Clinical Study of 8 Patients with Qi-dizziness(氣暈) (기훈(氣暈) 환자 8례에 대한 임상보고)

  • The aim of this study was to report eight patients with dizziness, diagnosed as Qi-dizziness. The cause of Qi-dizziness is seven emotions(七情), and seven emotions induce liver qi depression(肝氣鬱結), spleen deficiency(肝脾不和) and dual deficiency of the heart-spleen(心脾兩虛). Guibisoyo-san($Gu\={i}p\'{\i}xi\={a}oy\'{a}o-s\`{a}n$) modified formula, which can treat the above diagnosis, was given to patients three times a day. To evaluate the therapeutic effect, visual analog scale(VAS) and inconvenience degree (ICD) were examined. After treatment, the VAS score decreased 7.37 points on average and ICD score decreased 1.62 points on average in patients with both peripheral vestibular dizziness and nonvestibular dizziness. This study shows that herbal therapy by traditional Korean medicine has considerable effects on dizziness, regardless of the etiological cause.

  • Objectives : This paper aims to examine the clinical meaning of treating with sweet medicinals where acupuncture fails, through studying the verse, "In case of both Yin and Yang deficiency, treat with sweet medicinals" in the Huangdineijing. Methods : Related contents in the chapters 「邪氣藏府病形」, 「根結」, 「終始」, 「經脈」, 「九鍼論」 of the Lingshu were analyzed threefold. The circumstances of the application of the "In case of both Yin and Yang deficiency, treat with sweet medicinals" principle to the late Han East Asian medicine as written in the Shanghanzabinglun were examined, and its meaning was explored. Results : The 'Yin Yang' in the verse could be substituted with the Five Zhang and Six Fu, Blood and Qi, Form and Qi, Form and Jing, Form and Zhi, etc. In patients with deficiency in Qi, Blood, Yin and Yang, we can observe external symptoms such as narrow pulse pattern, symptoms in the throat or below the throat, thirst, and coarse voice. To apply sweet medicinals is to supplement the Jing from food, Spleen and Stomach, Middle Qi and Earth Qi which produces and maintains Qi, Blood, Yin and Yang. Therefore, it is essential in treating disease patterns with deficiency in both Qi, Blood, Yin and Yang, and cannot be substituted with other means of treatment such as acupuncture, moxibustion, and other manipulative therapies. Conclusions : Sweet medicinals were applied in disease patterns with throat thirst and narrow pulse patterns which could not be managed with general acupuncture or moxibustion in the time of the Huangdineijing's publication, as it holds the Earth virtue which could harmoniously supplement the body's Qi, Blood, Yin, and Yang. Later its application broadened, treating various conditions accompanying Qi, Blood, Yin, Yang deficiency, which expanded potential of medicine and contributed to the generalization of drug treatment.

  • Objectives : The purpose of this study was to investigate the interpretation of the Soyo-san(SYS) prescription in order to obtain the evidence for clinical applications. Methods and Result : We interpreted on the SYS-related contents based on 25 classic books, analyzed it according to followed categories ; meaning of name, alteration of herbal composition according to age, usage, origin, physiological and pathological situation of SYS, description of each herbal drug Conclusion : 1. So(逍) mean diminish(消), 遙 mean roll(搖). It reflected feature of SYS, that alleviate depression without impairing vital energy(正氣). 2. SYS was first mentioned in the prescription of peaceful benevolent dispensary(太平惠民和劑局方), its essential application was harminizing liver depression and spleen deficiency(肝鬱脾虛) 3. SYS originated from Sayuk-san. Sayuk-san was appropriate for soothe the liver and regulate the spleen(疏肝理脾), while SYS was suitable for depressed liver and blood deficiency(肝鬱血虛) 4. Most medical literature insisted that application situation of SYS is blood deficiency(血虛), depressed liver(肝鬱), spleen deficiency(脾虛). And there is strong correlation between three factors. 5. Bupleurum falcatum L.(柴胡), Mentha arvensis L.(薄荷) alleviates liver depression as "pungent dissipate-qi uprais (辛散氣升)" nature. Angelica gigas Nakai(當歸), Paeonia lactiflora Pall.(白芍藥) emoliates the liver by nourishing the blood. Atractylodes japonica Koidz. ex Kitam.(白朮), Poria cocos(茯苓), Glycyrrhiza uralensis Fisch.(甘草), Zingiber officinale Roscoe(生薑) cultivate the basis as tonifying earth(補土).

A comparative study on pattern identification by OMS-prime of Hwa-Byung group and Hwa-Byung with Major Depression double diagnosis group (화병과 화병 ${\bullet}$ 주요우울증 중복진단군의 OMS-prime을 통한 변증유형 비교연구)

  • The objective of tills study is to identify the difference of somatic characteristics between Hwa-Byung and Major Depression by comparing the pattern identification of Hwa-Byung group and Hwa-Byung with Major Depression group(double diagnosis) Method: According to Hwa-Byung Diagnostic Interview Schedule(HBDIS) and SCID, 17 patients as diagnosed Hwa-Byung and 20 patients as diagnosed Hwa-Byung with Major Depression group(double diagnosis) were recruited. and by depression scale like Hamilton Rating Scale for Depression(HRDS) & Montgomery-Asberg Depression Rating Scale(MADRS), we excluded patients complaining moderate & severe depression among Hwa-Byung group and excluded patients showing mild depression among Hwa-Byung with Major Depression group. After this evaluation, we analysed and compared the pattern identification of both groups by OMS-prime. Result: 1. There were no significant differences of demographic data between both groups. 2. In the result of 'analysis on pattern identification' for all participant used by OMS-prime, most frequent pattern was deficiency of Yin and Yang of the heart(49%). 3. In the result of 'analysis on most correlated pattern identification' used by OMS-prime, for Hwa-Byung group was deficiency of Yin and Yang of the heart(45%) and the next were disharmony of the liver and spleen (20%), generation of phlegm due to stagnation of Gallbladder(15%) deficiency of Qi and Yin of the heart(l0%), And for Hwa-Byung with Major Depression group(double diagnosis) was deficiency of Yin and Yang of the heart(53%), the next were generation of phlegm due to stagnation of Gallbladder(18%), and deficiency of Qi and blood of the heart(l2%), 4. In the result of 'analysis on significant pattern identification' used by OMS-prime, for Hwa-Byung group was deficiency of Yin and Yang of the heart(20%) and the next were disharmony of the liver and spleen(15%), generation of phiegm due to stagnation of Gallbladder(15%), deficiency of Qi and Yin of the heart(14%), And for Hwa-Byung plus Major Depression group(double diagnosis) was deficiency of Yin and Yang of the heart(18%), the next were deficiency of Qi and Yin of the heart(18%), deficiency of Qi and blood of the heart(l0%), generation of phiegm due to stagnation of Gallbladder(18%), Conclusion: Hwabyung is syndrome that have many different symptoms, but there is no difference between Hwa-Byung group and Hwa-Byung with Major Depression group(double diagnosis) on the side of symptoms. Therefore, Hwabyung could be a new model for research on depression in Korean.

Analysis of Oriental Obesity Pattern Identification Questionnaire on Overweight and Obese Korean Adult Women (과체중 및 비만 성인 여성의 비만변증 설문 결과 분석)

  • Objectives We aimed to explore obesity pattern among overweight and obese Korean adult women using oriental obesity pattern identification questionnaire. Methods This survey was performed using data of 83 overweight and obese women aged from 20 to 55 yrs (BMI ${\geq}\;23\;kg/m^2$ : n=18, BMI ${\geq}\;25\;kg/m^2$ :n=65) in Seoul, from 2007 to 2008. Subjects were given written consent and this study was performed under the permission of institutional review board of Kyung-Hee East-west Neo Medical Center. Results 1. The distribution of oriental obesity pattern identification did not show any differences between obese and overweight group(p>0.05). 2. The ratio of significantly-scored oriental pattern identification was ordered by Stagnation of the liver Qi(肝欝, 21.7%) > Indigestion(食積, 18.1%) > Spleen deficiency(脾虚, 16.9%) > Yang deficiency(陽虚, 14.5%) (n=83). 3. The frequency of top-scored oriental obesity pattern was ordered by Stagnation of the liver Qi(肝欝, 36.1%) > Indigestion(食積, 24.1%) > Yang deficiency (陽虚, 15.7%) (n=83). 4. The frequency of oriental obesity pattern identification was ordered by Stagnation of the liver Qi(肝欝, 41.7%) > Indigestion(食積, 29.2%) > Yang deficiency(陽虚, 12.5%) > Stagnation of the liver Qi and Yang deficiency(肝欝兼陽虚, 8.3%) (n=24). Conclusions In Korean adult overweight and obese women, Stagnation of the liver Qi(肝欝), Indigestion(食積), and Yang deficiency (陽虚) were found to be the main pathology based on oriental obesity pattern identification questionnaire. It suggests that not only physical status but also general condition and emotional problem should be concerned in treatment of obesity. This study could play a role as a preliminary data of oriental obesity pattern identification.

  • The oriental-western Literatural study of Amnesia, the results were as follows. 1. esia is caused by qi-depression resulted from excessive thought and deficiency of the kidney resulted from congenital deficiency and deficiency of the heart, the disharmony between the heart and the kidney, phlegm, stagnant blood, loss of the blood etc. resulted from deficiency of the heart blood. 2. The treatment method of Amnesia is as follows, the highest frequence was growing blood-tranquilization-regulating spleen, in descending order removing phlegm-stagnant blood-relaxing the mind and invigorate the heart-spleen-kidney and much tonification qi-blood and growing nutrient qi-manifesting source qi and regulating the harmony between the heart and the kidney and maintaining patency for the flow of gi were the most treatment method. 3. The treatment medicine of Amnesia is as follows, the highest frequence was Kuei Bi Tang(歸脾湯) in decending order Jeng Ji Whan(定志丸), Su Seng Whan(壽星丸), Chun Whang Boo Sim Dan(天王補心丹), Ju Jak Whan(朱雀丸), Doo Dam Tang(導痰湯), Yin Sin Kuei Sa Dan(引神歸舍丹), Ga Gam Go Bon Dan(加減固本丸), Ryung Ji Go(寧志膏), Jang Won Dan(壯元丹), Tong Ol Tang(通鬱湯). 4. In oriental medicine functional physiology and pathology was significant in differential diagnosis and treatment and in western medicine it was explained organically and psychologically. 5. In western medicine As one of memory disorder Amnesia is divided into psychogenic amnesia and organic amnesia, and organic amnesia is divided into anterograde amnesia and retrograde amnesia and psychogenic amnesia is divided into localized amnesia, generalized amnesia selective amnesia.

  • Objectives : This study examined the reliability of disease mechanism diagnosis, to evaluate items of questionnaires and inquire about the relationships between disease mechanisms and 'diagnosis program' questionnaires used for the objective diagnosis of Oriental medicine in the department of Oriental OB&GYN, Oriental Medical Hospital of Dongeui University. Method : We analyzed the results of questionnaires from 3504 outpatients of OB&GYN disease at the Oriental Medical Hospital of Dongeui University from April 2000 to April 2005. Results & Conclusions : 1. The research questionnaire had 188 questions, the summary questionnaire 137, and the diagnosis questionnaire 80. 2. The reliability of all questionnaires shows above 90% in deficiency of qi, deficiency of Yin, insufficiency of Yang coldness heat syndrome liver and spleen kidney in all, 8 case disease mechanisms. These are higher in the diagnosis questionnaires than in the research questionnaires and the summary questionnaires, except for kidney disease mechanism. 3. Cronbach a of the questionnaires decreased, especially blood deficiency, phlegm, heat syndrome, and insufficiency of Yang; these 4 case disease mechanisms were lower than 0.6. 4. For degree of correspondence of meeting points, both. the diagnosis and the summary questionnaires were above 80% with the exception of the 2 case disease mechanisms heart and blood deficiency. The meeting points of both the diagnosis and research questionnaires were above 80% in the to case disease mechanisms deficiency of qi blood stasis deficiency of Yim insufficiency of Yang damp dryness liver spleen kidney phlegm. 5. The change in the result values of questionnaires was a decreased level of deficiency of qi heat syndrome phlegm damp kidney and raised level of coldness heart disorder of qi dryness 6. The computation degree of disease mechanism in DSOM(r) D.1.1 was much lower on phlegm deficiency of qi heat syndrome disorder of blood, somewhat lower on insufficiency of Yang and higher on coldness than in the two different questionnaires.

Development of a Guideline for the Application of a Diagnostic Tool for Menopausal Syndromes Based on the Use of Systemic Review and Delphi Method (체계적 문헌고찰과 델파이 기법을 활용한 갱년기장애 변증(辨證)진단 도구 개발을 위한 기초 연구)

  • Objectives: This study was conducted towards developing a screening tool for syndrome differentiation in the diagnosis of menopause in menopausal and perimenopausal women. Methods: We conducted a literature review of studies on menopausal diagnosis based on syndrome differentiation, and examined well-founded differentiated syndromes and their respective clinical symptoms. Based on the findings, we created a questionnaire through consultations with Oriental medicine experts in physiology, pathology, and diagnostics. Finally, the research team conducted an expert Delphi study on differentiated syndromes and the associated clinical symptoms. Results: Seven differentiated syndromes were selected, including Liver Depression (肝鬱), Kidney Yin Deficiency (腎陰虛), Kidney Yang Deficiency (腎陽虛), Liver and Kidney Yin Deficiency (肝腎陰虛), Kidney Yin and Yang Deficiency (腎陰陽兩虛), Heart-Kidney Noninteraction (心腎不交), and Dual Deficiency of Heart and Spleen (心脾兩虛); 4 disease locations, including liver (肝), heart (心), spleen (脾), and kidney (腎); and 3 disease natures, including Yin Deficiency (陰虛), Qi Stagnation (氣滯), and Blood Deficiency (血虛). In addition, we added 3 supplemental disease natures, including Yang Deficiency (陽虛), Qi Deficiency (氣虛), and Heat (火熱), in consideration of syndrome differentiation categories that may possibly be added in a follow-up clinical questionnaire. Conclusions: This resulted in a total of 7 differentiated syndromes, 4 disease locations, and 6 disease natures. We translated the clinical symptoms of these 17 categories into Korean Hangeul. After consulting with 5 Oriental medicine experts and a psychology expert, we produced a questionnaire for use in diagnosing menopause based on syndrome differentiation. The calculation of scores for the syndrome differentiation screening tool will be confirmed through clinical research based on the results of a review of existing literature.

Exploratory Study on the Pre-and Post-Prandial Subjective Appetite and Plasma Gut Hormone Levels in Spleen Qi Deficiency (SQD) Syndrome (비기허증(脾氣虛證) 환자의 식사 전후 주관적 식욕과 Gut Hormone 혈중 농도에 대한 탐색적 연구)

  • Objectives The aim of this study was to investigate clinical factors of SQD syndrome by tracking plasma gut hormone (active ghrelin, active Glucagon-like peptide-1(GLP-1), pancreatic polypeptide(PP), total peptide YY(PYY)) profiling of pre-post prandial standard meal between SQD group and normal group. Methods A total of 24 adult participants were consecutively recruited on April 2014. They were diagnosed as either by SQD syndrome or normal by Spleen Qi Deficiency Questionnaire (SQDQ). On the experimental day, blood samplings of 2 ml were repeatedly collected at 6 points from 2 groups for measuring plasma levels of gut hormones. At every point, subjective appetite sensations were self-registered. Results & Conclusions 1. There were significantly lower subjective 'Appetite' (p=0.012) and higher 'Satiety' (p=0.012) in SQD group. At each time point, subjective 'Appetite' was significantly lower at 60 min after breakfast (p=0.034) and 'Satiety' were significantly higher at 15 min (p=0.020) and 120 min (p=0.044) after breakfast in SQD group. 2. There were no significant differences in plasma levels of gut hormones (active ghrelin, active GLP-1, PP, total PYY) between SQD and normal group. Also at each time point, there were no significant differences of plasma levels of gut hormones between SQD and normal group. 3. Changes in plasma levels of gut hormones compared to baseline were not significantly different at each time point between SQD and normal group. Plasma PYY levels compared to baseline increased in SQD group following 15 min and 30 min after breakfast but decreased in normal group. 4. Further investigation is needed to construct gut hormone profiling and in this perspective, we can approach evaluation tool on variable appetite in Traditional Korean Medicine (TKM) syndrome in the future.

  • Objective : This study was designed to investigate causes, symptoms and treatments for the convalescent thamuria of children with oriental medical literatures. Methods : We surveyed the oriental and western medical books from to recent published books that have articles on thamuria from one's illness. Results and Conclusions : The symptoms of convalescent thamuria of children is frequency of urination and no pain. The causes of convalescent thamuria of children are the deficiency of the spleen and pulmonary qi, the weakness of the kidney qi and inner heat caused by the deficiency of yin. The methods of treatment are tonifying the spleen and pulmonary(補益脾肺), warming the kidney and tonifying qi(溫補腎氣) and tonifying yin and removing heat(滋陰淸熱). The herbal-medications for treatment are Bojungikqitang and Onpaetang (補中益氣湯合溫肺湯), Wuquihwan(右歸丸) and Jayinganghwatang(滋陰降火湯).
