금연/흡연예방 프로그램의 방법에 따른 효과성 평가: 최근 3년 간 국내학술지 논문에 대한 체계적 문헌고찰 (Efficacy of Smoking Cessation and Prevention Programs by Intervention Methods: A Systematic Review of Published Studies in Korean Journals during Recent 3 Years)

  • Objectives: The purpose of this study was to evaluate program methods for effective smoking cessation and prevention through systematic review of studies published during recent 3 years in Korea. Methods: Electronic searches were conducted in DBPIA, RISS, KISS and other databases looking for studies published from 2010 to 2012 that dealt with the effects of intervention on smoking prevention and cessation. A total of 25 studies were included in the final analysis. Three independent reviewers critically evaluated the study quality and evidence of effectiveness based on The Community Guide methodology. Results: Multi-component intervention showed strong evidence of effectiveness for adolescents and sufficient evidence for patient smokers. Cognitive-behavioral intervention for adolescents and child-parent interaction program for adults demonstrated sufficient evidence of effectiveness on the smoking cessation and its determinants. There was insufficient evidence showing effectiveness of smoking prevention programs. Conclusion: Improving the quality and quantity of the intervention studies on smoking could help to expand evidence base for effective intervention methods and strategies on smoking.

  • Purpose: We reviewed all the articles on effect of smoking cessation intervention to find out the way of improvement of it. Method: 48 researches were published in Korean journals from 1980 to 2007. The research were analyzed according to: major of first author, subjects, application of theoretical framework, contents of intervention, a term of intervention, the time of evaluation on effect of interventions, research design, dependant variables, and effect of interventions. Results: Research on effect of smoking cessation intervention increased rapidly in the 2000's. At this time. 72.9% of research was published. Most of research subject were students. Theoretical framework were applied in 20.8% of articles. The most common intervention was education(39.4%). The effect of combined program on outcome variable was found to be stronger than only education program. Experimental design was used in 29.2% and quasi-experimental design was adapted in 43.8% of articles. 1- 6months intervention was in 35.4%. The long term intervention more than 1month was found to be effective. Health behavior related index was measured as a dependant variable in 62.3%. conclusion: Accordingly, long term program to apply to various subject is needed.

대학생 흡연자의 금연 준비단계 관련 요인 (Factors Associated with the Preparation Stage to Quit Smoking among College Smokers)

  • Purpose: This study was to compare the Transtheoretical Model components according to the stage of change in smoking cessation behavior and identify factors associated with preparation to quit smoking among college smokers. Methods: Data were collected from 224 undergraduate students using the self-report questionnaire. The survey variables comprised the stage of change in smoking cessation, self-efficacy, and decisional balance and process of change in smoking cessation. Results: There were significant differences in self-efficacy, cons of smoking, and the process of change according to the stage of change in smoking cessation behavior. Cons of smoking and self liberation were significant factors related to the preparation stage of smoking cessation. Conclusion: Strategies to enhance cons of smoking and self liberation in college smokers will be an important intervention component to prepare and plan smoking cessation in future studies.

Factors Associated with the Stage of Change of Smoking Cessation Behavior in Adolescents

  • Purpose. The purpose of this study was to identify the stage of change of smoking cessation behavior and investigate factors associated with the stage of smoking cessation behavior according to the transtheoretical model. Methods. The participants, 297 smokers & quitters were selected by a stratified random sampling from 127 high school sophomore students in B city. Data were collected from April 6th to 16th, in 2002 using the structured self-report questionnaire. Results. The subjects were distributed in each stage of change of smoking cessation behavior: there were 46 subjects (15.5%) in precontemplation, 73 subjects (24.6%) in contemplation, 67 subjects(22.3%) in preparation, 56 subjects (18.5%) in action, 55 subjects (18.5%) in maintenance. Compared to the precontemplation and contemplation, people in preparation tended to smoke daily more and smoked for a shorter time, and as precontemplation progressed to the maintenance, past 1 year smoking cessation frequency increased and friends smoking decreased. Smoking onset age was the earliest in preparation, and the latest in maintenance. Helping relationships and self relationships are used a lot in precontemplation and also in contemplation. In preparation, self liberation and helping relationships are used a lot, in action, self liberation and helping relationships, and in maintenance, self liberation and environmental reevaluation. At each stage, the score of negative affect situation was the highest, but the one of negative affect situation, positive social situation, habitual strength, weight control decreased as precontemplation progressed to the maintenance. While the score of social pros and coping pros decreased with increasing stage, the one of cons tended to increase. Through stepwise discriminant analysis, it was found that social pros, smoking onset age, delf-libration were the most influencing powers among factors associated with the stages of smoking cessation behavior. Conclusions. This study suggested that, in developing an effective smoking cessation intervention for adolescents, all the stage of a client's cessation had to be assessed prior to applying intervention programs. In addition, the results of this study will become a pillar of smoking cessation program planning and application.

성인 흡연자의 금연 프로그램 효과: 무작위대조군 실험연구의 체계적 문헌고찰 (The Effectiveness of Smoking Cessation Program in Adults: Systematic Review of Randomized Controlled Trials)

  • Objectives: The purpose of this study was to determine if smoking cessation programs (SCPs) are effective for adults through a systematic review of the results of such programs in large randomized controlled trials (RCTs). Methods: The PICO (Patient, Intervention, Comparator, Outcome) strategy was established, 1,160 literature from domestic and foreign electronic databases was reviewed, and 22 references were selected based on the inclusion and exclusion criteria. The quality of each reference was evaluated using the Scottish Intercollegiate Guidelines Network tool, and meta-analysis was carried out. Results: The SCPs were significantly effective for adult smokers. Smoking cessation counseling, education, and smoking cessation medications such as nicotine patch were more effective than the other interventions. However, the results showed short-term effects (within six months), and differences were observed among the SCPs. For the outcome measures for SCPs, the abstinence rate of seven days was mainly used, but differences were identified between the CO level and the cotinine-verified abstinence rate of smoking cessation. Conclusions: For a smoking cessation program for adult smokers, the strength of the evidence of the program's effectiveness in RCTs that provide the identified intervention strategies should be considered.

남자 고등학생 흡연자의 비타민 C 보충 및 금연교육 효과 평가 (Evaluation of Vitamin C Supplementation and Effectiveness of Smoking Cessation Intervention in Adolescent Male Smokers)

  • Smoking can increase oxidative stress and thereby change the antioxidant defense system in the body. Supplementation of antioxidant vitamins might improve antioxidant status in the body. The purpose of this study was to evaluate the effectiveness of vitamin C supplementation and smoking cessation education on changes of antioxidant status and psychosocial factors related to smoking. To obtain above purposes, we investigated the effectiveness of intervention for male adolescent smokers were evalnate by assessing changes in dietary intakes, plasma antioxidant vitamin concentration, and psychosocial factors related to smoking after program completion. Subjects, male adolescent smokers, were assigned into four groups : Control group(19 students), Educ. group(19 students), Vit. C supple. group(19 students), and Educ.+Vit. C suppl. group(19 students). The Educ. group and Educ.+Vit. C suppl. group received nutrition and smoking cessation education once a week for 5 weeks. Vit. C suppl. group and Educ.+Vit. C suppl. group received 500 mg per day of ascorbic acid for 35 days. All data were collected before and after intervention. Vit. B$_2$and Vit. C intakes of all groups were increased, but the only Ca intake was increased in the Educ. group. Plasma Vit. C concentration and Ratio(plasma Vit. C/Vit. C intakes) were increased in the Vit. C suppl. group and Educ.+Vit. C suppl. group, and the Vit. C deficiency status of these groups(Vit. C suppl. group and Educ.+Vit. C suppl. group) disappeared. Showing the effects of Vit. C supplementation, plasma $\alpha$-tocopherol was increased in the Educ. and Educ.+Vit. C suppl. group, and especially high increases were seen in the Educ.+Vit. C suppl. group. Psychosocial factors related to smoking changed after the education a little. This intervention program had an impact on nutrition intakes, plasma antioxidant vitamins, and some beliefs related to smoking in male adolescent students. Various programs of nutrition and smoking cessation education and vitamin supplementation for quitting smoking must be implemented for adolescent smokers, and further studies are needed regarding sorts and amount of antioxidant nutrients and supplementation periods.

성인남성의 금연동기 영향요인 (Influencing Factors on Smoking Cessation Motivation of Adult Males)

  • Purpose: This research analyzed the different factors that influenced smoking cessation motivation. Methods: Subjects were both current and past male adult smokers. Data collection was conducted in four small businesses in Seoul and one public enterprise in Gyeonggi-Province from December 10, 2011 to January 30, 2012. Reasons for Quitting (RFQ) scale which was developed by Curry et al. was used for data collection. There were 350 participants and 324 (93%) questionnaires were used for the statistical analysis. Results: Participant's level of motivation varied by present smoking, quit attempt, smoking cessation counseling, level of nicotine dependence, stages of smoking cessation, and types of smoking. Participant's drinking pattern, smoking cessation counseling, social seeking type (G) and stages of smoking cessation had a 13% explanatory power for being motivated to stop smoking. Conclusion: For participants who drink alcohol regularly, who do not participate in smoking cessation counseling and those who are in the pre-contemplation stage for smoking cessation are candidates for future smoking cessation intervention programs or smoking cessation motivation programs.

보건진료원의 금연지도활동에 영향을 주는 요인 - 광주$\cdot$전남지역을 중심으로 - (Predictors of Smoking Cessation Counselling Activities among Community Health Practitioners)

  • Purposes: The purposes of this study were to investigate the smoking cessation counselling activities among community health practitioners(CHP) and to identify the predictors of their smoking cessation counselling activities. Method: A descriptive-correlation study using self-administered questionnaires was conducted. Questionnaires were mailed to all the CHP in a community. A total of 330 CHPs participated in this survey. Results: Of the CHPs surveyed, 245(74.2%) returned completed questionnaires. Most CHPs(90.7%) believed that if a health professional advises their patient to quit, the patient's chances of quitting smoking are increased. While the majority of CHPs “asked, advised, and assessed” their clients, a minority of CHPs “assisted, arranged, and recorded”. In the final stepwise multiple regression model, attitude about smoking cessation policies and counselling activities, self-efficacy of smoking cessation counselling knowledge and skills, and perceived barriers of smoking cessation counselling activities were identified as significant predictors of smoking cessation counselling activities among CHPs. Conclusion: Smoking cessation counselling activities are not a routine part of CHP practice. Efforts should be made to increase the self-efficacy of smoking cessation counselling knowledge and skills among CHPs. Helping CHPs to overcome their barriers to smoking counselling may open up new channels for smoking intervention.

세이프약국 금연서비스의 성공요인 (Success Factors for the Smoking Cessation Service of the Safe Pharmacy)

  • Objective: Seoul City has implemented the Safe Pharmacy in 2013. This study aimed to ascertain the success factors of the smoking cessation service of the Safe Pharmacy. Methods: The data for the smoking cessation registration cards were obtained from the pharmacies which participated in 2014 Safe Pharmacy. The sample included 289 smokers in 6 districts who participated at least one sessions of smoking cessation service. the service included both behavioral intervention and nicotine replacement therapy. In order to identify the effectiveness of the smoking cessation service, logistic regression analysis was used. The dependent variable was the success of quitting smoking at the 4th week. The independent variables included age, sex, employment, chronic disease, district, smoking amount, registration path, supporters for quitting smoking and number of service sessions. Results: Fifty eight point eight percent of the sample successfully quit smoking at the 4th week. Unemployment, higher smoking amount, and less service sessions were negatively related to the success of quitting smoking. There were differences in the success rate across districts. Age, sex, chronic disease, registration path, and anti-smoking supporters were not related to the success rate. Conclusion: Pharmacy can be an effective community resource for smoking cessation. Factors that could increase the effectiveness of smoking cessation service of the Safe Pharmacy and possible ways to enhance the participation of pharmacies in smoking cessation services were also discussed.

The National "Smoking Cessation Clinics" Program in the Republic of Korea: Socioeconomic Status and Age Matter

  • Background: Between 1998-2009 South Korea experienced significant progress in reducing the male smoking rate from 66.3% to 46.9%. As part of a significant government effort in the area of smoking cessation intervention, the Korean government implemented the national "Smoking Cessation Clinics (SCC)" program in 2004. Materials and Methods: Data covered 804,334 adult male smokers participating in SCC program at 253 public health centers between 2006-2009. We examined participant cessation rates with the SCC program, their characteristics and program intervention components using health insurance status as a socioeconomic status (SES) indicator. Multivariate logistic regression analyses were performed correcting for intra-class correlations within public health centers. Results: The overall 6-month quit rate was high (46.8%). Higher odds of smoking cessation were positively associated with higher levels of behavioral counseling sessions, but not nicotine replacement therapy (NRT). Cessation rates were lower for Medicaid participants than for regular health insurance participants. Disadvantaged younger smokers were less likely to participate in the program. Older smokers were more likely to quit regardless of SES. Stress was cited as major reason for failure. Conclusions: SES inequalities across different age groups exist in smoking cessation among Korean adult male smokers. There is a need for intervention programs specifically targeting sub-populations of SES by different age groups.
