남성 근로자의 재흡연에 관련된 요인 (Factors Affecting Re-smoking in Male Workers)

  • Objectives: This study was performed to examine the factors affecting re-smoking in male workers. Methods: A self-administrated questionnaire survey was conducted during April 2003 to examine the smoking state of 1,154 employees of a company that launched a smoking cessation campaign in1998. Five hundred and eighty seven persons, who had stopped smoking for at least one week, were selected as the final study subjects. This study collected data on smoking cessation success or failure for 6 months, and looked at the factors having an effect on re-smoking within this period. This study employed the Health Belief Model as its theoretical basis. Results: The re-smoking rate of the 587 study subjects who had stopped smoking for at least one week was 44.8% within the 6 month period. In a simple analysis, the re-smoking rates were higher in workers with a low age, on day and night shifts, blue collar, of a low rank, where this was their second attempt at smoking cessation and for those with a shorter job duration (p<0.05). Of the cues to action variables in the Heath Belief Model, re-smoking was significantly related with the perceived susceptibility factor, economic advantages of smoking cessation among the perceived benefits factor, the degree of cessation trial's barrier of the perceived barriers factor, smoking symptom experience, recognition of the degree of harmfulness of environmental tobacco smoke and the existence of chronic disease due to smoking (p<0.05). In the multiple logistic regression analysis for re-smoking, the significant variables were age, perceived susceptibility for disease, economic advantages due to smoking cessation, the perceived barrier for smoking cessation, recognition on the degree of harmfulness of environmental tobacco smoke, the existence of chronic disease due to smoking and the number of attempts at smoking cessation (p<0.05). Conclusion: From the result of this study, for an effective smoking ban policy within the work place, health education that improves the knowledge of the adverse health effects of smoking and the harmfulness of environmental tobacco smoke will be required, as well as counter plans to reduce the barriers for smoking cessation.

Dokha: An Emerging Public Health Issue as a Form of Tobacco Smoking in the Middle East

  • Background: Tobacco use is one of the leading causes of preventable death across the world today and the smoking rates among men in the Arab world are amongst the highest in the world. Smoking of dokha, a traditional Arab tobacco is common in some of the Middle East countries such as the United Arab Emirates and Iran. This review focuses on the prevalence, pattern and health effects of dokha use in the Middle East. For this purpose an electronic search was performed in the following databases and websites: MEDLINE, EMBASE, Proquest and Google scholar up to December 2012. The search strategy was based on Internet search for the synonyms of dohka or midwakh. From the results of the review it emerged that younger people are the most common users of dokha. Hence effective outreach health education programs, targeting adolescents, especially school students before they take up the habit of smoking, may thus curb the emergence of the problem.

중학생들의 흡연 및 간접흡연 실태와 태도 (A Study on the State and Attitude toward Smoking and Passive Smoking Among Middle School Students)

  • This study was aimed at identifying on the state and knowledge toward smoking and passive smoking in middle school students. The data were collected from 6th. to 18th. August, 2001. Subject were 125 middle school students who were participated in the Smoking Cessation Program of S Health Agency in Busan. The collected data were analyzed by SPSS program for frequency, percent, mean, and chi-square test. The smoking related state and attitude of subjects were as follows; the nurmber of the smoking students are 10(8.0 %) at present, 19(15.2 %) at past experience, 53(42.4 %) having at present smoking friends, 73(58.4 %) having at present smoking family. Also the smoking starting time of present or past smoking experience was the middle school; 12(9.6 %) and elementary school; 11(8.8 %). They started because of the peer group members; 12(9.6 %), curiosity; 8(6.4 %), and looking good; 7(5.6 %). The passive smoking related state and attitude of subjects were as follows; the number of the passive smoking aware rates are very well level; 71(56.8 %), well level; 54(43.2 %). The exposed experience to passive smoking was always; 38(30.4 %), sometimes; 86(68.8 %), not experience; 1(0.8 5). The place of exposure to passive smoking was game rooms; 67(53.6 %), house; 30(24.0 %), fast food place or cafeteria; 10(8.0 %), and street or in a vehicle ; 3(2.4 %). The main smoker to passive smoking exposure was unknown person; 61(48.8 %), grand parents or parents; 43(34.4 %), brothers or friends; 8(6.4 %), teachers; 4(3.2 %). The chief complant of passive smoking exposure was dyspnea; 36(28.8 %), coughing; 34(27.2 5), dizziness; 21(16.8 %), no symptom; 18(14.4 %). The mostly affected smoker to passive smoking exposure was parents; 52(41.6 %), brothers or friends; 48(38.4 %). The health affected perception to passive smoking exposure was very affected; 109(87.2 %), not affected; 3(2.4 %). In conclusion, it can be well recognized that considerable number of middle school students is exposed to the passive smoking in their homes and by unknown persons, but, their knowledge and attitude toward the exposure to passive smoking is not effective. Therefore, it is hoped that regular smoking education program at school and restriction campaign in home and the public place is necessary.

공익광고를 통한 금연교육 효과 분석 (An Analysis of the Effect of Public Advertisement on Smoking Prevention Education)

  • Adopting the theory of enter-educate, this study was conducted to prevent the smoking by middle school students who had not started smoking yet. A famous pop music singer with high popularity among teenagers was cast on the public sponsored advertisement (PSA) on smoking prevention. This PSA was televised through major TV stations for two months. The effectiveness of this project was evaluated seven times. The effectiveness of the PSA, the role of the cast singer, the rate of seeing the PSA, and the impression about the PSA were evaluated. The viewer survey since the second period of the PSA revealed that most of the middle school students (79~94 percent) watched it. Among TV stations, MBC recorded the highest viewer rate as 69.6~72.0 percent of the middle school students watched the PSA at least once from this station. Also, the viewer rates of such cable TV stations as Mnet and OCN were quite high for their advertisement prices. The overall impression about the PSA was favorable. The contribution of the singer to this positive outcome was 60 percent and over. The effect of smoking prevention was also high as 93.7 percent of the middle school students replied that they would not start smoking. However, since this study dealt with the general public, it was not possible to control the effect of other factors systematically. Also, we could not measure the differences bewteen the pre-PSA and the post-PSA periods, so that the interpretation of the results has some limitation.

대전 지역 대학생의 6개월 금연 성공 관련 요인 (Factors Associated with the Continuous Abstinence Rate from Smoking on Smoking Cessation Program over 6 Months in College Students of Daejeon, Korea)

  • 대학생 흡연자가 중증고도 흡연자로 진입하는 것을 차단하고자 여러 가지 금연서비스를 지원하고 있지만 대학생의 금연시도율은 다른 연령대의 금연시도율보다 낮은 수준을 보인다. 본 연구는 대학생의 6개월 금연 성공과 관련된 변수를 도출하여 분석하는 데 그 목적이 있다. 연구 자료는 2015년 6월부터 2016년 12월까지 대전금연지원센터에서 제공하는 금연지원서비스를 이용한 대학생 781명에 대한 자료를 국가금연지원센터로부터 제공받아 분석에 활용하였다. 분석 결과, 4주, 12주, 24주 금연성공률에 모두 유의한 변수는 호기 중 일산화탄소 농도와 금연상담횟수였다. 호기 중 일산화탄소 농도가 10ppm 미만인 사람이 10ppm 이상인 사람보다 금연에 성공할 확률이 4주 2.53배, 12주 2.33배, 24주 2.13배 높은 것으로 나타났다. 상담횟수의 경우, 상담횟수 1회 증가 당 4주, 12주, 24주 각각의 OR은 12.39, 13.13, 12.21로 나타났다. 본 연구는 대학생을 대상으로 금연 중재의 효과성을 높이기 위해 금연 상담 횟수를 향상시키는 것이 필요함을 제안한다.

산업군별 고용형태에 따른 근로자 흡연율 변화 추이: 1992-2006 (Smoking Rate of Workers according to Employment Status and Industry: 1992-2006)

  • Objectives: The present study examined whether smoking rate has declined in 1992-2006 and who the high risk groups were on industry classification and employment type. Methods: Data from 91,263 persons aged 25-64 years were analyzed from three rounds of the Social Statistical Surveys of Korea between 1992 and 2006. Industry indicators were divided by the 9th Korean Standard Industrial Classification. Age-adjusted prevalence of smoking was calculated. Prevalence ratios(PR) and differences(PD) were estimated using log-binomial regression analysis. Results: Age-adjusted prevalence of smoking decreased between 1992 and 2006, specially the smoking prevalence of regular employees decreased most. PD in age-adjusted prevalence of smoking were the biggest between regular and daily employees. PR of the temporary employees', daily employees', self-employed persons' in order was wider than that of regular employees. PR increased significantly increased between 1999 and 2006 for those in manufacturing, construction, wholesale & retail trade, service industries. Increases in PR(regular/irregular) for women in service industry were statistically significant. Conclusions: Despite reducing overall cigarette smoking rates in males, the smoking rate was not reduced equally by industry classification and employment type in both genders. More adjustable antismoking policies and consideration of employment type are requested to reduce inequalities in smoking.

포커스 그룹 면담을 이용한 청소년 흡연의 습관화 과정 (The Processes of Habituating to Smoking among Teenagers)

  • Purpose: The smoking rates among teenagers in Korea grow gradually since 1980s despite of the numerous programs for youth smoking prevention and cessation have been developed and implemented. The purpose of this study was to explore and describe the processes of habituating to smoking among teenagers using the grounded theory methodology. Method: Qualitative data was collected via six focus group interviews. A total of 38 people, twelve teachers and 24 middle school students participated in this study. All focus group interviews were tape-recorded. transcribed. and analyzed according to the grounded theory methodology. Results: The overriding theme of the elicited grounded theory was "stepping into a quagmire by a merest chance". The student participants began smoking by a simple chance. The contingent factors to starting smoking were "discord within the family", "family member's smoking", "schoolwork stresses". or "a rebellious spirit". The conditions of smoking included accessibility. going around in group, and the lack of discipline. "Stigmatizing", "involved in mob violence", and "making a poor academic record" coexisted as the covariance of the smoking habituation. Conclusion: The findings of this study illustrated the comprehensive and insightful picture of the phenomena under investigation. Nursing implications and further directions for research were discussed.

Smoking Cessation Treatment and Outcomes in Medium to Heavy Cigarette Smokers being Treated for Cancer in Jordan

  • Background: Studies evaluating smoking cessation treatment outcomes in cancer patients are scarce, despite smoking cessation importance in cancer care. We sought to add to the literature by evaluating smoking cessation in a challenging group of cancer patients (medium-to-heavy smokers) visiting an out-patient smoking cessation clinic (SCC) in a cancer center in Amman, Jordan. Materials and Methods: Patients smoking >9 cigarettes per day (CPD) and referred to the SCC between June 2009 and May 2012 were studied. Clinic records were reviewed to measure demographic and baseline clinical characteristics, and longitudinal (3-, 6- and 12- month) follow-up by phone/clinic visit was conducted. At each follow-up, patients were asked if they experienced medication side-effects, if they had returned to smoking, and reasons for failing to abstain. Descriptive and multivariable logistic regression analyses were performed. Results: A total of 201 smokers were included in the analysis. The 3-month abstinence was 23.4% and significantly associated with older age, being married, and presenting with lower (${\leq}10ppm$) baseline carbon monoxide (CO) levels. On a multivariable level, lower CO levels, a higher income (relative to the lowest income group), being older, and reporting severe dependence (relative to dependence reported as 'somewhat' or 'not') were significant predictors of higher odds of abstinence at three months. Reasons for failing to quit included not being able to handle withdrawal and seeing no value in quitting. Long-term ARs did not reach 7%. Conclusions: In a sample of Jordanian smokers (>9CPD) with cancer and receiving smoking cessation treatment, ARs were low and further declined with time. Results underscore the need for more aggressive patient management and rigorous follow-up during and after smoking cessation treatment, particularly when this takes place in challenging settings. Observed reasons for failure to abstain should be used to tailor counseling practices.

서울 일지역 청소년 흡연관련 요인 분석 (A study of Adolescent Smoking Related Factors in the Seoul Area)

  • Purpose: The purpose of this study was to identify factors influencing adolescent smoking behavior in the Seoul area. Method: A total of 6.352 middle and high school students living in the Gangnam district of Seoul participated in the study. A self report survey method was used to identify factors related to smoking. Result: A stepwise logistic regression analysis identified four factors associated with adolescent smoking: living with a parent who smokes (OR=1.4), having friend (s) who smoke (OR=14.8), negative attitudes toward passive smoking(OR=4.8), and ignorance of the impact of smoking on health(OR=4.6). Conclusion: Based on the study results, components of effective programs to reduce adolescent smoking rates should include programs to deal with peer pressure to smoke and to reduce the impact of parents who smoke, to increase knowledge of the impact of smoking, and to promote positive attitudes toward anti smoking.

지역 간 흡연율 격차 영향요인 분석 및 금연사업 상대적 효율성 평가: Clustering Analysis와 Data Envelopment Analysis를 활용하여 (Analysis of Factors Affecting the Smoking Rates Gap between Regions and Evaluation of Relative Efficiency of Smoking Cessation Projects)

  • Background: Based on the importance of ceasing smoking programs to control the regional disparity of smoking behavior in Korea, this study aims to reveal the variation of smoke rate and determinants of it for 229 provinces. An evaluation of the relative efficiency of the cease smoking program under the consideration of regional characteristics was followed. Methods: The main sources of data are the Korean Statistical Information Service and a national survey on the expenditure of public health centers. Multivariate regression is performed to figure the determinants of regional variation of smoking rate. Based on the result of the regression model, clustering analysis was conducted to group 229 regions by their characteristics. Three clusters were generated. Using data envelopment analysis (DEA), relative efficiency scores are calculated. Results from the pooled model which put 229 provinces in one model to score relative efficiency were compared with the cluster-separated model of each cluster. Results: First, the maximum variation of the smoking rate was 16.9%p. Second, sex ration, the proportion of the elder, and high risk drinking alcohol behavior have a significant role in the regional variation of smoking. Third, the population and proportion of the elder are the main variables for clustering. Fourth, dissimilarity on the results of relative efficiency was found between the pooled model and cluster-separated model, especially for cluster 2. Conclusion: This study figured regional variation of smoking rate and its determinants on the regional level. Unconformity of the DEA results between different models implies the issues on regional features when the regional evaluation performed especially on the programs of public health centers.
