Purpose: The study was aimed at qualitatively enhancing and promoting a home visiting nursing program established in Korea on July 1, 2008, as part of the Long-Term Care Insurance for the Elderly program. Methods: Structural, procedural and consequential aspects of home visiting nursing care wereclassified on the horizontal axis by applying the standard notions for the evaluation of medical care (Donabedian, 1998). At the same time, the home visiting nursing care service support system and the service provision system weredivided on the vertical axis with reference to the accreditation standards for home visiting nursing care organizations suggested by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO, 2008). The data were collected from June 4, 2008 to October 27, 2008, and were analyzed using SPSS ver. 15.0. Results: Twenty-two (proposed) standards, centered on the standard elements under the conceptual framework of the study, were developed, and comprised structural aspects (n=10), procedural aspects (n=6) and consequential aspects (n=6). Those criteria and indicators underwent two content validity surveys among groups of home visiting nursing care research and training experts. The research produced 22 proposed standards, 50 proposed criteria and 166 proposed indicators. Conclusion: The home visiting nursing care standards developed pursuant to the Long-Term Care Insurance for the Elderly Act and the applicability of these standards need to be verified by home visiting nurses. These proposed standards should prove useful in developing an assessment tool to encourage the qualitative enhancement of visiting nursing care in Korea.

Purpose: This study was aimed to develop the multicomponent intervention for preventing delirium among postoperative patients in a surgical intensive care unit (SICU). Methods: Using a quasi-experimental pre & post-test design with a non-equivalent control group, a total of 88 hospitalized patients in a SICU participated in this study. The 44 patients were allocated in each experimental and control group. The experimental group received the multicomponent intervention for delirium prevention including a delirium assessment and nursing intervention using a checklist, whereas the control group was provided with a standard care. The primary outcome of this study was the delirium incidence during the course of hospitalization. Results: There were no significant differences in the demographic and clinical characteristics between the two groups. The delirium occurred in 19.2% in the experimental group, whereas 38.6% in the control group ($x^2=4.526$, p<.05). Conclusion: The findings of the study demonstrated an effect of the multicomponent delirium prevention intervention in decreasing the delirium incidence rate over the standard care among the patients in SICU.

Purpose: This study aimed to develop nursing practice standards of hemodialysis care according to admission types. Methods: The standards were developed in four phases. Phase 1: The preliminary standards of hemodialysis care were developed based on literature review and evaluation of professional experts. Phase 2: Content validity was evaluated by 34 professional experts and the standards were modified and revised. Phase 3: Clinical validity was evaluated by 212 hemodialysis nurses. Expert group validated and verified the final outcome. Phase 4: Final standards of hemodialysis care according to admission types were developed. Results: The standard of hemodialysis care for out-patients included 5 standards, 9 contents, 43 nursing activities. The standard for in-patients included 6 standards, 10 contents, 50 nursing activities. The standards for critical patients included 4 standards, 10 contents, 43 nursing activities. There were differences in nursing activities according to admission type. Time required to meet the standards were different according to admission types. Conclusion: The findings of this research demonstrated that the time required for hemodialysis care and appropriate number of nurse would be different according to admission types. Different nursing workload according to admission type of hemodialysis patients need to be considered in allocating nursing manpower.

Purpose: The purpose of this study was to investigate the level of knowledge, attitude and compliance with standard precautions and to identify predicting factors of compliance with standard precautions in ICU and ER nurses. Methods: The participants were 228 ICU and ER nurses working in one of three university hospitals and one general hospital. Collected data included general characteristics, features related to infection risk, and knowledge, attitude and compliance with standard precautions. For the final analysis, 218 questionnaires were used. Results: Within the previous one year, 103 (47.3%) nurses experienced pricking injury from syringe needles or other sharp materials, 111 (50.9%) nurses reported exposure to patients' blood and body fluid. In general, the scores for knowledge, attitude and compliance were all high. Compliance scores for nurses in intensive care units were significantly higher than those of nurses in emergency. There were significant correlations of knowledge, attitude, and compliance with standard precautions. Attitude and work place were significant factors predicting compliance with standard precautions. Conclusion: The results indicate that to increase nurse's compliance with standard precautions, continuous efforts to enhance positive attitudes and at the same time, improve work environments and use individualized approaches based on the work units are needed.
Purpose: To identify the perception and practices of kangaroo care in nurses and doctors working in neonatal intensive care units (NICU) in Korea. Methods: One hundred forty-nine nurses and nineteen doctors working in the NICU from six university hospitals completed a survey questionnaire. Results: Most agreed that Kangaroo care promoted attachment and parental confidence as well as physical health of the infant. However, nurses and doctors showed a negative perception in providing kangaroo care for premature infants under 1,000 grams or within several hours after birth. Major barriers for kangaroo care were worrying about extubation and safety problems of premature infants. Married or senior nurses showed a more positive perception than others. Also nurses who worked in hospitals where kangaroo care was provided had a lower barrier perception than other nurses. Conclusion: Nurses and doctors working in NICU worried about adverse effects of kangaroo care even though they perceived positive effects. Standard education programs and manuals should be developed before dissemination of kangaroo care in Korea.
This article aims to discuss the barriers hindering cancer patients from receiving early palliative care, which has been demonstrated to be more effective in improving quality of life and controlling symptoms. Specifically, there are barriers in four aspects of delivering early palliative care. First, the difficulty of starting discussions about early palliative care and the lack of adequate appointment time can impede communication between oncologists and patients and their family members. Second, determining the timing of referral and deciding upon and applying a standard for referral can be barriers in the process of referral from oncology to palliative care. Third, palliative care patients and their family members can face difficulties regarding in what format and by whom the services will be delivered. Fourth, biases, misinformation, and inaccurate beliefs can be barriers in the process of patients and their family members accepting care. In order to facilitate early palliative care, research and policy regarding these barriers are necessary, along with efforts made by medical staff.

We carried out this study to reduce the gaps between medical institutes and between medical personnels and help to improve medical service quality, by classifying diagnoses and related intervention through the development of standard nursing intervention and by computerizing protocols. We considered two processes: one is the development process of home nursing standard intervention, and the other is the process of computerizing its related protocols. For the former, research covered analysis of home health care practices, development of client assessment protocol, of patients diagnosis protocols, and of patients intervention protocol. For the latter, strategies for home health care information systems should be set up and it constituted four research contents of analysis, design, management and evaluation of the systems. We also trained and educated home nurses who work at home health service center, by making them use the manual of home health care information systems at a certain city of P. In this study, therefore, we developed elements of standard home health care mediation so that they could be included in the forms of home health information note, home health progress note, and home health progress summary, home health discharge summary. Because standard home health care intervention has been developed, it became easier to exchange information between different home heath service center offices, can prevent from missing or redundant information, and contribute to standardization of hospital terminologies when EMR and HMR are developed.
Recently, interest and social expectation for children's environment are on the rise. The needs for construction of safe and pleasant child care environment has bees agreed with. Especially, as the number of children who are wanted to go center jumped with additional support the government for child care business, centers have also seen a boost. But as number of child care centers increases rapidly in short period, the quality of centers is more inferior to before. and the space and facility level of many centers don't reach in legal minimum standard. In addition to, when child care centers are reviewed, it tend to be judged by one standard without considering connection with sense of values about child care or philosophy, child's traits, surrounding environment etc. Therefore, it is important to construct environment that can consider children's physical development and behavioral characteristic as well as basic element such thing as protection and support activity for current child care centers. This study presupposes that physical environment of child care centers makes important effect on teacher's motion and child's development, then it is to investigate it's characteristics in physically space, facilities, outdoor playground. And It is to identify space and accommodations, outdoors facilities which can support activity smoothly inside and outside. As a result, it is try to offer appliable basic information about the child-friendly environment of child care centers.
Purpose : The aim of this study was to identify the factors influencing nursing performance in caring for patients with delirium. Methods: This study included 166 nurses who worked for more than 6 months at 4 general hospitals in Jeju Province, South Korea. Patients were administered a self-reported questionnaire. The study was carried out from May 20, 2014, to June 19, 2014. Data analyses were conducted using stepwise multiple regression, Pearson's correlation coefficients, t-test, and analysis of variance using the SAS WIN 9.2 program (SAS Institute, Cary, NC, USA). Results: The factors associated with nurses' performance in delirium care were work position (${\beta}=.22$, t=2.58, p <.001) and having received education on delirium care (${\beta}=.16$, t=2.24, p=.026). Conclusion: The results showed that the nurses' work position and having received education on delirium care affected nursing performance in delirium care. In order to improve nurses' performance in delirium care, hospitals should provide a delirium education program for nurses and establish standard guidelines on delirium care.

The purpose of this study was to investigate unit care characteristics of large skilled nursing facilities for the elderly, which are located in city areas, and to discuss the way of encouraging home-like atmosphere in perspective of spatial unit. First of all, the analysis of five facilities opened before 2002 was performed, based on previous studies, to compare with them opened in recent 5 years. Most of the previous facilities did not have differentiation between their residential and nursing units, and the number of residents in each unit were excessive in comparison with the standard limit(12-28). On the contrary, the facilities established in recent 5 years had systematic spatial unit structure. The residential units had basically rooms and spaces for small group and the number of elderly residents per unit was appropriate on the basis of standard limit which was suggested by Kwon(2002). The nursing units were consist of nursing station, 2-3 residential units, spaces for large group of residents, hair dressing, nurse, living assistant, bathing, storage and etc. But, there was a problem In space usage even though they have distinct unit care systems. Some spaces for groups were empty without residents, furniture, and other equipments, which were necessary for unit care.
