용적변조회전 방사선치료에서 선량전달의 중단 및 재시작에 따른 정확성 평가 (The Accuracy Evaluation according to Dose Delivery Interruption and Restart for Volumetric Modulated Arc Therapy)

  • 목 적: 용적변조회전 방사선치료는 겐트리 회전과 다엽콜리메이터, 선량률이 연동되어 진행되며 치료 중 선량 전달의 중단이 발생한 경우 겐트리와 다엽콜리메이터가 최초위치부터 다시 동작하여 정지된 지점부터 선량전달이 재개된다. 본 연구는 용적 변조회전 방사선치료의 치료 중단과 재 진행에 따른 선량전달의 오차를 분석하고자 하였다. 대상 및 방법: 10명의 환자를 대상으로 전산화치료계획시스템(Eclipse V10.0, Varian, USA)을 이용하여 용적변조회전 방사선 치료 계획을 수립하였다. TRILOGY (Varian, USA)의 6 MV 선속을 이용하여 계획된 선량을 이차원 배열 검출기와 CUBE (IBA dosimetry, Germany)팬텀에 조사하였다. OmniPro I'mRT system (V1.7b, IBA dosimetry, Germany)을 통해 4회에 걸쳐 일반적인 선량전달의 일관성을 평가 하고 선량 전달의 중단 또는 도어 인터락 발생으로 인해 선량 전달이 중단되고 최초지점부터 다시 시작되어 조사되는 경우와 비교하여 감마지수(Gamma index)의 변화를 분석하였다. 결 과: 선량 전달이 정상적으로 이루어진 경우에 각각의 감마 평균 신호 값의 차이는 0.1로 나타났고, 선량 전달의 중단이 발생한 경우 0.128로 나타났으며 도어 인터락의 경우 0.141로 나타났다. 각각의 경우 감마 표준편차 값의 차이는 0.071, 0.091, 0.099로 나타났고, 감마 최대값의 차이는 0.286, 0.379, 0.413으로 나타났다. Gamma pass rate (3%, 3 mm)는 허용 오차 범위를 만족하였고, T검증 결과 95% 신뢰구간에서 P-value가 0.05 미만으로 유의한 차이를 보였다. 결 론: 본 실험에서 치료 중 선량 전달의 중단 및 재시작에 관한 정확성을 평가해 본 결과 통계적으로는 차이가 있으나 임상적으로 허용 오차 범위이내로 문제가 없다는 것을 알 수 있었다. 그러나 겐트리와 다엽콜리메이터, 선량률의 연동으로 정확성이 요구되는 치료방법인 만큼 선량적인 측면에서 치료 중단에 따른 차이는 간과할 수 없다. 그러므로 갑작스러운 치료 중단 상황이 발생한 경우 추가적인 정도관리 절차를 통해 정확한 선량평가가 이루어져야 할 것으로 사료된다.

Beak Trimming Methods - Review -

  • A review was undertaken to obtain information on the range of beak-trimming methods available or under development. Beak-trimming of commercial layer replacement pullets is a common yet critical management tool that can affect the performance for the life of the flock. The most obvious advantage of beak-trimming is a reduction in cannibalism although the extent of the reduction in cannibalism depends on the strain, season, and type of housing, flock health and other factors. Beak-trimming also improves feed conversion by reducing food wastage. A further advantage of beak-trimming is a reduction in the chronic stress associated with dominance interactions in the flock. Beak-trimming of birds at 7-10 days is favoured by Industry but research over last 10 years has shown that beak-trimming at day-old causes the least stress on birds and efforts are needed to encourage Industry to adopt the practice of beak-trimming birds at day-old. Proper beak-trimming can result in greatly improved layer performance but improper beak-trimming can ruin an other wise good flock of hens. Re-trimming is practiced in most flocks, although there are some flocks that only need one trimming. Given the continuing welfare scrutiny of using a hot blade to cut the beak, attempts have been made to develop more welfare friendly methods of beak-trimming. Despite the developments in design of hot blade beak-trimmers the process has remained largely unchanged. That is, a red-hot blade cuts and cauterises the beak. The variables in the process are blade temperature, cauterisation time, operator ability, severity of trimming, age of trimming, strain of bird and beak length. This method of beak-trimming is still overwhelmingly favoured in Industry and there appears to be no other alternative procedures that are more effective. Sharp secateurs have been used trim the upper beak of both layers and turkeys. Bleeding from the upper mandible ceases shortly after the operation, and despite the regrowth of the beak a reduction of cannibalism has been reported. Very few differences have been noted between behaviour and production of the hot blade and cold blade cut chickens. This method has not been used on a large scale in Industry. There are anecdotal reports of cannibalism outbreaks in birds with regrown beaks. A robotic beak-trimming machine was developed in France, which permitted simultaneous, automated beak-trimming and vaccination of day-old chicks of up to 4,500 chickens per hour. Use of the machine was not successful because if the chicks were not loaded correctly they could drop off the line, receive excessive beak-trimming or very light trimming. Robotic beak-trimming was not effective if there was a variation in the weight or size of chickens. Capsaicin can cause degeneration of sensory nerves in mammals and decreases the rate of beak regrowth by its action on the sensory nerves. Capsaicin is a cheap, non-toxic substance that can be readily applied at the time of less severe beak-trimming. It suffers the disadvantage of causing an extreme burning sensation in operators who come in contact with the substance during its application to the bird. Methods of applying the substance to minimise the risk to operators of coming in contact with capsaicin need to be explored. A method was reported which cuts the beaks with a laser beam in day-old chickens. No details were provided on the type of laser used, or the severity of beak-trimming, but by 16 weeks the beaks of laser trimmed birds resembled the untrimmed beaks, but without the bill tip. Feather pecking and cannibalism during the laying period were highest among the laser trimmed hens. Currently laser machines are available that are transportable and research to investigate the effectiveness of beak-trimming using ablasive and coagulative lasers used in human medicine should be explored. Liquid nitrogen was used to declaw emu toes but was not effective. There was regrowth of the claws and the time and cost involved in the procedure limit the potential of using this process to beak-trim birds.

경막외 진통법이 개흉술후 환자에게 미치는 영향 (Effect of Epidural Analgesia on the Post-thoracotomy Patient)

  • Postoperative hypoxemia in the absence of hypoventilation occurs more often after thoracic or upper abdominal surgery than lower abdominal operations or surgery on extremities. Although the factors which produce postoperative alveolar collapse have not been fully evaluated, the dominant factor of postoperative hypoxemia is shunt of blood passing collapsed alveoli and the postoperative pain is associated with restriction of depth of breathing, sighing and movement. In 1979, the first successful clinical usage of epidurally administered morphine for control of postoperative pain was reported by Behar and associates. This study was carried out for twenty patients who received posterolateral thoracostomy with bleb resection between May 1990 and May 1991 and who were primary spontaneous recurrent pneumothoraxes under general endotracheal anesthesia. For the relief of post-thoracotomy pain following of the general anesthesia, we selected ten patients as control group which were treated intermittently IM with injection of pethidine(50 mg) according to the conventional method and another ten patients as study group which were managed with thoracic epidural analgesia. The tip of the catheter was inserted to T4-5 epidural space through T12-L1 or L1-2 interspinous region before the induction of the general anesthesia and then the epidural analgesics(0.25% bupivacaine 15 ml+morphine 3 mg) was injected once a day via the catheter until 4 th POD in the study group. The epidural catheters were removed at postoperative 4 th day in study group. Clinical observations were done about vital signs, ABG, tidal volume, FVC and occurence of adverse effects during postoperative 2hr, 8hr, 1st day, 2nd day, 7th day in both groups. The results were as follows; (1) The values of $V_T$ and FVC were significantly improved in study group(85% and 66%) as compared with control group(76% and 61%) during the postoperative 4 day of the epidural analgesia. (2) After the end of the epidural analgesia(7th POD), the values of FVC were improved invertly rather in control group(98%) than study group(84%). It suggested that the reduction of FVC in study group were caused by the raised pain sensitivity following the end of epidural analgesia. (3) The side effects of epidural analgesia such as transient urinary retention(2 cases), itching sensation(1) and headache(1) were noted.

식생활라이프스타일에 따른 배달음식의 소비성향 분석 (Analysis of the consumption pattern of delivery food according to food-related lifestyle)

  • 배달음식 소비시장을 세분화하여 고객 성향 맞춤형 제품과 서비스 개발을 위한 기초자료로 활용하고자 배달음식 이용 경험이 있는 성인남녀 총 636명을 대상으로 설문조사를 실시하였다. 조사대상자의 식생활라이프스타일 평가항목에 대한 요인분석 결과 안전지향, 편의지향, 맛지향, 경제지향 요인이 도출되었고 요인분석 결과를 토대로 군집분석을 실시한 결과 조사대상 고객을 식생활 저관심군, 편의·경제 추구군, 미식·경제 추구군으로 분류하였다. 군집에 따른 조사대상자의 일반 특성에 대한 차이분석 결과 연령 (p < 0.001), 결혼 여부 (p < 0.001), 가족 유형 (p < 0.01), 주거 형태 (p < 0.05) 에서 유의적인 차이가 있었다. 식생활 저관심군은 기혼 (67.1%), 가족과 함께 거주하는 비율 (85.4%), 아파트에 거주하는 비율 (74.2%)이 다른 군집에 비해 높았고 편의·경제 추구군은 1인 가구 비율 (28.9%)이, 미식·경제 추구군은 미혼의 비율 (62.0%)이 다른 군집에 비해 높았다. 조사대상자의 연령은 편의·경제 추구군(32.3세)과 미식·경제 추구군 (32.5세)에 비해 식생활 저관심군 (40.0세)이 유의적으로 높았다(p < 0.001). 식생활라이프스타일 군집에 따른 배달음식 소비 실태에 대한 차이 분석 결과 주문횟수 (p < 0.001), 주로 주문하는 요일 (p < 0.05), 배달음식의 정보 습득 경로 (p < 0.001), 주문 방법 (p < 0.001), 같이 섭취하는 사람 (p < 0.01)에서 군집 간에 유의적인 차이가 있었으며, 편의·경제 추구군은 배달음식 주문횟수 (p < 0.001)와 배달음식에 대한 전체적인 만족도 (p < 0.001)에서 식생활 저관심군과 미식·경제 추구군에 비해 유의적으로 높았다. 따라서 음식점 경영자와 배달 플랫폼 업체는 배달음식 고객의 만족도를 극대화하기 위해서 고객의 식생활 라이프스타일 특성을 고려한 제품과 서비스를 개발하고 배달음식의 품질 향상을 위해 지속적으로 노력해야 할 것이다.

추천시스템의 효과적 도입을 위한 소셜네트워크 분석 (Social Network Analysis for the Effective Adoption of Recommender Systems)

  • 협업필터링은 다양한 분야에서 널리 활용되고 있지만 협업필터링의 추천 성능은 적용하는 기업의 비즈니스 형태나 발생하는 거래 데이터의 특성에 따라 다르게 나타나고 있다. 기업에서 협업필터링 추천시스템을 구축하려면 상당한 시간과 비용이 소요되기 때문에 구축된 추천시스템의 성과가 높지 않다면 기업 자원의 낭비를 초래할 뿐만 아니라 부정확한 추천서비스를 받는 고객들의 불만을 살 수 있다. 따라서 추천시스템 도입을 검토할 때 기업이 갖고 있는 데이터의 특성을 파악하고 이를 통해 추천시스템을 도입하는 것이 타당한지 사전에 예측할 수 있다면 불필요한 도입으로 인한 경제적 손실과 고객 만족도 저하를 막을 수 있을 것이다. 기존 연구에서는 협업필터링 추천 성과에 희박성, 우연성, 커버리지 등이 영향을 미칠 수 있다고 설명하고 있지만 이러한 요인들이 어떻게 얼마나 추천 성과에 영향을 미치는지, 요인들 간에 어떠한 상관관계가 있는지는 현재까지 구체적으로 밝혀진 바가 없다. 본 연구에서는 구매 트랜잭션으로부터 생성된 소셜네트워크로부터 밀도, 군집화계수, 집중도 등의 구조적 지표를 측정한 후 이들이 추천성과에 어떻게 영향을 미치는지 통계적 분석을 통해 실증적으로 규명한다. 이를 통해 협업필터링 추천시스템에 대한 도입 여부를 결정하고자 할 때 유용하게 사용될 수 있는 지침을 제공하고자 한다.

조경용 억새의 대량번식을 위한 플러그묘와 삽목번식법 (Seedling Plug and Cutting Method for Multi-propagation of Ornamental Miscanthus Spp.)

  • 억새(Miscanthus spp.)는 경관성이 우수하고 환경친화적이며 저관리가 가능한 종 들로 알려져 있다. 플러그묘를 통한 대량번식을 위해 4가지 품종의 억새(M. sinensis 'Andersson', 'Strictus', 'Gracillimus', 'Variegatus')와 삽목번식을 위한 3가지 품종의 억새(M. sinensis 'Gracillimus', M. xgiganteus, M. sacchariflorus)를 예비실험을 통해 선발하여 플러그 형성, 정식 후의 생육, 그리고 삽목 활착률 실험을 2015-2016년에 온실과 실험포장에서 실시하였다. 2가지 관수방법과 3가지 플러그 셀 크기로 종자로부터 플러그묘 형성과정 실험에서는 매트를 이용한 저면관수와 작은 크기의 플러그 셀이 양호한 결과를 보였다. 삽목실험에서 삽수채취 시기, 마디 부위, 삽수의 직경이 활착율에 미치는 영향을 분석한 결과 억새 공시 3종 모두 하부 마디에서 채취한 삽수가 높은 활착율을 보였으며 삽수 채취 시기는 화서가 발생 하기 전 그리고 목질화가 진행되어 삽수의 직경이 작은 하부마디가 높은 활착율을 보였다. 생존율의 차이가 뚜렷한 M. xgiganteus는 삽수채취 시기와 삽수 부위 및 삽수 직경에 따른 삽목의 성공여부(생존 혹은 고사)는 이분형 로지스틱 회귀모델(dichotomous logistic regression model)을 이용하여 분석 판정하였다. 그 결과 삽목 시기는 7월이 6월에 비하여 1.64배 정도 활착 성공률이 높았으며, 삽수 채취 마디부위는 1번째 마디를 기준으로 승산비(성공율/실패율)는 3번째 마디와 5번째 마디는 0.12와 0.02로, 각각 8배와 50배의 활착 실패율을 보였다. 즉 늦게 발달한 상대적으로 굵은 신생 줄기 마디는 목질화가 진행된 묵은 마디에 비해 직경은 크지만 생존율은 낮아졌다. 따라서 억새속 삽목번식은 화서가 분화되기 시작하는 9월 이전에 아래쪽 묵은 마디로 삽수로 사용하여 대량 번식을 하는 것이 바람직하다고 판단된다.

치과의료소비자의 라이프스타일에 의한 병원 만족도 (Dental Consumers Lifestyle of Dental Hospital Satisfaction)

  • 본 연구는 대상자의 라이프스타일에 따른 병원 만족도 신뢰도를 파악하고 일반특성에 따른 병원 만족도를 분석하여 라이프스타일에 따른 병원 만족도를 파악하였으며, 병원 재이용의 영향요인을 알아보기 위하여 2006년 10월 1일부터 12월 1일까지 전라북도 지역의 구강보건진료기관을 내원하는 의료소비자를 대상으로 자기 기입식으로 응답한 720부 설문지를 사용하였으며, 수집된 자료는 SPSS WIN 12.0 프로그램을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 연구대상자의 일반적인 특성에 따른 병원만족 요인을 분석한 결과 '신속성' 요인에서는 성별(p < 0.05), 소득(p < 0.05), 연령(p < 0.05)에서 통계적으로 유의한 차이를 보였다. '신뢰성' 요인에서는 주로 이용하는 치과진료기관(p = 0.004)에서 통계적으로 유의한 차이를 보였으며, '접근성' 요인에서는 연령(p < 0.001)에서 통계적으로 유의한 차이를 보였다. '경제성' 요인에서는 연령(p < 0.05)과 주로 이용하는 치과진료기관(p < 0.05) 에서 통계적으로 유의한 차이를 보였으며, '편리성' 요인에서는 주로 이용하는 치과진료기관(p < 0.01)에서 통계적으로 유의한 차이를 보였다. 2. 라이프스타일에 따른 치과진료기관 만족 요인의 차이는 신속성과 경제성 요인에 대해서는 건강라이프스타일에 따라 병원만족 요인을 지각하는 정도가 다르게 나타났으며, 신속성을 지각하는데 있어서 '건강실천형(I)'과 '건강무실천형(II)'간에 통계적으로 유의한 차이를 보였고(p < 0.05). 경제성 요인에서는 '건강실천형(I)'과 '건강무실천형(II)', '건강관심형(III)', '건강무관심형(IV)'간에 유의한 차이를 보였다(p < 0.001). 3. 병원 재이용의사에 따른 만족요인의 차이는 신속성의 경우 재이용의사가 있는 경우가 없는 경우보다 만족정도가 높게 나타났으며, 통계적으로 유의한 차이를 보였다(p < 0.05). 또한 신뢰성에서도 재이용의사가 있는 경우 만족정도가 높게 나타나 통계적으로 유의한 차이를 보였다(p < 0.001). 4. 건강라이프스타일과 병원만족도와의 관계를 살펴보면, 건강관심형, 건강실천형, 건강무관심형에서는 통계적으로 유의적인 정(+)의 관계를 보여 의미 있는 상관관계가 존재하는 것으로 나타났다.

식재료 아웃소싱이 경제적 주방에 미치는 영향에 관한 연구 - 특1급호텔 양식조리를 중심으로 - (Study on economic effects of outsourcing of food materials on the hotel kitchen - Focus on cooking Western food in the first class hotel -)

  • This study is designed to examine feasibility and limitation of outsourcing in cooking Western food in a hotel, to interpret importance of outsourcing(eg. outside order, outside procurement, outside supply) in a broad sense in order to reinforce the core capacity in the cooking department, and to know whether the cooking human power is efficiently used and how much the chefs recognize outsourcing of food materials. As many companies conduct restructuring to cut down its size, the reduction of human power led the Western food cooking in the hotel to lower core capacities, lower quality, and lower efficiency. In addition, the sagging morale of chefs undermined creativity. To change from the traditional kitchen to an economic kitchen needs to look into importance of outsourcing, cognitive attitude of chefs, relation with outside suppliers. Here suggests performance of positive changes in the structure The study examined feasibility and limitation of outsourcing in the hotel kitchen as well as chefs' cognitive attitude toward outsourcing of food materials to reinforce core capabilities of the hotel kitchen. 1. Companies of outsourcing are selected according to variability of price conditions, flexibility of contract conditions, popularity of the outsourcing company, and reputation of the outsourcing company. 2. The importance of outsourcing in the Western food cooking is divided into 4 factors such as standard of selecting outsourcing companies, policies of cooking manu, quality of cooking, and quantity of cooking. 3. The most feasible section in outsourcing of food materials is a process of kneading flour for bread, which shows that many Western-food chefs expect to put higher possibility of outsourcing on the kneading. In other words, when it comes to confectionery and bakery, there are many outside expert processing companies supplying high quality products. In the order of outsourcing feasibility, sauce is followed by processed vegetable, garnish of main dish, and soup. The least feasible section in outsourcing of food materials is appetize. Appetize includes a concept of a improvised dish and needs speed. Due to its color, freshness, and sensibility of taste, the appetize plays a key role in the Western food cooking. 4. When outsourcing is taken in place, the highest risk is to lower the inner cooking skills. Therefore chefs in charge of the Western food sequently recognize both internal problems including storage of cooking skills, unstability of layoffs, and loss of cooperation between departments, and external problems including inferior goods, difficulty of differentiating manu, delay of delivery, and expiration date. It shows that most of the Western food chefs consider risks of the internal problems at first. 5. A effective outsourcing needs appropriate selection of outsourcing companies, maintenance of credibility, active communication, check and management of hygiene. However regardless of their position or career, chefs in charge of the Western food have the same cognitive attitude toward selecting successful outsourcing companies after the outsourcing system is enforced. The core of cooking, or a final stage in the full process of so-called artistic cooking, should be treated with insourcing. Reduction of several cooking processes resulted in shortened cooking time, increased efficiency, faster cooking, cutting the waiting-lines, and finally more room for customers. The outsourcing system can reduce or eliminate the following processes in cooking: buying various food materials, checking, storing, preparing, and processing. Especially in the Western food cooking department of a hotel, the outsourcing system should be enforced to make an economic kitchen and to efficiently manage it. Wow it's time to change from the traditional kitchen to an economic kitchen in the hotel cooking department. For that, the cooking department should become a small but strong organization by outsourcing except its core work.

농촌지역의 일차보건사업 개발을 위한 기초조사 연구 - 경기도 여주군 금사면 산북부락을 중심으로 - (A Baseline Survey on Development of Primary Health Care in the Rural Korea -Sanpuk Village, Kumsa-Myun, Yuju- Gun, Kyunggi-Do-)

  • It is widely recognized that primary health care in the community is one of the most important and effective health measures in these days. However, it is reality that unsatisfactory health care system, ineffective utilization of health care by the community people in the rural area are hampering better understanding for primary health care. Therefore promoting health for the rural people and increasing understanding about primary health care, the baseline survey in the community focused in examination for safe community water supply was carried out. The survey was conducted through August 25-31, 1986 in order to find out health problems and relevant factors and to define the demographic characteristics of $^*$Sanpuk village, Kumsa-Myun, Yuju-Gun, Kyunggi-Do, Korea. Household survey was carried out for every home by trained interviewers. The major results are found out as follows : 1) 84.2%(400 houses) of total households were surveyed because 15.8%(75 houses) were unable to survey due to either refusal against interview or absence of family. These 400 households were composed of 1,697 residents(male:830, female 867). Educational level of respondents showed 34.1% as elementary school graduated. Religion distribution showed Buddism(23.8%) as the most dominant. 50.7% of respondents married in the area. 2) Most households(91.5%) have lived in their own house in Sanpuk area. Average family size showed 4.3. More than half of residents(64.2%) have used public supplied water tap. Only 1.5% of the households had a flush toliet. The rest of households have still used primitive insanitary latrines. 3) 32.5% of residents have used gas burner for cooking and for heating in the house, and the coal briquet were used for boiler. Lack of convenient public transportation was the chief complaint for their day life. 4) Each household occupied 1,990 pyungs of rice paddy and 1,170 pyungs of ordinary field in average. Beside farming products, mushroom was the highest product. 5) Sixth percent of households in the survey area regularly participated in community meeting one hand and on the other hand 39.5% never participated. Most of respondents closely contacted with their neighbours and they seemed very friendly each other. 6) The prevalence rate of illness and injury during recent 15 days showed 48.3 per 1,000. The prevalence rate of chronic illnesses during the past one year showed 74 per 1,000. Injury and accident lead the higher portion(22.0%) in the former and in the latter pain(arthritis, back-pain) showed 27.0% as the dominant sickness. 87.8% of the ill residents in the former received medical treatment. As the most frequently utilized medical facility, the clinic or hospital were counted. Among the residents suffering from chronic illnesses, 77.3% in Sanpuk area get some kind of medical treatments and they rarely utilized the clinic or hospital. The reason why the patient did not receive any medical care was found out the fact that symptoms of illness was light or mild and economic problems was serious. 7) Average age of marriage showed 21.6 years old in the women and the average duration of marital period was shown for 15.1 years. The married woman in reproductive age in Sanpuk area had experienced pregnancies 4 times in the aver-age including 0.7 time of pregnancy in average were interrupted by induced abortion and 0.3 time by spontaneous abortion respectively. The practicing rate of the family planning of the married woman during reproductive ages showed 70.7% and the tuballigation was found out as the most frequently used contraceptives. 8) Among woman who has children under 2 years old, 70.0% had received the prenatal care for the last pregnancy. However, the average number of prenatal care visitis per woman showed 3.3 times. Fifty-two % of woman who received the postnatal care for the last delivery showed only 37.5%. 9) Immunization rate of the children under 2 years old showed relatively high and looked successful. The breast feeding for these children showed dominantly in the most. Most of the mothers in Sanpuk area had started the supplementary diet during weaning period of their infants of 6th and 7th month after birth. * : Sanpuk area is a demstration area for community development which has been supported by the Community Development Foundation during the part 10 years. The village is relatively closer to urban area such as Seoul, However, it has a similar characteristics shown as a remote village because of geographical location and inconvenient transportation at present.
