The Development of the Integrated Nursing Practicum Education Matrix based on Learning Outcomes (학습성과기반 단계적 통합간호실습교육 매트릭스 개발)

  • Purpose: The purpose of this study was to develop an integrated nursing practicum education matrix based on the learning outcomes of each stage. Methods: In this Delphi technique study, 10 experts, consisting of six professors and four nurses, participated in the development of the matrix. The first step was an in-depth review for the composition of the questionnaire and the second step was the Delphi technique. The Delphi survey was conducted two times in order to complete the components of the matrix. The survey data was analyzed for statistical averages and standard deviations to decide the order of priority. Results: According to each stage (i.e. fundamental stage, competent stage, and proficient stage), the matrix was composed of education contents, methods, evaluation methods, and curriculum outcomes. Conclusion: The integrated nursing practicum education matrix of Y University was completed. The developed matrix will result in a reduction in the gap between nursing education and clinical practice and an improvement in nursing competency.

Task Analysis of Managers in the Customized Visiting Health Services (보건소 맞춤형 방문건강관리사업 전문인력의 직무분석)

  • Purpose: The aim of this study was to analyze the tasks of managers who were working in the Customized Visiting Health Services (CVHS) and to prioritize analyzed tasks according to performance frequency, perceived importance, and difficulty. Methods: Job analysis method by Hartley (1999) was used for task analysis and performance frequency, perceived importance, and difficulty were used for prioritize as criteria. A total of 85 managers in the CVHS of public health centers nationwide were recruited through e-mail and mail survey. Using SPSS/WIN 15.0, descriptive statistics, such as frequency distribution, means, median, and standard deviation, were conducted to examine each subject's general characteristics, the frequency, importance, and difficulty of the tasks as well as to prioritize the each task. Results: The job description of the managers revealed 12 duties, 35 tasks, and 104 task elements. Of the 85 managers, 84.8% were classified as nurses, 40.5% were home health care specialists, and 32.9% were social workers. Their coretasks were management of client cases, budget management, and management of work performance and quality assurance. Conclusion: Considering the analyzed managers' tasks and core tasks, we need to examine each manager's role precisely and provide various educational programs for improving overall manager competence.

Development of Competencies for New Nurses and Verification of Content Validity through a Delphi Survey (델파이조사를 통한 신규간호사 역량 도출 및 내용타당도 검증)

  • The purpose of this study was to develop and validate a competency tool for new nurses and to pilot-test it with new nurses. A Delphi survey was conducted to develop a competency tool, and a self-evaluation was conducted among new nurses who pilot-tested the finally derived competencies. The Delphi survey panel consisted of 18 people, including adjunct professors at the College of Nursing, nursing managers, and nurses with master's degrees. The Delphi survey asked about the validity of the competencies constructed in two rounds. After analyzing the Delphi results with mean, standard deviation, content validity ratio, degrees of convergence, and degrees of consensus, 12 core competencies and 36 enabling competencies were finally derived. The competencies consisted of clinical judgment and management (nine items), task competence (four items), patient orientation (five items), moral value orientation (three items), cooperation (two items), supply management (two items), professional development (three items), confidence (one item), self-control (two items), flexibility (two items), influence (one item), and nurturing others (two items). The finally derived competencies were pilot-tested with 229 new nurses who had worked for 2-12 months. The self-evaluation scores of new nurses were distributed differently according to their working period. In this study, the competencies required for new nurses were identified and the corresponding enabling competencies were identified. In the future, it is expected that a competency-based education program will be prepared based on these findings, and furthermore, it will be possible to provide high-quality medical and nursing services that meet patients' needs by improving the competency of new nurses and lowering the turnover rate.

Comparison of Education Programs for Hospice Volunteer Workers (호스피스 자원봉사자교육의 비교)

  • Purpose: The end of one's life can be one of the most important times in human life. However, physicians, nurses and volunteer workers are not sufficiently trained to understand the end-life care with competence and confidence. The purpose of this study was to compare various education programs currently carried out for volunteer workers. Methods: Nine curricula of hospice and palliative cares for volunteer workers at hospital and palliative care settings. Results: The mean time duration of theory education at nine institutes was 21.56 hours (range; 14-30). The common curricula of hospice and palliative cares for volunteer workers included 'Understanding of hospice and palliative care', 'Understanding of life and death', and 'Understanding of psychologic problem of end-of-life'. The education method comprised lectures, off-line 8 institutes and on-line 1 instiutute. Conclusion: It is necessary to develop the standard curriculum as well as regularly updated education program for volunteer workers.

Dynamic Reconstruction with Temporalis Muscle Transfer in Mobius Syndrome (뫼비우스 증후군에서 측두근 전위술을 이용한 역동적 재건)

  • Purpose: Mobius syndrome is a rare congenital disorder characterized by facial diplegia and bilateral abducens palsy, which occasionally combines with other cranial nerve dysfunction. The inability to show happiness, sadness or anger by facial expression frequently results in social dysfunction. The classic concept of cross facial nerve grafting and free muscle transplantation, which is standard in unilateral developmental facial palsy, cannot be used in these patients without special consideration. Our experience in the treatment of three patients with this syndrome using transfer of muscles innervated by trigeminal nerve showed rewarding results. Methods: We used bilateral temporalis muscle elevated from the bony temporal fossa. Muscles and their attached fascia were folded down over the anterior surface of the zygomatic arch. The divided strips from the attached fascia were passed subcutaneously and anchored to the medial canthus and the nasolabial crease for smiling and competence of mouth and eyelids. For the recent 13 years the authors applied this method in 3 Mobius syndrome cases- 45 year-old man and 13 year-old boy, 8 year-old girl. Results: One month after the surgery the patients had good support and already showed voluntary movement at the corner of their mouth. They showed full closure of both eyelids. There was no scleral showing during eyelid closure. Also full closure of the mouth was achieved. After six months, the reconstructed movements of face were maintained. Conclusion: Temporalis muscle transfer for Mobius syndrome is an excellent method for bilateral reconstruction at one stage, is easy to perform, and has a wide range of reconstruction and reproducibility.

A Study on the amendment of regulation of Korean Seamen's Act to exempt the seafarers who have completed advanced safety refresher training from basic safety refresher training (선원법상 상급안전 재교육 이수자의 기초안전 재교육 면제 규정의 개정에 관한 연구)

  • It is provided that seafarers qualified in accordance with STCW(International Convention on Standards of Training Certification and Watch-keeping for Seafarers) convention and code in safety training shall be required, every five years, to provide evidence of having maintained the required standard of competence, to undertake the tasks, duties and responsibilities listed in Chapter VI, section A-VI/1 of the STCW Code A. Every party to the STCW Convention has amended the relevant national regulations according to the above international convention and code. These amended Seaman's Act require the updating safety training which is only five years valid for existing seafarers serving onboard ship on international voyage. And above specified existing seafarers must, as of the 1st January 2017, have documentary evidence of either having completed the training course or updated their training within the last 5 years. In relation to the above international trend, the Korean Ministry of Oceans and Fisheries also has completed an amendment to its Seaman's Act. But, the Korean Seaman's Act has a supplementary provision to exempt the seafarers who have completed STCW advanced safety training from basic safety training. The purpose of this study is to define the problems of above mentioned exemption provisions and to make better policies for improvement. In order to complete the purpose, the research team has conducted an in-depth survey of various foreign cases on STCW safety training. Moreover, the team has suggested an improvement scheme in consonance with Korean shipping industry and society through out this report.

Relevance of Outsourcing S/W Process Quality and Product Quality with IT Adoption Factors (IT 도입요소를 고려한 외주 S/W 프로세스품질과 제품품질)

  • Due to the deterioration of the business environment, many companies become more interested in IT adoption to strengthen their internal competence. The recent expansion of the ERP adoption in the small and medium-sized enterprises (SMEs) can be interpreted as an example for this. However there are still a number of questions over the outsourcing effectiveness and its performance of IT adoption by characteristics of SMEs. This study aims to investigate the relationship between the software process quality and the product quality, and additionally the moderating effect of IT adoption factors, from the case of SMEs' ERP adoption by the outsourcing. For this purpose, the empirical analysis through the structural equation model has been carried out, based on the software quality model defined in ISO/IEC 15504 and ISO/IEC 9126. The result of analysis shows that the acquisition process quality can affect the product quality characteristics, currently applied in the national software quality certification, and this supports the practical usefulness of the ISO standard of software engineering. But another result shows that there is little moderating effect by IT adoption factors, in contrast to the expectation, and this can be explained due to the inappropriate measurement and the data constraint such as short duration-time of IT adoption. These can be used as an informative reference for continuing studies of the quality model application and the effect of IT adoption factors in SMEs.

Evaluation of the Effects of Feedback and Remediation after Formative Assessment in the Introduction to Clinical Medicine (임상실습 입문교육에서 형성평가 후 되먹임 및 재시험의 효과)

  • The authors investigated the effect of feedback and remediation after formative assessment (FRFA) by comparing the FRFA score and that of summative assessment (SA) in a course on clinical skills. In March 2015, 33 subjects underwent evaluation of their ability to perform a complex clinical skill using a real-time ready-made mobile assessment form tool, and through e-mail they were supplied with their feedback and final score (the pass group earned 2 points; the intermediate group earned 1 point; the nonpass group earned 0 points) followed by their self-reflection. The nonpass group underwent a re-test and e-mail feedback again until they passed the test, given the ease of performance. In December 2015, the 33 subjects took a 10-item SA, and one of the 10 items addressed a similar clinical skill. The difference between the first score on the FRFA and the score on the SA was evaluated statistically (p=0.05) through data analysis, variance distribution, correlation analysis, and linear regression analysis using SPSS software ver. 16. The increase from the score on the SA to that on the FRFA was statistically significant ($4.5{\pm}9.29$) in the pass group and the intermediate group, and was $29.7{\pm}11.49$ in the nonpass group of the formative evaluation (p<0.001). Using an FRFA could decrease the range in the standard deviation of the score and increase the minimum score among the subjects.

  • This study was carried out to analyze the job of care managers in their social welfare practicing fields about the frequency of care management duties they perform, types of duties considered to be most important and the reason, and types of jobs counted to be the most difficult and the reason by qualitative case study research methods. The job area of care management was elicited using DACUM method, and those 9 job areas derived from it were analysed according to frequency of performance, importance and difficulty. The subjects were 10 social workers serving more than 3 years with major duty of care management and chosen by reputational-case selection, and those collected data analysis was operated according to embedded analysis types. We discussed the implication of this research. : The experience of participants are the process of maintaining the tension, and search the equilibrium point between client centered service and institutional pressure. The Effects of Simulation Practicum using a Standardized Patient on Nursing Students' Communication Skills, Problem-Solving Skills, Critical Thinking Dispositions, and Clinical Competency (표준화 환자를 적용한 시뮬레이션 수업이 간호대학생의 의사소통능력, 문제해결능력, 비판적 사고성향 및 임상수행능력에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of standardized patient simulation on nursing students' communication skills, problem-solving skills, critical thinking dispositions, and clinical competency. Methods: A one-group pretest-posttest design was used. Data were collected from a convenient sample of 47 junior nursing students at W University. Scenarios to train SP and checklists to evaluate the students''competence were developed by our research team. The collected data were analyzed with descriptive analysis t-test, Pearson's Correlation Coefficient and Cronbach's ${\alpha}$ using SPSS WIN 21.0 Program. Results: The simulation practicum using a standardized patient was conducted based on four steps, namely, selections of scenario modules, standard patient training, implementation, and evaluation. The differences between the pre and post-test scores of problem-solving skills(t=-2.94, p=.005) and clinical competency(t=-2.84, p=.007) were statistically significant. But communication skill(t=-.32, p=.747) and critical thinking(t=-.68, p=.498) was not different in group. Conclusion: Standardized patient simulation in nursing education may be useful the improvement of problem-solving skills and clinical competency.
