군인, 의경의 흡연 특성과 비만 간의 관련성: 2009~2017년 금연클리닉 참여 군인, 의경을 대상으로 (Relationship between smoking characteristics and obesity among military personnel in Korea: Data from smoking cessation clinics, 2009~2017)

  • Objectives: This study was performed to investigate trends of smoking characteristics and to identify the relationship between smoking characteristics and obesity among military personnel in Korea. Methods: The study participants were 50,680 military personnel who participated smoking cessation clinic from January in 2009 to December in 2017(excluded in 2011). Obesity was defined as body mass index${\geq}25kg/m2$. Smoking characteristics was included cigarettes of smoked per day, age of initial smoking, smoking duration, and nicotine dependence. Binominal logistic regression analysis was performed to confirm the relationship of smoking and obesity among military personnel. Results: The prevalence of obesity of study subjects was 20.4%. The military personnel were more likely to be obese if they who smoked more than 20 cigarettes per day(adjusted OR 2.271, CI 2.027-2.545), who smoked for more than 10 years(adjusted OR 2.046, CI 1.820-2.299), and who smoked their initial smoking at later than 20 years(adjusted OR 1.357, CI 1.223-1.493). Conclusions: Obesity is closely related to cigarettes of smoked per day, age of initial smoking and smoking duration among military personnel. Thus, intervention included both smoking cessation and weight control are necessary for smokers with obese who are interested in losing weight among military personnel.

Association Between Tobacco Smoking and Dental Caries in the Indonesian Population: Results of a National Study in 2018

  • Objectives: The 2018 Basic Health Research (RISKESDAS), conducted by the Ministry of Health of the Republic of Indonesia showed a high prevalence of dental caries (88.8%) in Indonesia and suggested that smoking tobacco was associated with an increased risk of dental caries. This study analyzed the association between tobacco smoking and dental caries in the Indonesian population. Methods: This was a cross-sectional analysis of secondary data collected from RISKESDAS 2018. The study population included 35 391 Indonesians aged ≥10 years from all 34 provinces. The decayed, missing, and filled teeth (DMFT) index was used to measure dental caries. Smoking status was assessed qualitatively based on smoking activity, and the level of smoking exposure was assessed based on the Brinkman index. A multivariable logistic regression analysis was employed to examine the relationships of smoking status and smoking exposure levels with the DMFT index. Results: Of the population aged ≥10 years, 36% had a DMFT≥8 (females: 37.5%, males: 33.9%). Almost one-fourth (23.4%) were current smokers, and 4.1% were ex-smokers. Furthermore, 26.4% had a Brinkman index ≥400, indicating heavy smoking. According to the multivariate analysis, current smoking status was associated with the risk of DMFT≥8 in males (adjusted odds ratio [aOR], 1.40; 95% CI, 1.27 to 1.55; p<0.001) and overall (aOR, 1.07; 95% CI, 1.00 to 1.14; p=0.037). In females, ex-smoking was associated with a 41% higher risk of DMFT≥8 (aOR, 1.41; 95% CI, 1.07 to 1.84; p=0.014). Heavy smoking was associated with a higher risk of DMFT≥8 in males (aOR, 1.38; 95% CI, 1.25 to 1.52; p<0.001) and females (aOR, 1.24; 95% CI, 1.03 to 1.50; p=0.022). Conclusions: Tobacco smoking was associated with dental caries in the Indonesian population.

Prevalence and Factors Associated with Oral Pre-Malignant Lesions in Northeast Thailand

  • Background: Oral cavity cancer (OCC) is one of the most common cancers worldwide. No studies have reported on the prevalence and epidemiologic risk factors of oral premalignant lesions (OPLs) in Thailand. The purpose of this study was to investigate the prevalence of OPLs and associated factors in Roi Et Province, Thailand. Materials and Methods: To investigate the prevalence of OPLs, a cross-sectional descriptive study was conducted in which 2,300 subjects over 40 years of age were recruited and screened for the prevalence of OPLs. To identify factors associated with OPLs, a matched case-control study was used in which the subjects were 102 cases with OPL and 102 matched controls without OPLs. The studies were conducted in Roi Et Province during the period 1 February, 2014, to 30 April, 2014, and the data were collected by the use of a structured interview questionnaire and by extraction of information from medical records. Data analyses involved the use of descriptive statistics, McNemar's test, and conditional logistic regression. Results: The overall prevalence of OPLs was 3.8%, and no-one was diagnosed with more than one type of OPL. The factors found to be associated with a statistically significant higher risk of an OPL were betel nut chewing, smoking, and alcohol consumption. The associations with these factors were strong, especially for betel nut chewing and smoking. Conclusions: The habits of betel nut chewing, smoking, and alcohol use are confirmed as factors associated with OPLs in a population of Roi Et Province, Thailand. Campaigns to reduce such risk healthy behaviour are needed, but whether any actual decrease will prevent the eventual transformation of an OPL into an OCC remains an open question.

Re-examination of Opisthorchis viverrini Infection in Northeast Thailand

  • Background: Liver fluke infection caused by the parasite Opisthorchis viverrini (O. viverrini), a human carcinogen, is endemic in north-eastern Thailand and remains a major health problem. Objectives: The objectives of the study were to (1) resurvey the prevalence of O. viverrini infection in a field site from the Khon Kaen Cohort Study (in newly recruited subjects as well as previous cohort subjects surveyed in 1992); (2) investigate how subjects' lifestyle habits and their exposure to health promotion initiatives influence changes in prevalence of O. viverrini infection. Materials and Methods: The prevalence of O. viverrini infection in the cohort subjects (as well as new subjects) was investigated using faecal egg counts. Information on demographic factors, lifestyle and awareness of health promotion initiatives were obtained through questionnaires. Results: O. viverrini infection rates in the same individuals of the cohort were lower in 2006 than in 1992. Also, by studying the period effect, the current 35-44 year olds had a 12.4% (95% CI 3.9% to 20.9%) lower prevalence of O. viverrini infection than the 35-44 year olds in 1992 (24.2% versus 11.8%). Lifestyle choices showed that smoking and alcohol consumption were associated with an increased chance of acquiring O. viverrini infection with adjusted odds ratios of 10.1 (95%CI 2.4-41.6) and 5.3 (95%CI 1.2-23.0), respectively. Conclusions: Our study has demonstrated that although the prevalence of O. viverrini infection over a 14-year period has decreased, unhealthy lifestyle was common with smoking and alcohol consumption being associated with increased chances of infection, emphasising the double burden of disease which developing countries are facing.

Study of Oral Microbial Prevalence and Oral Health in Adults

  • Objective: This study performed a quantitative analysis using the real-time polymerase chain reaction technique to examine the oral microbial prevalence in adults and intended to examine the correlations between risk factors of periodontal disease and oral bacteria and correlation between oral test scores and oral microorganisms. Methods: We examined papillary marginal attached (PMA) index, modified patient hygiene performance (M-PHP) index, probing depth (PD), modified gingival index, and oral bacteria counts and surveyed 117, 20 years or older adult males and females who visited dental clinics in the Daejeon region to analyze the prevalence and oral health. Results: The prevalence was 100% for Fusobacterium nucleatum, meaning it was observed in all examined subject, 85.5% for Parvimonas micra, 76.1% for Prevotella intermedia, and 72.6% for Tannerella forsythia. The averages of P. gingivalis and T. forsythia increased as the examined subjects were older, and there was a statistically significant difference between T. forsythia and E. nodatum in relation to medical history, between P. intermedia and P. micra in relation to gender, and between P. intermedia and E. corrodens in relation to smoking (p<0.05). For a correlation between the oral test scores and oral microorganisms, P. gingivalis and F. nucleatum was highly correlated with PD (correlation coefficient of 0.51 and 0.41) (p<0.01) while P. gingivalis, P. micra, C. rectus, and E. nodatum were significantly correlated with M-PHP index, gingival index, PD, and PMA index (p<0.01, p<0.05). Conclusion: For oral health management of adults, the age, systemic disease, and smoking are closely related to oral bacteria, and P. gingivalis, T. forsythia, F. nucleatum, P. intermedia, P. micra, C. rectus, E. corrodens, and E. nodatum are considered to be the oral microorganisms that indicate periodontal health.

The Smoking, Drinking Behavior and Sexual Activity among Korean High School Students

  • The purpose of this study is to examine the prevalence and correlates of three types of risk-taking behavior among high school students in Korea in a cross-sectional national sample. A sample of 636 male and 622 female high school students is analyzed using descriptive statistics and logistic regression. Three types of risk-taking behavior are examined: drinking alcohol, smoking, and premarital sex. The results are as follows: Forty-three percent of students in the sample have ever drunk alcohol, 32% have ever smoked, 11% have experience premarital sex. Risk-taking behavior is more prevalent among boys than among girls. Boys and girls in Seoul are much more likely to have experienced three types of risk-taking behavior. Not having two parents in the family and having lived away from immediate family increase the likelihood of some risk-taking behavior. Students who have a friend with premarital sexual experience are much more likely to have experienced premarital sex themselves. Among sexually active students, having multiple sexual partners is common and prevalence of condom use is low. Reproductive health education at school has no effect on students' sexual behavior. The prevalence of risk-taking behavior among adolescent in Korea is likely to rise in the future. Substantial proportion of adolescents are exposed to the risks of STDs and substantial proportion of adolescent girls risk premarital pregnancy. Current health education programs are inadequate in protecting students from risk-taking behavior. Students whose families do not include both parents and students who live away from their immediate families need special programs to protect them from risk-taking behavior.

Peer Smoking and Smoking-related Beliefs Among College Students in Bangladesh

  • Objectives: Smoking is a significant public health issue in Bangladesh. The purpose of this study was to examine peer smoking and smoking-related beliefs among college students in Bangladesh. Methods: College students at two universities in Dhaka, Bangladesh participated in a self-administered survey in May and June 2017. Results: First, being a current or former smoker is associated with lower levels of beliefs among respondents that they would not smoke even with smoker friends or nervousness, and lower levels of intentions that they would not smoke, while current smokers and former smokers have different smoking-related beliefs. Second, having smoker friends is associated with lower levels of intentions that they would not smoke. Third, higher levels of normative beliefs that it is important not to smoke are associated with higher levels of beliefs that they would not smoke even with smoker friends or nervousness, higher levels of intentions that they would not smoke, and higher levels of avoidance of smoking. Conclusions: Smoking-related beliefs and perceived norms in individuals' social networks are important components in promoting tobacco cessation in Bangladesh. But it is challenging to prevent or intervene in smoking because of the high rates of smoking in this country and the high prevalence of smokers in individuals' social networks. Future studies should examine the most effective interventions to combat smoking in high-smoking social networks, such as using mobile apps or social media, and evaluate the effectiveness of such interventions.

일 농촌 지역 노인의 대사증후군 유병률과 관련요인 (Prevalence of the Metabolic Syndrome and Its associated Factors among Elders in a Rural Community)

  • Purpose: The purpose of this study is to identify the prevalence of the metabolic syndrome (MS) and factors associated with it among elders in a rural community. Methods: Data were collected from 683 subjects with a questionnaire, physical measurement, and blood test. The prevalence of the MS was determined by AHA/NHLBI and waist circumference cutoff points for Koreans. Results: The prevalence of the MS was 50.5% in total (41.6% in men, 56.3% in women) while the prevalence of 5 metabolic risk factors was 67.7% for elevated blood pressure, 51.0% for low HDL-cholesterol, and 50.2% for abdominal obesity. Risk factors associated with the prevalence of the MS included family history, BMI, and physical activity; significant factors associated with that of metabolic components included family history, BMI, smoking, drinking, and physical activity. Especially, a higher BMI was a strong risk factor of the prevalence of abdominal obesity as well as the MS and its components. Conclusion: The findings indicate that nurses should care for elders based on each metabolic component regarding its prevalence level and concentrate primarily on reducing elevated blood pressure, low HDL-cholesterol by controlling the main risk factor, abdominal obesity through lifestyle modification.

융복합 기반의 지역간 흡연율의 변이 분석 (Convergence-based analysis on geographical variations of the smoking rates)

  • 본 연구의 목적은 흡연율의 지역간 변이 정도와 흡연율에 영향을 미치는 요인을 살펴보는 데 있다. 분석을 위해 2009-2011년 질병관리본부의 지역사회건강조사 자료와 국민건강보험공단의 자료를 사용하였다. 흡연율에 영향을 미치는 요인을 파악하기 위해 상관분석, 다중회귀분석을 이용하였으며, 지역간 변이 분석을 위해 의사결정나무 모형을 이용하였다. 연구결과, 지역간 흡연율의 변이요인은 고위험음주율, 고혈압관리교육 이수율, 금연캠페인 경험율, 스트레스 인지율, 고혈압 유병률, 건강보험료, 당뇨병 유병률, 비만율, 근력운동 실천율로 나타났다. 융복합 기반으로 지역별 흡연율의 변이를 파악하는 것은 흡연율을 감소시키기 위한 지역별 맞춤형 보건프로그램을 수행하는데 매우 중요하다. 향후 흡연율이 높은 지역의 원인과 대상에 맞는 구체적인 관리방안을 모색하는 노력이 필요할 것이다. 본 연구의 결과는 효과적인 흡연관리사업 방안을 도출하는 데 필요한 기초자료로 의미있게 활용될 수 있으리라 여겨진다.

흡연 습관에 따른 폐암 발생의 차이 (The Difference of Lung Cancer Prevalence According to Smoking Habits)

  • 연구배경 : 흡연과 폐암 발생 사이의 인과관계는 잘 알려진 사실이지만, 흡연 습관 즉 흡연량 및 기간, 흡연을 처음 시작한 나이, 담배 연기의 흡입 정도, 그리고 담배의 종류 등 다양한 요소들이 폐암 발생에 더욱 크게 작용한다고 한다. 그러나 우리나라에서는 이에 대한 연구 특히 일상동안의 흡연으로 흡입된 타르, 니코틴이 폐암 발생에 미치는 영향에 대한 연구는 없는 실정이다. 방 법 : 연세대학교 의과대학 세브란스병원에 내원한 45세에서 75세 사이의 폐암 환자 150명과 나이와 성별을 일치시킨 폐암이 아닌 질병을 가진 환자 300명을 대상으로 흡연 습관에 따른 폐암 발생 및 폐암 조직형의 교차비, 일생동안 흡입한 티르와 니코틴 총량을 설문지를 통해 조사하여 산술적으로 합산하여 흡입 총량에 따른 교차비를 알아보고자 환자-대조군 연구를 시행하였다. 각 담배에 포함된 개피 당 타르, 니코틴 함량을 문헌을 통해 조사하였고, 개개인의 흡연 습관의 차이에 따른 일생동안 흡입된 타르와 니코틴의 함량을 구하였다. 결 과 : 폐암 환자군 150명 중 현재 흡연자가 가장 많았고, 여자들 중에서는 비흡연자가 가장 많았으며, 대조군도 유사하였다. 폐암 환자군의 흡연 습관 중 통계적으로 의미가 있었던 것은 흡연 기관과 총 갑년, 최근 2년간 피운 하루 당 개피 수이었고, 흡연 시작 나이와 필터 없는 담배를 피운 기간은 대조군과 차이가 없었다. 폐암 조직형 중 편평상피세포암과 소세포암은 남자와 현재 흡연자가 가장 많았으며, 선암은 여자와 비흡연자가 가장 많았다. 흡연 습관들 중 Kreyberg I에서는 흡연 시작 나이가 어릴수록, 흡연 기간이 길수록, 총 갑년이 높을수록, 최근 2년간 피운 하루 당 개피수가 많을수록, 필터없는 담배를 피운 기간이 길수록 교차비는 증가하였다. 또한 산술적으로 합산한 일생동안 흡입된 타르와 니코틴의 총량이 높을수록 교차비가 높았다. 그러나 Kreyberg II에서는 각 흡연 습관들의 교차비는 통계적으로 의미가 없었다. 결 론 : 본 연구는 단일 병원에서 조사되었고, 환자의 기억에 의존하는 제한점이 있으나, 흡연 습관이 폐암 발생, 폐암 조직형 등과 상당한 연관이 있는 것으로 조사되었고, 일생동안 흡연 습관의 변화를 타르와 니코틴의 흡입된 총량으로 산술적으로 계산하였다는 의의가 있다. 향후 이에 대한 대규모 연구가 필요할 것으로 생각된다.
