연구배경: 결핵의 유병률은 감소하는 추세이나 노인, 항암약물요법, HIV 감염 풍에 의해 면역기능이 약화된 환자에 있어서 결핵의 발생이 문제가 되고 있으며, 특히 다제내성균주의 출현은 큰 문제이다. 결핵진단법 중 체액에서 도말검사나 배양검사를 통해서 Myco-bacterium tuberculosis를 검출하는 것이 표준진단법이나, 도말검사는 상대적으로 민감도가 낮고 배양검사는 오랜시간이 소요된다. 중함연쇄반응을 이용하여 결핵균의 DNA를 검출하는 방법은 매우 민감하나 비용이 많이 소요된다. ELISA(enzyme linked immunosorbent assay)를 이용하여 혈청에서 결핵균 특이항원에 대한 항체를 검출하는 방법이 객담도말음성인 활동성 폐결핵환자를 다른 비결핵성 폐질환으로부터 감별하는데 유용한지를 확인하고자 하였다. 방 법: 연세대학교 세브란스 병원에 입원한 183명의 객담도 말음성인 폐질환환자에서 항산균에 대한 객담도말 및 배양검사와 결핵균 특이 항원인 recombinant 38 kilo-dalton 항원과 culture filtrate항원을 이용한 ELISA 검사를 시행하였다. 결 과: 35명의 활동성 폐결핵환자 중 18명에서 배양검사상 Mycobacterium tuberculosis를 검출할 수 있었다. 38 kDa와 culture filtrate에 대한 흡광도는 활동성 폐결핵환자들에서 비결핵성 폐질환자들보다 유의하게 높았다 (p<0.05). 활동성 폐결핵환자 중 객담배양양성과 음성군사이에 38 kDa와 culture filtrate에 대한 홉광도의 유의한 차이는 없었다 (p>0.05) 객담도말음성인 환자에 있어서 38 kDa과 culture filtrate의 민감도는 각각 20.0%와 31.4%였고 특이도는 각각 95.3%와 93.9%였다. 결 론: 객답도말검사 음성인 활동성 폐결핵환자에 있어서 ELISA를 이용한 38 kDa과 culture filtrate 항원에 대한 혈청항체검사법은 기존진단방법보다 낮은 민감도를 보여, 비결핵성 폐질환으로부터 활동성 폐결핵을 감별진단하는데 크게 도움이 되지 않을 것으로 여겨진다.
목적 : 청소년기 폐결핵의 침범위치에 따른 임상양상 및 배양률의 차이에 대해 알아보고자 하였다. 방법 : 2000년부터 2008년까지 광주기독병원에서 폐결핵으로 외래 및 입원치료 했던 10-20세 환자를 대상으로 의무기록지와 흉부방사선 소견을 통한 후향적 조사를 시행하였다. 결과 : 총 66명 중 90.9%가 15-20세였고, 평균나이는 16.82세(범위 10-20세)였다. 기침(74.2%), 객담(60.6%), 발열(39.5%) 순으로 많았다. 도말검사 42.4%, 배양검사 60.6%, PCR검사 69.7%의 양성률을 보였다. 공동형성이 18명, 가슴막삼출이 18명에서 보였다. 객담도말검사에서 침범엽수가 많을수록 양성률은 통계적으로 의미있게 높았다(P<.05). 치료기간은 평균 7개월이었다. 하폐야 결핵 12명(A군), A군을 제외한 폐병변이 있는 44명(B군), 결핵 가슴막염만 있는 10명(C군)으로 구분하였으며, 각 군간의 임상양상 및 배양 양성률의 차이는 통계적인 유의성은 없었다. 결론 : 청소년기 후반으로 갈수록 폐결핵이 증가하고, 폐결핵의 특징적인 방사선 소견으로 진단하기 어려운 하폐야 결핵에 대한 관심이 필요하겠다.
연구배경 : 최근 국내에서도 NTM 분리율 및 감염 빈도가 증가하고 있는 추세이다. 본 연구는 국내 한 삼차의료기관에서 1년 동안 항산균 검사가 의뢰된 호흡기 검체에서 NTM의 배양양상 및 분리비율을 살펴보고 그 임상적 의의에 대해 알아보고자 하였다. 방 법 : 2002년 1년간 서울아산병원에서 미생물검사실로 항산균 검사가 의뢰된 호흡기 검체를 대상으로 도말, 배양 및 동정 결과를 확인하고 그 임상적 의의를 분석하였다. 결 과 : 총 9,821명의 환자에서 26,820건의 항산균검사가 시행되었다. 결핵균이 동정 된 건수와 환자수의 비율은 각각 5.7%, 6.3%였고, 비결핵항산균은 각각 2.2%, 2.8% 이었다. 도말양성, 배양양성 중 결핵균 및 NTM의 비율은 각각 87.8%, 12.2%이었다. NTM 중 오염의 가능성이 높은 암색소성-저집락군은 31.8%(186/584 균주)에서 분리되었다. 암색소성-저집락군을 제외한 NTM 중 MAC, M. fortuitum complex의 분리비율이 42.1%(138/328), 19.5%(64/328)로 가장 높았다. 암색소성-저집락군을 제외한 NTM이 분리된 275명 중 미국 및 영국 흉부학회 진단기준을 만족하는 NTM 폐질환은 각각 8.4%(23명), 17.8%(49명)이었다. 결 론 : 국내 한 대학병원에서의 항산균 검사 결과 NTM의 분리 비율이 높고 일부는 NTM 폐질환의 원인이어서 항산균 도말 및 배양 양성의 경우 국내에서도 반드시 NTM의 가능성을 고려하여야 하겠다.
지난 1984년의 제1차 보고에 이어 1984년부터 1992년까지 9년간 서울 백병원 외래 및 입원환자 중 기생충검사를 의뢰한 52.552명에 대하여 포르말린-에테르 집란법 및 직접 도말법으로 대변검사를 시행하였던 바 그 결과는 다음과 같았다. 총 윤충류(13종) 충란 및 원충류(5종) 포낭 양성률은 각각 평균 6.5% 및 2.5%이었다. 윤충류 종별 양성률(양성자수)은 간흡충 3.2% (1.667). 편충 2.1%(1 089) 요꼬가와흡충 1 2%(613), 회충 0.2%(100) 동양모양선충 0.06%(34). 유무구조충 0.05%(28). 왜소조충 0.03%(18). 구충 0.03%(17). 극구흡충류 0.03%(15), 폐흠충 0.02%(12). 요충 0.02%(10) 분선충 0.01%(6), 광절열두조충 0.004% (2)이었다. 원충 별로는 대장아메바 1.1%(588). 왜소아메바 0.8%(402), 람블편모충 0.3% (173), 이질아메바 0.3%(164). 장트리코모나스 0.004%(2)로 나타났다. 조사년도별로 볼 때 회 충 및 편충 등 토양매개성 윤충은 뚜렷한 감소를 보이는 반면 간흡충을 비롯한 패류 매개성 윤충과 장내 원충류는 양성률의 감소를 인정할 수 없었다.
목적: 대학병원에 근무하던 중 활동성 폐결핵으로 진단된 전공의에게 노출된 의료종사자들에 대하여 실시한 접촉자 조사의 결과를 보고한다. 방법: 활동성 폐결핵 환자와 밀접한 접촉을 한 사람(접촉자) 101명 중 89명이 접촉자 조사에 응하여 조사를 받았다. 1차 접촉자 조사는 지표환자의 증상 개시 후 30일경에, 2차 조사는 1차 조사 10주 후에 기침, 발열, 인후통, 체중 감소와 같은 임상증상 확인 및 단순흉부촬영과 함께 결핵반응검사(Tuberculin skin test, TST)/QuantiFERON-TB Gold (QFT-G) 2단계 검사법을 시행하였다. 결과: 1차 TST 양성자는 34명(38.2%)이었고, TST 양성자 중 35세 이하 접촉자에서 시행한 QFT-G 양성률은 37.5% (6/16)이었다. 1차 TST 음성 대상자 41명에게서 시행한 2차 TST에서 17명(41.5%)이 양전을 보였고 그들 중 시행한 QFT-G 검사에서 3명(17.6%)이 양성이었다. 활동성 결핵으로 진단된 접촉자는 없었으며 지표환자에 노출되어 발생한 결핵 전파율은 2단계 검사법으로는 7.3% (3/41)였고, TST 진단법으로는 41.5% (17/41)였다. 결론: 국내에서 처음 보고되는 병원 내 결핵 접촉자 조사 연구로서 LTBI 발생률이 그 진단 기법에 따라 달랐으며 따라서 앞으로 발생할 수 있는 의료종사자들에 대한 병원 내 결핵 접촉자 조사를 위해 조직적이고 실용적인 가이드라인이 필요할 것이다.
The present study was undertaken to evaluate the prevalence rate of Taenia species infection and distribution of taeniasis caused by Taenia solium among Koreans in Seoul and Cheju Do In Korea during the period from August to December 1984. A total of 4,256 stool specimens from the students of Mapo Ku and 360 stool specimens from the inhabitants of Guro-Ku in Seoul was collected and examined by cellophane thick smear technique. On the other hand 1,015 stool specimens from the students of Jocheon Myun and 265 specimens from the students of Jocheon Myun and 265 specimens from the inhabitants of Aeweol Myun and Gujwa Myun in Cheju Do were also examined. The results were summarized as follows ; Four (0.1%) out of 4.256 students were positive and 2 (0.6%) out of 360 inhabitants in Seoul were positive (Table 1, 2). Positive rates of taeniasis in Cheju Do were 1.7% (17) out of 1,015 students and 12.5% (33) out of 5 villagers (Table 3, 4). In order to observe the distribution of Taenia solium infection, the scolex or a part of Taenia spp. were collected from the stool of positive cases by anthelmintic treatment. For the species identification, expelled proglottides were examined microscopically by the number of branches of the uterus, presence of vaginal sphincter or the accessory ovarian lobe etc. Three cases were infected with Taenia solium among 6 egg positive cases in Seoul. But only 1 case was infected with Taenia solium out of 7 students taking anthelmintics voluntarily by recommendation of Korean Association for Parasite Eradication (KAFPE). On the other hand, among 32 cases of egg positive cases of this study and 26 cases of KAFPE in Cheju Do, 13 cases (22.4%) were infected with Taenia solium. But 13 cases who were not examined and complained expulsion of proglottides in their stool were infected with Taenia saginata (Table 5). Among 62 persons infected with Taenia saginata, only 4 cases did not know their infection of this worm. Rut in 17 cases with Taenia solium, 7 persons did not know their infection until stool examinations were performed (Table 6).
Cerebrospinal fluid (CSF) cytology is an effective tool for evaluating diseases involving the central nervous system, but this technique is usually limited by its low cellularity and poor cellular preservation. Here we compared the manual liquid-base $Liqui-PREP^{TM}$ (LP) to the cytospin (CS) with using a mononuclear cell suspension and we applied both methods to the CSFs of pediatric leukemia patients. The cytopresevability, in terms of cell yield and cell size, and the clinical efficacy were evaluated. When 2000 and 4000 mononuclear cells were applied, LP was superior to CS for the cell yield, 16.8% vs 1.7% (P=0.001) and 26.2% vs 3.5% (P=0.002), respectively. The mean size of the smeared cells was 10.60 ${\mu}m$ in the CS, 5.01 ${\mu}m$ in the LP and 6.50 ${\mu}m$ in the direct smear (DS), and the size ratio was 1.7 (CS to DS), 0.8(LP to DS) and 2.1 (CS to LP), respectively. As compared to the cells in the DS, the cells in the CS were significantly enlarged, but those in the LP were slightly shrunken. Upon application to 109 CSF samples, 4 were diagnosed as positive for leukemia (positive), 4 had atypical cells and 101 were negative by CS; 6 were positive, one had atypical cells and 102 were negative by LP. For six cases, in which 4 were positive for leukemia and 2 of 4 had atypical cells by CS, they were positive by LP and they were also confirmed as positive according to the follow-up study. Three cases diagnosed as atypical cells (two by CS and one by LP), were confirmed as negative. In conclusion, these results suggest that LP is superior to CS for the cytopresevability and for rendering a definite diagnosis of cerebrospinal fluid.
Background: There are several active tuberculosis (TB) cases in Korean high schools each school year. The risk of transmission in schools is extremely high due to the considerable time spent in closed classrooms. We evaluated the control of latent tuberculosis infection in Korean high schools. Methods: When a student was identified with active TB, tuberculin skin testing was performed on their classmates and on students in their same school grade. When a student had a positive tuberculin skin tests (TST), they underwent follow-up testing with QuantiFERON-TB Gold In-Tube (QFT). The manufacturer recommended a cut-off of 0.35 IU/mL to determine QFT positivity was applied. Results: A total of 131 pulmonary tuberculosis (TB) patients were included based on the criteria for screening TB contacts in the National Tuberculosis Control Program. Seventy-five (57.2%) students tested smear positive. TST were performed on 7,109 students who were classmates of, or in the same grade as, a TB patient. Of the contacts, 1,231 students (17.3%) were TST positive and they were screened with QFT. Six hundred-sixty-six (55.0%) of the tested students returned a positive QFT result and the rate of positivity was significantly associated with the increasing size of TST indurations (p<0.0001). Conclusion: The use of QFT resulted in approximately 45% of TST positive students not being given chemoprophylaxis.
Cervical cancer is one of the most frequent cancers in women worldwide. Persistent infection with a human papillomavirus (HPV) is the main cause for cervical cancer. Vaccination and Pap smear screening are the best methods for prevention of the disease. The objective of this cross-sectional study was to assess the knowledge and views of upper secondary school female students in Kuala Lumpur, Malaysia, toward prevention of cervical cancer. This study was conducted from April 2009 to September 2009 in 8 schools in Kuala Lumpur area using pre-tested and validated questionnaires. Results indicated that the respondents had low knowledge of cervical cancer and its prevention although the majority of students (80.4%) had heard about the disease. The level of knowledge of cervical cancr and its prevention was significantly higher among students from the science stream (p<0.001) compared to students from the art stream. Most students (69.3%) agreed to take the vaccination if the service was available in schools. A high percentage of students (82.2%) agreed that the vaccination should be compulsory to the students. In conclusion, most students had low knowledge of cervical cancer and its prevention but they had positive attitude toward vaccination and agreed that vaccination should be compulsory. Therefore, suitable educational programmes should be developed to improve the knowledge of secondary school students on the prevention of cervical cancer.
Is a multiple number of sputum specimens necessary for the diagnosis of pulmonary tuberculosis? To answer this question, 6844 respiratory specimens obtained from previously untreated patients suspected of having pulmonary tuberculosis between 1998 and 2001 were evaluated retrospectively. All of the specimens were evaluated by acid fast bacilli smear and BACTEC 460 TB culture system. A total of 785 (11%) specimens from 353 patients were positive for Mycobacterium tuberculosis complex. For 76% (270/353) of these patients the organism was detected from sputum specimens collected sequentially for daily basis. Mycobacterium tuberculosis was isolated in the first, second and third samples of the majority (98%,195/199) of patients who had three or more sputum samples sent to the laboratory. Our results indicate that, we could carry out Mycobacterium tuberculosis isolation in the first, second and third sputum samples of the overwhelming majority of the patients and the diagnostic value of four or more sputum specimens submitted to the laboratory was very low (2%). We recommend that, for definitive and cost-effective diagnosis of pulmonary tuberculosis at least three sequential sputum specimens be collected for all patients suspected pulmonary tuberculosis.
