Regional Analysis of Soft Tissue Thickness on Korean Buttocks and Application to Fasciocutaneous Flap Design

  • Background Various shapes and designs of the gluteal artery perforator flap have been used for treating sacral pressure sores and reconstructing breasts. To establish the ideal fasciocutaneous flap design for use in the gluteal area, the soft tissue thickness distribution was measured. Methods Twenty-one buttocks of adult Korean cadavers were analyzed through rectangular subfascial dissection. Each buttock was divided horizontally into 10 sections and vertically into 10 sections, and then, the thickness at the corners of the sections was measured. For the sake of comparison and statistical verification with living bodies, computed tomography (CT) images of 120 buttocks of patients were randomly selected. Five horizontal sections and 4 vertical sections were made, and the thickness at each corner was recorded. Results According to the dissection and the CT images, the area with the thinnest soft tissues in the buttock was around the posterior superior iliac spine, close to the sacral area. The thickest area was the superolateral area of the buttock, which was 3.24 times and 2.15 times thicker than the thinnest area in the studies on cadaver anatomy and the CT images, respectively. Conclusions The thickness of the soft tissues in the buttocks differed by area. The superolateral area had the thickest soft tissues, and the superomedial area had the thinnest. This study includes information on the distribution of the thickness of the gluteal soft tissues of Koreans. The outcome of this study may contribute to the design of effective local flaps for pressure sore reconstruction and free flaps for breast reconstruction.

Catastrophic complications from filler injection on external genitalia

  • Soft tissue filler injections are widely used due to their immediate effects, predictable results, and high stability. However, as the use of soft tissue filler injections has increased, various complications have been reported. We report a life-threatening complication in a patient who developed sepsis and necrotizing fasciitis. A 45-year-old woman presented with right leg pain and discharge from the labia majora. The patient had received a soft tissue filler injection of unknown composition 1 year earlier and had recently undergone incision and drainage for an inflammatory cystic nodule. Antibiotic treatment was administered for cellulitis, but the infection progressed to necrotizing fasciitis and sepsis. Fasciotomy and intensive care unit treatment improved the systemic infection, but the soft tissue filler injection site did not respond to treatment for 1 month. Thus, the injection site was covered with a pedicled vertical rectus abdominis musculocutaneous flap after wide excision. The area of skin necrosis on the leg was covered with split-thickness skin grafts. Infections occurring after soft tissue filler injections are related to biofilms, and treatment is sometimes difficult. Therefore, although soft tissue filler injections have a favorable safety profile, it is important to be aware of the risk of life-threatening complications.

Quantitative cone-beam computed tomography evaluation of hard and soft tissue thicknesses in the midpalatal suture region to facilitate orthodontic mini-implant placement

  • Objective: To identify the most favorable sites that optimize the initial stability and survival rate of orthodontic mini-implants, this study measured hard and soft tissue thicknesses in the median and paramedian regions of the palate using cone-beam computed tomography (CBCT) and determined possible sex- and age-related differences in these thicknesses. Methods: The study sample comprised CBCT images of 189 healthy subjects. The sample was divided into four groups according to age. A grid area was set for the measurement of hard and soft tissue thicknesses in the palate. Vertical lines were marked at intervals of 0, 1.5, and 3.0 mm lateral to the midpalatal suture, while horizontal lines were marked at 2-mm intervals up to 24 mm from the posterior margin of the incisive foramen. Measurements were made at 65 points of intersection between the horizontal and vertical lines. Results: The palatal hard tissue thickness decreased from the anterior to the posterior region, with a decrease in the medial-to-lateral direction in the middle and posterior regions. While the soft tissue was rather thick around the lateral aspects of the palatal arch, it formed a constant layer that was only 1-2-mm thick throughout the palate. Statistically significant differences were observed according to sex and age. Conclusions: The anterolateral palate as well as the midpalatal suture seem to be the most favorable sites for insertion of orthodontic mini-implants. The thickness of the palate differed by age and sex; these differences should be considered while planning the placement of orthodontic mini-implants.

연조직-골 모사 팬텀에서 피질골 두께에 대한 유도초음파 위상속도의 의존성 (Dependencies of phase velocities of ultrasonic guided waves on cortical thickness in soft tissue-bone mimicking phantoms)

  • 노화 및 골다공증으로 인해 긴 뼈에서 발생하는 피질골의 두께 변화는 골절의 위험인자로 알려져 있다. 본 연구는 200 kHz의 중심주파수 및 12.7 mm의 직경을 갖는 한 쌍의 트랜스듀서와 함께 축방향 전파법을 이용하여 윗면이 2 mm 두께의 실리콘 고무층으로 덮인(1 mm부터 4 mm까지의 두께를 갖는) 아크릴판으로 제작된 7개의 연조직-골 모사 팬텀에서 피질골 두께에 대한 유도초음파 위상속도의 의존성을 고찰하였다. 모든 연조직-골 모사 팬텀에서 서로 다른 속도를 갖는 First Arriving Signal(FAS) 및 Slow Guided Wave(SGW)가 전파하는 것으로 일관되게 관찰되었다. FAS의 위상속도는 피질골 두께가 증가함에 따라 약간 감소하는 반면 SGW의 위상속도는 피질골 두께가 증가함에 따라 크게 감소하는 것으로 나타났다. FAS 및 SGW의 위상속도는 각각 실리콘 고무층을 갖지 않는 아크릴판에서 전파하는 S0 및 A0 램 모드의 위상속도와 거의 일치하는 것으로 나타났으며, 이는 아크릴판의 윗면을 덮고 있는 연조직 모사 물질(2 mm 두께의 실리콘 고무층)의 존재가 위상속도 측정에 큰 영향을 미치지 않는다는 것을 의미한다.

  • When X-rays were projected into a patient, there occured the phenomena such as penetration, absorption and scattering etc. The penetrating rays were recorded on films as X-ray image used for diagnosis but scattered rays caused the radiation hazard both to the patient, specialist and technicians. The soft tissue includes many organs which are sensitive to the radiation and in may occupy $40{\sim}50%$ of body weight. Therefore X-rays should be carefully projected to the patient and it is strongly recommended to analyse the distribution of X-rays, when ever the patient is exposed to X-rays. In this study, the distribution of X-ray according to the thickness, the radiation field and the tube voltages (kVp) in soft tissue, the following results were obtained: 1. Total transmitted rays which kept the step with X-ray tube voltage (kVp) increased in proportion to the increasing of X-ray tube voltage. 2. The scattered ray rate in the total transmitted ray was not significantly found with X-ray tube voltage. 3. The affecting factors of the scattered ray rate in total transmitted ray were shown through the radiation field and the thickness. 4. The dose of scattered ray by the angle was observed more in direction of primary ray ($0^{\circ}$) and back scattering ($160^{\circ}$) than in direction of $90^{\circ}$. 5. The more the distance from phantom to the patient should be less distribution of scattered ray.

Effect of Epidermal Growth Factor with Collagen Matrix on Increasing Gingival Thickness: A Pilot Preclinical Investigation

  • Purpose: To investigate the effect of epidermal growth factor (EGF) with collagen matrix (CM) for increasing gingival thickness. Materials and Methods: In five mongrel dogs, bilateral gingival defects were surgically made on the maxillary canines. After two months, either a subepithelial connective tissue graft (group SCTG) or CM with EGF (0.1 ug/ml, group EGF) was grafted, and the flap was coronally positioned to cover the graft materials. The animals were sacrificed after three months. Intraoral scanning was performed for soft tissue analysis. Histologic and histomorphometric analyses were performed. Result: Two animals exhibited wound dehiscence during the healing phase, leaving three for analysis. No statistically significant difference was found in soft tissue changes (P>0.05). The level of gingival margin (GM) increased in both groups (1.02±0.74 mm in group SCTG vs. 1.24±0.83 mm in group EGF). Linear increases at the GM pre-augmentation in the soft tissue profile were 1.08±0.58 mm in group SCTG and 0.96±0.73 mm in group EGF. Histomorphometric parameters (keratinized tissue height, tissue thickness, and rete peg density) were not significantly different between the groups (P>0.05). Conclusion: EGF loaded onto CM led to comparable gingival phenotype enhancement to SCTG.

E국민학교생 6~11세 아동에 있어서 악안면 연조직의 성장변화에 관한 누년적 연구 (A Longitudinal Study of The Soft Tissue Profile Changes in 6 to 11 Years Children)

  • The purpose of this study is to evaluate the chronologic changes of the soft tissue profile in 6 to 11 years children. The author analyzed 174 cephalometric roentgenograms of 13 boys and 16 girls taken from the ages of 6 to 11 to assess the growth changes of the soft tissue profile. The results were obtained as follow: 1. Means, standard deviation and coefficient of variation of children were obtained. 2. Items which showed difference between males and females in longitudinal study were prn. Sn. A', and Ls that were greater than females variation. 3. With age, thickness of the soft tissue were increased except Ls. 4. Nasolabial angle and facial convexity had no significant change with age and sex.

혼합치열기에 있어서 악안면연조직에 관한 X-선학적 연구 (ROENTGENOGRAPHIC STUDY ON MAXILLOFACIAL SOFT TISSUE IN THE MIXED DENTITION)

  • The purpose of this study was to obtain the cephalometric maxillofacial soft tissue measurements, and to define the differences that exist between males and females on the soft tissue profile who had normal occlusion in mixed dentitions. For the object of this study, cephalometric radiographs were obtained from the centric occlusion with the relaxed lip position. Copper filter was designed to obtain both hard and soft tissue structure on the same film. The subjects consist of 100 males and 100 females from 8 to 12.4 years with the normal occlusion and acceptable profiles. The author measured facial depth, vertical height from the tissue profiles in the mixed dentitions. The significant test compare males with females. The following results were obtained from the study 1. In facial convexity, much more larger females than that of males. 2. Inclination of the lip posture were more larger in maxilla (male) and in females more larger in the mandible. 3. The thickness of soft tissue were thicker in males, the height of nose were more prominent in females. 4. There were no significant differences in both sexes.

전외측대퇴근막 유리피판술을 이용한 수부의 재건 (Reconstruction of hand using anterolateral thigh fascial free flap)

  • Purpose: The Provision of thin and pliable tissue and the adequate coverage of tendon - gliding surface is necessary for a soft tissue defect of the hand with exposure of bone, tendon and muscle. This report will discuss our experience with anterolateral thigh fascial free flap for the reconstruction of the soft tissue defect of the hand. Methods: Between February 2004 and August 2008, seven patients with full - thickness soft tissue defects of the hand were reconstructed by means of a composite anterolateral thigh fascial free flap. There were soft tissue defects associated with trauma (n=5), scar contracture (n=1) and necrosis due to ischemia (n=1). Flaps were harvested from the anterolateral thigh as adipofascial flaps with only a small sheet of fascia and fatty tissue above it. The fascia and the skin of the donor site was closed directly and delayed split - thickness skin graft was performed. Result: All flaps survived completely. The size of the transferred flap ranged from $2{\times}4cm$ to $5{\times}8cm$. Thin flap coverage was possible without secondary debulking operations. It left minimal donor site morbidity with a linear scar. In one case, the thigh muscle herniation in the donor site was developed. Conclusion: The anterolateral thigh fascial free flap provided thin and pliable tissue which can establish a tendon - gliding mechanism, minimal bulk, minimal donor site morbidity. The disadvantages of this technique were the need for a skin graft and the muscle herniation of donor site.

부정교합 아동의 성장에 따른 연조직 측모의 변화 (A LONGITUDINAL STUDY ON THE SOFT TISSUE FACIAL PROFILE CHANGES - Study report from 9 to 13 years of age -)

  • 연조직 측모의 연령증가에 따른 변화양상을 정확히 규명하고자 정상적인 안모 및 교합을 가졌다고 생각되는 9세 아동 남자 29명, 여자 26명을 대상으로 2년 간격으로 3차례에 걸쳐 얻어진 두부방사선 규격사진을 분석하여 다음과 같은 결론을 얻었다. 남녀별 각 계측항목의 연령에 따른 평균치 및 표준편차를 산출하였다. 연조직 측모의 평가에서 soft tissue facial angle, total facial convexity angle은 연령증가에 따른 약간의 변화를 보여 주었으며 기타 항목에서는 큰 변화를 보이지 않았다. 연조직 후경은 연령증가에 따른 증가를 보여 주었으며 두안부의 하안면부(point B, pog) 보다는 구순부(point A, LS, LI)에서의 증가량이 더 크게 나타났다. 안면고경의 바율은 GL'-Sn/Sn-Me' 1:1, Sn-St/St-Me' 0.51:1, Sn-LI/LI-Me' 0.82 : 1 로 연령증가에 따른 큰 변화가 없었다.
