A Cost Evaluation Model for Developing FHIR-based Health Information Services to Support Massive Clients

  • Healthcare services converged with ICT technology are improving quality of life and satisfaction through various customized services. In ICT-based medical services, data interchange between medical services is important, and HL7 FHIR, a medical data standard, enables efficient medical data interchange. FHIR-based medical information services using wireless data broadcasting can efficiently support massive clients. This paper proposes a function point model to evaluate the implementation cost of FHIR-based health information services using wireless data broadcasting. The proposed cost evaluation model can effectively evaluate the development cost by applying the complexity of converting medical data into FHIR format and the complexity of organizing indexes to efficiently support massive clients. The comparison of the proposed feature point evaluation model with simple feature points shows the efficiency and suitability of the proposed cost evaluation model.

  • Objectives: The purpose of this study is to propose a smart work environment management service model that can measure and maintain work environments in real time. Methods: How existing private consignment business is being carried out was identified and a simpler method was applied to the model. Results: Common support was provided according to the Korea Standard Industrial Classification. Hazards suitable for the relevant industry classification were selected and information on safety and health education, etc. was provided. Theme-specific support provides services focusing on hazards that can be measured through applications. Hazards are evaluated by applying new standards divided into 'Good', 'Average', 'Inadequate', and 'Faulty'. Conclusions: This model is designed to help employers identify health and safety conditions in small businesses where it is difficult to hire health and safety professionals. Using the app proposed in this study, anyone can easily measure their work environment at any time.

Cost-Effectiveness of Intensive Vs. Standard Follow-Up Models for Patients with Breast Cancer in Shiraz, Iran

  • Background: Breast cancer is the most common type of cancer amongst women throughout the world. Currently, there are various follow-up strategies implemented in Iran, which are usually dependent on clinic policies and agreement among the resident oncologists. Purpose: A cost-effectiveness analysis was performed to assess the cost-effectiveness of intensive follow-up versus standard models for early breast cancer patients in Iran. Materials and methods: This cross sectional study was performed with 382 patients each in the intensive and standard groups. Costs were identified and measured from a payer perspective, including direct medical outlay. To assess the effectiveness of the two follow-up models we used a decision tree along with indicators of detection of recurrence and metastasis, calculating expected costs and effectiveness for both cases; in addition, incremental cost-effectiveness ratios were determined. Results: The results of decision tree showed expected case detection rates of 0.137 and 0.018 and expected costs of US$24,494.62 and US$6,859.27, respectively, for the intensive and standard follow-up models. Tornado diagrams revealed the highest sensitivity to cost increases using the intensive follow-up model with an ICER=US$148,196.2. Conclusion: Overall, the results showed that the intensive follow-up method is not cost-effective when compared to the standard model.

The Effects of Housing Poverty on the Depression of the Elderly: The Mediating Effect of Social Service (노년기 주거빈곤이 우울에 미치는 영향: 사회서비스의 매개효과)

  • The study looked into the effect of housing poverty on the depression level for the elderly in depth. In this study, we defined housing poverty as sub-minimum standard housing conditions, excess housing expenditure and housing instability. In order to verify the correlation of two variables, a mediating model structured by social welfare service was used which gave out the 4th Korea welfare panel data. When it came to our research methods, structured equation analysis was applied to verify the mediating effect and theoretical background. The results revealed that housing poverty of the elderly directly affected their depression level. Also the satisfaction of social service showed a partial mediating effect between housing poverty and depression level. But the mediating effect of social service experience between housing poverty and depression level was not statistically significant. The outcome of this study indicated the practical and social intervention to promote a mental health of the elderly by improving residential environment.

Performance Analysis of the DQDB Protocol (DQDB (Distributed Queue Dual Bus) 프로토콜의 성능분석)

  • In this paper, an analytical model of the message delay in the DQDB (Distributed Queue Dual Bus) network is investigated. The DQDB network has been adopted as a subnetwork for the IEEE 802 MAN (Metropolitan Area Network) standard. The DQDB network consists of two high speed undirectional buses and a series of stations attached to both of the buses. Massages arriving at each station consists of severla packets according to its size. This system is approximated into " $B^{[x]}$/G/1 with exceptional first service queueing " by defining the concept of service time on a packet. The service time for a packet is defined as the time from the instant the packet arrives at the transmission buffer until the time the packet is fully transmitted.   • We developed the conferencing system based on the conference service model which is defined in ITU-T T. 120 series standard. Our conferencing service system is operated over the local area network. The NetMeeting of Microsoft corporation is used as a terminal and the MCU is developed by ourselves. In this paper, we described the design and implementation method of transport protocol stack supporting Multipoint Communication Service (MCS). Also, we described the occurred problem during the interoperability test between MCU and NetMeeting. And then we suggested their solution.

  • The Web environment of the frontend domain is increasingly competitive to preempt the new standard of presentation layer. N-Screen, a service that enables users to seamlessly use one content in various devices in Korea, is competing for market preemption by recognizing it as a core service of the future. In the cloud computing, N-screen is a typical service type. However, most of the frontend research required for groupware in enterprise environments has been limited to responsive web design for the web and native apps for mobile. Gradually, the need for MVC design patterns is increasingly widening in enterprise environments to overcome the cultural differences of companies and to support one source multi-use strategy supporting multiple devices and development productivity. Therefore, in this study, we will analyze and design JPetStore with hybrid application design based on Spring MVC, e-government standard framework environment of next generation web standard, and provide reference model of frontend hybrid apps design in future enterprise environment.

Development of a Hospital Service-based Costing System and Its Application (병원서비스별 원가분석모형의 개발과 적용)

  • The managerial environment of hospitals in Korea characterized by low levels of medical insurance fees is worsening by increasing government regulations as to the utilization of medical services, rising costs of labor, material, and medical equipments, growing patient expectations concerning the quality of services, and escalating competitions among large hospitals in the market. Hospitals should seek for their survival strategies in this harsh environment and they should have information about costs of their products in doing so. However, it has not been available due to the complexity of the production process of hospital services. The objectives of this study were to develop a service-based cost accounting model and to apply the developed model to a study hospital to obtain cost information of hospital services. A model commonly used for the job-order product cost accounting in the manufacturing industry was modified for the use in hospitals in Korea. Actual costs, instead of standard costs, incurred to produce a unit of services during a given period of time were estimated in the model. Data required to implement the model included financial information, statistics for the allocation of supportive cost center costs to final cost centers, statistics for the allocation of final cost center costs to services, and the volume of each services charged to patients during a study period. The model was executed using data of a university teaching hospital located in Seoul for the fiscal year 1992. Data for financial information, allocation statistics fo supportive service costs, and the volume of services, most of them in electronic form, were available to the study. Data for allocation statistics of final cost center costs were collected in the study. There were 15 types of evaluation and management service, 2, 923 types of technical service, and 2, 608 types of drug and material service charged to patients in the study hospital during the fiscal year 1992. Labor costs of each of seven types of pesonnel, material costs of 611 types of drugs and materials, and depreciation costs of 212 types of medical equipments, miscellaneous costs, and indirect costs incurred in producing a unit of each services were estimated. Medical insurance fees for basic services such as evaluation and management of inpatients and outpatients, injection, and filling prescriptions, and for operating procedures were found to be set lower than costs. Infrequent services which use expensive medical equipments showed negative revenuse as well. On the other hand, fees for services not covered by the insurance such as CT, MRI and Sonogram, and for laboratory tests were higher than costs. This study has a significance in making it possible for a hospital to obtain cost information for all types of services which produced income based on all types of expenses incurred during a given period of time. This information can assist the management of a hospital in finding an effective cost reduction strategy, an efficient service-mix strategy under a given fee structure, and an optimum strategy for within-hospital resource allocations.

A Study on Open University's Library Service Quality Using the Kano Model and the Timko's Customer Satisfaction Coefficient (원격대학 도서관 서비스 품질에 관한 연구 - Kano 모델과 Timko 계수를 적용하여 -)

  • Many university libraries, facing financial difficulties, have been forced to operate on a tighter budget. Such trend has created the need for an objective standard that will prioritize current library services. This study derives service quality factor via open coding from internet postings posted in the last five years on the Korean National Open University's library website, and also calculates the customer satisfaction quotient and dissatisfaction quotient using the Kano Model and Timko's Customer Satisfaction Coefficient. The result draws 25 service quality factors across 3 categories consisting of: (i) 8 attractive qualities, (ii) 15 one-dimensional qualities and (iii) 2 reverse qualities. The study, through the calculation of Timko's Customer Satisfaction Coefficient per service quality, proposes an intensive service that can prioritize the efficient use of library budget.

Prediction of RC structure service life from field long term chloride diffusion

  • It is well-documented that the major deterioration of coastal RC structures is chloride-induced corrosion. Therefore, regional investigations are necessary for durability based design and evaluation of the proposed service life prdiction models. In this paper, four reinforced concrete jetties exposed to severe marine environment were monitored to assess the long term chloride penetration at 6 months to 96 months. Also, some accelerated durability tests were performed on standard samples in laboratory. As a result, two time-dependent equations are proposed for basic parameters of chloride diffusion into concrete and then the corrosion initiation time is estimated by a developed probabilistic service life model Also, two famous service life prediction models are compared using chloride profiles obtained from structures after about 40 years in the tidal exposure conditions. The results confirm that the influence of concrete quality on diffusion coefficients is related to the concrete pore structure and the time dependence is due to chemical reactions of sea water ions with hydration products which lead a reduction in pore structure. Also, proper attention to the durability properties of concrete may extend the service life of marine structures greater than fifty years, even in harsh environments.
