The Effect of Yangkyuk-Sanhwa-Tang on Cytokine Production in the Patients with Cerebral Infarction (중풍환자에서 양격산화탕에 의한 세포활성 물질 생성의 조절 효과)

  • Yangkyuk-Sanhwa-Tang(YST) has been widely used as a formula for the Soyangin cerebral infarction (CI) patients according to Sasang constitutional philosophy. Brain cells produce cytokines and chemokines during the inflammatory process after stroke both in animal models and in patients. Previously, regulation of serum cytokine levels by YSThas been observed in individuals at the acute stage of CI disease, but there have not been other scientific investigations on YST. The author investigated the effect of YST on theproduction of various cytokines using peripheral blood mononuclear cells (PBMCs)from the Soyangin (CI) patients, and Soyangin normal group. The cytokine production was analyzed using enzyme-linked immunosorbent assay (ELISA). The amount of interleukin (IL)-1, IL-1, IL-6, IL-8, and tumor necrosis factor (TNF)- in culture supernatant significantly increased in the LPS-treated cells compared with unstimulated-cells (P < 0.05). However, in LPS-stimulated PBMCs, cytokines level in CI patients group was higher than that of normal group. YST (1 mg/ml) significantly inhibited IL-1, IL-1, and IL-8 production in PBMCs stimulated with LPS (about 85% for IL-1, 87% for IL-1, and 53% for IL-8, P < 0.05), but did not significantly inhibit IL-6 and TNF- production in the CI patients group. We also show that YST significantly increased LPS-induced IL-1, IL-6, and TNF- production in the normal group. Thesedata suggest that YST has a regulatoryeffect on the cytokine production, which might explain its beneficial effect in the treatment of CI.

A Case-Control Study on Risk Factors of Obese Patients of Each Sasang Constitution (사상체질별 비만요인에 대한 환자-대조군 연구)

  • 1. Backgrounds and Aims Human being is divided into 4 constitution types(Taeyangin, Soyangin, Taeumin, and Soumin). According to Sasang Constitutional Medicine[SCM] Each constitution has its own incidence, treatment and prevention of disease. The purpose of this study is to find the relation of dietary habits, life styles and Psychosocial stress with obesity in each Sasang Constitution. 2. Methods 975 subjects who have received health examinations and diagnosis of Sasang Constitution at Kyung-hee Medical Center were divided into each constitutional group. Each constitutional group was then divided into obese group and normal group. We evaluated risk factors of obesity such as dietary habits, life styles, and Psychosocial stress. These data were statistically analysed to investigate the relations between risk factors and obesity. And then significant factors were analysed by multinomial logistic regression analysis to get each odds ratio. 3. Results In a multinomial logistic regression analysis adjusted for age and sex, promotion of appetite and overeating or excessive diet elevated risks of obesity in Taeumin, promotion of appetite and high speed of diet elevated risks of obesity in Soyangin, and Psychosocial stress elevated risks of obesity in Soeumin. 4. Conclusions The results suggest that promotion of appetite and overeating or excessive diet may be major risk factors for obesity in Taeumin, promotion of appetite and high speed of diet in Soyangin and Psychosocial stress and dyschezia in Soeumin.

Adverse Effects of Ephedra According to Sasang Typology in Healthy Adults : A Double-Blind Randomized Controlled Trial (마황복용 후 사상체질별 이상반응에 관한 임상연구 : 무작위배정 이중맹검시험)

  • Background : Ephedra (Ephedra sinica) has been widely used to treat respiratory disease in traditional medicine of East Asia for over a hundred years. Despite safety concerns raised by some, the use of ephedra in traditional medicine is documented over more than 1,800 years. It is well established that ephedra is one of the central medicines in Korean 'Seseng constitution' medicine. In Sasang constitution medicine, all humans can be divided into one of four types: Soeumin, Soyangin, Taeumin or Taeyangin, and each constitution type has their own typical characteristics. Accordingly, it is hypothesized that the adverse effects of ephedra differ depending on the Sasang constitution classification. Objectives : The aim of this study was to determine adverse effects of ephedra which is classified as a Taeumin herb, and to observe whether the response differs or not. according to Sasang constitution classification. Methods : The study design was a double-blind randomized controlled trial. The subjects were healthy adults 20 - 50 years old who agreed to participate in this study. They were allocated through randomization to either ephedra group (N=55) or placebo group (N=24). where ephedra extract (6 g of dried ephedra) and placebo with similar opaque capsules were given twice for one day. To compare the adverse events of ephedra according to Sasang constitution classification, we analyzed blood pressure (systolic and diastolic), pulse rate, the morning questionnaire, and patient's global assessment scale score for well known adverse events: palpitation, headache, sweating, tiredness, dyspepsia, and dry mouth. Results : After ingestion of ephedra, the pulse rate had a significant increase in all constitution types. The changes of diastolic pressure in Soeumin and the changes of pulse rate in Soeumin, Soyangin and Taeumin had a significant increase in the ephedra over the control group. In the ephedra group, the palpitation and dyspepsia score of the patients' global assessment scale had a significant increase in Soeumin, with palpitation and sweating score increasing in Soyangin. Others observations were insignificant results. Conclusion : The results of this study may confirm that the physical responses or adverse effects of herbs differ for each type of Sasang constitution. Future studies using other herbs will be required to ascertain the herbal drug reaction of Sasang constitutions.

The Origins, Changes, and Compositive Principles of Hyungbangjihwang-tang (형방지황탕(荊防地黃湯)의 기원(基源), 변천과정(變遷過程) 및 구성원리(構成原理))

  • Objectives This paper investigates the origins, changes, and the constructive principles of Hyungbangjihwang-tang(荊防地黃湯). Methods Hyungbangjihwang-tang and other related prescriptions were analyzed in terms of their pathological indications, based on previous literature including 『Gogeumuigam(古今醫鑑)』, 『Sanghannon(傷寒論)』, 『Uihagjeongjeon(醫學正傳)』, 『Donguisusebowon·Sasangchobongwon(東醫壽世保元·四象草本卷)』, 『Donguisusebowon·Gabobon(東醫壽世保元·甲午本)』, 『Donguisusebowon·Sinchukbon(東醫壽世保元·辛丑本)』. Results and Conclusions The origins of Hyungbangjihwang-tang can be seen in three sections depending on the classification of medicinal herbs. Schizonepeta tenuifolia Briq.(荊芥), Saposhnikovia divaricata Schischk.(防風), Notopterygium incisum Ting(羌活), Aralia cordata Thunb.(獨活), and Plantago asiatica L.(車前子) originated from Hyungbangpaedok-san(荊防敗毒散) included in 『Gogeumuigam』. Poria cocos Wolf.(茯苓) and Alisma canaliculatum(澤瀉) originated from Ohryoung-san(五苓散) included in 『Sanghannon』. Rehmannia glutinosa for hueichingensis(熟地黃) and Cornus officinalis S. et Z.(⼭茱萸) originated from Yukmijihwang-tang(六味地黃湯) included in 『Uihagjeongjeon』. Hyungbangpaedok-san was transformed into "Paedok-san" in 『Chobongwon(草本卷)』, Hyungbangpaedok-san in the 『Gabobon(甲午本)』 and 『Sinchukbon(辛丑本)』. Yukmijihwang-tang was mentioned as a Soyangin medicine in the 『Chobongwon』 and was later transformed into a Suhwagije-tang(水火旣濟湯) in 『Gabobon』. The meaning of Yukmijihwang-tang inherited in 『Sinchukbon』 as Dokhwaljihwang-tang and Hyungbangjihwang-tang. The medicinal herbs in Hyungbangjihwang-tang can be categorized in three sections by their functions, Schizonepeta tenuifolia Briq., Saposhnikovia divaricata Schischk., Notopterygium incisum Ting, Aralia cordata Thunb., and Plantago asiatica L. scatter the Pyohan(表寒) and make the Pyoeum(表陰) drop. Poria cocos Wolf.(茯苓) and Alisma canaliculatum(澤瀉) make the Pyoeum(表陰) drop through Lee-su(利水). Rehmannia glutinosa for hueichingensis(熟地黃) and Cornus officinalis S. et Z.(⼭茱萸) tonify Shin-won(腎元). The each principles of medicinal herbs corresponds to the stage of the "Cold-related Diarrhea in Soyangin Spleen Cold-based Exterior Cold disease(少陽人 脾受寒表寒病 亡陰病 身寒腹痛亡陰證)".

A Cross-sectional Study on Differences in Blood Homocysteine Levels of Acute Cerebral Infarction Patients Categorized by Sasang Constitutional Medicine (급성기 뇌경색 환자의 사상체질별 혈중 Homocysteine 농도에 대한 단면적 연구)

  • Objectives : This study aimed to clarify the difference in blood homocysteine levels of acute cerebral infarction patients categorized by Sasang constitutional medicine. Methods : The subjects were recruited from patients admitted to the Oriental Internal Medical Department at Kyunghee Medical Center between October 2005 and May 2007, who were classified as small vessel occlusion (SVO) patients according to the Trial of ORG 10172 in Acute Stroke Treatment (TOAST). The general characteristics along with total homocysteine levels were recorded and analyzed according to Sasang constitutional medicine. Results : A total of 151 patients were included in the trial. The prevalence of constitution was, in order, Soyangin, Taeumin, and Soeumin. No statistical significance was noted for any characteristic except body weight. There was no significant difference in blood homocysteine levels between constitutions. Conclusion : This study investigated the difference in blood homocysteine levels of acute cerebral infarction patients categorized according to Sasang constitutional medicine. Due to many limitations, the correlation between homocysteine levels and Sasang constitution was not clarified. Nevertheless, this study is significant in that it examined the largest study group to date in Oriental Medicine research history on the relation between stroke patients' homocysteine and Sasang constitution, and can be utilized in future as a basic material. Further research on the subject is needed.

  • Objectives This study was to identify the lifestyle associated with metabolic syndrome and to suggest a personalized health management according to the constitution to prevent disease by metabolic syndrome stage. Methods This study used the data of Korean medicine Data Center (KDC). A total of 8,985 data were searched for subjects who participated in Anseong and Ansan cohorts study from 2009 to 2012. We analyzed 2,602 participants that diagnosed with metabolic syndrome among the ages of 30 to 55. We divided into three groups, none, pre-metabolic syndrome (Pre-MetS), and metabolic syndrome (MetS), according to number of metabolic syndrome elements. Results The prevalence of metabolic syndrome was highest in Taeumin (Pre-MetS: 48.2%, MetS: 41.2%). The risk factors for metabolic syndrome are dietary amount, speed of eating, and sleep quality in Taeumin, and dietary amount and sleep quality in Soyangin. Conclusions The life style affecting the metabolic syndrome were different according to the constitution. It is necessary to manage life style considering the Sasang constitution

The Clinical Study of 12 Cases of Paralyric Ileus Patients Treated with Bo-Ryu Enema (보류관장을 시행한 마비성 장폐색환자 12례에 대한 임상적 고찰)

  • A clinial observation was done on 12 cases of paralytic ileus patients, treated by Bo-Ryu Enema(保留灌腸), who were hospitalized from May 1, 1995 to October 31, 1996 at the Department of Oriented Internal Medicine II, Oriental Medicine Hospital. Taejon University. The results were as follows; 1. The ratio between male and female was 1 : 1.4. The distribution of age. 70' years or over, 60', 50' years generation were revealed in turn. 2. In classiffication of human coporeal constitution, Soeumin(少陰人) were 9 cases(75.0%), Taeumin(太陰人) 2 cases (16.7%), Soyangin(少陽人) 1 case (8.3%). 3. In distribution of disease on admission, Stroke sequela was the most number with 7 cases(58.3%), Stroke 3 cases(25.3%). Hypertensive encephalopathy and Brain tumor were 1 case, each other. 4. The effect of treated by Bo-Ryu Enema was as follows: Each of Excellent(良好) and Good(好戰) were 6 cases(50%) but. Fair(別無好戰) and Poor(惡化) were no case.

  • 1. Objectives This study was to investigate correlation between the diagnosis of Sasang Constitutional Medicine(SCM) and the differentiation of syndromes according to the state of Qi, Blood, Body Fluid, Phlegm retention and the five viscera in storke inpatients. 2. Methods Stroke inpatients were investigated through questionaire of differentiation of syndromes and QSCC2. The results were analyzed statistically by one-way ANOVA on SPSS 12. 3. Results and Conclusions 1) Soeumin had a high grade on Qi-insufficiency than other consititutions. 2) Soyangin had a low grade on Qi-insufficiency, spleen disease than other consititutions. 3) There were not significant results in the investigation item of the sweating, constipation, diarrhea, anorexia, abdominal pain among the constitutions.

Distribution of Sasang Constitution and Prescriptions of Patients with Coldness in Hands and Feet (일개병원에서의 수족냉증환자에 대한 사상체질분포 및 처방)

  • Objectives This study was implemented to investigate the distribution of Sasang constitution and prescription of the patients with coldness in hands and feet.Methods Sixteen patients with coldness were analyzed in terms of age, Sasang constitution, season and prescriptions.Results The distribution of Sasang constitution of the patients with coldness in hands and feet was like Taeeumin 50%, Soeumin 43.8% and Soyangin 6.2%. And the date of consultation of the patients is different seasonally, 50% of the patients visited in winter, 18.8% in spring and fall respectively and 6.3% in summer. The frequency by age group is different was like 31.3% of the patients visited in 20s, 18.8% in 10s, 30s and 50s, 12.5% in 40s. In terms of Exterior and Interior diseases, Soyanging and Soeumin patients were categorized in Interior diseases while Taeeumin patients were categorized in Exterior disease.Conclusions Patients with coldness in hands and feet were mostly classified into Taeeumin and Soeumin and the prevalence of its symptom was high in their 20s. Patients were willing to visit clinics in winter. The Exterior/Interior diseases and mild/severe diseases were different according to Sasang constitution.

  • Spinal cord infarction in vascular impairment of spinal cord is rare in the world. and besides, because spinal cord has small cross-sectional area and almost all tract of sensory system and motor system pass away, spinal cord disease is one of the terrible diseases, that leaves usually permanent and severe neurological sequela. The case is about a patient who is 57 years old woman. Both of her legs were suddenly paralyzed and her case was diagnosed as spinal cord infarction in medical center. In our hospital, we focussed on her basic physiological condition-defecation, urination, dormitio, diet, etc. In the case of this patient, power and sense of both legs with physiolocal condition improved remarkably through Sasang Constitutional Medicine with rehabilitative therapy.
