Gateway design and performance evaluation based on health information standards in multi-platform environment (멀티플랫폼 환경에서 의료정보표준 기반 게이트웨이 설계 및 성능 평가)

  • Recently, to enhance the quality and efficiency of health care services, smart health care services utilizing a smart phone and tablet pc are mainly being introduced into the large hospital and the interest for personal health record service which manages personal medical information utilizing PC and smart phone and technology development is actively underway. In order to build a next generation hospital information systems, the interoperability of data between the different hospital information systems and UX consistency between the various user devices must be assured. Therefore, in this paper, medical information standard and the study for improving the performance is conducted to design the gateway system which converting and integrating the medical information under multi-platform environment. In this paper, CCR standard is adapted to the mapping method between the data which is the key feature of the gateway system and design the system utilizing multi-table-based mapping method. To evaluate the performance of the system, experiment was done and the result has been improved approximately 20 percent for response rate.

Study on the Transmission of Medical Information using Bluetooth Technology (블루투스를 이용한 의료정보 신호의 전송에 관한 연구)

  • In this paper, a system that transmits ECG signals which get from hand baggage ECG is implemented by using Bluetooth technology. Bluetooth technology is a close range wireless communication used wireless frequency 2.4GHz bandwidth. This technology consumes very small power and provides high reliability also self error correction with high speed frequency hopping. Because every device which uses Bluetooth protocol can communicate each other. These can connect between the system implemented and any devices such as mobile telephone with Bluetooth module, notebook, and the personal mobile device. Also, the paper proposes capability of transmission to the sever of hospital through each type of wireless communication device that acquired medical information signals in mobile medical machine. The system consists of hardware parts with Bluetooth module and host part, and software parts with blutooth protocol stacks. The host precesses a connection with other device and transmits ECG signals with bluetooth frequency hopping sequence.

  • Medical imaging modalities to image either anatomical structure or functional processes have developed along somewhat independent paths. Functional images with single photon emission computed tomography (SPECT) and positron emission tomography (PET) are playing an increasingly important role in the diagnosis and staging of malignant disease, image-guided therapy planning, and treatment monitoring. SPECT and PET complement the more conventional anatomic imaging modalities of computed tomography (CT) and magnetic resonance (MR) imaging. When the functional imaging modality was combined with the anatomic imaging modality, the multimodality can help both identify and localize functional abnormalities. Combining PET with a high-resolution anatomical imaging modality such as CT can resolve the localization issue as long as the images from the two modalities are accurately coregistered. Software-based registration techniques have difficulty accounting for differences in patient positioning and involuntary movement of internal organs, often necessitating labor-intensive nonlinear mapping that may not converge to a satisfactory result. These challenges have recently been addressed by the introduction of the combined PET/CT scanner and SPECT/CT scanner, a hardware-oriented approach to image fusion. Combined PET/CT and SPECT/CT devices are playing an increasingly important role in the diagnosis and staging of human disease. The paper will review the development of multi modality instrumentations for clinical use from conception to present-day technology and the application software.

  • Picture Archiving and Communication Systems(PACS) provide an integration of digital imaging information in a hospital, which encompasses various imaging equipment, viewing workstations, database archive systems, and a high speed fiber optic network. One of the most important requirements for integration is the standardization of communication protocols to connect devices from different vendors. Since 1985, the ACR-NEMA standard provides a hardware interface, a set of software commands, and a consistent set of data formats for point-to-point interconnection of medical equipment. However, it has been shown to be inadequate for PACS networking environments, because of its point-to-point nature and its inflexibility to allow other services and protocols in the future. Based on previous experience of PACS developments in The University of Arizona, a new communication protocol for PACS networks has been suggested to the ACR-NEMA Working Group VI. The defined PACS protocol is intended to facilitate the development of PACS's capable of interfacing with other hospital information systems. Also, it is intended to allow the creation of diagnostic information data bases which can be interrogated by a variety of distributed devices. A particularly important goal is to support communications in a multivendor environment. The new protocol specifications are defined primarily as a combination of the International Organization for Standardization / Open Systems Interconnection (ISO/OSI) protocols and the data format portion of ACR-NEMA standard. This paper addresses the specification and implementation of the proposed PACS protocol into network node. The protocol specification, which covers Presentation, Session, Transport, and Network layers, is summarized briefly. The implementation has natural extentions to Global PACS environments. The protocol implementation is discussed based on our implementation efforts in the UNIX Operating System Environment. At the same time, results of performance evaluation are presented to demonstrate the implementation of defined protocol. The testing of performance analysis is performed on the PACS prototype node.

  • Today all kinds of smart devices are being developed with various researches on wearable devices that support smart computing on the human body. Skin diseases continue to rise including freckles, pimples, atopy, and scalp trouble due to the environmental and genetic factors, and people pay bigger medical bills to treat their skin diseases. There is thus a need to develop a smart-phone or table-based smart healthcare imaging system of high portability and diagnostic accuracy capable of analyzing and managing various skin problems related to skin care. This study proposed an integrated system combining the Smart Mi Band, a wearable device using moisture and UV sensors based on IoT, on the hardware part with the sensor information monitoring software.

  • Among the fourth industrial revolution technologies, products related to healthcare using IoT and sensors are currently being developed. We design and develop an interaction system based on user standing posture recognition using multi-channel large area pressure sensors in this paper. To this end, first of all we investigate major sensor markets of the sensor industry and review technology trends and the current and future of smart healthcare. Based on this survey, we examine and compare cases developed at home and abroad for multi-channel large-area pressure sensors, which are key components of the system that we want to develop. We recognize the standing posture status of the user through the developed system and experiment with how effective it is actually in user posture calibration and apply the research results to various healthcare devices' medical fields based on this.

Determination of Flow Rate and Stability of 5-Fluorouracil in Disposal Infusion Device, $Anapa^{(R)}$ (일회용 약물 주입기구를 이용한 5-Fluorouracil의 지속주입효과와 용기 내 안정성 평가)

  • Disposal infusion device is known to be useful for chemotherapy. Anti-cancer drug can be released by the force of carbon dioxide or balloon. In this study, we compared the$Anapa^{(R)}$ (LC0020) with B Company (LV2 ml) in terms of infusion rate and stability. Infusion rate was determined every six minute using software, MSI08IH. Stability of 5-fluorouracil was examined periodically using a High Performance Liquid Chromatography. Infusion rates of gas-derived $Anapa^{(R)}$ device were 2.29, 1.86, 1.98 ml/hr and those of balloon-derived B Company device were 1.71, 1.58, 1.37 ml/min. There were no significant differences in stability of 5-fluorouracil between $Anapa^{(R)}$ and B Company devices. In summary, gas-derived $Anapa^{(R)}$ device is thought to be comparable or superior to balloon-derived B Company device as far as infusion rate and stability are concerned. We expect that $Anapa^{(R)}$ as a home infusion device can be employed to improve a quality of life and compliance of cancer patients.

Efficient Stack Smashing Attack Detection Method Using DSLR (DSLR을 이용한 효율적인 스택스매싱 공격탐지 방법)

  • With the recent steady development of IoT technology, it is widely used in medical systems and smart TV watches. 66% of software development is developed through language C, which is vulnerable to memory attacks, and acts as a threat to IoT devices using language C. A stack-smashing overflow attack inserts a value larger than the user-defined buffer size, overwriting the area where the return address is stored, preventing the program from operating normally. IoT devices with low memory capacity are vulnerable to stack smashing overflow attacks. In addition, if the existing vaccine program is applied as it is, the IoT device will not operate normally. In order to defend against stack smashing overflow attacks on IoT devices, we used canaries among several detection methods to set conditions with random values, checksum, and DSLR (random storage locations), respectively. Two canaries were placed within the buffer, one in front of the return address, which is the end of the buffer, and the other was stored in a random location in-buffer. This makes it difficult for an attacker to guess the location of a canary stored in a fixed location by storing the canary in a random location because it is easy for an attacker to predict its location. After executing the detection program, after a stack smashing overflow attack occurs, if each condition is satisfied, the program is terminated. The set conditions were combined to create a number of eight cases and tested. Through this, it was found that it is more efficient to use a detection method using DSLR than a detection method using multiple conditions for IoT devices.

Self-Organizing Middleware Platform Based on Overlay Network for Real-Time Transmission of Mobile Patients Vital Signal Stream (이동 환자 생체신호의 실시간 전달을 위한 오버레이 네트워크 기반 자율군집형 미들웨어 플랫폼)

  • To transmit vital signal stream of mobile patients remotely, it requires mobility of patient and watcher, sensing function of patient's abnormal symptom and self-organizing service binding of related computing resources. In the existing relative researches, the vital signal stream is transmitted as a centralized approach which exposure the single point of failure itself and incur data traffic to central server although it is localized service. Self-organizing middleware platform based on heterogenous overlay network is a middleware platform which can transmit real-time data from sensor device(including vital signal measure devices) to Smartphone, TV, PC and external system through overlay network applied self-organizing mechanism. It can transmit and save vital signal stream from sensor device autonomically without arbitration of management server and several receiving devices can simultaneously receive and display through interaction of nodes in real-time.

Dose perturbation measurements during the liver treatment with internal organ motion: Mathematical modeling and Experimental simulation (호흡에 의한 내부 움직임의 영향이 있는 간에서의 실험적 선량 측정)

  • Respiratory motion in the thorax and abdomen is an important limiting factor in high-precision radiation therapy. The lung tumor and tumor(pancreas, stomach) in abdomen therefore are internal motion due to breathing. We will perform to measurement of dose distributions for these moving tumors. In preliminary study, we investigated displacement of moving tumor such as liver, lung tumor in abdomen with previously reported papers. With reference data, internal movements of tumor are displayed with phantom and moving control device(MCD), which appear three dimension (3-D) motion such as x, y and z axis. These devices are used to access dose delivered in tumor with and without internal motion. The MCD and phantom were used to evaluate a delivered dose under similar condition, although there are not same internal tumor motion. In future, we will obtain the exact evaluation of dose if improved in programed software of moving control device and measure precise internal motion using image modality such as fluoroscopy, simulator in based on this study.
