  • The radiosurgery treatment is one time, non surgical approach to the treatment of patients with intracranial disorders whose conditions would be difficult or dangerous to treat with conventional sugical procedures. The LINAC based radiosurgery is based on the combination of multiple isocentric arc irradiation with small fields centered in the stereotactic target. The absorption of the beam in a tissue equivalent medium, such as water, as well as the uniformity, or profile, of the beam must be precisely documented. The beam characteristics and dosimetric measurememts of the 6MV X-ray beam from a ML-6M linear accelerator are examined. The percent depth dose (PDD) and beam profile (including flatness, symmetry and penumbra) is calibrated with the radiosurgery cone in water phantom. The cone is made of lead which size is from $10{\times}10mm{\phi}$ to $30{\times}30mm{\phi}$ All of these dosimetric measurements sufficiently characterized the beam to permit safe clinical use.

  • Purpose : In the radiosurgery, to obtain CT image to find more accurate tumor position during respiration, and using them, to increase the accuracy of radiation treatment by applying image guided. Materials and Methods : Using the self-made vacuum cushion for the body SRS, CT images were obtained three for each patient during respiration (shallow, inhalation, exhalation). They were transformed to the RTP computer and then were fused. Global GTVs were delineated on the fused images and more appropriated treatment planning was established. Results : We can find the tumor position is moving toward cranio-caudal with max 10 mm margin and volume is transformed. As a result from the comparision of DVH (pre & post radio surgery), we observed about 100% dose to tumor. Conclusion : BSRS was skeptical due to the tumor movement during respiration. More accurate by the combination of the development of immobilization devices and BSRS based on Image Guide, it will be applied to more cases for BSRS.

Comparison of Linac-based VMAT Stereotatic Radiosurgery and Conventional Stereotatic Radiosurgery for Multiple Brain Lesions (Linac 기반 VMAT 정위적 수술 뇌 병변 연구와 기존의 정위적 방사선 수술 비교)

  • Portal Dosimetry was verified using EPID to secure the clinical application and reliability of the existing research dose evaluation. The dose distribution of Geant4 was compared with the measured value by 360° rotational irradiation with a 2.5 cm cone for stereotactic brain surgery. To confirm the dose distribution of patients with brain metastasis, the dose distribution investigated by inserting a Gafchromic EBT film into the parietal phantom and the dose distribution obtained from the parietal phantom using VMAT are compared and applied to actual patients. As a result of the analysis, it was confirmed that the accuracy of the beam center and the center of the couch coincide accurately with an error within 1mm as a result of QA through a pin ball. In addition, it was confirmed that the EBT3 film has excellent linearity in the range of 0 to 10 Gy according to various dose irradiation. In the same setting as the two cervical phantoms, we confirm that the implementation and simulation results calculations of dose calculations based on Geant4 using photon beams match the experimental data within the treatment planning volume (PTV). Therefore, volume modulated arc treatment (VMAT) 360° rotational irradiation was performed, and the result of iso-dose distribution analysis by rotational irradiation confirmed that it is appropriate to include a virtual tumor.

  • We have desingned multi channel dosimetry system with Intel single-chip microprocessor. We considered that this system is very useful for patient dose measurement, measurement of sealed source dose distribution and calibration of small field for stereotatic radiosurgery system We have designed that this system use commercially available semicondutor detector and personal computer can control this system and process data through RS-232C serial port.

Verification of Stereotactic Target Point Achieved by Acquisition of MR Image in Actual Treatment Position of Radiosurgery (정위적 방사선 수술시 치료위치에서의 정위적 표적점 확인을 통한 자기공명영상 획득의 정확도 연구)

  • To make practical application of the MR image for stereotactic radiosurgery, the target point-achieved by acquisition of MR image in stereotactic radiosurgery planning system must agree with the actual isocenter of irradiation in real treatment. And the amount of distortion of the MR image must be known to make a correction for the agreement. A radish containing abundant water content was chosen as a homogeneous phantom for the purpose of verification of the agreement in this experiment. A dosimetric film was firmly attached to the small specially fabricated acryl plate and needle puncture was made through the film just into the acryl plate and a drop of oil was dropped into the hole of the film. The acryl plate with film was inserted into the radish and the dorp of oil represented the target point in MR image. After the image acquisition by stereotatic radiosurgery planning system, we achieved stereotactic coordinate of the target point represented by the oil drop. And we proceeded to actual irradiation to the target point according to the procedure of stereotactic radiosurgery. After the irradiation, the film in the radish was developed and processed and the degree of coincidence between the center of the radiation distribution and the target point represented by the hole in the film was measured. The discrepancy between two points was under 0.5 mm. so we could confirm good coincidence in homogeneous phantom such as radish. On the other hand, authors tried to use our home-made device for estimation of distortion of MR image.

Postoperative radiotherapy for ependymoma

  • Purpose: To evaluated the patterns of failure, survival rate, treatment-related toxicity and prognostic factors in postoperative radiotherapy of patients with ependymoma. Materials and Methods: Thirty patients who underwent surgery and postoperative radiotherapy for ependymoma between the period of June 1994 and June 2008 were reviewed retrospectively. The age of patients ranged from 21 months to 66 years (median, 19 years). Seventeen patients had grade II ependymoma, and 13 had grade III anaplastic ependymoma according to the World Health Organization grading system. The postoperative irradiation was performed with 4 or 6 MV photon beam with median dose of 52.8 Gy (range, 45 to 63 Gy), and radiation field including 2 cm beyond the preoperative tumor volume. Median follow-up period was 51 months (range, 12 to 172 months). Results: Fourteen out of 30 (46.7%) patients experienced recurrence, and 12 of those died. Among those 14 patients who experienced recurrence, 11 were in-field and 3 were out-of-field recurrence. The 5-year overall survival (OS) and progression-free survival (PFS) rates were 66.7% and 56.1%, respectively. On univariate analysis, tumor grade was a statistically significant prognostic factor for OS and PFS. There were two complications after surgery and postoperative radiotherapy, including short stature and facial palsy on the left side. Conclusion: We observed good survival rates, and histologic grade was a prognostic factor affecting the OS and PFS. Almost all recurrence occurred in primary tumor site, thus we suggest further evaluation on intensity-modulated radiotherapy or stereotatic radiosurgery for high-risk patients such as who have anaplastic ependymoma.
