Suicide Related Indicators and Trends in Korea in 2020 (2020년 자살 관련 지표들과 추이)

  • The aim of this study was updating suicide-related indicators, including the number of suicidal deaths, suicide rate, and the prevalence of suicidal ideation and suicide attempts. We observed trends in suicide-related indicators based on up-to-date information. This study used five data sources to evaluate the trends of suicide-related indicators: Statistics Korea (1983-2020), Korean Wealth Panel Study (KOWEPS, 2012-2020), and Korea Health Panel Survey (KHP, 2010-2013, 2016-2018), Korean National Health and Nutrition Examination (KNHANES, 2007-2013, 2015-2020), and Korean Community Health Survey (KCHS, 2008-2009, 2013, 2017). The suicide rate per 100,000 population decrease from 26.9 in 2019 to 25.7 in 2020. Based on recently available data, the rates of suicidal ideation were 1.48% (KOWEPS, 2020), 2.61% (KHP, 2018), 4.62% (KNHANES, 2019), and 6.96% (KCHS, 2017). Those of suicide attempt as were 0.07% (KOWEPS, 2020), 0.44% (KNHANES, 2020), and 0.32% (KCHS, 2017). Annual percentage change (APC) of suicide rate was -1.87% (Statistics Korea, 2011-2020). APC of suicidal ideation was -10.7% (KOWEPS, 2012-2020), -11.5% (KHP, 2010-2013, 2016-2018), -14.7% (KNHANES, 2007-2013, 2015, 2017, 2019), and -2.5% (KCHS, 2008-2009, 2013, 2017). APC of suicide attempt was -11.3% (KOWEPS, 2012-2019), -5.2% (KNHANES, 2007-2013, 2015-2020), and -4.4% (KCHS, 2008-2009, 2013, 2017). Although the suicide rate in Korea has decreased compared to 10 years ago, it is still at a high level. Thus, continuous observation and appropriate suicide prevention policies are needed.

Low Serum Cholesterol Level in Major Depression Patients with Suicidal Attempt (자살을 시도한 주요우울증 환자에서 혈청 콜레스테롤 농도의 저하)

  • Objective : Several studies hove suggested that reduction of cholesterol concentration and low cholesterol level increases suicide, homicide, and depression. The authors sought to determine whether low cholesterol is associated with a history of suicidal attempts among major depression patients. Method : The subjects were 105 patients with major depression, diagnosed according to the DSM-III-R criteria, and 105 age, sex matched healthy controls. Blood was token following overnight fast and serum cholesterol concentration were measured by a enzymatic method Results : The serum cholesterol level was significantly lower in the patients with major depression than in healthy controls($180.4{\pm}43.4mg/dl$ vs $199.7{\pm}35.8mg/dl$). And the serum cholesterol level was significantly lower in major depression patients with suicidal attempt than in major depression patients without suicidal attempt($156.7{\pm}38.2mg/dl$ vs $ 187.8{\pm}42.5mg/dl$) Conclusion : It is hypothesized that low cholesterol level is associated with depression by modifying the serotonin metabolism and the production of interleukin-2, low cholesterol concentration should be further investigated as a potential biological marker of suicidal risk in major depression. Prospective study with serial cholesterol determinations should be done.

Association Between Suicidal Attempt and Internet Addiction in Korean Psychiatric Patients: A Case-Control Study (한국 정신질환자에서의 자살시도와 인터넷 중독 간의 연관성에 관한 환자-대조군 연구)

  • Objective : Current study investigated the association between internet addiction and suicidal attempts in psychiatric patients. Methods : Patients who visited emergency department or psychiatry outpatient unit of Chungnam National University Hospital from January 2020 to July 2021 were included in this study. Total of 192 participants, 96 suicidal attempt cases and 96 psychiatric control patients without history of suicidal attempt were matched by gender and baseline diagnosis. Depression and internet addiction were assessed by using Patient Health Questionnaire-9 (PHQ-9) and Young's Internet Addiction Test (IAT). Independent t-tests, and logistic regression analyses were conducted. Results : IAT scores were significantly higher in suicidal attempt case group (41.09±18.196 years) compared to psychiatric control patients without history of suicidal attempt (36.63±17.016 years) (p=0.040). Internet addiction (adjusted Odds Ratio [aOR]=2.049, 95% Confidence Interval [CI]=1.004-4.539, p=0.0493) was an independent risk factor for suicidal attempt in psychiatric patients after controlling age, gender, and sociodemographic factors, such as marital status, education level and employment status. Conclusion : This is the first study to investigate the relationship between internet addiction and suicidal attempt in psychiatric patients in Korea. Internet addiction is significantly associated with suicidal attempt in psychiatric patients, after adjusting sociodemographic variables. This study provides evidence to future studies to further investigate on causality of internet addiction and suicidality in psychiatric patients.

Effects of Depression and Quality of Life on Suicidal Intention of the Elderly (우울과 삶의 질이 노인의 자살 의도에 미치는 영향)

  • Purpose : Recently, Korea has entered an aging society due to the steadily increasing proportion of the elderly population. Additionally, elderly suicidal rate in Korea is the highest among OECD member countries. Because old age is a time when all functions are weakened physically, mentally, and socially, negative experiences, including depression and poor quality of life in the elderly, may lead to suicidal intentions. This study attempts to understand the relationship between depression and quality of life experienced by the elderly and their suicidal intentions. Methods : The survey was conducted from January 2 to March 31, 2019 by administering a questionnaire targeting people aged ≥ 65 years, who were registered at public health centers located in U-city, Gyeonggi-do. In total, 1,015 copies of data were used for the final analysis. The collected data were analyzed using the SPSS/WIN 23.0 program. The χ2 test was performed to investigate the relationship between the demographic characteristics and the suicidal intentions of the elderly subjects. The t-test was performed to determine the relationship between suicidal intentions, depression, and quality of life in elderly subjects. Results : The study results revealed that 168 people (16.6 %) had thought of suicide and 3.2 % had actually tried to commit suicide. The reasons for suicidal thoughts were family problems in 60 (5.6 %), economic problems in 49 (4.6 %), and physical illness in 44 (4.1 %). Those with suicidal intentions had higher levels of depression (t=11.712, p<.001) and higher quality of life scores (t=-9.565, p<.001). Conclusion : Based on these results, it is proposed that a suicide prevention policy and program that can improve the elderly's depression and quality of life is necessary.

A Review on Brain Imaging Studies of Suicide in Youth (청소년기 자살에 대한 뇌영상 연구)

  • Suicide is a leading cause of death worldwide, especially among adolescents and young adults. Considering this fact, it is imperative that we understand the neural mechanisms underlying suicidal thoughts and behaviors in youth from a neurodevelopmental perspective. In this review, we focused on the magnetic resonance imaging studies that examined the neural correlates of suicidal ideations (SI) or attempts (SA) in youth. We reviewed twenty-three cross-sectional studies reporting the structural and functional alterations in association with SI or SA among adolescents and young adults with various mental disorders. The previous literature suggests that the dorsolateral prefrontal cortex, anterior cingulate cortex, and ventral frontolimbic circuit, may play an important role in the pathophysiology of suicidal behavior in youth through altered top-down control over emotion and impulsivity. Future studies with a longitudinal design and using multimodal imaging techniques may be of help to identify novel therapeutic targets specific for youth with suicidal thoughts and behaviors.

Factors Related with Suicidality in High-School Students : Focused on Perceived Academic Performance (고등학생 자살 경향성에 영향을 미치는 요소 : 학생 스스로 지각하는 학업성취도를 중심으로)

  • Objectives : This study investigated factors related to suicide in high school students and focused on perceived academic performance. Methods : We surveyed 293 grade 10 high-school students in Jeju and Gyeonggi provinces and gave them self-assessment questionnaires. To evaluate the suicide risk, the questionnaire included questions about suicidal thoughts, plans, threats, deliberate self-injury, and suicide attempts from the Scale for Suicide Ideation (SSI). Also, it included the Beck Depression Inventory (BDI), Beck Hopelessness Scale (BHS), Locus of Control Scale (LOC), Rosenberg's Self Esteem Scale (RSE) and Perceived Academic Performance (PAP) to identify factors related to suicide in high school students. Results : Depression, hopelessness, and low self-esteem were related with suicidal thoughts, plans, attempts, and severe suicidal ideation (SSI score${geq}$16). In the multivariate analysis, a low level of PAP (OR=3.796, 95%CI 1.280-11.257) was significantly associated with severe suicidal ideation (SSI score${geq}$16). Deliberate self-injury was most prevalent in failing the PAP group and also having a high level of PAP (OR=6.161, 95%CI 1.474-25.760). Conclusion : The authors found that the suicide risk of high school students is related to depression, hopelessness, low self-esteem and a low level of PAP. These results provide significant indicators in planning an adolescent suicide prevention program.

Gray Matter Volume Reductions Were Associated with TPH1 Polymorphisms in Depressive Disorder Patients with Suicidal Attempts

  • Objective Structural changes of brain areas have been reported in depressive disorder and suicidal behavior (SB), in which TPH1 also has been known as a promising candidate gene. We investigated gray matter volume (GMV) differences, TPH1 rs1800532 and rs1799913 polymorphisms previously found to be associated with depressive disorder and SB, and the relationship between the two markers. Methods Thirteen depressive disorder patients with suicidal attempts (SA) and twenty healthy controls were included. We examined GMV differences using a voxel-based morphometry and regions of interest analysis. Direct sequencing was used for genotyping. Results The patients showed significant GMV reduction in left cerebral region including middle frontal gyrus, inferior frontal gyrus, and anterior cingulate cortex; in right middle temporal gyrus; in left cerebellar tonsil; and in right cerebral region including precentral gyrus and postcentral gyrus (corrected p<0.005). The right precentral and postcentral gyri GMV values of AA and CA genotypes patients were significantly decreased compared to those of CC genotype subjects (corrected p=0.040). Conclusion These findings show the possibility that both GMV reductions and TPH1 rs1800532/rs1799913 A allele may be involved in the pathogenesis of depressive disorder patients with SA.

Effects of A Body Shape Index and Body Mass Index and Suicidal Behaviors (체형지수와 체질량지수가 자살행동에 미치는 영향)

  • The purpose of this study was to determine the association of the Body Shape Index (ABSI) with suicide ideation and suicide attempts by controlling the body mass index (BMI), as well as general characteristics that may affect suicide. The study has used The Sixth Korea National Health and Nutrition Examination Survey conducted by the Korea Centers for Disease Control and Prevention from 2013 to 2015. A total of 13,155 participants were studied, excluding those who had no information on variables that could affect suicidal ideations and suicidal attempts. The chi-squared test and the multivariate logistic regression analysis were performed. The results of the analysis showed that those who had obesity in BMI were more likely to commit suicide and suicide than those who were in 'standard' in BMI. In ABSI, Q3 group was more likely to commit suicide than Q1 group. The number of suicide attempt was significantly higher in the group with Q3 in ABSI than in Q1 group (3.623 times (95% CI 1.027-12.772)). The results of this study suggest that a higher ABSI compared to BMI means that it is possible to make a more extreme choice of suicide attempt rather than suicide ideation.

Clinical Characteristics of NSSI and Predictors of Suicide Attempts in Clinically Depressed Korean Adolescents (일 대학병원에 방문한 우울한 청소년에서 비자살성 자해행동의 임상적 특성과 자살 시도 예측요인)

  • Objectives : The purpose of this study was to examine the prevalence and clinical characteristics of nonsuicidal self-injury (NSSI), and its association with suicide attempts among clinically depressed adolescents in Korea. Methods : In total, 113 depressed adolescents aged 12-18 years in South Korea were enrolled in this study. We assessed sociodemographic and clinical characteristics including suicidality and non-suicidal self-injury (NSSI) using various self-reported scales and semi-structured interview for diagnosis of psychiatric disorders. Demographic and clinical characteristics of the subjects were compared between NSSI and non-NSSI groups. We examined significant predictors of suicide attempts using logistic regression analysis. Results : Among 113 depressed participants, 48 (42.1%) adolescents were classified into the NSSI group. In the NSSI group, there were significantly more females, showed higher depression, higher state-anxiety, and more suicide ideation. The most predictive factors of suicide attempts were history of NSSI, observed suicide/NSSI behaviors of their family or friends, and total state anxiety score. Conclusions : NSSI is more common problem among clinically depressed adolescents and history of NSSI is a significant predictor of present suicide attempts. To include the assessment of NSSI for clinically depressed adolescent may be crucial for intervention programs for high risk adolescents of suicide in Korea.

Suicide Related Indicators and Trends in Korea in 2017 (2017년 자살 관련 지표들과 추이)

  • Suicide is a major public health issue that causes over 800,000 deaths each year globally. Korea ranks high in suicide rates, in which around 24.3 per 100,000 individuals are reported to have died by intentional self-harm in 2017 according to Statistics Korea. The aim of this study was to examine the current status and trend of suicide ideation and attempt using data from the following five sources: Korean National Health and Nutrition Examination (KNHANES, '07-13, '15-17), Korean Community Health Survey (KCHS, '08-09, '13, '17), Korean Wealth Panel Study (KOWEPS, '12-17), Korea Health Panel Survey (KHP, '10-13), and Statistics Korea (1983-2017). Suicide ideation and attempts were also further examined based on equalized household income levels. Data published by Statistics Korea were used to show the updated suicide rate and number of deaths by intentional self-harm. The rate of suicide ideation at the recent year was 4.73% (KNHANES, '17), 6.96% (KCHS, '17), 1.63% (KOWEPS, '17), and 5.39% (KHP, '13). That of suicide attempts as recent year was 0.71% (KNHANES, '17), 0.32% (KCHS, '17), and 0.09% (KOWEPS, '17). Annual percentage change of suicidal ideation was -15.4% (KNHANES, '07-17), -2.5% (KCHS, '08-17), -8.6% (KOWEPS, '12-17), and -10.9% (KHP, '10-13). Annual percentage change of suicide attempts was -4.0% (KNHANES, '07-17), -4.4% (KCHS, '08-17), and -14.9% (KOWEPS, '12-17). Individuals with lower income levels were more likely to experience suicide ideation and attempts. Considering that Korea still shows a high suicide rate despite the continuously decreasing trend of suicide ideation and attempt, continuous observation and appropriate policy implementation regarding suicide related problems are necessary.
