Objectives : This study was to examine the satisfaction level of participants in a oral exercise program geared toward the improvement of oral function, their reuse intention of the program and their willingness of recommendation in an effort to facilitate the advancement of the program. Methods : The subjects in this study were 85 senior citizens who used senior welfare centers and nursing homes in Seoul. A oral exercise program was implemented 24 times from April to June 2009, twice a week, and the selected elderly people participated in the program 20 times or more. An interview survey was conducted by two trained dental hygienists, and the collected data were analyzed. Results : 1. As for satisfaction level with each part of the program, the elderly people investigated expressed the best satisfaction with the service supplier(a mean of 3.81), followed by the usefulness of the procedure of the program(3.77) and oral exercise(3.64). 2. The satisfaction level with oral exercise had a significant impact on their overall satisfaction with the program, reuse intention and willingness of recommendation, and the entire satisfaction level with the program was correlated to reuse intention and willingness of recommendation(p<0.001). 3. Out of the demographic characteristics, marital status and whether to live with their families or not made a significant difference to their satisfaction level with oral exercise(p<0.05). Conclusions : It took 20 minutes to get oral exercise, which consisted of warming-up, exercise for each part of oral and cooking-down, and they found it appropriate to spend that time and had no difficulties in repeating after the instructor.
The medical expenses review system in Korea has developed under fee-for-service system with its own unique structure. The importance of reviewing medical expenses has been emphasized, as the size of medical expenditures moving through the health insurance legal context and its weight in the national economy have increased very rapidly. It is, however, analyzed that the feuds and arguments continue among the stakeholders for the lack of laws supporting the medical expenses review system. The medical expenses review is a series of administrative procedures, deciding whether claims from medical care institutions to the insurer are legal and valid or not. It mainly controls the increase of unnecessarily excessive health insurance claim and prevents fraudulent claim and abuse and checks the less use or unsuitable use of medical resources. It also works a function guarantees medical benefits for the appropriate treatment according to the object of health insurance system as a social insurance scheme. The dispute on legal base of the medical expenses review is about the source of law in the medical expenses review. There are the Health Insurance Act and administrative laws as jus scriptum and the guidelines of review as administrative orders. The medical expenses review should reflect various factors, such as the development of medical healthcare technologies, the health expenditures distribution, the financial situation of the health insurance, and the evaluation on the level of appropriate benefits. It is also likely to adapt to the traits of characters of medicine, and trends and transition, Besides it should judge the legality and the validity of medical benefits expenditures by synthesizing these all factors. And the evaluation system of appropriateness of medical benefits was administrative procedure which was consecutive with reviewing the medical expenses system and it was intended to make up for the result of reviewing the medical expenses in more comprehensive levels.

Objective: Using biofeedback in instructing pelvic floor muscle (PFM) activation is a great method to provide information on muscle contraction. This study aimed to determine the immediate effectiveness of a mobile game-based instruction with an extracorporeal biofeedback device (EBD) to improve PFM contraction in healthy subjects. Design: Cross-sectional study. Methods: Sixteen healthy subjects (4 men and 12 nulliparous women; age, 31±5 years) were enrolled. The subjects were randomly categorized into two groups, those who were to receive instructions on how to contract the PFM by using the EBD (n=7) and those who were to undergo a transabdominal ultrasonography (TAUS; n=9) with biofeedback. PFM function was measured as the displacement (mm) of PFM elevation by using the TAUS before and after the instructions in each group. Results: The EBD and TAUS groups showed a significant increase in the displacement of PFM elevation after the instructions from 5.93±4.03 mm to 7.62±3.77 mm and from 5.27±3.39 mm to 7.47±2.79 mm, respectively (p<0.05). No significant differences were found between the two groups. Conclusions: The results of this study indicated that instructions for PFM contraction using the EBD and TAUS showed an immediate effect; however, no significant difference in effectiveness was observed between the two instruction methods. Therefore, not only can the TAUS method be used but the EBD method can also be used as a PFM instruction method for noninvasive procedures. However, further studies are needed to demonstrate the effectiveness of training and exercise on larger sample sizes that includes patient populations with PFM dysfunction.

Objective: This study aimed to compare muscle structure, balance, and gait parameters between healthy adults and persons with stroke and to analyze the correlation among these variables. Design: Cross-sectional study. Methods: Twenty persons with stroke (11 male, 9 female) and twenty healthy participants (9 male, 11 female) were included. Ultrasound images of the triceps surae and the tibialis anterior were acquired in sitting resting, sitting co-contraction, and standing resting positions and also during the functional reach test (FRT) and single leg anterior reaching test (SLART). Muscle thickness and fascicle length were measured. Spatiotemporal parameters of gait were measured using a pressure walkway. Gait speed, cadence, step length, stride length, stance time, and swing time were measured. Results: Changes in percent fascicle length were significantly greater in the gastrocnemius and soleus (SOL) muscles of healthy adults in the sitting co-contraction position (p<0.05). The percent fascicle length of the SOL in FRT and SLART were significantly greater in healthy adults (p<0.05). The mid-stance phase of stroke patients was shorter than healthy adults (p<0.05). A negative correlation was observed between percent fascicle length of the SOL in the sitting co-contraction position and the proportion of the mid-stance phase (p<0.05). Conclusions: The function of the triceps surae is affected in persons with stroke when compared with healthy adults. This can lead to difficulty in performing tasks that involve forward transfer of weight. If the triceps surae is not sufficiently secured, the possibility of compensation in the stance phase increases during gait.

The purpose of this study is to investigate the effects of work-family conflict and spousal support resources on parenting stress among working mothers with pre-school children. This study involved a total of 232 working mothers with children enrolled in child care centers in Seoul and Gyeonggi-do as conducting a survey. First, the higher the work-family conflict (inter-role conflict caused by the impact of work on family life), the higher the family-work conflict (inter-role conflict caused by the impact of family life on work). Moreover, the lower the level of spousal support resources (emotional spousal support resources, instrumental spousal resources), the higher the work-family conflict and family-work conflict. The level of parenting stress was higher in working mothers experiencing greater work-family conflict and family-work conflict both and lower levels of spousal support resources for parenting. Second, the number of children and the amount of work-family conflict and instrumental spousal support resources had a statistically significant impact on parenting stress. Thus, the higher the number of children, the greater the work-family conflict, and the lower the level of instrumental spousal support resources, the higher the level of parenting stress. The findings of this study highlight the relative effects of role conflicts between work and family and of spousal support resources on parenting stress in working mothers. Husbands' being involved in parenting, in particular, was identified as an important support function for healthy child-rearing by successfully combining career and family responsibilities.
Purpose: Hemiplegia patients who were attacked by a stroke suffer from hemiplegic disabilities as well as motor disabilities. For them, rehabilitation cure is being carried out broadly. However, it is not enough for them to use the upper extremity than the lower extremity. For the use of the upper extremity, we examined the effect of constraint-induced movement therapy developed in this research on patients who experienced a stroke following hemiplegia. Method: For this study we selected 36 stroke patients who were registered at the community health center through accidental sampling, and assigned 21 of them to the experimental group, and 15 to the control group. The experimental group had constraint-induced movement therapy for 5 days and 7 hours a day from 9 to o'clock in the morning 9 to 4 o'clock in the afternoon 4 including warmup exercise and main exercise in the rehabilitation room, whereas the control group were restricted. Result: As a result of constraint-induced movement therapy, affected side elbow joint flexion range, side shoulder joint extension range and side shoulder joint of the flexion range of motions increased obviously in the experimental group compared to those in the control group. Conclusion: The result above clearly shows that constraint-induced movement therapy is an effective intervention for the rehabilitation of hemiplegia patients in increasing affected side elbow joint of the flexion range of motion, the shoulder joint extension, and the increase of flexion range of motion.
Objective: The purpose of this study was to investigate the effect of the hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on the clinical symptoms and the treatment effects in balance, sit to stand, and gait abilities in patients with TBI. Design: A single case study. Methods: A 13-year-old adolescent with quadriplegia and hip joint control impairment participated in this four-week training intervention. The patient, diagnosed with TBI, wastreated with hip joint strengthening exercises using PNF. In the first week, we focused on strengthening the body, relaxing the hip flexors and activating the hip extensor muscles in order to solve the patient's physical function and body structure. From the 2nd and 4th week, we improved the motivation through the task-oriented method, and then weight-bearing training of the right lower extremity was proceeded by kicking a soccor ball with the left lower extremity. The exercises were performed for 4 weeks, 5 days a week, for 60 minutes with the exercise intensity gradually increased according to the subject's physical abilities. Results: As a result of the study, the patient demonstrated improvements in the physical examination, which were evaluated before and after intervention and included the manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5-time sit to stand test, and the 10 meters walk test. Conclusions: The results of this case suggest that a hip joint strengthening exercise program using PNF may improve hip control ability, balance, sit to stand and gait ability in a patient with TBI.
Recent studies have investigated anger or anger-related problems in adolescents from various perspectives. However, understanding of anger expression types or the role of mediating variables in relation to anger and anger expressions have been disregarded. The purpose of this research was to understand the relationships between adolescents' anger, problem solving ability, and three types of anger expressions (anger-in, anger-out, anger-control) and to focus on the mediator function of problem-solving ability. Participants comprised 596 adolescents (283 males and 313 females). Anger was significantly related to all three types of anger expressions. Adolescents who showed high anger levels tended to indicate high anger-in and anger-out expressions but low anger-control expression, while controlling for age and gender. The findings also provided evidence for the mediator role of problem-solving ability on the relationship between anger and anger-control expression. From the results of this study suggestions and limitations were also discussed.
This study is related with support system plans of public-private convergence for emergency recovery services to fire victims. The current issues are analysed from the condition and problem analysis results of emergency recovery activities for fire victims. And building plans of an integrated support system based on the government and private sector cooperation are proposed. Overall, the system shows that the government is responsible for the functionality of management and supervision for the system operation, and the private is responsible for the functionality of specialized agencies for practical emergency recovery. Consequently, support system plans include with infrastructure maintenance of laws and regulations, contracting-out of the support system, stable supply of operating budget and function strengthening of public-private convergence for emergency recovery. Especially, plan for stable supply of operating budget in detail including with establishment of the fund and self-financing composition of private corporations for emergency recovery services is proposed. The results of this study will lead strengthening of fire safety welfare and social safety nets at future.

The aim of this study is to suggest an improvement of BTL in Educational Facilities applying the concept of n-BPPP(non-Benefits Public-Private Partnerships). The research is based on four of ninety-nine completed projects of Mixed-use Educational Facilities since 2001. Ninety-nine projects are analyzed and categorized to the typology of architectural program. The architectural programs planned are uniform in function, such as gym, swimming pool, library, info-center, parking etc. The public parking is a main program in the BTL projects, where district offices are involved as partner. The difference between BTL and n-BPPP for educational facilities is the business units. The n-BPPP concerns in regional or district units in order to expand public infrastructure facilities based on the network of schools and local community. And the fund for n-BPPP does not come from the government but from the investors. The economic interest is to reinvest on the maintenance of the facilities. The benefits of n-BPPP will be not only for the Governments in terms of social welfare budgets but also for local residents and students in terms of a variety of high-quality public services. The concept of n-BPPP can be an alternative way for the Mixed-use Educational Facilities.
