Study on Diagnostic Value of Straight Leg Raising Test between Traffic Accident Patients Group and Non-Traffic Accident Patients Group in Diagnosing Herniated Lumbar Intervertebral Disc (요추간판탈출증 진단에 있어 교통사고 환자와 비교통사고 환자의 하지직거상 검사상 진단적 가치 비교 연구)

  • Objectives : Straight leg raising test(SLR) is useful clinical test to diagnose herniated lumbar intervertebral disc. The purpose of this study is to analyse diagnostic value of straight leg raising test between traffic accident patients group and non-traffic accident patients group. Methods : The study involved 26 traffic accident patients group and 45 non-traffic accident patients group with low back pain or radiating pain into the leg. All had a result of straight leg raising test and lumbar computed tomography(CT) or magnetic resonance imaging(MRI). The associations between straight leg raising test and herniated lumbar intervertebral disc were analysed. Results and Conclusions : 1. There is not significant associations between straight leg raising test and herniated lumbar intervertebral disc in traffic accident patients group. 2. There is significant associations between straight leg raising test and herniated lumbar intervertebral disc in non-traffic accident patients group. 3. In traffic accident patients group, and negative predictive value are higher than non-traffic accident patients group. 4. In non-traffic accident patients group, sensitivity, positive predictive value and odds ratios are higher than traffic accident patients-group.

Effect of 1Hz Motor Nerve Electrical Stimulation on Joint Range of Motion

  • Objective: This study aims to compare the range of motion of the joints by applying the contraction and relaxation techniques used in manual therapy as electrical stimulation treatment. Based on this, we would like to propose the possibility of using motor nerve electrical stimulation therapy for musculoskeletal physical therapy. Design: Single-arm interventional study Methods: Active and passive straight leg raising tests were performed on 20 healthy men and women in their 20s to measure the angle of hip joint flexion. Then, the electrical stimulation time was set to 10 seconds and 5 seconds of rest, and motor nerve electrical stimulation of 1 Hz was applied with the maximum strength that could withstand the hamstring muscles for 10 minutes. After electrical stimulation, straight leg raising tests again to confirm the range of motion of the hip joint flexion. Results: As a result of this study, it was confirmed that the joint range of motion was significantly improved for both active and passive straight leg raising tests after application of motor nerve electrical stimulation(p<.05). Conclusions: With a strong electrical stimulation treatment of 1 Hz, the effect similar to the contraction and relaxation technique used in manual therapy was confirmed through the joint range of motion. In the future, motor nerve electrical stimulation therapy can be used for musculoskeletal physical therapy to provide a new approach for patients with reduced pain and joint range of motion due to muscle tension.

Comparison of Six Tests for Assessing Hamstring Muscle Length (슬괵근 유연성 평가에 관한 연구)

  • Background and Purpose. Objective measurements of hamstring muscle length are needed to quantify baseline limitations and to document the effectiveness of therapeutic interventions. Several indirect clinical tests for measuring hamstring muscle length are available, but influence of their test procedure is not well documented. The purpose of this study were 1) to describe hamstring muscle length as reflected by use of six tests(active straight leg raising(ASLR), passive straight leg raising(PSLR), passive straight leg raising with the lower back flat(PSLRB), active knee extension(AKE), passive knee extension(PKE), hip joint angle(HJA). 2) to examine the correlation among the tests. Subjects, Sixty subjects(30 men. 30 women) ranging in age from 18 to 25 years(mean 20.2 years) and with no limitation hamstring flexibility and no neurological and orthopedical problems. Methods. All subjects performed six tests. A inclinometer was used to determine the end point of range of motion. HJA was measured using an inclinometer placed over the sacrum. PSLRB were tested PSLR with the low back flat and the opposite thigh slightly flexed and support on pillows. Results, A mean ASLR value of 85.9 degrees, PSLR value of 99.9 degrees, PSLRB value of 109.8 degrees, AKE value of 77.2 degrees PKE value of 83.1 degrees and HJA value of 73.0 degrees were obtained for all subjects. A dependent t-test showed significant difference between the angles of ASLR and PSLR(p<0.001). There was a significant difference between the angles of PSLR and PSLRB(p<0.001). There was a significant difference between the angles of AKE and PKE(p<0.001). The highest correlation was between PSLR and PSLRB(r=0.915, p<0.001). All SLR tests were significants related(p<0.001), as well as AKE and PKE(p<0.001). The lowest correlation was between PKE and HJA(r=0.171. p>0.05). Conclusion and Discussion. The results indicated that the hip flexion angles for ASLR, PSLR and PSLRB were a difference, and the knee extension angles for AKE and PKE were a difference.

A Study of the Relationship between Straight Leg Raising, Valsalva Test and Size, Position of Lumbar Disc Herniation (요추부 추간판 탈출 정도와 SLR, valsalva test의 관계)

  • Objectives : This study investigated the relationship between straight leg raising(SLR), valsalva test and size, position of lumbar disc herniation. Methods : We took SLR and valsalva test on 105 patients with lumbar disc herniation. According to the result of MRI findings, this study classified three groups of 105 patients with lumbar disc herniation, bulging, protrusion and extrusion. According to the position of lumbar disc herniation, 72 patients that were diagnosed protrusion and extrsuion were sorted 4 groups, lateral, lateral postero-lateral, central postero-lateral, central. The association size, position of lumbar disc herniation and SLR, valsalva test were analysed. Results : The bigger size of disc herniation, the more positive result of SLR and valsalva test, the lower angle of SLR test. There was not significant association between the position of lumbar disc herniation and the angle of SLR test. Conclusions : The SLR and valsalva test is an useful physical examination to speculate about the degree of lumbar disc herniation.

  • Purpose: This research was conducted to compare the effects of a muscle strengthening exercise program on pain, straight leg raising ability, passive range of motion, independent walking time and satisfaction with total knee arthroplasty(TKA). Method: A nonequivalent control group posttest only design was used for this study. A total of 62 patients were randomly divided into the experimental group(31) and the control group (31). The experimental group received a 1-hour exercise intervention, while the control group received the usual care. Data was analyzed by t-test using the SPSS/WIN 11.0. Results: The experimental group showed significant improvement in pain score, straight leg raising ability, passive range of motion of knee joint, independent walking time, and the patient's satisfaction. Conclusion: The muscle strengthening exercise program was effective on decreasing pain and increasing mobility and satisfaction in patients with TKA.

The Correlation of the Pressure of Biofeedback Unit and Lumbopelvic Motion During Straight Leg Raising in Healthy Subjects (정상인에서 수동적인 뻗은다리올림 시 생체되먹임 장치의 압력과 허리골반 움직임의 상관관계)

  • Background: Passive straight leg raising (PSLR) is the common clinical test to measure of hamstring muscle length. Hip flexion angle contributes to change the lumbopelvic rotation during PSLR. Pressure biofeedback unit (PBU) is commonly used to detect lumbopelvic movement during lower limb movements. Thus, there may be the relationship between pressure of PBU and lumbopelvic motion during PSLR. Objects: The objective of this study was to determine the relationship between pressure of PBU and lumbopelvic motion during PSLR. Methods: Thirty two subjects participated in this study. A three-dimensional motion analysis system were used to measure the lumbopelvic angle during PSLR, while recording the pressure of PBU according to angle of PSLR by 10 degree increments. Pearson product moment correlations and linear regression analysis were used to describe the relationship between variables. Results: The results showed that there was a significant relationship between the lumbopelvic and angle of PSLR (Pearson's r=.83, p<.05), between the pressure of PBU and angle of PSLR (Pearson's r=.75, p<.05), and between lumbopelvic motion and pressure of PUB (Pearson's r=.83, p<.05). Linear regression equation using lumbopevic angle as an independent factor was as follows: Pressure of PBU = 47.35 + (2.55 ${\times}$ angle of lumbopelvic motion) ($R^2=.69$, p<.05). Conclusion: Results of the present study indicate that pressure of PBU can be used to indirectly detect the amounts of lumbobevic motion during muscle length test or stretching of hamstring.

The Clinical Study on the Pain Improvement of Lumbago Patients with Electro-acupuncture Therapy using Visual Analogue Scale (Visual Analogue Scale을 사용하여 분석한 전침시행 요통환자의 통증 호전에 대한 임상 연구)

  • Objective : To evaluate the pain improvement of the lumbago patients taken the electro-acupuncture therapy using Visual Analogue Scale. Methods : The 25 patients of 46 lumbago patients visited department of Acupuncture & Moxibustion in National Medical Center from 1st March to 20th August had taken the electro-acupucture therapy more than 5 times. We checked on their age, sex, onset, radiating pain, back pain past history and Straight Leg Raising test. And we evaluated their improvement of back pain using Visual Analog Scale(VAS) at each visit. Then we analyzed the pain improvement by their age, sex, onset, radiating pain, back pain past history, Straight Leg Raising test, visiting frequency and Visual Analog Scale on 1st visit(VAS1) Results : VAS of 24 patients has improved and VAS of one patient has not been changed. There's no patient getting worse. The women and the positive group at SLR Test had better result of pain improvement. Conclusion : The lumbago patients taken electro-acupuncture more than 5 times showed significantly different pattern of pain improvement according to the sex and the SLR test.

The Effect of Korean Medicine with Needle-embedding Therapy on a Lumbar Herniated Intervertebral Disc : A Case Report (요추 추간판 탈출증 환자에 대한 매선을 포함한 한의 복합치료의 효과 : 증례보고)

  • Objectives : The purpose of this study was to report the clinical effects of needle-embedding acupuncture therapy combined with Korean medicine on a lumbar herniated intervertebral disc. Methods : We treated one patient with a lumbar herniated intervertebral disc with needle-embedding acupuncture therapy combined with Korean medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and straight leg raising test(SLRT). Results : In this case, the numeric rating scale decreased, while the straight leg raising test results improved. Conclusion : Needle-embedding acupuncture therapy could be effective for pain-reduction for a lumbar herniated intervertebral disc.

A Study on the Change in the Results of the Straight Leg Raising Test according to the Intensity of Microwave Diathermy

  • Purpose: Although microwave diathermy is widely employed, research conducted in South Korea indicates a lack of theoretical understanding concerning the appropriate application intensity. Consequently, there is a need to enhance the objectivity of microwave diathermy by conducting quantitative studies to determine its effective application intensity and provide suitable application parameters. Methods: The study was performed on 60 participants, divided equally into 40W or 80W microwave diathermy intervention groups. Microwave diathermy was applied at 2,450MHz to iliocostalis and longissimus muscles for 15 minutes in each group. Ranges of motion were compared before and after intervention using straight leg raise (SLR) test results. The analysis was performed using the paired t-test and the independent t-test. Results: SLR angles increased significantly in both groups after intervention, but the 80W group exhibited a greater post-intervention angle increase. While no significant intergroup difference was observed between pre-intervention angles, post-intervention angles were significantly different. Conclusion: The study confirms that microwave diathermy treatment at 40W for 15 minutes effectively increases SLR angles but that treatment at 80W has a greater effect. Further research is warranted to establish quantitatively the optimal time and intensity settings for microwave diathermy.

Clinical Reports on Correlation between the Different Herniated Type and Oriental Medical Treatment (요추(腰椎) 추간판(椎間板)의 탈출형태(脫出形態)와 한방치료(韓方治療)의 상관성(相關性)에 관한 임상보고(臨床報告))

  • Herniated lumbar intervertebral disc(H.I.V.D) is the most common reason causing low back pain and leg radiating pain. Objective : The purpose of this report is to observe the effects of oriental medical conservative treatment in the different herniated type. Methods : We investigated 30 patients suffering from low back pain with sciatica which were admitted to Taejon Cheonan O. M. hospital from Nov 1, 2000 to Apr 30, 2001. 30 patients had a diagnosis of herniated lumbar interver tebral disc by Lumbar-C.T and Lumbar-M.R.I. we treated 30 patients by oriental medical conservative treatment (Ex: acupuncture, herb-med, physical theraphy, bed-rest, etc.) Results : 1. The mean hospitalization of patients was 21.1 days. that in the bulging type was l4days, protruded type 21days, mixed type 27days, extruded type 30days. that shows the less herniation of lumbar intervertebral disc, the mean hospitalization was more short. 2. In the distribution of the clinical symptoms admitted at that time, low back pain, leg radiating pain were showed in the all types(29patients, 96.6%). and sensory disorder, muscle powerlessness were showed least in the bulg ing type(0%, 37.5%), but that were showed most in the extruded type(50%,75%). 3. In the result of treatment due to clinical syptoms, bulging type was more execellent than any other other types. 4. The less herniation of lumbar intervertebral disc, angle of straight leg raising test was higher. In the result of treatment due to angle of straight leg raising test, bulging type was more execellent than any other other types. 5. In the distribution of physical. test, positive case in the Peyton sign, Ankle Dorsiflexion were showed least in the bulging type(25%, 12.5%), but that were showed most in the extruded type(100%, 75%). 6. In the result of treatment due to physical.test, bulging type was more exec ellent than any other other types. 7. The efficacy of total treatment was 90%(when we set a standard things more than fair), that in the bulging type was 100%, protruded 92%, mixedtype 80%, extruded type 75%. Bulging type had a more remakable effectsthan any other types. Conclusions : The less herniation of lumbar intervertebral disc, clinical syptoms were slighter, physical. test were better, and efficacy of oriental medical conservative treatme nt was remakable high.
