A case report of Madelung's disease

  • Madelung's disease (MD) otherwise known as Launois-Bensaude syndrome, multiple symmetrical lipomatosis, or benign symmetric lipomatosis, is a rare disease characterized by abnormal diffuse lipomatosis in proximal upper limbs and neck. Here, we report a rare case of MD. A 66-year-old man presented with massive growth of soft tissues in the cervico-occipital region of more than 2 years duration. Physical examination showed diffuse enlargement of the anterior neck (Madelung's collar) and three huge humps at the posterior neck. Under a diagnosis of MD, lipectomy via a single anterior transverse incision and liposuction were performed. This rare case report may be helpful for assessing patients with abnormal diffuse lipomatosis in the neck and proximal upper limbs.

Fat Injection of Functional Velopharyngeal Insufficiency as the Supportive Treatment (기능성 연구개 인두부전증의 보조 치료로서의 지방 삽입술)

  • Background: The results of treatment in functional velopharyngeal insufficiency (VPI) was not good compared to physician's common practice. Objectives: Authors conducted this study to evaluate the efficacy of fat injection on posterior pharyngeal wall in the functional velopharyngeal insufficiency as the supportive treatment. Materials and Methods: The preoperative assessment includes history of patients, the perceptual analysis of patient's voice, nasopharyngoscopic finding of velopharyngeal movements, nasometer, movement findings of soft palate during phonation and swalling. Fat which was taken from umbilical area was injected in 5 patients with conducted functional velopharyngeal insufficiency. Results: All 5 patients had good results in voice quality after fat injection. Conclusions: Fat injection is a good treatment method in functional velopharyngeal insufficiency as a supportive method.

A Noninvasive Estimation of Hypernasality using Linear Predictive Model (선형 예측 모델을 이용한 비관혈적 과비음성 추정)

  • 연구개에 결함이 있는 사람의 발음은 부적절한 비음이 섞이게 되어 과비음성 비음이 되어 연구개를 복원해주는 시술을 하게 되는데, 과비음성 비음을 정량적으로 측정할 수있다면 시술 결과를 객관화 할 수 있게 된다. 현재 임상적으로 사용되고 있는 방법들은 관혈적이거나 고가의 장비를 필요로 한다. 본 논문에서는 비음의 특징인 스펙트럼에서 zero 의 존재와 비강에 의한 포만트의 존재 사실, 그리고 선형 예측 모델을 이용하여 마이크로폰과 사운드 카드가 장착된 PC로 구현할 수 있는 새로운 과비음성 비음 추정 알고리즘을 제안하였다. 음성 신호의 스펙트럼에 zero가 존재하는 경우, 낮은 차수(order)의 선형 예측 모델이 그 음성을 발음한 성도 시스템에 정확히 적용되지 않는다는 점을 이용하여, 같은 음성에 대한 높은 차수의 선형 예측 모델과의 차이를 이용해서 과비음성의 정량화를 시도했다. 본 논문에서는 제안된 알고리즘은 기존의 Teager Operator를 이용한 알고리즘에 비해서 Nasonmeter 의 측정결과와 더 높은 통계적 상관관계를 보여주었다.

Partial Mandibulectomy for Oral Cavity and Oropharyngeal Carcinoma (구강 및 구인두암의 수술에 있어서 하악골 부분절제술에 대한 고찰)

  • Segmental mandibulectomy causes severe functional disability and cosmetic problem. Many methods of reconstruction have been used but none of these procedures provides the ideal solution. There has been increased interest in the possibility of preserving a portion of the mandible and still carrying out a adequate tumor resection. We experienced four cases of marginal with sagittal mandibulectomy in patients with floor of mouth. buccal. and soft palate carcinoma, and sagittal mandibulectomy in 8 patients with tonsillar carcinoma and a case of marginal mandibulectomy in tonsillar cancer patient. In all cases mandible facing the tumor were successfully resected and repair of the mandible. postoperative facial contour were satisfactory. We think. that these sagittal procedures are oncologically sound in its removal of tumor. biomechanically secure and maximize postoperative rehabilitation while maintaining the normal contour of the face.

A Case of Acquired Nasopharyngeal Stenosis (후천성 비인두 협착증 1례)

  • Nasopharyngeal stenosis is an obliteration of the normal communication between the nasopharynx and the oropharynx resulting from the fusion of the tonsillar pillars and soft palate to the posterior pharyngeal wall. It is a rare but serious problem. The most common etiology is currently the surgical trauma associated with uvulopalatopharyngoplasty or adenotonsillectomy. It can range in severity from a thin band to a complete obstructing cicatrix, Symptoms vary from mild hyponasal speech to almost complete nasal obstruction with oral breathing, We present a case of a 16 year-old male with nasopharyngeal stenosis after radiofrequency-assisted adenoidectomy in this paper. This patient was managed by synechiolysis, obturator and buccal mucosal graft.

First Record of the Black Snoek Thyrsitoides marleyi (Pisces: Gempylidae) from Korea

  • A single specimen of Thyrsitoides marleyi (818 mm in standard length), belonging to the family Gempylidae, was collected for the first time in Korea from the coastal waters off Busan, Korea using longline with hooks on 27 January 2012. This species is characterized by the following morphological traits: palate with three fangs, dark dorsal spine membrane, ending part of soft dorsal and anal fin rays finlet-like, and tip of the jaw with a cartilaginous process. We suggest a new Korean name "Gal-chi-ggo-chi" for T. marleyi.

Spindle Cell Lipoma: A Rare, Misunderstood Entity

  • Spindle cell lipoma, a rare variant of lipoma, is a benign tumor found in the posterior neck and shoulder. A 24-year-old man with a close family history of malignant lymphoma had presented with a large, firm, nodular mass found in the right supraclavicular area. Excision of the deeply located mass revealed a pale yellow, rubbery nodule which grossly resembled an enlarged lymph node, with a variant of lymphoma as a primary suspect. However, pathological studies revealed the lesion to be a spindle cell lipoma. Although atypical in location, spindle cell lipoma should always be kept in differential diagnosis of a newly-noted soft tissue mass, as this entity may be easily cured by simple excision.

  • Oral appliance therapy is a simple, reversible way for improving snoring and/or obstructive sleep apnea. It may be indicated for the patients who are unable to tolerate continuous positive airway pressure (CPAP) or who have potential risks for surgical intervention. Oral appliance therapy increases airway space by the providing stable anterior positioning of the mandible, pulling out tongue, lifting up soft palate, or changing the muscle activity of the genioglossus. Currently, more than 80 different types of oral appliances have been introduced for snoring and/or obstructive sleep apnea. They are classified by their characteristics such as mode of action, adjustability and material used. This article provides a detailed clinical protocol and treatment procedure for oral appliance therapy.

Anatomical repair of a bilateral Tessier No. 3 cleft by midfacial advancement

  • Background: Bilateral Tessier number 3 clefts are extremely rare, and their surgical treatments have not been well established. Case presentation: The authors describe the case of a patient with a right Tessier number 3, 11 facial cleft with microphthalmia, a left Tessier number 3 facial cleft with anophthalmia, and cleft palate. We repaired simultaneously the bilateral soft tissue clefts by premaxillary repositioning, cleft lip repair, facial cleft repair by nasal lengthening, midfacial advancement, and an upper eyelid transposition flap with repositioning both the medial canthi. Postoperatively, the patient showed an esthetically acceptable face without unnatural scars. Conclusions: We achieved good results functionally and esthetically by midfacial advancement with facial muscle reposition instead of traditional interdigitating Z-plasties. The surgical modality of our anatomical repair and 3 months follow-up results are presented.

DISTRACTION OSTEOGENESIS IN PATIENTS WITH HEMIFACIAL MICROSOMIA (반안면 왜소증 환자에서의 골신장술)

  • Distraction osteogenesis is a technique of bone lengthening by gradual movement and subsequent remodeling. Distraction forces applied to bone also create tension in the surrounding soft tissues, distraction histiogenesis. Distraction osteogenesis is used to correct facial asymmetry, such as patients with hemifacial microsomia, maxillary or mandibular retrusion, cleft lip & palate, alveolar defect and craniofacial deficiency. Hemifacial microsomia is characterized by unilateral facial hypoplasia, often with unilateral shortening of the mandible and subsequent malocclusion. This report describes two cases of hemifacial microsomia(type IIB). In these two cases, distraction osteogenesis was used to correct a facial asymmetry. Two patients underwent unilateral mandibular distraction osteogenesis of ascending ramus of the mandible with extraoral devices. Successful distraction osteogenesis was achieved in the patients with hemifacial microsomia.
