A 73-year-old man, who, in an inebriated state, had slipped in a flowerbed and was wounded on the left flank, was transferred to Trauma Center, Gil Medical Center, Gachon University College of Medicine. Based on the chest and abdominopelvic computed tomography, he was diagnosed with multiple rib fractures and hemopneumothorax on the left hemithorax and was found to have a bony fragment in the spleen. He had not presented peritonitis and exsanguinous symptoms during the observation period. Seven days later, computed tomography of the abdomen showed suspected diaphragmatic injury and a retained foreign body in the spleen. On exploration by video assisted thoracoc surgery (VATS), a herniated omentum through the lacerated site of the diaphragm was observed. After omentectomy using Endo Gia, the foreign body in the spleen was observed through the lacerated site of the diaphragm. Traumatic diaphragm rupture with a foreign body, in the spleen, was successfully managed by video assisted thoracic surgery via the lacerated site of the diaphragm.

This study deals with the biological changes in mice after ${\gamma}-irradiated$. Four weeks old BALA/c mice were irradiated with 6.5Gy of ${\gamma}-ray$ on the fifth day after oral administration of radioprotectants such as ascorbic acid, tocopherol and cysteine. Control group was irradiated with 6.5Gy without pre-administration of radioprotectors. Blood cells and sperm cells were counted and body, testis and spleen were weighed 3 days after irradiation. And also liver antioxidant activity and range of spleen immune cells were measured. Differences in most biological parameters were not clearly distinguished between experimental groups. However, the relative spleen weight, the relative testis weight and the population size of spleen immune cells such as T helper cells, B cells and macrophages measured by means of FACS showed significant difference between irradiated and radioprotectant administered group. It is concluded that the relative spleen weight, the relative testis weight and the population size of spleen immune cells are easy and useful parameters for assessing the effect of radioprotective substances and for quantifying biological damage of radiation, as well.

Objectives: The purpose of this study was to investigate the effect of Jeokbaekhaogwanjung-tang (JGT) distillate on the immune activity of spleen cells of aged rats. Methods: Spleen cells of 10w, 52w 72w old SD rats were cultured and treated with JGT distillate and the levels of IL-2, IL-4, IL-10 and IFN-${\gamma}$ were measured. Results: 1. The levels of IL-2 and IFN-${\gamma}$ in the spleen cells of 52w old rats were significantly decreased by JGT treatment. 2. The level of IFN-${\gamma}$ in the spleen cells of 72w old rats were significantly decreased by JGT treatment. Conclusion: These results suggest that Jeokbaekhaogwanjung-tang distillate has an immune regulative effect by way of suppressing the change of immune activity caused by aging in rats.

A total number of 31 types of seaweed were assessed with regard to their effects on the proliferation of mouse spleen and thymus cells in a culture, using an MTT reduction assay. Acetone:dichloromethane (1:1) extracts of three seaweed plants: Derbesia marina, Sargassum sp., and Hisikia fuziformis, exhibited significantly positive effects on the survival of mouse spleen and thymus cells in vitro. The acetone:dichloromethane (1:1) extracts of Sargassum sp., in particular, much more potent effects on thymus cell activation than did any of the other types of seaweed. However, the methanol extracts of Sargassum ringgoldianium and Chondrus crispus exerted a stimulatory influence only on the proliferation of mouse spleen cells, whereas the methanol extracts of Grateloupia lanceolata exhibited significant cell proliferation properties in both spleen and thymus cells.

The effects of antlers have long been known in traditional Asian medicine. However, few studies have investigated the effects of antlers on immunity. In this study, we investigated whether fermented antler extract (FAE) has immunomodulatory effects on spleen cells. FAE enhanced the activity of spleen cells in a concentration dependent manner compared to antler extract. Interestingly, FAE significantly increased the production of interleukin-12, a representative cytokine of cell-mediated immunity, while it marginally increased that of tumor necrosis factor-alpha. Flow cytometry analysis demonstrated that FAE can protect spleen cells from spontaneous cell death without a significant proportional change in subsets, mainly lymphocytes. Taken together, the results of the present study showed that FAE has beneficial effects on spleen cells, a major type of immune cell, indicating that it can function as an immunomodulator without significant cytotoxicity. These data may broaden the use of FAE in basic research and clinical areas.

In an attempt to investigate the current of clinical researches on spleen yang or vital energy deficiency syndrome, the results were as follows. 1. It is possible to occure spleen deficiency syndrome which come from genetic factor. 2. The absorption disturbance in spleen deficiency syndrome can be likely caused by gastrointestinal mucosa injury, disorder of vagus nerve funtion and impairment of excretion of exocrine gland in pancreas. 3. Owing to the failure of tansporting and converting funtion of spleen, minerals, hematogenic substance and nutritional substance are scanty and then imbalanced metabolism state which heat production is decreasing is appeared. 4. By the failure of vital energy and blood growth, decreasement of $O_2$ transportation ability of RBC, disoder of blood coagulation, immune system disturbance which humoral immunity is enhanced and cellular immunity is decreased, are noted. 5. While there is not still an attemt to study the spleen deficiency sydrome in muscle disease or disease of four extremities, but it is likely suggested that spleen-stomach supplyment thereapy is very excellent effect on muscle disease and disease of four extremities.

저자들은 발열, 복통 등의 임상증상을 보이며 복부 CT촬영 결과 다발성의 저음영 병변을 보였으며 초음파를 통한 비장 생검 결과 육아종성 염증소견을 보여 항 결핵제 치료 후에도 호전없어 비장절제를 시행한 비장결핵 1예와 항 결핵제로 치료하고 있는 비장결핵 1예를 각각 경험하였기에 이를 보고하는 바이다.

In review of "invigorating spleen and supplementing qi" of clinical and experimental studies on malignant tumor, we obtained the conclusions as follows 1. Asthenic splenic qi is an important factor in mutation, occurrence and development of tumor. 2. The anti-tumor mechanism of "invigorating spleen and supplementing qi" is improvement of immune suveillance, controling cell proliferating period and enhancing body metabolism. 3. "Invigorating spleen and supplementing qi" is often used with "nourishing kidney" or "expelling pathogen" for treating cnacer. 4. In experimental studies, "invigorating spleen and supplementing qi" has effects on inhibiting occurrence and development of tumor, protecting mutation, inhibiting recurrence and metastasis, immune activity, enhancing metabolism, promoting bone marrow hemopoietic cell proliferation, increasing anti-tumor effect and protecting normal cells. 5. In clinical studies, "invigorating spleen and supplementing qi" has effects on prolonging the survival period of cancer patients, improving clinical symptoms and quality of life of cancer patients, degrading the side effects of western therapie(operation, chemotherapy and radiotherapy). 6. "Invigorating spleen and supplementing qi" is an extensive discipline of syndrome differentiation used to inhibit occurence, development, recurrence and metastasis.

This is an attempt to evaluate normal spleen size in the Koreans. Spleen scans were obtained in 29 patients who were admitted to the Seoul National University Hospital because of trivial illnesses. All scans were performed by use of $^{51}Cr$-labelled, heat-treated erythrocyte. As a simple screening criteria for the presence of splenomegaly, vertical height of the spleen was considered to be the best one. After statistical analysis it was concluded that the upper limit of normal spleen height is 13cm in anterior view and 14cm in lateral view.

본 연구는 간경변을 보유한 환자의 복부 CT 영상을 이용하석 비장의 웨이브 패턴(wave pattern)을 관찰하였는데 정상간을 보유한 환자의 복부 CT 영상과 차이가 있음을 발견하였다. 간경변을 가지고 있는 환자의 복부 CT 영상에서 비장은 왼쪽 측면에 깊은 웨이브 부분이 존재하였다. 정상간의 경우에도 왼쪽 측면에 웨이브가 존재하기도 하나 깊이가 깊지 않음을 알 수 있다. 그러므로, 간경변을 가진 비장의 웨이브 부분들의 면적의 합이 정상간을 가진 비장의 웨이브 면적보다 크다는 것을 알 수 있다. 나아가, 간경변을 보유한 영상에서 비장의 웨이브 부분을 추출하여 원형성(circularity)을 살펴보았을 때 정상간에 수반된 비장의 웨이브 부분보다 더 원형에 가깝다는 것을 알 수 있었다. 본 논문은 위와 같이 관찰된 원리를 바탕으로 복부 CT 영상에서 비장의 웨이브 패턴을 이용하여 간경변을 효과적으로 진단하는 새로운 방법을 제시한다. 이는 꼬리엽과 우엽의 비율 테스트를 생략하고 비장만으로 간경변 보유 간을 자동 판정해 낼 수 있음을 말해주는 것이다
