 유의한 변수로 선정되었으며(설명력 22.3%), 농촌지역학생에서는 성별, 부모와 언쟁 유무, 최근1년 자살생각유무, 자살태도가 유의한 변수로 선정되었다(설명력 37.1%). 자살태도에 영향을 미치는 관련요인으로 도시지역학생은 학교계열, 자살 인식도가 유의한 변수로 선정되었으며(설명력 20.6%), 농촌지역학생은 성별, 흡연유무, 자살 인식도가 유의한 변수로 선정되었다(설명력 38.8%). 따라서 자살에 대한 인식과 태도에 영향을 주는 가정환경 및 학교생활 변인을 적극적으로 관리하는 프로그램 개발 및 자살예방을 위한 지속적인 관심과 지도가 필요하다.
Depression has a relatively high lifetime prevalence rate in spite of a genetic influence on its etiology and a high mortality rate in untreated cases. This suggests the possibility that depression gives us evolutionary benefits which we do not exactly know yet. There have been several hypotheses which tell us what evolutionary advantages depression could give us. The psychic pain hypothesis considers sad or depressed mood as a negative reward just like physical pain which we have to avoid for our protection and survival. The social rank hypothesis holds that depressed mood is very similar to the emotional state of the defeated in social competition which prevents him from further protesting or fighting that might cause additional damage to him. The inclusive fitness hypothesis views suicide as a sacrifice phenomenon to contribute to inclusive fitness. These hypotheses gives us new insights into mood disorders and also some suggestions about the conditions in which depressed mood or suicidal behaviors increase and the ways to reduce them.
자살은 우리 사회의 주요한 사회문제이며 현재 우리나라의 자살률은 OECD 국가 중 가장 높다. 자살의 주요 요인 중 하나로 노동시장에서의 고용불안정성이 자살 생각에 영향을 미친다고 알려져 있다. 무직 또는 한시적·일시적 고용형태의 근로자는 불안정한 고용 때문에 자살 생각이 높으며 향후 자살 위험군 또는 고위험군이 될 가능성이 높으므로 고용안정성이 자살에 미치는 요인 및 이를 완충시키는 보호요인에 대한 주의 깊은 연구가 필요하다. 이에 본 연구는 고용안정성이 자살생각에 미치는 영향을 살펴보고 보호요인으로서 가족기능의 조절효과를 검증하였다. 이를 위해 전국규모의 종단연구인 '사회적 관계 내에서 자살을 경험한 자살생존자의 정신건강 추적연구'의 자료(N=1,235)를 분석하였다. 분석결과 고용안정성은 자살생각과 유의한 연관성이 있었으며(B=-.904, p<.001) 무직자가 가장 높은 자살생각을 가진 것으로 나타났다. 또한 가족기능의 조절효과가 유의하여(B=-.218, p<.05) 불안정한 고용상태가 자살생각에 영향을 미치는 관계에서 가족기능이 완충작용을 함이 나타났다. 이러한 결과에 근거해 불안정한 고용상태에서 가족 차원의 자살예방 개입을 위한 정책적, 실천적 함의를 논의하였다.
우리나라 자살률은 Organization for Economic Cooperation and Development 국가 중 압도적 1위이며, 특히 아동·청소년들은 삶의 만족도가 낮고, 자살률 또한 매년 증가하고 있다. 아동·청소년 시기는 어느 발달단계보다 격동적이고 충동적인 시기로 자살 그 자체가 치명적이고, 전염성이 높다. 또한 의미 있는 작업을 잃어버렸을 경우에도 자살률이 높은데, 이는 자살고위험군 아동·청소년도 마찬가지이다. 정신건강복지법이 개정되고, 정신건강전문요원에 작업치료사가 포함되기 시작하면서 정신건강서비스에 있어 작업치료를 포함한 다학제적 접근이 강조되고 있다. 아동·청소년 자살을 막기 위한 선별도구로 자살사고척도(Suicidal Ideation Questionnaire), Beck의 자살사고척도(Beck Scale for Suicide Ideation), 콜롬비아 대학 자살 심각성 평가 척도(Columbia-Suicide Severity Rating Scale), 청소년용 삶의 이유척도(The Reasons for Living Inventory for Adolescents), 학생 정서·행동 특성검사(Student Emotional and Behavioral Screening Test)가 사용되었고, 작업치료평가도구인 Canadian Occupational Performance Measure도 사용되었다. 또한 다양한 학문과 기관에서 다양한 자살예방프로그램이 제시되고 있다. 하지만 자살고위험군 현실적 특성상 신속하고 적기에 서비스를 제공해야 하기 때문에 다른 자살예방프로그램보다 상대적으로 그 수가 적으며 이 또한 대부분 인지행동적 측면에 초점이 맞춰져 있다. 따라서 본 연구는 아동·청소년 자살고위험군을 대상으로 정신건강서비스를 제공하는 전문가들에게 다양한 자살예방프로그램을 소개하고, 각기 처해 있는 실정에 맞게 현장에서 활용할 수 있도록 하는 데 목적이 있다.
우리나라는 2004년부터 지금까지 OECD 국가 가운데 자살사망률 1위이며, 지속적으로 자살자 수와 자살사망률이 증가하고 있다. 효과적인 자살정책을 확인하기 위해 AHP(Analytic Hierarchy Process) 기법을 이용하여 자살예방의 주요 접근방법별 세부정책들의 우선순위를 조사하였다. 설문은 2013년 8월 23일, 26일 두 차례에 걸쳐 총 30명을 대상으로 워크숍 형식으로 실시하였다. 이중 학계는 20명(사회복지사 4명, 정신보건학자 7명, 정신의학자 5명, 임상심리학자 4명), 정책결정자는 5명, 공급자는 5명이였다. 2013년 9월 9일부터 12일까지 4일간 이메일로 사회복지분야와 임상심리분야에서 각각 5명씩 추가하여 총 40명을 대상으로 조사를 하였다. 세부 정책들의 중요도는 '자살시도자 관리,' '공공자살예방서비스 인프라 구축,' '유가족 지원사업,' '위험집단의 위기관리' 등의 정책에서 높은 것으로 나타났다.
Objectives: This study investigated the association between oral health and suicide attempts in adults aged 65 years or older using the 7th Korea National Health and Examination Survey (KHANES, 2016-2018). Methods: Of the total 24,269 participants of the 7th KHANES (2016-2018), 1,488 adults aged 65 years or older who had an oral health examination were selected. Variations in suicide attempts with demographic characteristics and perceived oral health status were analyzed using the χ2 test, and the association between oral health and suicide attempts was analyzed using the logistic regression model. Results: The suicide attempt rate was higher among older adults with chewing (p<0.01) and speaking (p<0.05) difficulties. The odds ratio for a suicide attempt was 1.05 times higher among those with tooth loss (p<0.05). Conclusions: Our results showed that older adults with tooth loss had an increased risk of suicide. Thus, it is important to prevent tooth loss, in the first place, through proper oral health management. If tooth loss occurs, appropriate treatment should be prioritized to facilitate functional recovery. Finally, national support is required for older adults with limited access to dental care.
