Efficacy of Aloclair$^{TM}$ for Treatment of Intraoral Ulcer Caused by Orthodontic Appliance (교정장치에 의해 유발된 구내궤양 치료에 대한 Polyvinylpyrrolidone-sodium hyaluronate (Aloclair$^{TM}$)의 임상적 효과)

  • This experimental study was undertaken to evaluate the efficacy of polyvinylpyrrolidone-sodium hyaluronate(Aloclair$^{TM}$) in decreasing pain in orthodontic procedures. A total of 60 patients who were use Aloclair$^{TM}$ ordered to were included. These patients were categorized by 3 cases; 1. Patients who were bonded with their first brackets. 2. Patients who were in pain with additional appliance such as anchorage devices. 3. Patients who got ulcer caused by appliances. We collected information about pain and assessed it using visual analogue scale. According to questionnaire, it took 3.5 min. for pain to subside and pair relief durated for 2.7 hours. Most patients in the study reported rapid pain relief and found Aloclair$^{TM}$ easy and pleasant to use than wax. Accordingly Aloclair$^{TM}$ provided raid relief of resting and provoked ulcer pain.

Effects of Molecular Weights of Sodium Hyaluronate on the Collagen Synthesis, Anti-inflammation and Transdermal Absorption (히알루론산나트륨의 분자량 크기에 따른 Collagen 합성, 항염증 및 피부 흡수에 미치는 영향)

  • In this study, we examined the effects of various molecular weights (1, 10, 50, 100, 660, and 1500 kDa) of sodium hyaluronate (HA), which were prepared by enzyme hydrolysis, on the collagen synthesis, anti-inflammation and skin absorption. These HA did not significantly affect the viability of human dermal fibroblast Hs68 cells. Among them, 1500 kDa, 50 kDa HA most significantly increased collagen production by 59%, and 50% in the Hs 68 cells, respectively. Whereas 1500 and 660 kDa HA hardly pass through mouse transdermis membrane, lower molecular weights (1, 10, or 50 kDa) of HA showed time-dependent increase in skin permeation. HA of 50 kDa showed highest anti-inflammatory effects by reducing nitric oxide and tumor necrosis factor-${alpha}$ production in the RAW 264.7 cells, comparing to other HA (1, 10, and 100 kDa HA). Recently, there is no report about anti-wrinkle and anti-inflammatory effects and skin permeation of different molecular weights HA (1, 10, 50, 100, 660 and 1500 kDa), which were produced by enzyme hydrolysis. These results suggested that 50 kDa HA can be potent candidates for the development of effective anti-aging and anti-wrinkle cosmetic agents. The results of this study demonstrated that among those HA with different molecular weights, 50 kDa HA showed highest anti-inflammatory activity, significant capability to induce collagen synthesis and high level of skin permeation.

Effects of Intra-articular Injection of Ketorolac in Degenerative Arthritis of Knee (퇴행성 슬관절염에서 Ketorolac의 관절강내 주사 효과)

  • Background: Intra-articular injection is a commonly performed procedure in patients with degenerative osteoarthritis of the knee. Several drugs are used for relief of pain in such cases. Local anesthetics, clonidine and steroids have been confirmed to be effective when used in an intra-articular injection. Ketorolac has recently become one of the most commonly used and potent NSAIDs. There have been many studies about the effect of ketorolac. Methods: Sixty-four patients were divided into 2 groups. In Group I (n = 31), 0.5% bupivacaine 3 ml and sodium hyaluronate 20 mg were used and in Group II (n = 33), 0.5% bupivacaine 3 ml, sodium hyaluronate 20 mg and ketorolac 5 mg were used. We observed the pain relief scale (PRS) at 15 minutes, 1 week and 1-3 months after injection. Results: After 15 minutes, PRS scores were $6.6{\pm}2.7$ (Group I) and $5.1{\pm}3.1$ (Group II), so there was a statistical difference between the two groups. After 1 week, the PRS scores were $5.9{\pm}2.0$ (Group I) and $5.8{\pm}2.4$ (Group II). At 1-3 months later, PRS scores were $5.6{\pm}3.0$ (Group I) and $5.1{\pm}2.7$ (Group II). Thus, there were no significant statistical differences between the two groups at 1 week or 1-3 months later, although some patients were more satisfied with pain relief in Group II at 1-3 months. Conclusions: Ketorolac provides more rapid pain relief of degenerative arthritis when used in intra-articular injection. And there was no statistical difference of effect after 1 week or 1-3 months later. Further studies are required on the effect of the intra-articular use of ketorolac.

  • In this study, we tried to design and synthesize using natural polymers (hyaluronic acid and sodium alginate) and also to make some kinds of scaffolds as sponge type for reducing the burst effect of loaded drug from them. Photo-dimerizable group was incorporated to hyaluronic acid and degradable hydrogel was prepared by the UV radiation of the polymer. The pore size and its distribution of scaffold were controlled by changing microsphere production conditions such as solution concentration and spraying pressure. It was found that drug release behavior from synthesized scaffolds was affected by hybridization of two naturally originated polymers (cinnamoylated tetrabutylammonium hyaluronate: CHT and cinnamolylated sodium alginate: CSA) and the obtained scaffolds were degraded in fairly long time (about 2 months) under in vitro environment. Therefore, we expect that obtained scaffolds can be applicable for the tissue regeneration scaffolds in the fields of orthopaedic surgery.

Characterization of Cross Linked Hyaluronic Acid Microbeads by Divinyl Sulfone (Divinyl Sulfone으로 가교된 히알루론산 마이크로비드의 특성평가)

  • Hyaluronic acid(HA) microbeads were synthesized by dropping the sodium hyaluronate(Streptococcus) solutions in NaOH into a solution mixture of divinyl sulfone(DVS) in 2-methyl-1-propanol, followed by stirring, cleaning and drying process at room temperature. The initial experimental conditions are crosslinking time(CLTi) of 5 h, crosslinking temperature(CLTe) of room temperature, injection air pressure(IAPr) of 5 psi, and DVS concentration( DVSc) of 0.2 vol%, respectively. Then, parametric studies were performed by varying the parameters to investigate the morphology, the porosity, the swelling ratio and the size of the beads. The microbead size pattern was not regular to function of the degree of crosslink. It was observed that the swelling ratio, the degree of crosslink, and the pore size can be controlled by adjusting the CLTi, CLTe and DVSc. Among the parameters investigated, the smallest bead size can be achieved by varying the CLTi parameter. The lowest swelling ratio, as an indication of the highest degree of crosslink, can be obtained by varying CLTe.

Dependence of Crosslinking Temperature on Swelling Behavior of Hyaluronic Acid Porous Microbeads Synthesized by a Modified Spray Method (노즐 낙하법으로 제조한 히알루론산 다공성 마이크로비드의 가교온도에 따른 팽윤특성)

  • Hyaluronic acid (HA) microbeads were synthesized by dropping 0.5 wt% of sodium hyaluronate dissolved in NaOH into 0.2 vol% of divinyl sulfone dissolved in 2-methyl-1propanol at a speed of 0.005 ml/min. HA microbeads were collected from a divinyl sulfone crosslinker solution stirred at 200 to 400 rpm for 5 h at temperatures from room temperature to $60^{\circ}C$ at intervals of $10^{\circ}C$. The crosslinked microbeads were then cleaned thoroughly using distilled water and ethanol. SEM results revealed that the microbeads were white-colored spheres. The 3-D porous network structure of the microbeads became dense with an increase in the crosslinking temperature; however, no dependence of the crosslinking temperature on the microbead size was detected. The extent of swelling decreased from 970% to 670% with an increase in the crosslinking temperature from room temperature to $60^{\circ}C$, most likely due to the increase in the degree of crosslinking.

  • The liposomes have been developed in many drugs and cosmetics fields. The liposomes prepared with main compounds of the intercellular lipids and lecithin. Amphiphile nonionic surfactants used for (PEG) n-sitosterol(n=5), diethanolamine cetylphosphate. The effect of gelation for liposomes have been on swelling reaction which have been mixed phospholipid with polyol-group at the high temperature. There were very good encapsulated properties of the active ingredients whether hydrophilic-group(magnesium ascorbyl phosphate, allantoin, sodium hyaluronate) and hydrophobic-group(vitamin-E acetate, vitamin-A palmitate). Optimum condition of liposomes were passed five times in the microfluidizer(700bar), wetting reaction temperature was at $95{\pm}5^{\circ}C$ for a hours. Particle size distribution of the vesicles should be within range 50-560nm(mean 200nm). The stability of liposomes for the course of time was stabilized for six months at $45^{\circ}C$. Application of the cosmetic was prepared moisturizing cream with liposomes of the phospholipid base.

The Effect of Alloderm on Prevention of Adhesions following Tenorrhaphy in the Rabbits (알로덤이 건 봉합술 후 발생되는 유착 방지에 미치는 효과)

  • Purpose: Peritendinous adhesion is one of the most notorious complication after the flexor tendon injury. In this study, $Alloderm^{(R)}$(LifeCell Corp., Branchburg, N.J.), which is the decellularized human dermal analogue with its intact native basement membrane components, was used for the prevention of peritendinous adhesions following flexor tendon repair. Methods: Thirty New Zealand white male rabbits were divided equally into 3 groups. In all groups, the flexor digitorum profundus of the third finger of the right back foot was cut totally and repaired by modified Kessler suture technique. Following tendon repair, $Alloderm^{(R)}$ was wrapt around the repaired tendon in the first group and sodium hyaluronate gel was sprayed to the operation field in the second group. In the control group, no external material was applied. The right back foot were immobilized for 6 weeks to optimize the formation of adhesion ingrowth. After death, the third finger that repaired tendons and sheaths was removed en bloc. We checked range of motion. and studied histologically for all groups. Results: The experimental groups had better range of motion than the control group. We checked that the range of motion was 73.5 degrees in $Alloderm^{(R)}$ group, 55.9 degrees in the hyaluronic acid group, and 38.3 degrees in the control group. in the histological study, the experimental group had less adhesions compared with the control group. Conclusion: This study concludes that $Alloderm^{(R)}$ can decrease peritendinous adhesions following flexor tendon repairs in rabbits. We think the method could be used in clinical cases.

New Anti-aging & Moisturizer Ingredients of Exopolysaccharides by Grifola frondosa

  • In this study, in an attempt to search for functional cosmetic ingredients from higher fungal, we have produced exopolysaccharides (GF-l, approximately carbohydrate 75%, protein 25%) and polysaccharide (GF-2) of mycelium extract, by submerged culture of Grifolafrondosa. For applications in anti-aging cosmetic field, we investigated the diverse biological activities. Antioxidant activity and inhibition of Matrixmetalloproteinases (MMPs) were investigated enzymatic assays by measuring the superoxide scavenging activity using xanthine-xanthine oxidase system and the proteolytic activity of MMPs using EnzChek Collagenase/Gelatinase kits, respectively. GF-l polysacchairde showed inhibition of superoxide radical by 90% at a concentration of 0.2% (w/v) and inhibition of collagenase by 45% at 0.2% (w/v). GF-2 polysaccharide of mycelium extract also exhibited good antioxidant activity. However, MMPs inhibition activity was relatively lower level compared to GF-l polysaccharides. The treatment of human dermal fibroblast (HDF) with GF-l and GF-2 polysaccharides increased the proliferation of fibroblast by approximately 23-25% at a concentration of 0.5% (w/v), also showed collagen synthesis increase in HDF by about 50% at 0.5% (w/v) compared to that of untreated control. We also report the moisturizing effects of polysaccharides in cosmetic products (O/W emulation) and its own ingredient, in vitro and in vivo. The GF-1 polysaccharide showed higher moisturizing ability than sodium hyaluronate, which is the most commonly used moisturizers ingredient. These results suggest the GF-l polysaccharide, protein-bound polysaccharide, may be used as an ingredient for new moisturizing and anti-aging cosmeceuticals.
