임신 기간에 따른 임산부 보행의 운동학적 요인과 하지 관절모멘트 패턴 비료 (Comparison of Gait Patterns on Pregnant's Kinematic Factors and Lower-Limb Joint Moments During Pregnant Period)

  • The purpose of this study was to compare gait patterns during pregnancy. Because of the changes in hormone levels and anatomical changes such as body mass, body-mass distribution, joint laxity, and musculotendinous strength that result from pregnancy, it was possible that there would be certain gait deviations associated with these changes. Three-dimensional gait analyses were performed from a self-selected pace, and six subjects(height : $163{\pm}5.3cm$, mass : $61.3{\pm}3.80kg$, $65.3{\pm}5.14kg$, $70.2{\pm}4.98kg$) participated in the three times(the early, middle and last years). 7 cameras(Proreflex MCU-240, Qualisys) and 2 force plates (Type 9286AA, Kistler) were used to acquire raw data. The parameters were calculated and analyzed with Visual-3D and Joint moments computed using inverse dynamics. In conclusion, pregnant women's gait patterns were changed during pregnancy period because pregnancy makes them physical changes. The main changes were joint moments and kinematic factors during pregnancy period. The pregnancy transformed normal gait pattern Into toe out position. Therefore, exercise programs to improve muscle activity were necessary where joint moments were small. The development of simulator should be studied for pregnant women's tailored shoes and accessories in future.

한쪽 상지의 D2 패턴 운동동안 반대측 상지, 하지 및 체간 근육의 활성도에 미치는 영향 (The Effects of Contralateral Upper and Lower Limb and Trunk Muscle Activation During Ipsilateral Upper Limb D2 Pattern Exercise)

  • Purpose: The aim of this study was to examine the activation of the contralateral upper and lower extremities and trunk muscle during ipsilateral upper extremity diagonal isokinetic exercise. Methods: Twenty-one healthy male subjects with no history of shoulder injury undertook ipsilateral diagonal isokinetic exercise at 60, 120, and $180^{\circ}/sec$, utilizing a standard Biodex protocol. Muscle activation amplitudes were measured in the upper trapezius, pectoralis major, biceps brachii, rectus abdominis, external oblique, rectus femoris, adductor longus, and biceps femoris muscles using electromyography. A one-way analysis of variance and paired t-tests were conducted, and the data were analyzed using SPSS, version 21.0. Results: The results revealed no statistically significant interaction between motion and angular velocity and no statistically significant contralateral muscle activation according to angular velocity (p>0.05). However, they revealed statistically significant contralateral muscle activation according to motion (p<0.05). Conclusion: These results suggest that the movements involved in contralateral upper extremity diagonal isokinetic exercise can enhance muscle strength in patients affected by stroke, fracture, burns, or arthritis.

비타민D 보충 요법이 폐경 후 여성 근력에 미치는 효과: 메타분석 (Effect of Vitamin D on Muscular Strength in Postmenopausal Women: A Meta-Analysis)

  • Purpose: Postmenopausal women are a unique population with high risk for the degeneration of muscle. The aim of the present meta-analysis was to accurately evaluate the effects of vitamin D on muscular strength in postmenopausal women. Methods: A review was conducted using electronic databases, including PubMed, EMBASE, Ovid Medline, CINAHL complete, and the Cochrane Library from inception through 19 March 2019. Included studies were selected by two independent reviewers. The meta-analysis were performed using Review Manager 5.3 software. Results: A total of nine randomized controlled clinical trials were included in this review. Vitamin D interventions led to no changes in the upper limb muscle strength (mean difference -0.16, 95% CI: -1.09 to 0.77), lower body muscle strength (standard mean difference 0.08, 95% CI: -0.11 to 0.26), and back/hip muscle strength (standard mean difference 0.06, 95% CI: -0.05 to 0.17). Conclusion: Pooled results from eight studies indicated that supplementation of vitamin D did not increase muscle strength in postmenopausal women. Apparently, the present review suggests that supplementation of vitamin D alone had no didn't show any beneficial effects on muscle strength in postmenopausal women.

선천성 심장질환아의 수술후 활동제한 기간에 따른 사지근 위축 변화에 관한 연구 (A study on the change of limb muscle atrophy by the period of limited activities following the heart surgery in congenital heart disease children)

  • In order to investigate the effects of a decreased activity on skinfold thickness, circumference and muscle strength of the extremities during the recovery period following heart surgery, skinfold thickness, circumference and muscle strength of the extremities were measured on days 0, 3, 6, and 9 following the surgery, and compared with those on the arrival day of intensive care unit. Skinfold thickness was measured using a skinfold caliper(Saehan Cor., Korea), circumference of the limbs were measured with a tape measure, upper extremity strength was determined using the Takei grip dynamometer and lower extremity strength was measured by pressing the flatfoot on an electronic digital health meter while tying on a bed. Results from this study were thus : 1. Skinfold thickness of triceps, quadriceps and gastrocnemius muscle on days 3, 6, 9 following the heart surgery was not significantly different from that of on the day of operation. 2. Circumference of midupperarm and midthigh on days 3, 6, 9 following the heart surgery was not significantly different from that of on the day of operation. Circumference of midcalf on days 3, 6 following the heart surgery was not significantly different from that of on the day of operation, while that of midcalf on day 9 following the surgery decreased significantly compared with that of on the day of operation. 3. Muscle strength of the upper extremity was not significantly different from that of on the day of operation, while that of the lower extremity on day 9 following the surgery decreased significantly compared with that of on the day of operation. From these results, it may be concluded that circumference and muscle strength of lower extremity can be decreased due to the postoperative inactivity following heart surgery in congenital heart disease children.

불안정판을 부착한 경사침대에서 하지운동 시 정상성인의 근력 특성 분석 (Characteristic Analysis of Lower Limbs Muscles in Young Normal Adults on a Tilting Bed Using an Unstable Platform)

  • We investigated an early rehabilitation training system that increase the intensity of patient rehabilitation training to shorten the time it takes for patients to progress to a secondary rehabilitation training stage by allowing patients incapable of self-ambulation. It consisted of tilting bed, unstable platform using strong springs and training program for lower limb rehabilitation. We performed experimental study on the muscular activities of tibialis anterior(TA), soleus(SO), gastrocnemius(GA) in the lower extremities during training of straight line, circle, quardrangle pattern during tilting angle of $30^{\circ}$, $60^{\circ}$. The muscle activities were higher during tilting angle of $30^{\circ}$ than $60^{\circ}$. In straight line pattern, the muscle activities were higher by SO, GA and TA during medio-lateral direction, however, by TA, SO and GA during anterio-posterior direction. In circle and quardrangle pattern, the muscle activities were higher by TA, SO and GA during clockwise and counterclockwise direction. The results indicate that the early rehabilitation training system could be applied to improve the lower extremity muscular strength for elderly and patients, especially, stroke.

PNF을 이용한 엉덩관절 강화운동이 피부 결손을 동반한 개방성 정강뼈 골절 환자의 균형과 앉았다 일어서기, 보행에 미치는 영향 - 단일사례연구 - (The Effect of a Hip Joint Strengthening Exercise using PNF on Balance, Sit-to-Stand Movement, and Gait in a Tibia Fracture Patient with Skin Defects - A Single Case Study -)

  • Purpose: Patients with tibial fractures can have functional problems with balance and gait, as well as lower extremity muscle weakness. This case report aimed to describe the effect of hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on balance and gait and lower limb function in a patient with tibia fracture. Methods: One patient diagnosed with tibial fracture was treated for seven weeks with the basic procedure, pattern, and technique of PNF for a hip joint strengthening exercise. Results: The results of pre- and post-intervention treatment showed improvements in physical function and structure in the clinical tests, including the manual strength test; the modified Ashworth scale; sensory evaluation; balance, sit-to-stand, and gait performance; and evaluation of lower limb function. Conclusion: Based on the results of this study, it is suggested that the use of theory-based proprioceptive neuromuscular stimulation for hip joint strengthening exercises positively affects patients' functional improvement in tibial fracture patients, and this may be used as a therapeutic exercise method for those with orthopedic problems in the lower extremities. One limitation of this study was that it was performed on only one tibia fracture patient, which makes it difficult to extend the treatment effects to all patients with this condition.

Immediate Effect of Postural Control of the Contra-Lateral Side on Exercise-Induced Fatigue of the Ipsi-Lateral Plantar Flexor Muscle

  • Purpose: The purpose of the current study was to examine the effects of exercise-induced fatigue of the plantar flexor muscle in the dominant ankle on the plantar flexor strength and postural control function of the contra-lateral side. Methods: Twenty-one young adults (male: 10, female: 11) volunteered to participate in this study. An exercise-induced fatigue protocol to induce fatigue was performed in the plantar flexor of the dominant ankle. For the fatigue protocol, the participants were instructed to raise their heels as high as possible in the position with one leg stance of the dominant lower limb, and the heel was then downed after holding for 1 second. The muscle strength of the contra-lateral plantar flexor was measured using a digital muscle strength test device, and the static and dynamic postural control were tested by acquiring the center of gravity velocity while performing one leg standing. A paired t-test was used to identify the differences between the pre- and post, and the data were analyzed using SPSS 12.0 software. Results: Comparison of the pre- and post-test data revealed a significant difference in the plantar flexor strength and dynamic postural control after exercise-induced muscle fatigue in the dominant side. On the other hand, there was no significant difference in the static postural control. Conclusion: These findings have practical implications, suggesting that unilateral muscle fatigue affects the ankle muscle strength and postural ability of the contralateral side.

K3 리그 축구선수의 시즌별 운동기술체력에 관한 비교 연구 (Seasonal Variation in Physiological Fitness of a Semi-Professional K3 League Soccer Team)

  • 이 연구는 K3 리그 축구선수들의 시즌 중 체력 상태를 점검하기 위한 목적으로 종목 특이적인 체력측정을 시즌 초반과 후반에 실시하였다. 평가항목은 신체구성, 순발력, 민첩성, 지구력 및 등속성 하지근력 등으로 구성하였다. 측정 결과 시즌 초반과 비교하여 시즌 후반에 향상된 기능을 보여준 항목은 골격근량, 순발력, 민첩성 등이며, 신체조성, 지구력 및 등속성 하지근력은 차이점이 발견되지 못하였다. 시즌 후반부에 향상도를 보여준 항목은 시즌 중 반복적인 훈련과 시합 참여에 의한 학습효과 및 다양한 생리학적 기전에 의한 것으로 생각된다. 마지막으로 지구력과 하지근력은 별도의 특이적인 훈련을 시즌 중에 실시하지 못한 이유로 인하여 시즌 후반에 향상도를 보여주지 못한 것으로 판단된다. 이상의 결과를 종합한 결과, 선수들의 체력상태를 주기적으로 점검하는 연구를 통하여 시합기, 전이기 및 준비기에 적합한 체력 훈련프로그램의 개발 및 적용이 필요한 것으로 생각된다.

우세측에 따른 상지와 하지의 기능 비교 (Comparison of Upper and Lower Limb Functional Ability Depends on Dominants)

  • Purpose. The purpose of this study was compared of left dominant and the right dominant hands about coordination. Methods. The participants were 30 occupational therapy students in M university in Changwon, from April 28th 2015 to May 1. The test tools were Jebson-Taylor Hand Function Test, Hand Strength & Pinch, Grooved Pegboard Test, Box & Block and Balance Test. Results. Right dominant hand was higher than left dominant in hand grip and dexterity, hand manipulation skills. And one leg standing and toe standing were higher left foot than right foot dominant. Conclusion. Most of all evaluation items showed a better result right dominant hand than left dominant. But shows that there is no functional difference between left-hand dominant than right-hand. So left-hand dominant coordination was evaluated by a better than right-handed.

런지 동작 시 발 위치의 앞뒤 간격에 따른 하지 근위부 근육의 근 활성도 비교 (Comparison of Muscle Activity in Proximal Muscle of Lower Extremities during Lunge according to the Anterior-posterior Distance of Foot Position)

  • PURPOSE: This study was conducted to compare the muscle activity of the proximal muscles of the lower limb according to the distance between the front and rear foot during lunge and to determine the most effective foot position for activation of the proximal muscle in the limb. METHODS: A total of 49 young adults were enrolled in this study. All subjects performed lunge by positioning the big toe of the back foot and the heel of the front foot at intervals of 40%, 60%, and 80% of the subject's own leg length. Muscle activity of the vastus medialis oblique, rectus femoris (RF), vastus lateralis oblique (VLO), gluteus medius, biceps femoris, and semitendinosus (ST) was then measured during three intervals of lunge operation. Each operation was measured three times for 10 seconds each, after which the average value was calculated and analyzed. RESULTS: There were significant differences in muscle activities of RF, VLO, and ST among the three intervals of the foot (p<.05). Post hoc, comparisons revealed lunge at 40% intervals resulted in higher RF and VLO activity than at 60% and 80% intervals (p<.05). In the semitendinosus muscle, 80% leg length intervals showed higher muscle activity than 40% (p<.05). CONCLUSION: Strengthening of the proximal muscles of the lower extremities during lunge exercise is considered to be most effective when placing the fore- and rear foot at intervals corresponding to 40% of the leg length.
