I made inquires about mammographic equipments and circumstances of mammography rooms in the 64 medical facilities in areas of Seoul and Kyong Gi Do. Moreover I had experments about exposure dose with patients and radiologic technologists. so there is the data indicated follows. 1. There are inclined to improve in quality and function of mammographic equipments, it has been proven that s/f system exchanged to DR system. 2. It is certain that the number of examinations are becoming increasingly significant. 3. The Space Scattered Dose of mammography rooms are much more larger than portable equipments. 4. I worry about the affection of expose dose about Space Scattered Dose of mammography room. 5. There is need of study how to cope with the situation about increasing exposure dose of radiologic technologists in small space and numeruous number of examinations.
The purpose of this study is to survey the present status of portable radiography and the result of free space scattered dose rate when taking a radiography at the general hospital or the university hospital in Taejon city. The results were as follows; 1. The number of cases using portable radiography for three years increased to averages 16.2%, 7.7% per year from January 1st in 1996 to December 31st in 1998. 2. The average of distance of adjacent patients was 219.1 cm at the ward. 3. For portable chest radiography, the free space scattered dose rate was 10.5 mSv/hr at 50 cm distance, 1.8 mSv/hr at 100 cm distance, and 0.2 mSv/hr at 200 cm distance. Therefore, in case of portable chest radiography at the ward, the average of distance of adjacent patients is 219.1 cm, so it does not have influence on the adjacent patients. But during the portable radiography, a guardian who is close to the patient, doctor, nurse and radiologic technologists has to set up the shield to prevent from the unnecessary radiation or the distance should be as great as possible from the mobile X-ray equipment.
We tried to study in order to furnish the data for medical exposure dose and scattered ray in radiography. As the tables(from 1 to 3) show, we can presume, by means of a concrete numerical value, the amount of results affected by patient radiation exposure dose and somatic effect in radiography. However, there are many difficulties in the difference of exposure factor in each hospital, the accuracy of measuring by tracebility, shortage of exposure dose data especially in the area of children, and portable radiography, etc. In the radiation examination, it is considered if the gained benefit to the patient due to radiation is more than the risk of radiation, then the medical exposure is thought to be justified. Therefore, the radiotechnologists should continually make an effort to develop and study new techniques so as to reduce patient exposure dose.
High energy photon beams from medical linear accelerators produce large scattered radiation by various components of the treatment head, collimator and walls or objects in the treatment room including the patient. These scattered radiation do not provide therapeutic dose and are considered a hazard from the radiation safety perspective. Scattered dose of therapeutic high energy radiation beams are contributed significant unwanted dose to the patient. ICRP take the position that a dose of 500mGy may cause abortion at any stage of pregnancy and that radiation detriment to the fetus includes risk of mental retardation with a possible threshold in the dose response relationship around 100 mGy for the gestational period. The ICRP principle of as low as reasonably achievable (ALARA) was recommended for protection of occupation upon the linear no-threshold dose response hypothesis for cancer induction. We suggest this ALARA principle be applied to the fetus and testicle in therapeutic treatment. Radiation dose outside a photon treatment filed is mostly due to scattered photons. This scattered dose is a function of the distance from the beam edge, treatment geometry, primary photon energy, and depth in the patient. The need for effective shielding of the fetus and testicle is reinforced when young patients ate treated with external beam radiation therapy and then shielding designed to reduce the scattered photon dose to normal organs have to considered. Irradiation was performed in phantom using high energy photon beams produced by a Varian 2100C/D medical linear accelerator (Varian Oncology Systems, Palo Alto, CA) located at the Yonsei Cancer Center. The composite phantom used was comprised of a commercially available anthropomorphic Rando phantom (Phantom Laboratory Inc., Salem, YN) and a rectangular solid polystyrene phantom of dimensions $30cm{\times}30cm{\times}20cm$. the anthropomorphic Rando phantom represents an average man made from tissue equivalent materials that is transected into transverse 36 slices of 2.5cm thickness. Photon dose was measured using a Capintec PR-06C ionization chamber with Capintec 192 electrometer (Capintec Inc., Ramsey, NJ), TLD( VICTOREEN 5000. LiF) and film dosimetry V-Omat, Kodak). In case of fetus, the dosimeter was placed at a depth of loom in this phantom at 100cm source to axis distance and located centrally 15cm from the inferior edge of the $30cm{\times}30cm^2$ x-ray beam irradiating the Rando phantom chest wall. A acryl bridge of size $40cm{\times}40cm^2$ and a clear space of about 20 cm was fabricated and placed on top of the rectangular polystyrene phantom representing the abdomen of the patient. The leaf pot for testicle shielding was made as various shape, sizes, thickness and supporting stand. The scattered photon with and without shielding were measured at the representative position of the fetus and testicle. Measurement of radiation scattered dose outside fields and critical organs, like fetus position and testicle region, from chest or pelvic irradiation by large fie]d of high energy radiation beam was performed using an ionization chamber and film dosimetry. The scattered doses outside field were measured 5 - 10% of maximum doses in fields and exponentially decrease from field margins. The scattered photon dose received the fetus and testicle from thorax field irradiation was measured about 1 mGy/Gy of photon treatment dose. Shielding construction to reduce this scattered dose was investigated using lead sheet and blocks. Lead pot shield for testicle reduced the scatter dose under 10 mGy when photon beam of 60 Gy was irradiated in abdomen region. The scattered photon dose is reduced when the lead shield was used while the no significant reduction of scattered photon dose was observed and 2-3 mm lead sheets refuted the skin dose under 80% and almost electron contamination. The results indicate that it was possible to improve shielding to reduce scattered photon for fetus and testicle when a young patients were treated with a high energy photon beam.
Mobile X-ray generators are used not in the radiation area but in open space, which causes the exposure of secondary radiation to the healthcare professionals, patients, guardians, etc., regardless of their intentions. This study aimed to investigate the shielding effect of the developed radiation restrictor to block the secondary radiation scattered during the use of mobile X-ray generator. Upon setting the condition of mobile X-ray generator with chest AP, spatial doses were measured by the existence of human equivalent phantom and radiation restrictor, and measured by the existences of phantom and radiation restrictor at the same length of 100 cm. Measurements were taken at intervals of 10 cm every $30^{\circ}$ from $-90^{\circ}$ (head direction) to $+90^{\circ}$ (body direction). Upon the study results, spatial doses in all direction were increased by 45% on average when using phantom in the same condition, however, they were decreased by 64% on average when using the developed radiation restrictor. The dose at 100 cm from the center of X-ray was $3.0{\pm}0.08{\mu}Gy$ without phantom and was increased by 40% with $4.2{\pm}0.08{\mu}Gy$ after phantom usage. The dose when using phantom and the developed radiation restrictor was $1.4{\pm}0.08{\mu}Gy$, which was decreased by 66% compared to the case without using them. Therefore, it is considered the scattered radiation can be shielded at 100-150 cm, the regulation of the distance between beds, effectively with the developed radiation restrictor when using mobile X-ray generators, which can lower the radiation exposure to the people nearby including healthcare professionals and patients.

The present study used a digital angiography x-ray device to measure the space dose and exposure dose of patients and practitioners using x-ray tube shielding devices developed in our laboratory. The intent of the study was to reduce the space dose within the test room, and to reduce the exposure dose of patients and practitioners. The patient and practitioner exposure doses were measured in five configurations in a human body model. The glass dosimeter was placed on the eye lenses, thyroid glands, left shoulder, right shoulder, and gonads. The beam was collimated at full size and at a 48% reduction for a comparative analysis of the measurements. The space dose was measured with an ion chamber at distances of 50 cm, 100 cm, and 150 cm from the x-ray tube under the following conditions: no shielding device; a shielding device made of 3-mm-thick lead (Pb) [Pb 3 mm shield], and a shielding device made of 3-mm-thick Pb (outside) and 3-mm-thick aluminum (Al) (inside) [Pb 3 mm+Al 3 mm shield]. The absorbed dose was the lowest when the 3-mm-thick Pb+3-mm-thick Al shield was used. For measurements made with collimated beams with a 48% reduction, the dose was the lowest at $154{\mu}Gy$ when the 3-mm-thick Pb+3-mm-thick Al shield was used, and was $9{\mu}Gy$ lower than the measurements made with no shielding device. If the space dose can be reduced by 20% in all situations where the C-arm is employed by using the x-ray tube shielding devices developed in our laboratory, this is expected to play an important role in reducing the annual exposure dose for patients, practitioners, and assistants.
Current medical institutions with the development of medical technology to the increased demand for health use of radiation equipment is increasing rapidly. Direct radiation from the patient receives the aim of reducing exposure as much as possible is important and the spatial dose of scattered radiation with in the space to engage in reducing healthcare physician, radiation workers and carers need to reduce indirect exposure. X-ray radiation workers and caregivers in the X-ray room to wearing of protective clothing is advised. However Radiation worker sand caregivers of patients with secondary is done, by wearing protective clothing to wear protective clothing because of the weight and discomfort have been neglected. In this study, based on the presence or absence of clothing scattered radiation from space to measure distances, depending on the horizontal and height by measuring the angle of the importance of wearing protective clothing were investigated.
Aircrews and passengers are exposed to radiation from cosmic rays and secondary scattered rays generated by reactions with air or aircraft. For aircrews, radiation safety management is based on the exposure dose calculated using a space-weather environment simulation. However, the exposure dose varies depending on solar activity, altitude, flight path, etc., so measuring by route is more suggestive than the calculation. In this study, we developed an instrument to measure the cosmic radiation dose using a general-purpose Si sensor and a multichannel analyzer. The dose calculation applied the algorithm of CRaTER (Cosmic Ray Telescope for the Effects of Radiation), a space radiation measuring device of NASA. Energy and dose calibration was performed with Cs-137 662 keV gamma rays at a standard calibration facility, and good dose rate dependence was confirmed in the experimental range. Using the instrument, the dose was directly measured on the international line between Dubai and Incheon in May 2023, and it was similar to the result calculated by KREAM (Korean Radiation Exposure Assessment Model for Aviation Route Dose) within 12%. It was confirmed that the dose increased as the altitude and latitude increased, consistent with the calculation results by KREAM. Some limitations require more verification experiments. However, we confirmed it has sufficient utilization potential as a cost-effective measuring instrument for monitoring exposure dose inside or on personal aircraft.
This study was conducted to evaluate the dose of the space to the controller located within the mammography room conducted a research on ways to the reduction exposure to the radiation workers. Results, the dose of 6.18 mGy/year was measured when there is no difference in the hilar area of the controller position, the dose of 2.35E-11 mGy/year was measured when installing the Shielding door. In addition, when the direction of the X-ray tube anode be heading this direction controller, low average level measured was 0.30 mGy/year. Based on this study, the mammography should be considered when installing the anode and cathod directions. And, by installing the shielding door, it must be able to completely separate shooting space and control room. This is the best way radiation protection method in radiation workers.

In the case of designing a high dose rate remote controlled afterloading treatment room with existing hospital facilities. We must construct the effective protective barriers so as to reduce the primary and scattered radiation up to the maximum permissible dose level. It is difficult to reinforce the barrier thickness of the shielding requirements because of the limited space and the problem of the existing building structure at the surrounding area. Therefore we can reduce the intensity of primary radiation to the required degree at the location of interest with installing the appropriate I shaped Pb barriers between the radiation source and the shielding wall of the concrete. As a result, it was possible to reduce the intensity of the primary radiation below the M.P.D level by using additional Pb barriers instead of increasing thickness of concrete wall.
