뇌성마비아 어머니의 양육 스트레스, 우울 및 양생과의 관계 (Relationship Between Parenting Stress, Depression, and Yangsaeng in Mothers with Cerebral Palsy Children)

  • Purpose: This study examines parenting stress, depression, and Yangsaeng in mothers with cerebral palsy children, providing information that may be helpful in the development of a health intervention program that may improve the lives of the mothers. Methods: The subjects of this study were 124 mothers with cerebral palsy children; the subjects were selected by convenience sampling. The collected data were processed by using the SPSS program and analyzed by using descriptive statistics and Pearson's correlation. Results: Parenting stress in mothers with cerebral palsy children was $91.84\pm20.75$, depression was $20.54\pm11.30$, and Yangsaeng was $92.77\pm15.79$ There was a positive relationship between parenting stress and depression. Parenting stress and depression were negatively related with Yangsaeng. Conclusions: The results suggest that Yangsaeng as an oriental health care regimen can lower the level of parenting stress and depression. In this regard, to promote a healthier life for mothers with cerebral palsy children, it would be beneficial to develop a nursing intervention program that incorporates the use of Yangsaeng.

보육교사의 안전에 대한 태도, 지식, 실천 및 관련요인 (Attitude, Knowledge, and Practice of Safety and Related Factors of Teachers in Child Care Centers)

  • Purpose: To investigate the attitude, knowledge, and practice of safety and related factors of teachers in child care centers. Methods: The total sample consisted of 116 teachers from child care centers in Kyunggi Province. A questionnaire and checklist were used to collect data, which was analyzed with SPSS 11.5 Win program. Results: Most of the teachers were anxious about child accidents, and thought of safety as a primary concern. The mean score for knowledge on safety was 10.23, and the rate of correct answers ranged from 34.8% to 98.3%. The mean score on practice of safety was 42.01. Teachers with higher education showed significantly higher scores in safety efficacy. Teacher's knowledge on safety was significantly different depending on the experience of safety education, but this did not apply to safety practice. Safety practice was significantly related to confidence in safety performance, health beliefs on safety, stress, and social support, but not related to knowledge of safety. Conclusion: Psychosocial factors were found to be important in safety practice. Related factors found in this study should be considered when providing safety programs for child care centers.

일 대학병원 임상간호사의 건강관리 경험 (The health care experience of the clinical nurse in a university hospital)

  • 본 연구는 일 대학병원 임상간호사의 건강관리 경험의 본질을 이해하기 위해 2015년 10월 4일부터 10월 30일까지 S시에 소재한 일 대학병원 간호사 9명의 참여로 그들의 경험의 의미와 실체를 파악하기 위하여 Colaizzi(1978)가 제시한 현상학적 연구 방법을 적용하여 자료를 분석하였다. 연구 결과로는 참여자의 건강관리 경험이 '신체건강관리에 대한 양가감정', '신체운동에 대한 부담감', '교대근무로 인한 어려움', '스트레스 관리' '자신의 재발견'의 5개의 범주와 이들 범주에 포함된 14개 주제모음 및 42개 주제로 확인할 수 있었다.

  • Nutritional assessment and support are often overlooked in the critically ill due to other urgent priorities. Unlike oxygenation, organ dysfunction, infection, or consciousness, there is no consensus of indicators. Making it difficult to evaluate the effectiveness of an intervention. Nevertheless, appropriate nutritional support in the critically ill has been associated with less morbidity and lower mortality. But, nutritional support has been considered an adjunct, for body weight maintenance and to help patients during the inflammatory phase of illness. Thus, it has been assigned a lower priority, compared to mechanical ventilation or hemodynamic stability. Recent findings have shown that nutritional support may prevent cellular injury due to oxidative stress and help strengthen the immune response. Large-scale randomized trials and clinical guidelines have shown a shift from nutritional support to nutritional therapy, with an emphasis on the importance of protein, minerals, vitamins, and trace elements. Nutrition is also important in neurocritically ill patients. Since there are few studies or recommendations with regard to the neurocritical population, the general recommendations for nutritional support should be applied.

Transfer anxiety in parents of children transferred from pediatric intensive care units to general wards in South Korea: a hybrid concept analysis

  • Purpose: This study aimed to analyze the concept of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Methods: The hybrid model by Schwarz-Barcott and Kim was used to analyze the characteristics of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Results: Transfer anxiety was defined by the following attributes: 1) stress concerning the adaptation process, 2) concern about the child's condition worsening due to the parent's caregiving, and 3) involuntary changes in daily life due to the treatment. Transfer anxiety has the following antecedents: 1) uncertainty; 2) a lack of knowledge about the illness, medical devices, and caregiving; and 3) a lack of social support. It resulted in 1) caregiver burden, 2) a decrease in the capacity for coping with caregiving, 3) delays in the child's physical and psychological recovery, and 4) decreased quality of life. Conclusion: It is necessary to develop an assessment scale that considers the attributes of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Furthermore, an effective nursing intervention should be developed to reduce transfer anxiety.

The art of diabetes care: guidelines for a holistic approach to human and social factors

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

정상 신생아 어머니와 고위험 신생아 어머니의 신생아에 대한 지각 및 교육요구도 비교 (The Educational Needs and Perception of the Mothers of High Risk Infant and Normal Neonate)

  • The purpose of this study is to compare the educational needs and perception of mothers of normal neonates and high risk infants. This research was designed as a descriptive study. Data were collected for two months from April 2002 to March 2002. Subjects were 41 mothers of high risk infants and 60 mothers of normal neonates in one general hospital in Seoul participated in the study. Measurement tools used in this study were the educational needs scale developed by Cho Kyoul Ja et al and the neonatal perceptive inventories scale developed by Broussard. They ask mothers to rate each item on a four point Likert type scale. The collected data were analyzed using SPSS 8.0 program. The variables were listed as frequency, mean, standard deviation, X2 test, t-test, ANOVA. The results were as follows: The educational needs of the mothers of primi pregnancy was higher than the mothers of multi pregnancy. The educational needs on management of diseases such as prevention of infection, symptoms of disease, mental development, attachment promotion, congenital metabolism test, management of convulsion, care of vomiting and fever were higher than general care of infants such as immunization, measurement of temperature, hiccough care, follow up care. The informations must be included in nursing intervention program to reduce the mothers' stress level. In conclusion, in order to promote positive mother infant relationship, nurse need to give information and educate the mothers of high risk infant and normal neonate.

신생아중환자실에서의 가족중심돌봄 기반 가족중재연구에 대한 통합적 문헌고찰 (An Integrative Review of Family Interventions based on a Philosophy of Family-Centered Care in Neonatal Intensive Care Units)

  • Purpose: The aim of this study is to analyze the characteristics of family interventions based on a philosophy of family-centered care conducted in neonatal intensive care units through an integrative literature review. Methods: We searched the PubMed, CINAHL, RISS, KISS, and DBpia databases; a total of 20 studies, published between January 2013 and May 2018, was selected according to our criteria. Results: Mothers accounted for a greater proportion of participants in family interventions than did fathers. Family interventions described in the studies were categorized into four educational and sixteen non-educational interventions. Among non-educational interventions, skin-to-skin-contact interventions, such as kangaroo care, accounted for the highest proportion. Only one paper employed a theoretical framework. Conclusion: More family interventions based on theoretical frameworks should be conducted as these frameworks serve as guidelines for nursing research. As the stress patterns experienced by parents in neonatal intensive care units showed gender differences, more programs tailored for fathers are needed. Moreover, further research should be conducted to evaluate feasibility as an outcome variable, and studies of family interventions based on a philosophy of family-centered care should be performed more actively in the neonatal intensive care units in Korea.

보육교사가 지각하는 부모와의 의사소통 수준이 직무스트레스와 이직의도에 미치는 영향: 교사의 적극적인 스트레스 대처방식의 조절효과 검증 (Examination of the Moderator Effect of Active Stress Coping on the Influence of Parent-Childcare Teacher Communication on Teacher's Job Stress and Turnover Intention)

  • Objective: This study aimed to investigate the effect of parent- teacher communication on childcare teacher's job stress and turnover intention. This study also examined if the level of active stress coping used functions as a moderator in relationships among parent-teacher communication, childcare teacher's job stress, and their turnover intention. Methods: To achieve this, this study used data from 223 surveys from childcare teachers and analyzed the data with SEM and multi-group SEM analysis methods. Results: First, parent-childcare teacher communication influenced the teacher's turover intention through job stress. Second, the effect of parent-childcare teacher communication on job stress was not found in the higher-level of the active stress coping group. This was different from the significant effect in the lower-level of the active stress coping group. Also, parent-childcare teacher communication influenced teacher's turnover intention through job stress in the lower-level of the active stress coping group. In contrast, the mediation effect was not shown in the higher group. Conclusion/Implications: The results of this study show the effect of parent-childcare teacher communication on teacher's job stress and their turnover intention. Also, this study shows that the level of active stress coping could moderate the relationship among parent-childcare teacher communication, teacher's job stress, and their turnover intention.

장애아동 양육을 위한 어머니의 정보요구에 관한 연구 (Information Needs Expressed by Mothers of Young Children with Disabilities)

  • Objectives: This study aims to determine fostering stress and mental health state that mothers of handicapped children perceive as primary care givers and to analyze their demand for information assistance in order to release their stress so that it can provide materials that contribute to establishment of assistance system for families with handicapped children. Methods: The research subjects were 340 mothers whose children went to a nursery for special children, 3 general nurseries and 6 special schools in Daegu, and the data were collected using structures questionnaires. The survey analyzed mothers' fostering stress, their demand for fostering information assistance, children's daily activity abilities. Component concepts of each scale was validated by adopting confirmatory factor analysis, and factors affecting demand for fostering information assistance were analyzed by adopting covariance structural analysis. Results: Younger mothers tend to have higher demand for information, and mothers with younger children or children with double handicaps also have higher demand. Mothers under 30 have the lowest demand for public health and medical care assistance and for home and community life assistance, while mothers with children with physical handicaps have the highest. The validity of component concepts was verified by categorizing as cognitive structure models fostering stress, information demand, children's daily activity abilities, and their appropriateness was evaluated through confirmatory factor analysis using structural equation modelling. And then, GFI (more than 0.9), CFI (more than 0.9), TLI (more than 0.9) and RMSAE (less than 0.08) were used to evaluate the appropriateness. It was found that all the component concepts are valid, as every item is within appropriate range. The result of analyzing information demand demonstrated that children's handicap levels significantly affect their mothers' mental health, while fostering stress significantly affect mothers' metal health, information demand. As well, it was confirmed that mothers' mental health has a significant effect on information demand. Conclusions: Therefore, to reduce special children's mothers' uncertainty, helplessness and fostering burden, it is necessary to provide them with information on children's challenges, development and fostering and to offer them quality public health, medical care and welfare assistance along with family and local community life assistance.
