Musculoskeletal disorders are a major cause of occupational disabilities. Approximately thirty percent state that the major reason for an inability to work is a musculoskeletal problem. A farm work is associated with increased stress and trauma to joints. Chronic overuse injuries are a result of stresses that exceed the body's adaptive or healing capabilities. They can occur in bone, tendons and muscle-tendon-bone junctions. The aim of the investigation was to the study the frequency of pain, ache, and discomfort in the musculoskeletal system among farmers, to find possible correlations between these symptoms and various working positions and different working actions. A questionnaire was answered by 138 farmers. Of those who answered the questionnaire 82% had pain and discomfort from back, shoulders, arms in orders, The result showed that musculoskeletal pain predominate in the farming seasons, and of those who had pain and discomfort 89% thought that farm works had a correlation with the pain and discomfort of musculoskeletal system. But only 17% of quationnaire were put to periodic medical examinations, and 50% of those who had pain and discomfort consulted a doctor. Education in effective pain treatment should therefore be intensifide to ascertain that farmers in rural areas have satisfactory knowledge of the musculoskeletal pain as a chronic overuse syndrome.
Object : Since autonomic nerve system dysfunction was known as the mechanism of panic disorder, many researchers used heart rate variability (HRV) as means of measuring autonomic nerve function of patients with panic disorder. We aimed to examine the effect of paroxetine medication for 3 months on symptom improvement and change of heart rate variability of the patients with panic disorder. Methods : The subjects were patients with panic disorder who visited the psychiatric outpatient clinic of Samsung Medical Center in Seoul. We included panic disorder patients who were aged from 20 to 50 and in normal BMI range (from 18 to 30) to minimize the effect of age and weight on HRV data. We excluded the patients with EKG abnormalities, hypertension or other major psychiatric disorders. They took 20-40 mg paroxetine medication a day for 3 months. Alprazolam was used only during the first month to control the acute panic symptoms and was tapered off after that. We measured the acute panic inventory (API), Hamilton rating scale for anxiety and depression (HAM-A & HAM-D), Spielberger state-trait anxiety inventory (STAIS, STAIT), and Beck depression inventory (BDI) in order to assess clinical improvement of the patients. And we measured time and frequency domain HRV in the resting, standing and cognitive stress states to assess the change of HRV. All measurements were done before and after paroxetine treatment. Result : After paroxetine medication, patients showed significant improvement in all psychiatric scales. In time domain of HRV, standard deviations of all R-R intervals (SDNN) were significantly increased in all states. In frequency domain of HRV, the ratio of high frequency to total power (HF/TP) in the standing state was significantly increased. Conclusion : After 3 months paroxetine medication, panic disorder patients showed significant clinical improvement and change in HRV data such as SDNN in all states and HF/TP ratio in the standing state. This result suggests that paroxetine medication is effective for the improvement of autonomic nerve system dysfunction in panic disorder patients.

Background: Breast self examination (BSE), performed regularly every month, is one of the most important methods in the early diagnosis of breast cancer. This study was performed with the aim of establishing the knowledge, attitudes, and behavior of women working in government hospitals within the province of Samsun regarding BSE. Materials and Methods: This cross-sectional study was conducted between January-March 2012, on a total of 550 women (500 health personnel, and 50 general administration services (GAS) workers) from 7 government hospitals and the Cancer Early Diagnosis, Screening, and Education Centre (CEDSEC). Percentages were used for the descriptive statistics, and the chi-square test for the evaluation of statistical importance. Values of p<0.05 were accepted as significant. Results: The mean age of the participants was $36.2{\pm}15.3$, and 42.5% were in the 30-39 year old age group, 78.0% being married. Seventy-eight point four percent (78.4%) of the health personnel and 76.0% of the GAS workers performed BSE. However, the rates of performing BSE regularly every month were only 25.6% and 5.0%. Within the health personnel, 1.4% stated that they did not perform BSE because they found it unnecessary as they had no history of breast cancer in their family, 3.6% did not do so due to fear and stress, 13.2% because they forgot, and 14.6% because they had no complaints. Some 22.2% of the health personnel and 52.0% of the GAS workers had undergone mammographic evaluation, the difference being significant (p<0.05), 84.1% of the health personnel and 61.9% of the GAS workers knowing symptoms of breast cancer. Conclusions: Women in society should be brought to a certain level of awareness and knowledge regarding BSE. It is of the utmost importance that health personnel, who carry the responsibility for counseling and enlightening society, should interiorize the necessary knowledge, attitudes and behavior.

Objectives The objective of this study is to investigate differences of clinical characteristics between a healthy female control group and female panic disorder (PD) patients with early sexual abuse history (PD+S) and without early sexual abuse history (PD-S). Methods We examined data from 83 patients diagnosed with PD and 20 healthy control subjects. We divided the patients with PD into PD+S (32 patients) and PD-S (51 patients) to compare demographic and clinical characteristics. The following instruments were applied: the Stress coping strategies, the Beck Depression Inventory (BDI), the Panic Disorder Severity Scale, the Anxiety Sensitivity Index-Revised (ASI-R), the Albany Panic and Phobia Questionnaire (APPQ) and the NEO-neuroticism. Results Compared to the PD-S, the PD+S group showed higher scores in neuroticism and the APPQ. And, in the PD+S group, the scores of neuroticism were correlated with the ASI-R and APPQ subscale scores and the APPQ total scores were associated with the scores of BDI. Conclusions This study shows that female PD+S patients have higher scores in neuroticism and the APPQ than the PD-S group, and these factors are associated with the panic-related symptoms severity. It emphasizes the need of specific strategies considering the childhood abuse history such as early sexual abuse in clinical approach among patients with PD.
Objectives : In this paper, lung atrophy syndrome[肺痿] in 『Huangdineijin(黃帝內經)』 and 『Jinguiyaolue(金匱要略)』 were compared, followed by examining its relation with upper wasting thirst[上消]. Also, ways in which psychological factors that contribute to lung atrophy syndrome could cause upper wasting thirst were studied. Methods : Verses from 『金匱要略·肺痿肺癰咳嗽上氣病脈證治』 and 『素問·痿論』 were analyzed based on various annotators's opinions to determine the cause and mechanism of lung atrophy syndrome and its relationship with upper wasting thirst. Results : In 『Jinguiyaolue(金匱要略)』, lung atrophy syndrome is described as the heat of the upper body entering the lungs to dry it out. The description in 『Suwen(素問)』 differs in that it accompanies atrophy symptoms, but the mechanism is the same. Lung atrophy syndrome in 『Jinguiyaolue』 could come from wasting thirst, while wasting thirst can be accompanied in deficiency caused by chronic lung atrophy syndrome. Heat in the lungs is caused by psychological factors where the person has lost its subject of possession or was unable to attain what was desired. When expanded to include heart atrophy syndrome[心痿] and lung atrophy syndrome[肝痿], the reason for upper wasting thirst could include immense sadness or excessive indulgence in pleasure due to unmet desires. Conclusions : Although diabetes and wasting thirst are not identical, application of wasting thirst pattern differentiation to diabetes treatment and management could lead to tailored treatment of each patient. Moreover, the five zhang pattern differentiation from the 『Suwen(素問)』 could increase treatment efficacy when applied to conditions caused by stress and emotional disorder, which are increasingly playing larger roles in causing wasting thirst, or diabetes.
Objectives: The objective of this study was to investigate the correlation between demographic factors and oral health factors and mental health factors in Korean adolescents. Methods: The study was based on the 11th Korea Youth Risk Behavior Web-Based Survey (2015). The final participation rate in the survey was 96.7%. of a total of 70.362 adolescents (age, 12~18 years) who had participated in the survey, 68,043 adolescents were selected for analysis. Results:As a result of examining the stress awareness of adolescents and the experience of oral disease symptoms, it was found that the number of brushing times per day was less than 1 time (p<0.001), tingling and throbbing (p<0.001), gum pain & blood (p<0.001) and bad breath (p<0.001) were more stressed than those who had experience. On the other hand, the lack of experience in oral health education was more stressful (p<0.001). As a result of examining the factors influencing subjective happiness, it was found that the female, the lower the grade, the higher the academic performance and economic level, the less experience of oral disease in the last one year, The level of awareness was high. Conclusions: These results indicate that the demographic factors, mental health factors, and oral health factors of adolescents are correlated to one another. Therefore, when developing a program for promoting adolescent health or establishing a national business plan, it should be considered in a multi-faceted way.
Stress or march fractures among military personnel, especially recruits, has been appreciated for many years. According to the classical references, the second metatarsal was one of the first sites identified as a focus for march fractures and radiological evidence of fracture appeared as late as several weeks. The purpose of this study was to document the clinical feature of march fractures in Korean infantry soldiers. From 1997 to 1998, at one infantry medical company of OO infantry corps in Korea, 15 (19cases) patients with march fracture were detected among infantry soldiers. There were some different finding in the fracture site and its clinical features from the previous foreign reports. 1. There were pain and local swelling in all cases as clinical manifestation. By physical examination, direct point tenderness on the location of the fractured metatarsal shaft was characteristic. 2. On roentgenographic examination, cortical fissuring or break was seen one week after onset of symptoms and external callus was seen from two weeks or at the least four weeks. Oblique view was more useful than AP view in the diagnosis of march fractures. 3. The third metatarsal was the most frequently involved site(7 cases, 48%). and the second metatarsal was Jess frequent(3 cases, 20%). This difference of frequent site with previous reports might be attributed to the relatively long shaft of the third metatarsal, but should be analyzed in further study. 4. The incidence of the development of march fracture was 1 per 104 infantry soldiers.
Purpose: The purpose of this study is to assess the clinical and radiological results of the early primary repair for acute ankle sprains. Materials and Methods: From October 2002 to September 2005, nine patients with acute ankle sprain were analyzed. Among them, eight patients took the inversion stress X-ray at local clinics, and the mean talar tilting angle was 28 degrees. We observed avulsion fragment near lateral malleolus in the other. The average age at the time of operation was 24 years and average follow-up period was 29 months. We evaluated postoperative symptoms by Hasegawa's clinical rating system, postoperative complications, and compared the talar tilting angle and anterior draw distance between both ankles at the final follow-up X-rays. Results: Anterior talofibular ligament was ruptured at fibula in 4, at midsubstance in 3, at talus in 1 and at fibula and midsubstance simultaneously in 1. Calcaneofibular ligament was ruptured at fibula in 3 including a case of avulsion fracture, at midsubstance in 2, and at calcaneus in 4. And posterior talofibular ligament was ruptured at midsubstance in 2. Clinical results were rated as excellent in all. We did not find major postoperative complications except for one sural nerve irritation. Both (injured ankle/uninjured ankle) talar tilting angle averaged 6.8/8.2 degrees and anterior draw distance averaged 2.9/3.7 mm at final follow-up X-rays. Conclusion: Early primary repair is recommended for treating acute severe ankle sprains and in case found avulsion fracture in X-ray taken after ankle sprain.
Previous studies have demonstrated that repeated administration of the exogenous stress hormone corticosterone (CORT) induces dysregulation in the hypothalamic-pituitary-adrenal (HPA) axis and results in depression and anxiety. The current study sought to verify the impact of catechin (CTN) administration on chronic CORT-induced behavioral alterations using the forced swimming test (FST) and the elevated plus maze (EPM) test. Additionally, the effects of CTN on central noradrenergic systems were examined by observing changes in neuronal tyrosine hydroxylase (TH) immunoreactivity in rat brains. Male rats received 10, 20, or 40 mg/kg CTN (i.p.) 1 h prior to a daily injection of CORT for 21 consecutive days. The activation of the HPA axis in response to the repeated CORT injections was confirmed by measuring serum levels of CORT and the expression of corticotrophin-releasing factor (CRF) in the hypothalamus. Daily CTN administration significantly decreased immobility in the FST, increased open-arm exploration in the EPM test, and significantly blocked increases of TH expression in the locus coeruleus (LC). It also significantly enhanced the total number of line crossing in the open-field test (OFT), while individual differences in locomotor activities between experimental groups were not observed in the OFT. Taken together, these findings indicate that the administration of CTN prior to high-dose exogenous CORT significantly improves helpless behaviors, possibly by modulating the central noradrenergic system in rats. Therefore, CTN may be a useful agent for the treatment or alleviation of the complex symptoms associated with depression and anxiety disorders.

Hwabyung is a culture-bound anger syndrome in Korea. It manifests as one or more of a wide range of physical symptoms, in response to emotional stress. This case describes a 26-year old woman diagnosed as Hwabyung by HBDIS. She complained chiefly of binge eating and sleep disorder. She also complained of chest discomfort, abdominal discomfort, foreign body sensation in the throat, burning sensation and so on. We treated the patient with herbal medication, acupuncture, cupping, and moxa. After treatment, her condition improved. Standardized patients (SP) were used in teaching and practicing interviewing and physical examination skills for This report suggests that traditional Korean treatment such as herbal medication, acupuncture, cupping, moxa is effective for the treatment of Hwabyung patients.
