Risk Factors for Suicidal Ideation and Attempts in Adolescents

  • Objective: Although suicide is a serious public health concern for adolescents, there is a lack of studies that explore its risk factors in the Republic of Korea. The present study aims to investigate risk factors associated with suicidal behaviors in Korean adolescents. Methods: Participants consisted of 2258 middle and high school students who completed a series of questionnaires regarding suicide ideation or attempts, non-suicidal self-injuries, depression, impulsivity, drinking behaviors, and negative life events, including peer bullying. Results: Among the participants, 8.3% of students reported suicide ideation, while 3.2% reported a history of a suicide attempt in the past 12 months. Depression, peer-victimization, internet-related delinquency, and positive attitudes toward suicide were associated with suicidal ideations and attempts. Adverse life events were also associated with suicide ideation, but not attempts, while not living with both parents and poor family relationships were associated with suicide attempts, but not ideations. Non-suicidal self-injuries were associated with both suicide ideations and attempts. Students with suicidal ideations and attempts can be differentiated depending on the presence of self-injury. Conclusion: In addition to depression or behavioral problems, non-suicidal self-injuries and lack of support from family may also play significant roles in suicide attempts in adolescents. To facilitate the prevention of suicide in adolescents, longitudinal studies should be followed to confirm the risk factors identified in this study.

Influence of Famous Entertainer's Suicides on Suicide Attempts of the Public (유명 연예인 자살보도가 일반인의 자살시도에 미치는 영향)

  • Purpose: The study explored the influence of the suicide of two famous entertainers on the suicide attempts of the public. Method: The subjects of the descriptive study were 888 suicide attempt patients recruited from October 2007 to September 2009 in an emergency centre located in S-city. The research tools were developed at the Centre for Disease Control. Results: The frequency of suicide attempts during the 6 months and 12 months preceding and following the gas-related suicide of celebrity A were ${\chi}^2$=18.14 and ${\chi}^2$=25.97, respectively, and both were significant(both p<.001). The frequency of suicide attempts of the 6 and 12months before and after the release of report of celebrity B's hanging suicide were ${\chi}^2$=3.85 and ${\chi}^2$=3.47, respectively, and both were significant (both p<.05). Conclusion: These results showed that reports of suicide of celebrities influences suicide attempts of the public. Multiple factors that can influence suicide attempts should be further identified and researched.

Epidemiology of Suicide by Hanging in Fars Province, Iran (2011-2019): A Population-based Cross-sectional Study

  • Objectives: Hanging is a common method of attempted suicide. This study investigated the epidemiological profile of attempted and completed suicides by hanging in southern Iran. Methods: This cross-sectional study was performed on 1167 suicide attempts by hanging between 2011 and 2019. All data related to suicide attempts by hanging were collected from the Fars Suicide Surveillance System. The trends in suicide cases and the mean age of attempted and completed suicides were plotted. The chi-square test was used to identify suicide-related factors. Crude rates of incidence, mortality, and standardized fatality during the study period were calculated. Finally, logistic regression was used to identify the predictors of death in individuals who attempted suicide. Results: The mean age of those who attempted suicide was 33.21±16.82 years; the majority were male (80.5%). The rate of attempted and completed suicide by hanging were 3.50 and 2.79 per 100 000 people, respectively. The case-fatality rate was calculated as 79.34%. The results of our study indicated an increasing trend in suicide attempts by hanging. The likelihood of death was 2.28 times higher in individuals with a previous history of suicide attempts and 1.85 times higher in those with a psychological disorder. Conclusions: The findings of this study suggest an increasing trend in attempted and completed suicide by hanging, especially among individuals with a history of suicide attempts and psychological disorders. It is necessary to take action to reduce the rate of suicide attempts and identify the underlying causes of suicide attempts by hanging.

Predictors of Suicide Attempts in the Korean Adolescent Population (청소년의 자살 시도 예측 요인 - 수도권과 광역시 거주 청소년을 대상으로 -)

  • Purposes: The purposes of this study were ① to identify the rate of suicide attempts, and ② to investigate relevant variables(family dynamic environment, personality factors) and risk factors for attempting suicide among Korean adolescents. Method: Data were collected through a questionnaire survey. Internal consistencies for this questionnaire ranged from 0.63 to 0.88. Nine hundred and twenty two adolescents were surveyed (delinquent : 367, student : 555), using a proportional stratified random sampling method. Statistical methods employed were chi-square, t-test, and logistic regression analysis. Results: ① The rate of suicide attempts was 10.8% for the total sample, and the delinquents(19.6%) had a higher rate of suicide attempts than the students(5.1%). ② Adolescents attempting suicide had a greater dysfunctional family dynamic environment and more maladaptive personality than those who did not attempt suicide. ③ Risk factors for suicide attempts among Korean adolescents were way of coping, psychosomatic symptoms, and parental child-rearing attitudes, in that order. Conclusions: Suicide and suicidal behaviors are multifaceted events. For suicide prevention, there is a need to make an independent assessment of the variables such as familial problems, personality and dynamic environment of the families of the adolescents.

Influence of the Werther Effect: An Increase of Intentional Carbon Monoxide Poisoning (베르테르 효과의 영향; 의도적 일산화탄소 중독의 증가)

  • Purpose: Suicide attempts are known to be influenced by mass media reports. The purpose of this study was to evaluate the effects of mass media reporting celebrity suicides on an increase of intentional carbon monoxide (CO) poisoning and suicide attempts. Methods: We retrospectively reviewed the medical records of the consecutive patients who presented with suicide attempts to the Emergency Department of Ajou University Hospital during a 24 month period. We obtained the demographic data, any past history of suicide attempt and the methods of suicide attempts from the medical records of the suicide attempters. Time series analysis was conducted for evaluating the influence of mass media reporting of celebrity suicide on the suicide rates. Results: We finally enrolled 770 patients during the study period. The total number of suicide attempts by CO poisoning was 18 and the average number of suicide attempts by CO was $0.33{\pm}0.73$ per week. All of the suicide attempts by CO poisoning occurred after a celebrity committed suicide using CO from burning charcoal. Conclusion: This study showed that celebrity suicide by CO poisoning resulted in the Werther effect, which made the rate of intended CO poisoning increase, and the study provided further evidence for the need to actively restrain mass media reporting of suicide to decrease the Werther effect.

Determining Attributes of Suicide Attempts in Korean Elderly People: Emphasis on Attribute Selection Techniques

  • In order to prevent the elderly people from committing suicide attempts, it is necessary to verify attributes that affect the suicide attempts. It is noted that previous studies have focused on qualitative approaches, and simple correlation analyses to determine the attributes related to the suicide attempts in the elderly people. However, such previous approaches had led to insufficient performance when facing with complicated data sets. In this sense, this study suggests an alternative method in which attribute selection techniques are adopted to determine more relevant attributes of the suicide attempts occurring in Korean elderly people. To verify empirical validity of our proposed method, we used Korea National Health and Nutrition Examination Survey (KNHANES) from January 2007 to December 2012. Empirical results proved that the proposed attribute selection techniques showed better predictive effectiveness; 94.4% compared to the simple statistical methods. This study proposes a way to determining the elderly suicide and preventing it to happen.

A Study on the Determinants of Suicide Attempts among the Elderly Living Alone: Focused on Seniors in Seoul and Gyeonggi Province (독거노인 자살시도 영향요인에 관한 연구: 서울 및 경기지역 노인을 중심으로)

  • The purpose of this study was to examine the characteristics of suicide attempts and non-attempts of the elderly living alone and the subjects in this study were 690 elderly persons living alone in Seoul and Gyeonggi Province. For data analysis, this study used descriptive analysis, mean comparison correlation analysis, and binominal logistic regression analysis using PASW 20.0. To explain the suicide attempts of elderly persons who live alone, variables such as sex, age education level, monthly household income, religion, subjective economic status, subjective health status, ADL/IADL, depression, hopelessness, social support, and stress were used. The findings of the study were as follows: First, 9.2 percent(64) of the elderly living alone had attempted suicide and 90.8 percent(626) had never attempted suicide. Second, monthly household income, hopelessness, and social support were identified as the factors that affected their suicide attempts. In other words, the elderly living alone who had lower monthly household income, less social support, and greater feelings of hoprlessness had made more suicide attempts. As it is a cross-sectional research using data from non-probability sampling, this has a limitation in generality sampling, this has a limitation in generalizing the study results. To overcome this limitation, longitudinal research using data from probability sampling is necessary.

Self-poisoning as a Target Group for Prevention of Suicide (자살 예방을 위한 집중관리 대상으로서의 중독)

  • Purpose: The Korean government has tried to decrease the suicide death rate over the last decade. Suicide attempts, particularly non-fatal attempts, are the most powerful known risk factor for a completed suicide. An analysis of suicide attempt methods will help establish the effective preventive action of suicide. Fit prevention according to the method of suicide attempt may decrease the incidence of suicide death. Self-poisoning is suggested as a major method of both suicide attempts and suicide death. The aim of this study was to determine if a self-poisoning patient is a suitable target for the prevention of the suicide. Methods: This was retrospective analysis of a prospective cohort, which included patients who presented to the emergency department (ED) after a self-harm or suicide attempt from Jan 2013 to Dec 2017. The proportion of methods in suicide attempts, psychological consultation, and fatality according to the suicide attempt method were analyzed. The types of poison were also analyzed. Results: Poisoning was the most common method of suicide attempts (52.1%). The rate of psychological consultations were 18.8% for all patients and 29.1% for poison patients (p<0.001). The rate of mortality in poisoning was 0.6%. Psychological consultation was performed more frequently in admission cases than discharged cases. The most common materials of poisons was psychological medicines and sedatives that had been prescribed at clinics or hospital. Conclusion: Self-poisoning is a major method of suicide attempt with a high rate of psychiatric consultation, low mortality rate, versus others methods. The prevention of suicide death for suicide attempts may focus on self-poisoning, which is the major method of suicide attempts. A suitable aftercare program for self-poisoning may be an effective method for preventing suicide if an early diagnosis and management of psychiatric disorders through psychiatric consultation can be made, and early connection to social prevention program for non-fatal patients are possible.

Suicide Related Indicators and Trends in Korea in 2017 (2017년 자살 관련 지표들과 추이)

  • Suicide is a major public health issue that causes over 800,000 deaths each year globally. Korea ranks high in suicide rates, in which around 24.3 per 100,000 individuals are reported to have died by intentional self-harm in 2017 according to Statistics Korea. The aim of this study was to examine the current status and trend of suicide ideation and attempt using data from the following five sources: Korean National Health and Nutrition Examination (KNHANES, '07-13, '15-17), Korean Community Health Survey (KCHS, '08-09, '13, '17), Korean Wealth Panel Study (KOWEPS, '12-17), Korea Health Panel Survey (KHP, '10-13), and Statistics Korea (1983-2017). Suicide ideation and attempts were also further examined based on equalized household income levels. Data published by Statistics Korea were used to show the updated suicide rate and number of deaths by intentional self-harm. The rate of suicide ideation at the recent year was 4.73% (KNHANES, '17), 6.96% (KCHS, '17), 1.63% (KOWEPS, '17), and 5.39% (KHP, '13). That of suicide attempts as recent year was 0.71% (KNHANES, '17), 0.32% (KCHS, '17), and 0.09% (KOWEPS, '17). Annual percentage change of suicidal ideation was -15.4% (KNHANES, '07-17), -2.5% (KCHS, '08-17), -8.6% (KOWEPS, '12-17), and -10.9% (KHP, '10-13). Annual percentage change of suicide attempts was -4.0% (KNHANES, '07-17), -4.4% (KCHS, '08-17), and -14.9% (KOWEPS, '12-17). Individuals with lower income levels were more likely to experience suicide ideation and attempts. Considering that Korea still shows a high suicide rate despite the continuously decreasing trend of suicide ideation and attempt, continuous observation and appropriate policy implementation regarding suicide related problems are necessary.

The Association between Attention Deficit/Hyperactivity Disorder Symptoms and the Number of Suicide Attempts among Male Young Adults with Unipolar and Bipolar Depression

  • Objectives: Mood disorder is highly comorbid with attention deficit/hyperactivity disorder (ADHD), and comorbid ADHD symptoms are associated with suicide risk. The aim of this study was to examine the association between comorbid ADHD symptoms and the number of suicide attempts among patients with unipolar and bipolar depression. Methods: In this cross-sectional study, 100 patients with either unipolar or bipolar depression constituted the final sample. Pearson's correlation and multiple regression analyses were conducted to examine the association between comorbid ADHD symptoms and suicide risk. Results: Among patients with bipolar depression, the number of suicide attempts was significantly correlated with ADHD symptoms (r=0.324, p<0.01). ADHD symptoms significantly predicted the number of suicide attempts (β=0.249, p<0.05). Conclusion: Our findings underscore the importance of screening and assessing ADHD symptoms in male young adults with depressive disorders.
