Comparison of Central and Peripheral Refraction in Myopic Eyes after Corneal Refractive Surgery and Emmetropes (굴절교정수술을 받은 근시안과 정시안에서 중심부 및 주변부의 굴절력 비교)

  • Purpose: To evaluate changes in central and peripheral refraction along the horizontal visual fields in myopic corneal refractive surgery group compared with emmetropes. Methods: One hundred twenty eyes of 60 subjects ($23.56{\pm}2.54$ years, range: 20 to 29) who underwent myopic refractive surgery and 40 eyes of 20 emmetropes ($22.50{\pm}1.74$ years, range: 20 to 25) were enrolled. The central and peripheral refractions were measured along the horizontal meridianat $5^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$, $25^{\circ}$ in the nasal and temporal areas using an open-field autorefractor. For analysis of post-op group, the group was classified by pre-op spherical equivalents of < -6.00 D and ${\geq}-6.00D$ as two post-op groups. Results: Pre-op spherical equivalent was $-4.56{\pm}0.92D$ (rang: -2.50 to -5.58 D) in post-op group 1, and $-7.09{\pm}0.96D$ (rang: -6.00 to -9.00 D) in post-op group 2. Spherical equivalent (M) in the emmetropes ranged from $-0.20{\pm}0.22D$ at center to $-0.64{\pm}0.83D$ at $25^{\circ}$ in the temporal visual field and to $-0.20{\pm}0.67D$ at $25^{\circ}$ in the nasal visual field; M in post-op group 1 ranged from $-0.16{\pm}0.29D$ at center to $-5.29{\pm}1.82D$ at $25^{\circ}$ in the temporal visual field and to $-4.48{\pm}1.88D$ at $25^{\circ}$ in the nasal visual field; M in post-op group 2 ranged from $-0.20{\pm}0.32D$ at center to $-7.98{\pm}2.08D$ at $25^{\circ}$ in the temporal visual field and to $-7.90{\pm}2.26D$ at $25^{\circ}$ in the nasal visual field. Among the three groups, there was no significant difference in M at central visual field (p=0.600) and at $5^{\circ}$ in the temporal visual field (p=0.647), whereas, there was significant difference in M at paracentral and peripheral visual field (p=0.000). Conclusions: Emmetropes had relatively constant refractive errors throughout the central and peripheral visual field and showed myopic peripheral defocus along the horizontal visual field. On the other hand, in myopic corneal refractive surgery group, there were significant differences in refractive errors between the central and peripheral visual field compared with differences in the central and peripheral refraction patterns of emmetropes.

Comparative Effect of Spectacles and Orthokeratology Lenses on Axial Elongation in Children with Mild-to-Moderate Myopia (중등도 이하 소아 근시에서 안경과 각막굴절교정렌즈 착용이 안축장 성장에 미치는 영향 비교)

  • Purpose: To assess the effect on axial elongation and associated factors between spectacles and of orthokeratology lens (OK) wearing in children with mild to moderate myopia. Methods: A total of one hundred subjects, ranging in age from 6 to 13 years, and with mild to moderate myopia no more than -4.50 diopters in spherical equivalent, visited our clinic from 2013 to 2015. The OK group (75 eyes) and the spectacles group (64 eyes) were compared and analyzed on the axial elongation and associated factors. Results: In the OK group, axial length was elongated in 1 year period with a mean increase of $0.24{\pm}0.29mm$. In spectacles group, axial length was elongated in 1 year period with a mean increase of $0.42{\pm}0.20mm$. The statistically significant suppression of axial elongation was observed in OK group compared to the spectacles group (Mann-Whitney U test, p < 0.05). For OK group, the age of starting OK (Pearson's correlation, r = -0.481, p < 0.05) was the only influencing factor on axial elongation, which had negative correlation with axial elongation. In spectacles group, the age of starting spectacles had negative correlation with axial elongation (Pearson's correlation, r = -0.462, p < 0.05) and baseline spherical equivalent, spherical diopter, cylindrical diopter from manifest refraction had positive correlation with axial elongation. Comparison of axial elongation in orthokeratology lens group and spectacles group by age groups (6 to 9 years [28 eyes], 9 to 13 years [47 eyes]), 9 to 13 years of orthokeratology lens group had the stronger suppression of axial elongation (Mann-Whitney U test, p < 0.05). Conclusions: The OK effectively suppresses axial elongation compared to the spectacles. Although the patients are in age from 9 to 13 years, the axial elongation was effectively suppressed.

Three-Dimensional Dosimetry Using Magnetic Resonance Imaging of Polymer Gel (중합체 겔과 자기공명영상을 이용한 3차원 선량분포 측정)

  • Purpose : Three-dimensional radiation dosimetry using magnetic resonance imaging of polymer gel was recently introduced. This dosimetry system is based on radiation induced chain polymerization of acrylic monomers in a muscle equivalent gel and provide accurate 3 dimensional dose distribution. We planned this study to evaluate the clinical value of this 3-dimensional dosimetry. Materials and Methods: The polymer gel poured into a cylindrical glass flask and a spherical glass flask. The cylindrical test tubes were for dose response evaluation and the spherical flasks, which is comparable to the human head, were for isodose curves. T2 maps from MR images were calculated using software, IDL. Dose distributions have been displayed for dosimetry. The same spherical flask of gel and the same irradiation technique was used for film and TLD dosimetry and compared with each other. Results : The R2 of the gel respond linearly with radiation doses in the range of 2 to 15 Gy. The repeated dosimetry of spherical gel showed the same isodose curves. These isodose curves were identical to dose distributions from treatment planning system especially high dose range. In addition, the gel dosimetry system showed comparable or superior results with the film and TLD dosimetry. Conclusion : The 3-dimensional dosimetry for conformal radiation therapy using MRI of polymer gal showed stable and accurate results. Although more studies are needed for convenient clinical application, it appears to be a useful tool for conformal radiation therapy.

The Study of Abnormal Refraction Eye on Women Population of University Students in South-East Korea (한국 동남지역 여대생들의 굴절이상 눈에 대한 연구)

  • Purpose: To research the study of abnormal refraction eye on women population of university students in South - East Korea. Methods: Between March 2007 and October 2007, the refraction test of eye glasses wearer was evaluated on women population (367 students, aged 19~22 years) of university in living on Kyung-Nam and Pusan province. Data was analysed with T-test. Results: On the abnormal refraction status, Compound Myopic Astigmatism was 76.72%, simple myopia was 10.90%, mixed astigmatism was 6.27%, respectively. On the prevalence of myopic power, low was 59.57%, moderate was 24.93%, high was 15.49%, respectively. On the myopic equivalent power, the right eye had more increase to compare to left eye. but these was not a statically significant correlation (p<0.5) between the right and left eyes. On the anisometropia of spherical equivalents and cylinder power, most subjects was under 1.0 diopter. On the other hand, the type of astigmatic axis was with-the rule (70.79%), against -the rule (18.41%), and oblique (10.80%). Conclusions: This study identify that the refraction status of abnormal refraction eye on women population in university students in South-East Korea have been more increased prevalence the myopia and astigmatism.

The Efficacy and Safety of Periocular Acupoint Stimulation on Myopia Progression in Children: A Study Protocol (소아근시에 대한 눈 주위혈 지압의 유효성 및 안전성 평가 연구)

  • Objectives Myopia has a higher prevalence rate in eastern countries, which also have a higher rate of educational fever compared to western countries. Considering this, social costs paid for myopia will increase rapidly in Korea. Although the development of myopia treatment is necessary, there has been a lack of relevant studies. Thus, this study aims to produce data to prevent unnecessary treatments and medical expenses. Methods The objective is to evaluate the effect of periocular acupressure exerted by a medical massager for myopia. This is an open-label, prospective, single-arm, and pre and post superiority study. The subjects are 7~12-year-old myopia patients with under 5 D (diopter) of spherical equivalent. A total of 56 subjects were enrolled. The selected subjects will wear the massager for 15 minutes twice a day for 24 weeks. The primary endpoint is the refraction change. The secondary endpoint is the axial length change. Statistical analysis was performed at a significant level of 0.05, using a two-tailed test. The criterion for significantly improved refraction was $-0.17{\pm}0.50$ D/6 months and that of axial length change was 0.126 mm. Results and Conclusions This study did not include a control group because children represent a vulnerable group. This objective study will bring some impact on Korean medical myopia treatment. A long-term confirmatory clinical study may be necessary in future.

Study of the Axial Length/Corneal Radius Ratio in Determining the Refractive State of the Eye (굴절이상과 안축장/각막곡률반경 비와의 관계에 관한 연구)

  • The purpose of this study is to evaluate the relationship between axial length/corneal radius ratio and refractive error for human eye. Ocular components were measured Baush & Lomb keratometer, Holden-Payor pachometer, and Stoz Compuscan. Refractive error was measured by subjective refraction. The results were as follows; 1) Spherical equivalent refractive error and axial length/corneal radius ratio was very highly correlated with the correlation coefficient for -0.89. 2) Axial length/corneal radius ratio and axial length, vitreous chamber depth were highly correlated that the correlation coefficients were 0.82, 0.80 respectively. 3) Axial length/corneal radius ratio and anterior chamber depth, corneal power, corneal radius, lens power were correlated with the correlation coefficients for 0.57, 0.40, -0.39, -0.35 respectively. 4) There were no significant correlation between axial length/corneal radius ratio and lens thickness, and corneal thickness.

  • The experimental study on the shape and behavior of bubbles in U-tube deep shaft of 32m has been performed. According to the results in this paper, the equivalent spherical diameter of bubbles was 0.422-0.722cm and the shape of bubbles was almost ellipsoidal for bubbles to be generated ta single-orifice of diameter 0.06cm in the downcomer of the deep shaft system. Gas hold-up and gas-liquid contact area of bubbles in the deep shaft was decreased in accordance with depth, Also downcomer velocity of bubbles was slower than circulation velocity of liquid and riser velocity of bubbles was fast than circulation velocity. The bubble-size-distribution of bubbles in the deep shaft was the logarithmic probability distribution. The number of bubbles in the deep shaft was decreased in accordance with depth and the number of bubbles in the downcomer is more than the bubbles in the riser.

  • This study proposes finite element modeling of dislocation punching at cooling after consolidation in order to calculate the strength of particle-reinforced aluminum composites. The Taylor dislocation model combined with strain gradient plasticity around the reinforced particle is adopted to take into account the size-dependency of different volume fractions of the particle. The strain gradients were obtained from the equivalent plastic strain calculated during the cooling of the spherical unit cell, when the dislocation punching due to CTE (Coefficient of Thermal Expansion) mismatch is activated. The enhanced yield stress was observed by including the strain gradients, in an average sense, over the punched zone. The tensile strength of the SiCp/Al 356-T6 composite was predicted through the finite element analysis of an axisymmetric unit cell for various sizes and volume fractions of the particle. The predicted strengths were found to be in good agreement with the experimental data. Further, the particle-size dependency was clearly established.

  • Recently, technical advances have been made in high efficiency gas turbine power plants. In domestic gas turbine facilities, the material properties of the blade and vane are degraded by the daily start-stop operations arising from the thermo mechanical cycle. We surveyed the time dependent degradation of the HP blade and vane to gather basic data for life assessment and damage analysis. The EOH(equivalent operating hours) of the blades were 23,686, 27,909, and 52,859 and the EOH of the vanes were 28,714 and 52,859, respectively. With increased operating hours, the shape of the primary ${\gamma}$' precipitate transformed from cubic to spherical, and its average size also increased. The leading edge area of the blades and the center of the vanes had the worst morphology, and this tendency agrees with the microhardness results. The thickness of the thermally grown oxide at the outer surface of the bond coat increased with increased operating hours.

  • To assess the oil-containment effectiveness of tandem oil fences placed in currents, the movement of oil droplets in the fore and aft region of the fences is investigated by experimental and numerical methods. The effect of the flexibility of the fence skirt of single fence on the fence effectiveness is also investigated. Laboratory experiment is conducted to trace the path of a spherical solid particle of equivalent density to an oil droplet which was released in a uniform stream ahead of a model oil fence. Depending upon the releasing position and the flow condition there, it was observed that the particle floated up to the free surface, collided with the fence, or escaped below the fence. By analyzing the droplet trajectories, a numerical method is developed to predict the region ahead of the fore fence where an oil droplet initiating its motion eventually escapes beneath the fence. The effect of the relative sizes of the drafts of the fore and aft fences, the fence separation, and the bottom depth of the sea bed on the effectiveness of tandem fences is investigated using the numerically obtained trajectories of oil droplets.
