산욕후기 모성의 산후우울 정도에 따른 주관적 수면의 질에 관한 연구 (Subjective Sleep Quality in Depressed and Non-Depressed Mothers During the Late Postpartum Period)

  • Purpose: This study aimed to explore the subjective sleep quality of depressed and non-depressed mothers in the late postpartum period and to determine the relationship with their health promoting behaviors, family functioning, parenting stress. Method: A non-probability sample of 128 mothers completed a self-administered questionnaires at 4-6weeks postpartum. The Edinburgh postnatal Depression Scale (EPDS) and Pittsburgh Sleep Quality Index were used to measure mother's experiences of depression symptoms and sleep. Related factors of sleep quality were measured by the Korean Family Functioning Scale, Health Promoting Lifestyle Profile, and Parenting Stress Index. The data was analysed using t-test, one-way ANOVA, and the Pearson's correlation coefficients. Result: The results indicated that the depressed mothers (EPDS$\geq$ 10) had poorer sleep quality than the non-depressed mothers(EPDS < 10), reported shortened sleep duration, and experienced more daytime dysfunctions. Depressed mothers who had no job, did not drink coffee, and were primipara tended to report poorer sleep quality. There were significant correlation between poorer sleep quality and lower health promoting behaviors, higher family intimacy and lower family communication, and higher parenting stress among depressed mothers. Conclusion: Our findings support the view that depressed mothers' experiences of poor sleep are much higher than non-depressed mothers and multi-faced. Nurse professionals should screen for sleep problems in the depressed mothers with a different biopsychosocial and behavioral aspect from the non-depressed mothers in the late postpartum period.

Effect of Foeniculum Vulgare (Fennel) on Sleep Quality of Menopausal Women: A Double-blinded, Randomized Placebo-controlled Trial

  • Objectives: Aim of present study was designed to investigate the soporific effect of fennel among menopausal women. Methods: The present double-blinded and placebo-controlled trial examined the fennel effect on Pittsburgh Sleep Quality Index (PSQI). Total score and relevant 7 components, including sleep duration, sleep latency, use of sleeping medication, subjective sleep quality, sleep disturbances, daytime dysfunction and habitual sleep efficiency among 50 menopausal women compared to control group within a 12-week follow-up. Results: The patients in both groups reported no certain side effects and all subjects completed the study. The mean actual sleep duration was 5 hours and 66 minutes. Intergroup comparison revealed no statistically significant differences in the mean total PSQI score (P = 0.439), subjective sleep quality (P = 0.826), habitual sleep efficiency (P = 0.127), sleep disturbances (P = 0.130), use of sleeping medication (P = 0.52) and daytime dysfunction (P = 0.439). A tendency toward significant between 2 groups was seen concerning the sleep duration (P = 0.059). Intergroup comparison showed significantly borderline levels (P = 0.059). Conclusions: The treatment of 12 weeks with fennel caused a slight effect that did not reach to significant. These findings should be considered cautiously because of small sample size, short-term follow-up and subjective measure of sleep quality.

한국 중고령층의 폐쇄성 수면무호흡증 위험과 주관적 건강 및 건강 관련 삶의 질 간의 연관성 (Association between Risk of Obstructive Sleep Apnea and Subjective Health and Health-Related Quality of Life of the Korean Middle-Aged and Elderly Population)

  • 연구배경: 최근 국내 중고령층 수면무호흡증 환자 수는 증가되는 추세이며, 그 중 가장 임상적으로 흔히 나타나는 폐쇄성 수면무호흡증은 전반적인 건강 및 웰빙과 연관된다. 이에 본 연구는 한국 중고령층의 폐쇄성 수면무호흡증 위험과 주관적 건강 및 건강 관련 삶의 질 간의 연관성을 파악하고자 하였다. 방법: 2019-2020년 국민건강영양조사(Korea National Health and Nutrition Examination Survey VIII) 전체 응답자 22,559명 중 40세 이상 성인을 추출하여, 결측치가 없는 총 6,659명의 중고령층을 대상으로 데이터를 2차 분석하였다. 그리고 로지스틱 회귀분석과 다중회귀분석을 통해 폐쇄성 수면무호흡 위험 여부와 주관적 건강 및 건강 관련 삶의 질 간의 연관성을 조사하였다. 결과: 폐쇄성 수면무호흡 비위험군에 비해 위험군인 경우 주관적 건강이 저하될 오즈비는 1.84배(p<0.001) 통계적으로 유의하게 높았고, 건강 관련 삶의 질은 0.02점(β, -0.02; p<0.001) 통계적으로 유의하게 낮은 것으로 나타났다. 특정 변수에 대해 하위그룹 분석을 실시한 결과, 성별, 수면시간, 우울증 여부, 가구소득, 가구원 수에 따라 폐쇄성 수면무호흡 위험과 주관적 건강 및 건강 관련 삶의 질 간의 연관성이 통계적으로 유의하게 나타났다. 폐쇄성 수면무호흡 위험군을 기준으로 하였을 때, 남성에 비해 여성에서 주관적 건강이 낮을 연관성이 더 높았고 건강 관련 삶의 질 점수가 낮았다. 수면시간이 8시간 이상이거나 6시간 이하인 경우 6-8시간인 경우보다 주관적 건강이 낮을 연관성이 더 높았고 건강 관련 삶의 질 점수가 낮았다. 우울증이 있는 경우 없는 경우보다 주관적 건강이 낮을 연관성이 높았다. 가구소득 수준이 낮을수록, 가구원 수가 감소할수록 주관적 건강이 낮을 연관성이 높아지고 건강 관련 삶의 질 점수가 낮아졌다. 결론: 폐쇄성 수면무호흡증 위험이 단순히 수면장애로만 직결되는 것이 아니라 개인의 주관적 건강과 건강 관련 삶의 질과도 연관성이 있다는 점을 인식할 수 있도록 사회적인 지원 및 교육이 제공되어야 한다. 특히 여성, 낮은 가구소득, 1인 가구원, 우울증이 있는 취약계층을 대상으로 폐쇄성 수면무호흡증 예방 및 관리프로그램을 통해 주관적 건강과 건강 관련 삶의 질을 향상시킬 수 있도록 해야 할 것이다.

수지요법이 수면장애를 가진 성인의 수면의 질에 미치는 효과 - 수면다원검사 및 뇌혈류 검사를 통한 검증 - (Effects of Hand Acupuncture Therapy on Sleep Quality in Sleep Disrupted Adults - Verification by Polysomnography and Cerebral Blood Flow Test -)

  • Purpose: The purpose of this study was to identify the effects of hand acupuncture therapy on sleep quality by means of a sleep questionnaire, polysomnography and a cerebral blood flow test in $30{\sim}59$ year old adults. Methods: The study was a sham controlled design. Twenty-two adults were assigned to the pellet stimulating group (11) or sham group (11). The pellet stimulating group received hand acupuncture therapy using New Seoam Press Pellets number 1 for 4 weeks. On the other hand, the sham group used the same Adhesive tape in terms of shape, size and quality as New Seoam Press Pellets number 1 for 4 weeks. A Transcranial Doppler Ultrasonography and Carotid Duplex Ultrasonography examination were used for evaluating cerebral blood flow. Data was analyzed using the SPSS 12.0 version program with $X^2-test$, Fisher's exact test and Mann Whitney U-test. Results: In the pellet stimulating group, subjective sleep quality significantly improved more than that of the sham group. Among the sleep indices of the polysomnography, total sleep time and sleep latency of the sham group significantly improved. The cerebral blood flow test didn't show any differences. Conclusion: These results suggest that hand acupuncture therapy is effective for subjective sleep improvement only, not polysomnographical sleep indices and cerebral blood flow.

매트리스의 종류가 건강한 성인의 주관적, 객관적 수면의 질에 미치는 영향에 대한 탐색적 연구 (Comparative Effects of Mattress Type on Subjective and Objective Sleep Quality : A Preliminary Study)

  • 목 적 : 일반스프링 매트리스에 비교하여 지지력 등을 향상시킨 컨투어 코일 매트리스가 주관적 및 객관적인 수면의 질을 향상시킬 수 있는지 알아보고자 하였다. 방 법 : 건강하게 수면하는 일반인 대상자 15명을 모집하였다. 연구대상자들은 두 종류의 매트리스에서 잠을 자면서 야간수면다원검사를 시행하였고 검사 전, 후 주관적인 수면의 질에 대한 자가설문지를 작성하게 하였다. 한 연구대상자가 각각 다른 매트리스에서 2일에 걸쳐 야간수면다원검사를 시행하여 얻어진 객관적인 수면변인들과 주관적인 자가 설문지 변수들로 교차설계분석을 시행하였다. 결 과 : 야간수면다원검사에서 측정된 수면잠복기가 컨투어 코일 매트리스에서 잠을 잔 경우 일반스프링 매트리스에서 잠을 잔 경우보다 평균 1.9분 감소하였다. 이외에도 주관적으로 느낀 수면 중 각성횟수, 깊게 잠든 느낌 그리고 잠자리의 편안함이 컨투어 코일 매트리스가 유의하게 더 나은 결과를 보였다. 결 론 : 매트리스가 종류에 따라서 주관적인 수면의 질뿐만 아니라 객관적인 수면의 질에도 영향을 미친다고 할 수 있다.

사상체질별 수면 소증(素證)에 대한 체계적 고찰연구 (Systematic Review on the Sasang Type-specific Pathophysiological Symptoms of Sleep)

  • Objectives: The purpose of this study was to review previous clinical studies on underlying mechanisms of sleep-related type-specific pathophysiological symptoms among the Sasang types. Methods and Procedure: We reviewed seven research databases from December 2003 to August 2015 with the keywords Sasang typology, constitution and sleep. The Sasang type-specific sleep-related symptoms were analyzed based on seven categories, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medications and daytime dysfunction. Results: Total of twelve studies were included in the analysis. The Tae-Yang type showed low subjective sleep quality, long sleep latency and frequent awakening, and the So-Yang type showed long sleep latency. The Tae-Eum type presented high subjective sleep quality, short sleep duration, frequent snore, toss and turn during the sleep, and insomnia and low frequency of dream. The So-Eum type showed long sleep latency in male and high frequency of dream. The Eum type combining Tae-Eum and So-Eum types had higher subjective sleep quality, longer sleep duration and higher frequency of dream than the Yang type combining Tae-Yang and So-Yang types.Conclusions This study reviewed type-specific sleep-related characteristics and discussed possible pathophysiological mechanisms involved. Differences in sleep characteristics among the Sasang types might stem from type-specific temperaments and require further study.

일부 종합병원 간호사들의 교대근무와 수면문제 및 피로자각증상과의 관련성 (Relationship between Shift Work, and Sleep Problems and Fatigue Symptoms of Nurses for General Hospitals)

  • Purpose: To find out the relationship between the shift work, and disrupted sleep and consequent fatigue symptoms. Methods: The questionnaires were given to 345 nurses of 4 hospitals with over 400 beds in Daejeon. Results: The shift workers showed worse subjective quality of sleep than the daytime workers, and had significantly higher frequency of "cannot get to sleep within 30 minutes", "wake up in the middle of the night" and "wake up in the early morning". The shift workers had significantly higher scores of subjective symptoms of fatigue than the daytime workers, and the fatigue was found to be the mental type. The subjective symptoms of fatigue scores were significantly higher in the group who had "cannot get to sleep within 30 minutes", "wake up in the middle of the night" and "wake up in the early morning". Conclusion: The study results indicated that the shift workers had the higher level of subjective symptoms of physical fatigue as well as the worse quality of sleep than the daytime workers. It is speculated that the factors attributable to the poor quality of sleep or sleep induction disorders of nurses of hospitals could be due in major part to adaptational difficulties resulting from frequent night shifts.

고등학생의 정신건강인식과 주관적인 수면 질과의 관련성 (Relationship between High School Students' Awareness of Mental Health and Subjective Quality of Sleep)

  • Objectives: This study aims to identify the effect between high school students' recognition of mental health on sleep quality to present basic data for better health. Methods: The study used data obtained through the 2007 online research on adolescents' health behaviors, targeting a total of 35,228 high school students (18,628 of male students and 16,600 of female students). Results: Seventy six point ninety six percent of the male respondents and 79.67% of female respondents answered their sleep quality was not satisfactory. The subjects who had stress showed significantly lower quality of sleep than those who answered they had no stress. For female students, those who answered they had intention to suicide had significantly lower quality of sleep than those who answered they did not have, but for male students, there was no significant relations between them. There were no significant relations between sense of dispair and sleep quality both for male and female subjects. Conclusion: Factors of mental health related with sleep quality were stress and intention of suicide. The more they had stress and intention of suicide, the significantly lower sleep quality was. Therefore, in order to improve sleep quality of high school students, it is suggested that management programs based on education and counselling with experts should be provided and further studies on other mental health factors and sleep quality should be conducted.

성인의 지각된 스트레스, 수면의 질, 주관적 행복감이 웰니스 생활양식에 미치는 영향 (The Effects of Perceived Stress, Sleep Quality and Subjective Happiness on the Wellness Lifestyle among Adults)

  • 본 연구는 성인을 대상으로 웰니스 생활양식의 정도와 이에 영향을 미치는 요인들을 파악하고자 시도되었다. 연구대상은 경기도 A시에 거주하는 성인 390명으로 지각된 스트레스, 수면의 질, 주관적 행복감, 웰니스 생활양식을 설문조사하였다. 연구결과 대상자의 웰니스 생활양식 정도는 정신적 웰니스 3.60점, 지적 웰니스 3.58점, 사회적 웰니스 3.51점, 정서적 웰니스 3.41점, 신체적 웰니스 2.94점 순으로 나타났다. 또한 웰니스 생활양식에 유의한 영향을 미치는 요인은 주관적 행복감, 수면의 질, 지각된 스트레스로 나타났으며 웰니스 생활양식에 대한 변수의 설명력은 34%였다. 결론적으로 성인의 웰니스 생활양식을 높이기 위해서는 행복감을 향상시키는 것이 중요하고, 동시에 수면의 질을 높이고 스트레스를 완화시키는 전략이 도움이 될 것으로 사료된다. 따라서 후속연구에서는 이러한 전략들을 적용한 웰니스 건강증진 프로그램을 개발하고 적용효과를 확인할 것을 제언한다.

Influencing Factors for Sleep Quality among Firefighters: Based on Objective and Subjective Evaluation

  • Purpose: The purpose of this study was to assess insomnia and the quality of sleep, investigate the concordance between objective and self-report sleep patterns, and identify physiological, psychological, and situational factors influencing insomnia and sleep quality among firefighters. Methods: A descriptive, cross-sectional study was conducted with 103 firefighters in Korea. The collected data were analyzed using SPSS 23.0. Descriptive statistics, the independent t-test, and hierarchical logistic regression analysis were performed. Results: Insomnia was found in 66 (64.1 %) of the total subjects, and the average quality of sleep (PSQI) was 5.65 (SD=2.57). Total sleep time (401.00 minutes) and sleep latency (21.60 minutes) measured using self-reported scales were longer than the ones measured using objective measurements by approximately 48.70 and 17.10 minutes, respectively. Factors related to insomnia included the role as a paramedic (OR=4.28, 95% CI: 1.02~17.92), anxiety (OR=1.12, 95% CI: 1.01~1.24), and sedentary lifestyle (OR=0.85, 95% CI: 0.78~0.94), and factors related to sleep quality were physical illness status (OR=5.17, 95% CI: 1.53~17.51) and social support (OR=0.86, 95% CI: 0.78~0.95). Conclusion: The results show a high prevalence of insomnia, poor quality of sleep and the discrepancy between objective and subjective sleep patterns among firefighters. To promote sleep quality and health, early screening and treatment of anxiety and physical illness are required. It is necessary to conduct further studies examining the relationship between physical activity level and sleep.
