Effects of a Smoking Cessation Program on Processes of Change, Situational Temptation and Decisional Balance in Male University Student Smokers (금연프로그램이 흡연 남자대학생의 변화과정, 유혹상황, 의사결정균형에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of a smoking cessation program on processes of change, situational temptation, and decisional balance in male university student smokers. Method: Utilizing a quasi-experimental design, 78 male university students consisted of an experimental group (n=48) and a control group (n=30). A smoking cessation program was applied to the experimental group for a period of 6 months while no program was given to the control group. Study questionnaires were used to measure outcome variables before and after the program. Descriptive analysis, $\chi^2$-test, t-test, and ANCOVA were used with SPSS 18.0 program for data analysis. Results: After treatment with this program, the experimental group showed significantly higher scores for processes of change (experiential and behavioral processes of change) than the control group. The experimental group also showed significantly lower scores for situational temptation (positive social, negative affect, and habit strength). Conclusion: The smoking cessation program may have positive effects on process of change and smoking temptation for male university student smokers.

Influential Variables on Intention and Action to Quit Smoking between Adolescent Smokers and Adult Smokers-Based on the Transtheoretical Model (금연의 준비단계 영향변인: 범이론적(Transtheoretical model) 모형을 중심으로)

  • Purpose: This study identified and compared influential variables on intention and action to quit smoking between adolescent smokers and adult smokers. Methods: For the selection of variables, the transtheoretical theory was used. A survey was conducted with 376 adolescent smokers in 4 high schools and 451 adult smokers in community settings in South Korea. Discriminant analysis was used for data analysis Results: The variables of adolescent smokers that predicted an intention to quit smoking were: smoking temptation, self re-evaluation, counter conditioning and stimulus control. The variables that predicted an action to quit smoking were: self-efficacy for smoking abstinence, pros for smoking, self reevaluation, and self liberation. The variables of adult smokers that predicted an intention to quit were: smoking temptation, pros for smoking, cons for smoking, self reevaluation. The variables that self liberation and predicted an action to quit smoking were: self efficacy for smoking abstinence, smoking temptation, and counter conditioning. Conclusions: Developing stage specific smoking intervention methods based on different ways of how individuals make a decision to quit smoking within their contexts needs to be done.

The Effects of Knowledge about the Harmfulness of Smoking and Attitude Toward Smoking Temptation for Middle School Students (일개 시 중학생들을 대상으로 흡연에 관한 유해성지식, 태도가 흡연유혹에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of knowledge about the harmfulness of smoking and attitude toward smoking temptation for middle school students. Methods: A survey was conducted in this study on 745 students from 12 middle schools in the Y City. Data were analyzed with SPSS/WIN 12.0 using descriptive statistics, t?test, ANCOVA, Scheffe's test and multiple regression analysis. Results: There was statistically significant difference in knowledge about the harmfulness of smoking (F=2.88, p<.05) and attitude toward smoking (F=54.61, p<.001). And there was statistically significant difference in smoking temptations among tempting factors such as negative mood of individual (F=63.61, p<.001), positive mood of social situation (F=67.08, p<.001), habitualness (F=57.68, p<.001), weight control (F=33.01, p<.001), and others' smoking (F=73.13, p<.001). Conclusion: These results suggest that smoking?cessation programs to enhance the effectiveness of nursing interventions should be tailored to the level of smoking temptation and that educational programs should be developed for middle school students.

Differences in Processes of Change, Decisional Balance, and Temptation Across the Stages of Change for Smoking Cessation (금연 변화 단계에 따른 변화 과정, 의사결정 균형, 흡연 유혹의 차이)

  • Purpose: The purpose of this study was to investigate smoking cessation behavior of male taxi drivers in Korea on the basis of the Transtheoretical model(TTM), and to validate the usefulness of TTM. Method: Data were collected using a self-reported questionnaire including smoking history and major factors of TTM from 208 subjects who were current smokers or ex-smokers. Data were analyzed by descriptive statistics and ANOVA. Result: Most subjects ($85.1\%$) were current smokers. Stages of change were precontemplation ($44.7\%$) and contemplation ($27.4\%$). Subjects in precontemplation stages had the lowest mean score in processes of change and the highest mean scores in decisional balance(pros) and temptation(positive affective, habitual/craving). According to stages of change, there were statistically significant differences in processes of change, decisional balance, and temptation. Conclusion: This study supported the generalization of TTM. As this study showed that the subjects didn't have motivation in smoking cessation, applying tailored smoking cessation programs for taxi drivers is needed.

Effect of Empirical Smoking Prevention Program for Female High School Students (인문계 여고생을 대상으로 한 체험적 흡연예방프로그램의 효과)

  • Purpose : This was aimed to investigate the effects of empirical smoking prevention program on the knowledge of the harmfulness of smoking, attitude to smoking, smoking temptation among female high school students. Methods : This study used nonequivalent control group pre and post test design. The subjects were recruited from the first and second grade at the two girls' high schools located in Ulsan city, and randomly assigned to two groups(113 in the experimental group and 117 in the control). The intervention was both the events such as nonsmoking promulgation and experiential learning programs and student-oriented educational sessions(once a week for six weeks). Data was analyzed with X2 test, t-test and ANCOVA. Results : The mean knowledge was 12.5 for pretest and 13.7 for posttest in the experimental group, and 12.7 and 13.4 in the control group, retrospectively. The mean attitude was 71.0 for pretest and 72.2 for posttest in the experimental group, and 72.3 and 72.6 in the control group, retrospectively. The mean temptation was 15.0 for pretest and 14.7 for posttest in the experimental group, and 13.9 and 14.0 in the control group, retrospectively. There's no difference in the change of the knowledge of the harmfulness of smoking, attitude to smoking, smoking temptation between experimental and control groups. Conclusion : As the possible reasons, the subjects already had sufficient knowledge on the harmfulness of smoking, the research questionnaires were too plain to get any difference, the effect of intervention was too small to show a difference in such as short-term, and finally, there were many external factors. Hence, we recommend further studies with appropriate questionnaire for longer time.

Effectiveness of Public Health Center Smoking Cessation Counseling Program using the Transtheoretical Model (범이론적 모형을 적용한 대면적 금연상담 프로그램의 효과)

  • Purpose: The purpose of this study was to identify the effectiveness of public health center smoking cessation counseling program using the transtheoretical model on the process of change, smoking temptation, decisional balance and stage change transition. Methods: A nonequivalent control group non-synchronized design was used for this study. The study population was 115 adult smokers. The counseling program was administered to the experimental group and a smoking cessation program from the Korean Ministry of Health & Welfare was administered to the control group. Descriptive analysis, $\chi^2-test$, t-test, and ANCOVA were used with the SPSS 12.0 program to analyze the data. Results: After treatment with this program, the experimental group showed significantly higher scores for process of change (t=4.148, p<.001), smoking temptation (t=-2.988, p=.003), and stage change transition ($\chi^2=5.871$, p=.031) compared to the control group. Experimental group members also showed significantly lower score for Pros of smoking (t=-3.151, p=.002). Conclusion: The findings indicate that this program could have positive effect on process of change, smoking temptation, decisional balance and stage transition for adult smokers. Based on these findings, the authors suggest additional counseling program focusing on smokers in specific stages.

Types of Smoking Decision Making-Temptation in Adolescents and Related Characteristics (청소년기 흡연의사결정-유혹 유형과 유형별 흡연 관련 특성)

  • Purpose: This study was done to identify types of smoking decision making-temptation in adolescents and characteristics related to type among student smokers. Method: Data collection was done from March to July. 2006. A survey was administered to 275 students in 13 high schools and 15 middle schools in Seoul, South Korea. To identify types and characteristics smoking decision making-temptation in adolescents, cluster analysis using the K-mean method was employed. Characteristics of the influential variables according to the identified types of adolescent smokers were evaluated using ANOVA. Results: Four types of smoking pattern in adolescents were identified: habitual craving (17.7%), nicotine dependence (35.8%), feeblemindedness (28.4%), and self control (18.1%). The score for nicotine dependency was higher in the habitual craving type than any other type (F=11.79, p=.001), while the score for self efficacy for smoking abstinence was higher in the self control type (F=23.06, p=.000). Conclusions: Findings from this study suggest that effective interventions for smoking cessation in adolescents require not only active implementation of nicotine replacement therapy but also development of individualized approaches for each person targeting change in the social environment that may lead to positive smoking decisional balance.

Types of Smoking in Adult Smokers and Influential Variables Related to Smoking (성인기 흡연유형과 흡연영향변인)

  • Purpose: This study attempted to identify influential variables on types of smoking in adult smokers, which have been derived from Q-study. Methods: A survey was conducted with 328 adult smokers at community settings in Seoul, South Korea from April 2006 to July 2006. To identify the influential variables related to smoking, variables from the Trans-theoretical model(i.e., self-efficacy for smoking abstinence, smoking temptation, process of change, decisional balance(pros and cons)) and nicotine dependency were analysed by ANOVA. Post-comparison tests were also done. Results: The scores of nicotine dependency and smoking temptation were higher in habitual craving types than any other types, while the scores of self-liberation and self-efficacy for smoking abstinence were higher in self control types than any other types. Conclusion: Specific smoking intervention methods for each of the smoking types needs to be developed, based on how different groups of individuals gets into a decisions to quit smoking within their contexts.

A Study on the Smoking Cessation Self-efficacy and Counseling Satisfaction of University Students Using a University Smoking Cessation Clinic (일부 대학교보건소 금연클리닉 이용 대학생의 자기효능감과 상담만족도)

  • This study analyzed the smoking and smoking cessation characteristics, smoking cessation self-efficacy and counseling satisfaction of 140 college students using smoking cessation clinics in 2 universities, located in D Metropolitan City and C Province. The results can be summarized as follow; As motives for signing up for a smoking cessation clinic, 47.8% responded that they were recommended by others. 61.4% said that they had tried to stop smoking for the past one year. As the first reason for stopping smoking, 45.7% responded that it was to prevent diseases in the future. As factors affecting the smoking cessation self-efficacy, the smoking temptation score was the most significant variable, followed by the smoking cessation confidence score and the smoking amount. As a factor affecting the smoking cessation counseling satisfaction, whether to perceive health problems was the most significant variable, followed by the smoking cessation confidence score and the smoking temptation score.

A Study on the Variables that Influence Stages of Change in Smoking Cessation (금연실천단계의 영향변인에 대한 연구)

  • Purpose: The purpose of this study was to identify variables that influence smoking cessation based on the transtheoretical model. Methods: Variables that influence the stages of change in smoking cessation were selected based on transtheoretical model. Data were collected from 176 men aged 30-64 who smoked, Data collection was done in community settings in Seoul and vicinity, Korea between December, 1999 and June, 2000. Data were analyzed by descriptive statistics and logistic analysis using the pc-SAS program. Results: According to the logit model, the influence of significant variables on stage of change for smoking cessation suggest that ; When the scores for Pros in decision making for smoking cessation are high, the possibility that the subjects are in the action, precontemplation and contemplation stage of change will be lower than the possibility that the subjects are in the maintenance stage of change. When the scores of habitual craving in smoking temptation is high, the possibility that the subjects are in the precontemplation and contemplation stage of change will be higher than the possibility that the subjects are in the maintenance stage of change. When the scores of smoking temptation in negative affect situations are high, the possibility that the subjects are in the contemplation stage of change will be higher than the possibility that the subjects are in the maintenance stage of change. When the scores of cognitive process in the process of change are high, the possibility that the subjects are in the contemplation stage of change will be higher than the possibility that the subjects are in the maintenance stage of change. Conclusions : The results provide evidence that physical and psychological dependence on smoking are main barriers to smoking cessation. Therefore nursing interventions based on stage that focus on increasing motivation and intention for smoking cessation need to be developed.
