아동기 외상의 정신과적 개관 (CHILDHOOD TRAUMA:PSYCHIATRIC OVERVIEW)

  • 아동에 가해지는 정신적 외상의 충격은 아동의 심리발달에 지울 수 없는 상처를 남김으로써 향후 심각한 정신장애를 일으키는 결정적 원인이 된다. 아동기 외상의 현상학적 특징과 외상에 관여하는 신경생물학적 기전을 살펴보았고, 아동기에 경험하는 외상이 인격의 발달에 미치는 영향과 그와 관련한 정신병리를 알아보았다. 외상에 노출된 아동은 공통적으로 자신이 경험한 공포사건을 시각적으로 재경험하거나 그 기억을 반복하며, 놀이나 행동을 통해서도 외상사건을 반복하려는 경향을 보인다. 일회의 충격적 사건에 노출된 아동은 사건에 대한 지나친 기억과 집착을 보이고 지각왜곡이 심한 반면 장기간 반복되는 외상을 경험하는 아동은 정신적 무감각과 자기최면, 해리, 분노의 경향이 두드러진다. 소아·청소년에서는 외상 후 퇴행이 보다 현저하고 시간감각의 왜곡이 더 심하게 나타나며 아동의 연령이 어릴수록 자율신경계의 반응성이 증가하는 경향을 보인다. 외상적 충격에 노출되면 뇌내에서 즉각적이고 방대한 신경전달물질의 방출이 있게되는데, 자율신경계, 면역계, 시상하부-뇌하수체-부신피질 축(hypothalamic-pituitary-adrenal axis)이 활성화된다. 외상시 internal opiate가 활발한 작용을 하면서 아동으로 하여금 외상에 반응하지 않도록 하고 외상에 대한 감정반응 자체를 봉쇄할 수 있도록 해준다. 아동기 외상의 경우 central noradrenergic system에 변화가 초래되어 카테콜아민에 대해 과민감한 비정상적 소견을 보이게 된다. 체내 순환되는 cortisol 수치는 감소하고, glucocorticoid 수용체 농도와 반응성이 증가한다. 외상 후 발견되는 기억력 장애는 해마의 구조적 변화와 관련이 있는 것으로 보고되고 있다. 아동기 외상은 자기(self)와 대상(object)에 대한 내적 표상(internal representation)이 지속적으로 분화 발전하는 발달과정에서 일어난다는 점에서 인격의 발달에 심각한 영향을 미치게 된다. 해리, 신체화, 자학성, 자기애 장애의 정신병리로 발전하면서 경계성 인격장애, 자기애성 인격장애, 다중인격장애 등을 초래하게 된다.

좁은잎 엉겅퀴 추출물의 산화방지 활성 및 산화적 스트레스에 대한 PC12 세포 보호효과 (Cirsium japonicum Extracts Show Antioxidant Activity and PC12 Cell Protection against Oxidative Stress)

  • HPLC/MS를 이용하여 국내 자생 엉겅퀴(Cirsium japonicum)의 페놀성 화합물을 분석한 결과 제주지역 엉겅퀴가 가장 많은 페놀성 화합물을 함유하고 있었으며, chlorogenic acid가 73.15 mg/g dry weight, linarin이 76.67 mg/g dry weight 그리고 pectolinarin이 12.98 mg/g dry weight으로 확인되었다. 엉겅퀴의 기능성 식품으로서의 가치를 확인하기 위하여 산화방지 활성 및 신경세포 보호효과를 평가한 결과 DPPH, ABTS 및 FRAP assay에서 엉겅퀴의 강력한 산화방지 활성이 나타났다. 엉겅퀴 추출물이 DPPH 및 ABTS 라디칼의 50%를 저해하는 농도는 각각 $567{\mu}g/mL$$130{\mu}g/mL$으로 나타났다. DPPH, ABTS, FRAP법을 통한 총산화방지능은 각각 11.32, 100.15, $12.76{\mu}g/mL$ trolox equivalents 나타났다. 과산화수소로 산화적 손상을 유도한 PC12 세포에 대하여 MTT와 LDH assay를 이용하여 세포생존률을 측정한 결과 농도 의존적으로 세포 보호 활성이 나타났으며, 마찬가지로 활성산소종 생성률을 측정한 결과 농도 의존적으로 활성산소종 생성이 감소되어 세포 보호활성이 확인되었다. 본 연구를 통하여 엉겅퀴의 우수한 산화방지 활성 및 신경세포 보호효과가 검증되었다. 따라서 엉겅퀴는 안전한 식품 재료로서 꾸준히 섭취하였을 때 천연 산화방지제로 작용하여 신경퇴행을 예방함으로써 알츠하이머병, 파킨슨병 및 헌팅턴병 등의 질병 위험을 줄일 수 있을 것으로 판단된다.

Hydrogen peroxide attenuates refilling of intracellular calcium store in mouse pancreatic acinar cells

  • Intracellular calcium ($Ca^{2+}$) oscillation is an initial event in digestive enzyme secretion of pancreatic acinar cells. Reactive oxygen species are known to be associated with a variety of oxidative stress-induced cellular disorders including pancreatitis. In this study, we investigated the effect of hydrogen peroxide ($H_2O_2$) on intracellular $Ca^{2+}$ accumulation in mouse pancreatic acinar cells. Perfusion of $H_2O_2$ at $300{\mu}M$ resulted in additional elevation of intracellular $Ca^{2+}$ levels and termination of oscillatory $Ca^{2+}$ signals induced by carbamylcholine (CCh) in the presence of normal extracellular $Ca^{2+}$. Antioxidants, catalase or DTT, completely prevented $H_2O_2$-induced additional $Ca^{2+}$ increase and termination of $Ca^{2+}$ oscillation. In $Ca^{2+}$-free medium, $H_2O_2$ still enhanced CCh-induced intracellular $Ca^{2+}$ levels and thapsigargin (TG) mimicked $H_2O_2$-induced cytosolic $Ca^{2+}$ increase. Furthermore, $H_2O_2$-induced elevation of intracellular $Ca^{2+}$ levels was abolished under sarco/endoplasmic reticulum $Ca^{2+}$ ATPase-inactivated condition by TG pretreatment with CCh. $H_2O_2$ at $300{\mu}M$ failed to affect store-operated $Ca^{2+}$ entry or $Ca^{2+}$ extrusion through plasma membrane. Additionally, ruthenium red, a mitochondrial $Ca^{2+}$ uniporter blocker, failed to attenuate $H_2O_2$-induced intracellular $Ca^{2+}$ elevation. These results provide evidence that excessive generation of $H_2O_2$ in pathological conditions could accumulate intracellular $Ca^{2+}$ by attenuating refilling of internal $Ca^{2+}$ stores rather than by inhibiting $Ca^{2+}$ extrusion to extracellular fluid or enhancing $Ca^{2+}$ mobilization from extracellular medium in mouse pancreatic acinar cells.

전방머리자세와 중립자세에 대한 인위적 자세변화가 호흡에 미치는 영향 (Effect of the Changes in Forward Head Posture and Neutral Head Posture on Respiratory)

  • Purpose : Forward head posture is typical neck disorders occur in all people. And this attitude causes a shortening and weakening of the muscles in the body. It also causes excessive extension acts as a reward. This attitude has to change if the pain occurs around the neck and shoulders, and are subjected to unusual stress. Patients with chronic neck pain associated with forward head posture was found to be the more severe the fall of the respiratory, forward head posture poor quality of much breath. The purpose of this study was to compare the effect of changes in forward head posture and neutral head posture on respiratory. Method : Forty volunteers were participated in study and divided into two groups [forward head posture group (n=20) and neutral head posture group (n=20)]. We measured cervical alignment with global postural system to find out a forward head posture. Respiratory function was measured with a SPIROVIT SP-1 equipment and we found out a forced vital capacity (FVC), forced expiratory volume at one second (FEV1) and FEV1/FVC. A pared t-test was used to determine a statistical significance for the pulmonary function variation and a independent t-test was used to determine a statistical significance in the two groups. Results : In the experimental group, FVC and FEV1 were significantly higher in the artificial neutral head posture than in the forward head posture. In the control group, FVC and FEV1 were significantly higher in the neutral head posture than in the forward head posture. FVC, FEV1, and FEV1/ FVC were significantly higher in the neutral head posture of control group than the artificial neutral head posture of experimental group and higher in the artificial forward head posture of control group than the forward head posture of experimental group. Conclusion : In conclusion, neutral head posture is considered to be an important factor in correct posture and improvement of lung function and continuous study of posture correction program for posture imbalance will be needed.

The Indian Magical Herb 'Sanjeevni' (Selaginella bryopteris L.) - A Promising Anti-inflammatory Phytomedicine for the Treatment of Patients with Inflammatory Skin Diseases

  • Objectives: Selaginella bryopteris L. (family: Selaginaceae), is often used in traditional Indian systems of medicine for the prevention and cure of several disorders and for the treatment of patient with spermatorrhoea, venereal disease, constipation, colitis, urinary tract infections, fever, epilepsy, leucorrhoea, beri-beri and cancer. It is also used as a strength tonic. This study aimed to evaluate the mechanisms underlying the anti-inflammatory effects of topically administered aqueous, polar and non-polar methanolic fractions ($10mg/20{\mu}L$) of Selaginella bryopteris. Methods: An acute oral toxicity study of Selaginella bryopteris at doses from 250 to 2,000 mg/kg body weight (bw) was performed. Aqueous, polar and non-polar methanolic extracts ($10mg/20{\mu}L$) applied topically for 5 days were evaluated for their anti-inflammatory effects against 12-tetra-O-decanoyl phorbol acetate (TPA)- induced inflammation by using the redness in the ear, the ear's weight (edema), oxidative stress parameters, such as lipid-peroxide (LPO) and nitric oxide (NO), and the pro-inflammatory cytokines involved in inflammation, such as tumour necrosis factor $(TNF)-{\alpha}$, interleukin $(IL)-1{\beta}$ and IL-6. Indomethacine ($0.5mg/20{\mu}L$) was used for the positive control. Results: Selaginella bryopteris produced no mortalities when administered orally at doses from 250 to 2,000 mg/kg bw. Topical treatment with the non-polar methanolic fraction ($10mg/20{\mu}L$) significantly suppressed redness ($2.4{\pm}0.5$) and edema ($30.4{\pm}1$) and effectively reduced the LPO level ($32.3{\pm}3.3$). The NO level was ($8.07{\pm}0.55$), and the $TNF-{\alpha}$, $IL-1{\beta}$, and IL-6 levels were decreased to $69.6{\pm}15.5$, $7.7{\pm}4.8$ and $82.6{\pm}5.9$, respectively. Conclusion: This study demonstrated for the first time the mechanisms underlying the anti-inflammatory effect of medicinal plants like Selaginella bryopteris and quantified the pharmacological interactions between them. The present study showed this herbal product to be a promising anti-inflammatory phytomedicine for the treatment of patients with inflammatory skin diseases.

허혈/재관류 세포 손상에서 청폐사간탕의 보호 효과 (Protective effects of Chungpesagan-tang against ischemia/reperfusion induced cell injury)

  • 세포열을 이용한 허혈/재관류 환경에서 청폐사간탕의 세포보호능을 측정하였다. 청폐사간탕 추출물은 허혈/재관류 환경하에서 발생하는 세포 독성으로부터 대표적 수용성 항산화제인 ascorbic acid보다 높은 세포 보호 활성을 나타내었으며, 산화적 손상의 지표로서 사용되는 지질과산화물(TBARS)를 측정한 결과에서도 ASA와 유사한 활성을 나타내었다. 또한, 허혈/재관류 환경하에서 활성이 증가하여 세포에 산화적 손상을 일으키는 활성 산소종을 유발하는 것으로 알려진 xanthine oxidase 활성 측정에서는 청폐사간탕이 ASA보도 높은 xanthine oxidase 활성 억제능을 보였으며, xanthine oxidase 효소 표품을 이용한 활성 억제능 측정에서도 ASA보다 뛰어난 결과를 보였다. 따라서, 청폐사간탕은 허혈/재관류 환경하에서 세포 보호능이 있는 것으로 추측이되며, 이러한 보호능은 항산화 활성과 더불어서 xanthine oxidase 활성 억제능이 공동 작용의 결과로 사료된다.

Protective Effect of a 43 kD Protein from the Leaves of the Herb, Cajanus indicus L on Chloroform Induced Hepatic-disorder

  • Cajanus indicus is a herb with medicinal properties and is traditionally used to treat various forms of liver disorders. Present study aimed to evaluate the effect of a 43 kD protein isolated from the leaves of this herb against chloroform induced hepatotoxicity. Male albino mice were intraperitoneally treated with 2mg/kg body weight of the protein for 5 days followed by oral application of chloroform (0.75ml/kg body weight) for 2 days. Different biochemical parameters related to physiology and pathophysiology of liver, such as, serum glutamate pyruvate transaminase and alkaline phosphatase were determined in the murine sera under various experimental conditions. Direct antioxidant role of the protein was also determined from its reaction with Diphenyl picryl hydraxyl radical, superoxide radical and hydrogen peroxide. To find out the mode of action of this protein against chloroform induced liver damage, levels of antioxidant enzymes catalase, superoxide dismutase and glutathione-S-transferase were measured from liver homogenates. Peroxidation of membrane lipids both in vivo and in vitro were also measured as malonaldialdehyde. Finally, histopathological analyses were done from liver sections of control, toxin treated and protein pre- and post-treated (along with the toxin) mice. Levels of serum glutamate pyruvate transaminase and alkaline phosphatase, which showed an elevation in chloroform induced hepatic damage, were brought down near to the normal levels with the protein pretreatment. On the contrary, the levels of anti-oxidant enzymes such as catalase, superoxide dismutase and glutathione-S-transferase that had gone down in mice orally fed with chloroform were significantly elevated in protein pretreated ones. Besides, chloroform induced lipid peroxidation was effectively reduced by protein treatment both in vivo and in vitro. In cell free system the protein effectively quenched diphenyl picryl hydrazyl radical and superoxide radical, though it could not catalyse the breakdown of hydrogen peroxide. Post treatment with the protein for 3 days after 2 days of chloroform administration showed similar results. Histopathological studies indicated that chloroform induced extensive tissue damage was less severe in the mice livers treated with the 43 kD protein prior and post to the toxin administration. Results from all these data suggest that the protein possesses both preventive and curative role against chloroform induced hepatotoxicity and probably acts by an anti-oxidative defense mechanism.

외상 후 스트레스장애 환자의 악몽치료에서 Terazosin의 효과 : 예비 연구 (Effect of Terazosin on Posttraumatic Nightmares : A Pilot Study)

  • Objective : Chronic posttraumatic stress disorder (PTSD) is defined by the coexistence of symptoms (reexperiencing, avoidance and hyperarousal), persisting for more than months or years that causes significant impairment in social occupational functioning. This study was conducted to evaluate the effects of terazosin on posttraumatic nightmares which is one of chronic PTSD symptoms. Methods : Twenty patients with chronic PTSD and severe trauma-related nightmares were treated with terazosin (3-7 mg/day) for 8 weeks. Recurrent distressing dreams item of the Clinician administered PTSD Scale (CAPS), Total CAPS score, Clinical Global Impressions-Severity Scale (CGI-S) and Clinical Global Impressions-Change Scale (CGI-C) were performed at baseline, 4-week and 8-week. Mississippi Scale for Combat-Related PTSD and Combat Exposure Scale (CES) were used to evaluate PTSD symptom pattern and degree of exposure. Results : Significant decrease in recurrent distressing dreams item, reexperiencing, avoidance and hyperarousal symptom score and total CAPS score were at 4-week and 8-week treatment compared to baseline (recurrent distressing dreams item : $2.70{\pm}1.88$ and $1.25{\pm}1.49$ ; reexperiencing, avoidance and hyperarousal symptom score : $2.30{\pm}2.49$, $1.20{\pm}1.05$, $3.10{\pm}1.68$ and $4.00{\pm}3.59$, $1.55{\pm}1.50$, $5.60{\pm}3.23$ ; total CAPS score : $8.20{\pm}6.26$ and $5.40{\pm}5.89$). There were significant correlation between dosage of terazosin and improvement of PTSD symptoms. Adverse effects such as fatigue and orthostatic hypotension were showed, which were mild and self-limited. Conclusion : These results suggest that terazosin may improve severe trauma-related nightmares and overall PTSD symptoms. Randomized controlled study with more subjects would be necessary in the future.

Nutrition Practice to Alleviate the Adverse Effects of Stress on Laying Performance, Metabolic Profile, and Egg Quality in Peak Producing Hens: I. The Humate Supplementation

  • This experiment was conducted to determine the effects of cage density (CD) and humate supplementation (HS) on laying performance, metabolic profile, and egg quality during the peak production period in hens. Lohman layers (n = 180, 46 wks of age) were blocked according to the location of cages and then allocated randomly to two levels of CD (4 or 6 hens per cage or 540 vs. 360 $cm^2$/hen) and three levels of HS (0, 0.15, and 0.30%). Egg production (EP) and feed consumption (FC) were measured daily; egg weight was measured bi-weekly; and BW was measured before and after the experiment. Blood and additional egg samples were obtained at the end of the experiment for determination of metabolic profile and egg quality. The data were analyzed using two-way ANOVA as repeated measures. Except for FC, CD did not affect laying performance parameters. Hens placed in high-density cages had lower FC than hens placed in normal-density cages. Increasing HS level linearly increased FC, EP, and feed conversion ratio (FCR). There was a CD by HS interaction effect on FC and EP. Hens placed in high-density cages had greater serum glucose, total protein, albumin, globulin, Ca, and P concentrations and tended to have greater serum corticosterone concentration than hens placed in normaldensity cages. Increasing HS level linearly increased serum glucose, total protein, albumin, globulin, creatine, and Ca concentrations and linearly decreased serum triglyceride and very low-density lipoprotein concentrations. There was a CD by HS interaction effect on serum glucose and albumin concentrations. There were no alterations in egg quality parameters in response to increasing CD. Albumen index and Haugh unit decreased linearly and other egg quality parameters did not change as HS level increased. In conclusion, increased caging density adversely affected metabolic profile, despite insignificantly deteriorating laying performance. Moreover, benefits from humate supplementation seem to be more noteworthy for hens housed in stressing conditions than for hens housed in standard conditions.

피로와 우울.불안증 환자에서 Fatigue Severity Scale의 임상적 유용성 (Clinical Usefulness of Fatigue Severity Scale for Patients with Fatigue, and Anxiety or Depression)

  • 연구목적: 피로는 일차 진료에서 흔한 증상이지만 비특이적 주관적 특성상 명확한 정의와 객관적 평가가 어렵고, 특히 기질적 질환이 없는 피로의 평가는 더욱 힘들다. 이에 저자들은 기질적 질환이 없는 피로 환자에게 Fatigue severity scale (FSS) 을 적용하여 그 유용성을 알아보고자 하였다. 방법: 피로환자 44명, 우울 또는 불안증의 정신과 환자 43명과 대조군 45명에게 피로도, 스트레스, 우울 불안 척도를 이용하여 FSS의 신뢰도와 타당도를 조사하였다. 결과: FSS의 신뢰도 계수는 0.935이었고 재검사의 상관관계 계수는 0.916이었다 (p<0.01). FSS 점수는 피로 군에서 유의하게 가장 높았고 대조군에서 가장 낮았다 (p<0.01). FSS와 Chalder fatigue scale의 상관관계 계수는 0.782로 높았다 (p<0.01). 피로군과 대조군에 대한 FSS index의 최적의 절단점은 민감도 84.1%. 특이도 85.7%인 3.22로 조사되었다. 결론: FSS는 일차진료에서 기질적 질환이 없는 피로환자와 우울 또는 불안증 환자에서 피로도를 평가할 수 있는 유용한 도구이며, 향후 보다 많은 환자를 대상으로 한 연구가 필요하리라 생각된다.
