규칙적 신체활동을 수행하지 않는 고령자의 주관적 건강평가에 영향을 미치는 요인 (Determinants of Self-rated Health in Sedentary Older Adults)

  • Purpose: The objective of this study were to examine the determinants of self-rated health, specially focused on the effect of functional capacity of community dwelling sedentary older adults on self-rated health. Method: The data has been collected from 654 community-dwelling sedentary older adults (mean age: 75 years) during the period from April to June in 2007. The data were collected by the in-person interview and direct measurement of functional capacity. The data were analyzed using chi-square test and multiple regression analysis with the SPSS 9.1 program. Result: The elderly rated their health as very good (3%), good (28%), fair (38%), poor (29%0, and very poor (2%). The higher average daily walk minutes ($\beta$=0.12, p<.01), number of chair stand ($\beta$=0.10, p<.05), scores of self-efficacy ($\beta$=0.16, p<.001) and the lower number of disease ($\beta$=-0.44, p<.001) show better self-rated health. Conclusion: Self-rated health is the most commonly used indicators in social epidemiology and geriatric research because it has been known as the good predictor of mortality and reflects health related disability. The finding suggested that daily walking habits, lower body strength, physical self-efficacy should be considered to improve the senior's self-perception of health. The community-based intervention associate increase these factors should be considered.

Drinking Patterns Among Korean Adults: Results of the 2009 Korean Community Health Survey

  • Objectives: In Korea, the proportion of deaths due to alcohol is estimated at 8.9%, far exceeding the global estimate of 3.8%. Therefore, this study was performed to examine the factors associated with low-risk, moderate-risk, and high-risk drinking patterns in Korean adults and to identify target populations for prevention and control of alcohol-related diseases and deaths. Methods: We analyzed data from 230 715 Korean adults aged 19 years and older who participated in the 2009 Korean Community Health Survey. Multinomial logistic regression analysis was used to examine associations between socio-demographic and health-related factors and patterns of alcohol use. Results: A substantially larger proportion of men than women engaged in high risk (21.2% vs. 3.4%) and moderate-risk alcohol use (15.5% vs. 8.2%). In both sexes, moderate- and high-risk uses were associated with younger age, higher income, being currently employed, smoking, being overweight/obese, and good self-rated health. Conclusions: Given the large proportion of the population that is engaging in moderate- and high-risk drinking and given the social norms that support this behavior, public health policies and campaigns to reduce alcohol consumption targeting the entire population are indicated.

The Prevalence and Risk Factors for Diabetic Retinopathy in Shiraz, Southern Iran

  • Globally, diabetic retinopathy (DR) is one of the leading causes of blindness, that diminishes quality of life. This study aimed to describe the prevalence of DR, and its associated risk factors. This cross-sectional study was carried out among 478 diabetic patients in a referral center in Fars province, Iran. The mean${\pm}$standard deviation age of the participants was $56.64{\pm}12.45$ years old and DR prevalence was 32.8%. In multivariable analysis, lower education levels (adjusted odds ratio [aOR], 0.43; 95% confidence interval [CI], 0.24 to 0.76), being overweight (aOR, 1.70; 95% CI, 1.02 to 2.83) or obese (aOR, 1.88; 95% CI, 1.09 to 3.26), diabetes duration of 10 to 20 years (aOR, 2.35; 95% CI, 1.48 to 3.73) and over 20 years (aOR, 5.63; 95% CI, 2.97 to 10.68), receiving insulin (aOR, 1.99; 95% CI, 1.27 to 3.10), and having chronic diseases (aOR, 1.71; 95% CI, 1.02 to 2.85) were significantly associated with DR. In conclusion, longer diabetes duration and obesity or having chronic diseases are strongly associated with DR suggesting that control of these risk factors may reduce both the prevalence and impact of retinopathy in Iran.

Developing a Job Exposure Matrix of Work Organization Hazards in the United States: A Review on Methodological Issues and Research Protocol

  • Background: Most job exposure matrices (JEMs) have been developed for chemical and physical hazards in the United States (US). In addition, the overall validity of most JEMs of work organization hazards using self-reported data in the literature remains to be further tested due to several methodological weaknesses. Methods: This paper aims to review important methodological issues with regard to a JEM of work organization hazards using self-report data and to present a research protocol for developing a four-axis (job titles, hazards, sex, and time) JEM of major work organization hazards using the US General Social Survey-Quality of Work-Life (GSS-QWL) data (2002-2018; N = 7,100 workers). Results: Five methodological weaknesses in existing JEMs of work organization hazards using self-report data were identified: having only two axes (hazard and occupation), using psychometrically weak items and scales, including scales having little interoccupational variability, unresolved optimal minimum numbers of subjects per occupation, and low accessibility. The methodological weaknesses were successfully addressed in the proposed research protocol. Conclusion: The work organization JEM to be developed will significantly facilitate and strengthen occupational epidemiological studies on work organization hazards and major health outcomes, improve national and occupational surveillance of work organization hazards, and promote interventions for a healthy work environment in the US.

The role of nutritional status in the relationship between diabetes and health-related quality of life

  • BACKGROUND/OBJECTIVES: The association between nutritional status and health-related quality of life (HRQoL) among patients with type 2 diabetes mellitus (T2DM) is not fully understood. This study was conducted to understand the role of nutritional status on HRQoL among people with and without T2DM. SUBJECTS/METHODS: Structured survey and direct measurement of anthropometric data were conducted among people with and without T2DM. Nutritional status was measured with Mini Nutritional Assessment tool and HRQoL was measured with a 36-item Short Form Healthy Survey. Data collection was conducted in Chuncheon, South Korea with 756 participants who are older than 40 yrs of age. RESULTS: This study found that overall HRQoL were significantly lower in people with T2DM than people without T2DM after controlling for key covariates. When stratified by nutritional status, a greater degree of negative impact of T2DM on overall physical HRQoL was observed among well-nourished or at risk of malnutrition, whereas significant and more evident negative impact of diabetes on overall psychological HRQoL was observed only among malnourished. CONCLUSIONS: The study results suggest the role of nutritional status among people with T2DM on overall, especially psychological aspects of HRQoL. Future longitudinal or intervention studies are warranted to test the impact of nutritional status on HRQoL among people with T2DM.

보건학 석사과정 이수자들의 제특성 및 사회활동에 관한 조사연구 (A Study on the Characterics and Social Activities of Graduates from A School of Public Health in Seoul)

  • The purpose of the present study was to identify the charateristics and social activities of graduates from a School of Public Health in Seoul and to provide basic information which might contribute to the improvement of new established Schools of Public Health in Korea. The Subjects were composed of 199 persons who graduated from a School of Public Health in Seoul during the period of 1987 through 1989. The response rate was 59.8% (199 graduates). The survey was conducted from December 1st 1991 to the end of February 1992 with a self-administered questionnaire by mail. Descriptive analysis with Chi-square test was employed for analysis. The major results were as follows : 1. 462% of students were the group aged 24-28 and 76.5% of them were married at the time of entrance to the School. 2. The major motives to come to the School were continuous study in a graduate school(41.2%), and strengthening of job opportunities(35.3%). 3. The major expectation of students were to acquire new professional knowledge(31.9%), to improve present work performance(22.7%) and to become an educator(22.7%). 4. 84.9% of students had some kinds of expectation at the entrance to the School, but only 48.8% of them met their expectations they had had. 5. The most preferred subjects were Health Statistics and Epidemiology in order. 6. 59.7% of students insisted that a professor should advise 4-5 students in maximum for a thesis. 7. Students suggested that the candidate for scholarship should he selected from those who had earned high credits in the previous semester(33.6%), who were low economic status(31.l%) or who were recommended by academic adviser(24.4%). 8. 57.1% of students were positive in utilizing their major subjects studied in their current works. 9. Students expressed that if the School of Public Health is to be further developed full-time professors should be added (37.9%) and excellent students be secured (33.6%). 10. After graduation from the school, some of the respondents changed their job placement, the number of persons working in research institutes increased from 6 to 14 persons, those working in educational institutes from 16 to 35, and Ministry of Health and Social Affairs and its affiliated organization from 10 to 13.

가축질병 방역활동 지원을 위한 국가동물방역통합시스템 시공간 정보 활용 (Utilizing Spatial and Temporal Information in KAHIS for Aiding Animal Disease Control Activities)

  • 고병원성 조류인플루엔자(Highly Pathogenic Avian Influenza: HPAI)는 전염력이 매우 높아 발병 시 전국적으로 확산되어 막대한 사회경제적 피해를 유발하는 제1종 법정가축전염병이다. 국가동물방역통합시스템(Korea Animal Health Integrated System: KAHIS)은 축산차량등록제를 통하여 축산관련차량의 축산관련시설 출입정보를 실시간으로 기록함으로써 가축질병 발생 시 신속한 방역업무를 지원하기 위하여 구축한 통합시스템이다. KAHIS는 가축질병 발생 시 차량이동에 의한 확산경로를 효과적으로 추적하기 위한 시공간 정보를 포함한다. 이러한 시공간 정보를 바탕으로 시설들 간 연결 관계를 나타내는 접촉 네트워크를 구축할 경우 가축질병의 시공간 확산과정 추론에 유용하게 활용할 수 있다. 본 연구의 목적은 가축질병 방역활동 지원을 위하여 KAHIS에 내재된 시공간 정보를 접촉 네트워크로 구축하여 일차적 활용방안을 실증적으로 제시하는 것이다. 구축한 접촉 네트워크를 토대로 가축질병의 확산이 이루어지는 잠재적 시공간적 권역을 추정함으 로써 차단 방역 관련 의사결정을 돕기 위한 분석정보를 도출하고 해석을 통하여 정책적 함의를 제시하였다. 아울러 좀 더 고도화된 의사결정 지원정보 도출을 위한 접촉 네트워크의 심층적 활용 방안을 제시하였다.

Women's Knowledge, Attitudes, and Practices about Breast Cancer in a Rural District of Central India

  • Background: Breast cancer accounted for almost 25% of all cancers in women globally in 2012. Although breast cancer is the most prevalent cancer in India, there is no organised national breast cancer screening programme. Local studies on the burden of breast cancer are essential to develop effective context-specific strategies for an early detection breast cancer programme, considering the cultural and ethnic heterogeneity in India. This study examined the knowledge, attitudes, and practices about breast cancer in rural women in Central India. Materials and Methods: This community-based cross sectional study was conducted in Wardha district, located in Maharashtra state in Central India in 2013. The sample included 1000 women (609 rural, 391 urban) aged 13-50 years, selected as representative from each of the eight development blocks in the district, using stratified cluster sampling. Trained social workers interviewed women and collected demographic and socio-economic data. The instrument also assessed respondents' knowledge about breast cancer and its symptoms, risks, methods of screening, diagnosis and treatment, as well as their attitudes towards breast cancer and selfreported practices of breast cancer screening. Chi-square and t-test were applied to assess differences in the levels of knowledge, attitude, and practice (the outcome variables) between urban and rural respondents. Multivariable linear regression was conducted to analyse the relationship between socio-demographic factors and the outcome variables. Results: While about two-thirds of rural and urban women were aware of breast cancer, less than 7% in rural and urban areas had heard about breast self-examination. Knowledge about breast cancer, its symptoms, risk factors, diagnostic modalities, and treatment was similarly poor in both rural and urban women. Urban women demonstrated more positive attitudes towards breast cancer screening practices than their rural counterparts. Better knowledge of breast cancer symptoms, risk factors, diagnosis, and treatment correlated significantly with older age, higher levels of education, and being office workers or in business. Conclusions: Women in rural Central India have poor knowledge about breast cancer, its symptoms and risk factors. Breast self-examination is hardly practiced, though the willingness to learn is high. Positive attitudes towards screening provide an opportunity to promote breast self-examination.

의학교과서를 통해 본 북한예방의학 내용분석에 관한 연구 (The State of the Art of Preventive Medicine in North Korea With Reference to the Content Analysis of a Medical Textbook)

  • Objectives : The most frequently cited health related slogan in North Korea is that socialistic medicine is preventive medicine. It implies that North Korea puts preventive medicine at the operational center of its national health care system. This study aimed at examining and comparing preventive medicine practices in North Korea with those of South Korea. Methods : Efforts have been made to obtain a textbook for analysis its contents. Many people have iassisted in the study by joining the interview. Some of these people are as follows : a former professor of PyongYang Medical School, NK physicians living in South Korea, WHO staffs, diplomatic officials, etc. The major items of analysis consisted of industrial medicine and hygiene, nutrition, school health, epidemiology, health statistics and heath policy & management. Results : Public health philosophy is finely noted and well integrated in the operation of the North Korean national health care system, particularly in the area of industrial medicine and hygiene. Preventive medicine with a strong health surveillance system spanning a number of broad social organizations is a major tool to improve the health of the people in North Korea. The emphasis on preventive medicine has a close relationship with the 'Juche Philosophy' and the shortage of pharmaceuticals and medical equipment. To cope with the shortage problem, North Korean health workers are encouraged to grow medicinal herbs. We have found that they put little effort into teaching newly emerging diseases, such as AIDS, VDT syndrome, hazards of EMF, and agricultural chemical poisonings. Of the subjects of the preventive medicine text, 78.9% coincide with those of South Korean industrial health manuals and 34.2% with South Korean epidemiology texts. However, an absolute difference was found to exist between the health policies and management systems. Conclusion : In North Korea, the concept of preventive medicine functions as the basic philosophic strategy of the national health care system. It differs greatly from the South Korean system in both practice and educational content. Its contribution to society is simply incomparable to that of South Korea. More communication and further study is called for in order to improve the preventive medicine practices in the future.

2009년 국내 응급실 중독환자 다기관조사: 두 번째 연차보고 (Multicenter Survey of Intoxication Cases in Korean Emergency Departments: 2nd Annual Report, 2009)

  • Purpose: The purpose of this study was to examine the occurrence of toxic exposure cases in Korean emergency centers using a toxic exposure surveillance system-based report form and to provide guidelines for the prevention and treatment of toxic exposures. Methods: We retrospectively reviewed the medical records of toxic exposure patients who had visited emergency centers from January 2009 to December 2009. Epidemiology data points for the toxic exposure cases included age, gender, type of exposure, number and kind of substances involved, reason and route of poison exposure, management of the patients in the emergency departments, and the clinical outcome. Results: A total of 3,501 patients from 12 emergency departments were enrolled in the study. 50.0% of the total exposure patients were male and 63.0% of the total cases were fatal. Acute intoxication occurred in 91.3% of the total patients and suicidal intent was the most common (43.3%) reason for exposure. The most common route of exposure was ingestion (75.9%). Of the total cases, pesticides were involved in 26.3%, sedatives/hypnotics/antipsychotics were involved in 22.0%, and bites and envenomations were involved in 15.7%. Conclusion: We provided a database of patients who were admitted to emergency departments after poisoning incidents. We recommend that toxicology professionals develop a classification scheme for toxicants which is adequate for Korean domestic circumstances and initiate a toxic surveillance system for all types of exposures. With support of a psychiatric surveillance system for suicidal patients and establishment of social mediation for pesticide poisoning, major reductions in poison exposures can be achieved.
