Standardized surgical strategy for the treatment of preauricular sinus to reduce recurrence

  • Background: Preauricular sinus (PAS) is a common congenital anomaly, and complete excision is recommended to prevent recurrence. However, PAS has a high recurrence rate as a result of incomplete removal due to the high variability of the sinus ramifications, making its treatment challenging. In this study, we standardized the surgical procedure to reduce the complications and recurrence rate and compared the postoperative results between the non-standardized and the standardized groups. Methods: This retrospective study included 97 patients (120 ears) who had undergone PAS excision by a single surgeon between October 2014 and September 2022 and underwent at least 6 months of follow-up. After October 2018, all patients were treated using the standardized method, which comprised the use of magnifying glasses, exploration with a lacrimal probe, the use of methylene blue staining, and excision of a piece of surrounding normal tissue and related cartilage in continuity with the specimen. There were 38 patients (45 ears) in the non-standardized group and 59 patients (75 ears) in the standardized group. Results: Recurrence was observed in six of 120 ears, indicating an overall recurrence rate of 5.0%. Recurrence occurred in five ears (11.1%) in the non-standardized group and one ear (1.3%) in the standardized group. The standardized group had a significantly lower recurrence rate (p= 0.027) than the non-standardized group. Conclusion: We defined a standardized sinectomy protocol and used it for the surgical treatment of PAS. With this standardized method, we were able to reduce the rates of complications and recurrence without the use of a long incision.

Behaviorpharmacological Studies of Standardized Ginseng Extract G115 on the Central Dopaminergic Activity(I) (표준화된 인삼추출물 G115의 중추도파민신경계에 대한 행동약리학적 연구(I))

  • Central dopaminergic activity of standardized ginseng extract G115 was investigated in comparison with those induced by haloperidol in rats. The effects of G115 on the locomotor activity and, stereotyped behavior induced by apomorphine, which interacts directly with dopamine receptor were observed. Apomorphine(2 mg/kg) significantly decreased locomotor activity, whereas it showed a markdly increased incidence of stereotyped behavior. Standardized ginseng extract G115(100 mg/kg) and haloperidol(1 mg/kg) showed a significant decrease in locomotor activity but not induced stereotyped hehavior. Locomotor activity induced by apomorphine was markdly decreased by haloperidol(1 mg/kg), but that was significantly increased by standardized ginseng extract G115(50 mg/kg). Stereotyped behavior induced by apomorphine was completely supressed haloperidol(1 mg/kg), but was not changed by standardized ginseng extract G115. These results suggest that standardized ginseng extract G115 plays an important role in central dopaminergic activity, and haloperidol and standardized ginseng extract G115 seem to have a different action in behavior.

  • There are six approaches to constructing standardized coefficient for logistic regression. The standardized coefficient based on Kruskal's information theory is known to be the best from a conceptual standpoint. In order to calculate this standardized coefficient, the coefficient of determination based on entropy loss is used among many kinds of coefficients of determination for logistic regression. In this paper, this standardized coefficient is obtained by using four kinds of coefficients of determination which have the most intuitively reasonable interpretation as a proportional reduction in error measure for logistic regression. These four kinds of the sixth standardized coefficient are compared with other kinds of standardized coefficients.

Contrast and geometric correction of non-standardized radiographs in digital subtraction radiography (디지털 공제술에서 비표준화 방사선사진의 대조도 및 기하학적 보정에 관한 연구)

  • The purposes of this study were to develop the computer program for the contrast and geometric correction in digital subtration radiography with the IDL (Interactive Data Language) and compare the results with this program for the correction of the non-standardized radiographs to those of standardized radiographs and those with "Emago" software, the commercial program for the correction. The procedures were written for the contrast correction and subtraction with the geometric correction, using IDL. 32 pairs of periapical radiographs of premolar and molar portion of two dry human mandibles were taken at two different occasions with XCP film holder(nonstandardized films) and another 32 pairs with customized XCP film holder(standardized films). Subtraction of standardized film pairs was performed. Subtraction after the contrast and geometric correction of non-standardized films was performed using the newly developed program and Emago software. Standard deviations of grey levels of the subtracted images by the newly developed program were compared with those of the standardized group and Emago-corrected group. Standard deviations of grey levels of new program-corrected group were much smaller than those of the Emago-corrected group (p<0.001) and slightly larger than those of standardized group (p<0.05). However, the difference was very minute. This study indicates that the newly developed program written with IDL may substitute the mechanical standardization for digital subtraction radiography.

  • This research investigated the present state of the dress forms used by clothing manufacturers and proposed procedure to bring out the standardized body type and figure for women in twenties. The result of questionnaire showed chief pattern maker's thoughts that dress form should be differently made for each age group and should be more reflected the standard body than the idea body. The standardized body type was made from the standardized posture and size which were analyzed from the interquartile range(IQR) of 314 subjects. The standardized size could be verified as for the balanced body shape of twenties through making the standard dress form. The standardized form(S form) was compared with three kinds of forms(A, K and P) for investigating the suitability of forms. K form was very similar to S form, Educational P form and manufacturer's A form had quite big differences from S form at the important areas for making clothing. The side shapes of A, K and P forms showed differences at back bending, neck angle and hip shape from S form. A form was comparatively smooth and flat. This results could be used as the practical standard to improve the suitability of size and shape in the dress forms of manufacturers and educators.

A Study on the Development of Standardized Nursing Care Plans for Computerized Nursing Service (간호업무 전산화를 위한 표준화된 간호계획의 개발에 관한 연구)

  • A central issue in the development of nursing practice is to describe the phenomenon with which nursing is concerned. To identify the health problems which can be diagnosed and managed by the nurse is the first step to organize and ensure the development of nursing science. Therefore the academic world has been discussing the application of the nursing diagnosis in nursing practice as a means of improving quality of care. The objectives of this study were to develop a standardized nursing care plan for ten selected nursing diagnoses to form a database for computerized nursing service. The research approach used in the study was (1) the selection of the ten nursing diagnoses which occur most frequently on medical-surgical wards, (2) the development of a standardized nursing care plan for the ten selected nursing diagnoses, (3) application of the plan to hospitalize patients and evaluation of the content validity by the nurses, and (4) evaluation of the clinical effects after the use of the standardized nursing care plans. The subjects were 56 nurses and 395 hospitalized patients on two medical and two surgical unit. The results of this study were as follows ; 1) The ten selected nursing diagnoses for the development of the standardized nursing care plans were “PAIN, SLEEP DISTURBANCE, ALTERED HEALTH MAINTENANCE, ALTERATION IN NUTRITION, ANXIETY, CONSTIPATION, ALTERED PATTERNS OF URINARY ELIMINATION, DISTURBANCE IN BODY IMAGE, POTENTIAL FOR ACTIVITY INTOLERANCE AND ACTIVITY INTOLERANCE”. 2. The developed standardized nursing care plans included the nursing diagnosis, definition, defining characteristics, etiologic or related factors that contribute to the condition, recording pattern, desired outcomes and nursing orders (nursing interventions). 3. The plan was used with hospitalized patients on medical - surgical wards to test for content validity. The patient's satisfaction with the nursing care and nurses' job satisfaction were investigated to evaluate the clinical effects after the use of the standardized nursing care plans. A comparison of patient satisfaction with nursing care before and after the introduction of the standardized nursing care plans showed a statistically significant higher level of satisfaction with the standardized care plans. There was no difference in the level of job satisfaction expressed by the nursing staff before and after the standardized nursing care plans were introduced. However, when opinions about the use of the standardized nursing care plans were examined it was found that there was a positive effect on clarity in defining the nursing problems, determining nursing cost, more feasible goal setting, effective and systematic nursing records and indications for nursing research. The results of this study suggest that in order to increase the use of nursing diagnoses in the clinical area, it would be effective to select some wards as a pilot project, give the nurses training in the use of nursing diagnosis and develop and use the standardized nursing care plans. In addition to the ten diagnosis used in this study it is recommended that continual development of nursing diagnoses be done using diagnoses that are appropriate to Korea and testing them for validity through standardized care plans.

A Study on a Standardized Scoring System for College Interview Entrance Examination (대학입시에서의 면접점수 표준화에 관한 연구)

  • A standardized scoring system for college interview entrance examination means we standardize the score of examination to adjust the degree of difficulty among questions and difference of panel's disposition. A standardized scoring system were newly enforced at college interview entrance examination from 2001. Colleges want to choose the most suitably qualified students, taking full advantage of interview examination. Also they should always prepare for questions, plan the answers and a standardized scoring system so that all candidates get a fair shake. The main purpose of this paper is to provide a standardized scoring system for interview examination. The results of interview examination are ranked from highest to lowest and each candidate have different rank from several panels. So some unit scores from panels are given for each candidate using standard normal distribution. Then we calculate the mean unit score for each candidate and final interview entrance examination scores are given using the mean unit score for each candidate.

Challenges and suggestions of resource planning for standardized concurrent construction

  • Concurrent construction offers considerable improvement for shorten the project duration of its production process. Therefore, standardized concurrent construction is widely applied in building construction projects. However, resources planning for standardized concurrent construction project is manually developed by construction manager. This practice is not effective since it is time-consuming and error-prone for managers to identify all project-specific information, distinguish different activity-resource types, interpret these types and analyze how they affect resource allocated on an ad hoc basis. Therefore, this research investigates the opportunity for leveraging activity modeling to enable automated resource planning for standardized concurrent construction during project development, with identifying the characteristics of construction activities under standardized concurrent planning and determining the activity-resources types that affects resource planning. Both will function as a basis for modeling these construction activities in a computer-interpretable manner and for automation in resource planning.

Analysis of Related Factors and Regional Variation of Mortality in Seoul (서울특별시 사망률 변이 및 관련 특성 분석)

  • Background: Health is affected by various local factors. This study aims to investigate the age-standardized mortality variation of Seoul as well as the characteristics of the factors related to the mortality variation. Methods: The Korea Community Health Survey data, Seoul Survey data, Seoul statistics, and e-regional indicators of the National Statistical Office were used. To investigate the basic boroughs standardized mortality variation in Seoul, external quotient, coefficient of variation (CV), and systematic component of variation (SCV) values were suggested; correlation analysis and multiple regression analysis were conducted to investigate the characteristics related to standardized mortality rate. Results: The highest and the lowest standardized mortality rate of Seoul by boroughs had as much as 1.4 times difference; a low level of variation was shown in CV by 8.2; and was shown in SCV by 79. As a result of the multiple regression analysis of the factors that affect standardized mortality variation, the higher the rate of householders with college or higher, the lower the standardized mortality rate, and the higher the high-risk drinking rate, the higher the standardized mortality rate. Of the two, the rate of householder with a degree equivalent or higher than college was shown to have the biggest impact, followed by high-risk drinking rate. Conclusion: We found a variation in age-standardized mortality rate of boroughs in Seoul. The results suggest that policy makers should take into account socioeconomic environmental characteristics of community in developing community-based health promotion rather than focusing on lifestyle changes of residents.

Cross Mapping of Standardized Nursing Diagnoses and Problems with NANDA-I in 4 Tertiary Hospitals (표준화된 간호진단 및 문제와 NANDA-I 교차분석: 4개 상급종합병원 사례를 중심으로)

  • Purpose: To explore the scope and method of applying standardized nursing terminologies to nursing diagnosis and problems used in nursing practice. Methods: A descriptive study was done with a retrospective analysis of the nursing records of 141,420 patients that were hospitalized in 4 tertiary hospitals. The nursing diagnosis and problems collected from the records were standardized, and the standardized nursing diagnosis and problems cross mapped with NANDA-I, confirmed in a nursing focus group. Results: 65 (67.7%) of the 96 standardized nursing diagnosis and problems were equal with NANDA-I and included in the 10 domains of NANDA-I. Among 86 nursing diagnosis and problems excluded from the cross mapping with NANDA-I, the 63 terms (73.3%) related to surgery/procedure were the most common. Conclusion: It is meaningful that multi-tertiary hospital nursing diagnosis and problems were standardized and cross mapping with standard nursing terminologies was performed. As for the method of applying standardized nursing terminologies in nursing practice, it is appropriate to use several standardized nursing terminologies complementarily.
