스마트폰을 이용한 응급환자 인식 및 구조요청 시스템설계 및 구현 (Design and Implementation of Identifying and Requesting Rescue System for Emergency Patient Using Smartphones)

  • For emergency patients as heart disease, fast treatment is very important. This paper describes a smart phone software to detect emergency circumstance and request rescue. Detection of emergency condition of heart disease patients is based on physical motion and biological heart signals as electrocardiogram. Most smart phones have three axis acceleration sensor. On emergency condition, patients remain stationary. Thus the software detect stationary condition by using acceleration sensor data. For more precise detection, it combines electrocardiogram of patients. To request rescue, it sends help messages to designated persons. In addition, it generates emergency sound to surrounding people and plays a video of emergency measure that any person on the place can help the patient temporarily.

Development of Retina Healthcare Service System Using Smart Phone

  • In this paper, we have developed a Retina Healthcare Service System through which the patient himself/herself can manage his/her retina health. In the case of conventional portable ophthalmic cameras, patients cannot check their eye health on their own because most of them are used by doctor in environments where ophthalmography cannot be performed properly. This system consists of web, app and camera modules, and when a patient mounts a camera module for fundus photography on his / her smart phone and then photographs his / her fundus through the app, the image is transmitted to a server, and the transmitted image reads the fundus the patient's fundus image status in the fundus image reading model learned using deep learning. When the doctor expresses his/her opinions about the patient 's eye condition based on the reading result and the fundus photograph, the patient can check through the app and judge whether to receive ophthalmologic treatment.

  • With the advance of Information Technology (IT) and dynamic requirements, diverse application services have been provided for end users. With huge volume of these services and information, users are required to acquire customized services that provide personalized information and decision at particular extent of time. The case is more appealing in healthcare, where patients wish to have access to their medical record where they have control and provided with recommendation on the medical information. PHR (Personal Health Record) is most prevailing initiative that gives secure access on patient record at anytime and anywhere. PHR should also incorporate decision support to help patients in self-management of their diseases. Available PHR system incorporates basic recommendations based on patient routine data. We have proposed decision support service called "Smart CDSS" that provides recommendations on PHR data for diabetic patients. Smart CDSS follows HL7 vMR (Virtual Medical Record) to help in integration with diverse application including PHR. PHR shares patient data with Smart CDSS through standard interfaces that pass through Adaptability Engine (AE). AE transforms the PHR CCR/CCD (Continuity of Care Record/Document) into standard HL7 vMR format. Smart CDSS produces recommendation on PHR datasets based on diabetic knowledge base represented in shareable HL7 Arden Syntax format. The Smart CDSS service is deployed on public cloud over MS Azure environment and PHR is maintaining on private cloud. The system has been evaluated for recommendation for 100 diabetic patients from Saint's Mary Hospital. The recommendations were compared with physicians' guidelines which complement the self-management of the patient.

  • In this paper, we propose a telemedicine system based ECG data using a bio-signal meter and a smart device for treating faraway patients. This system is composed of a patch-shaped portable bio-signal meter, patient's smart device application, and doctor's PC software. Using these components, doctors and patients can do telemedicine. First, a patient measures his own ECG signal with a bio-signal meter and send the data to a doctor using a smart device application. Then, the doctor checks the ECG data, and make and send a diagnosis chart to web server. Likewise, doctors and patients can be offered a medical environment without time and space restraints. Applying this system to real medical system can improve the problem of low accessibility and efficiency and also can reduce medical expenses.

  • 최근 노인 인구가 급격하게 증가하면서 의료시설 내외에서 환자의 안전을 모니터링하는 시스템에 대한 요구가 증가하고 있다. 센서 및 정보통신기술의 발달로 치매 환자를 위한 다양한 모니터링 시스템이 제안되고 있지만 치매 환자에게 부착되는 장치에 의존하여 단편적인 기능을 제공하는 것에 그치고 있다. 본 논문에서는 환자의 상태를 모니터링하는 스마트 기기에서부터 사용자 편의적인 UI/UX, 병원정보시스템 연동까지 포함하는 통합시스템을 설계한다. 제안된 시스템을 통해서 의료진과 보호자는 환자의 상태와 관련된 정보를 실시간으로 전달받을 수 있으며 실내외에서도 치매 환자의 위치를 추적할 수 있다.

  • Purpose: The purpose of this study was to develop and implement the Hospice Smart Patient Program and to evaluate its effectiveness. Methods: It was quasi-experimental non-equivalent pre-post study. Breast cancer patients who underwent surgery, chemotherapy or radiotherapy, or who needed palliative care, participated in the study. Participants were divided into two groups, experimental and control groups based on their preferences. The program was developed after literature review and discussion among experts on hospice and palliative care. Participants who were in the experimental group received either face-to-face or phone "Hospice Smart Patient" Service at least once a week for 5 months. Results: There was a significant difference in quality of life and communication skill between the two groups after the service was provided. In addition, participants in experimental group showed improved decision making skills, mastery sense, and understanding of hospice and palliative care, which would be beneficial in improving their quality of life. Conclusion: We have concluded that the "Hospice Smart Patient" Program is useful for cancer patients in decision making, improving self-control and choosing hospice care to improve their quality of life.

  • Purpose: The purpose of the study was to develop an educational program reflecting the educational needs of Hospice Smart Patient service providers. Method: The description, goal, curriculum, method, and process evaluation of the educational program were constructed based on Modified Tyler-type Ends-Means Model followed by the analysis of current curriculum and needs of service providers. Results: The curriculum was constructed based on hospice volunteer program currently offered in Korea and the recommendations of hospice service volunteers and experts. A total of 90 hr was required to complete the curriculum that was composed of 'Introduction to cancer', 'Treatment and treatment complications of cancer', 'Post-treatment nutritional care', 'Helpful information', 'Introduction to hospice and palliative care', 'Comprehension of life and death', 'Holistic hospice and palliative care', 'How to communicate as a smart patient', 'Hospice and ethics', 'Pediatric hospice', 'Bereavement management', and 'Clinical practicum'. Conclusion: It is necessary to implement the developed educational program and evaluate its effectiveness, as well as making the service available to a greater number of cancer patients.

Performance evaluation study of a commercially available smart patient-controlled analgesia pump with the microbalance method and an infusion analyzer

  • Background: Patient-controlled analgesia (PCA) has been widely used as an effective medical treatment for pain and for postoperative analgesia. However, improper dose errors in intravenous (IV) administration of narcotic analgesics from a PCA infusion pump can cause patient harm. Furthermore, opioid overdose is considered one of the highest risk factors for patients receiving pain medications. Therefore, accurate delivery of opioid analgesics is a critical function of PCA infusion pumps. Methods: We designed a microbalance method that consisted of a closed acrylic chamber containing a layer and an oil layer with an electronic balance. A commercially available infusion analyzer (IDA-5, Fluke Co., Everett, WA, USA) was used to measure the accuracy of the infusion flow rate from a commercially available smart PCA infusion pump (PS-1000, UNIMEDICS, Co., Ltd., Seoul, Korea) and compared with the results of the microbalance method. We evaluated the uncertainty of the flow rate measurement using the ISO guide (GUM:1995 part3). The battery life, delay time of the occlusion alarm, and bolus function of the PCA pump were also tested. Results: The microbalance method was good in the short-term 2 h measurement, and IDA-5 was good in the long-term 24 h measurement. The two measurement systems can complement each other in the case of the measurement time. Regarding battery performance, PS-1000 lasted approximately 5 days in a 1 ml/hr flow rate condition without recharging the battery. The occlusion pressure alarm delays of PS-1000 satisfied the conventional alarm threshold of occlusion pressure (300-800 mmHg). Average accuracy bolus volume was measured as 63%, 95%, and 98.5% with 0.1 ml, 1 ml, and 2 ml bolus volume presets, respectively. A 1 ml/hr flow rate measurement was evaluated as 2.08% of expanded uncertainty, with a 95% confidence level. Conclusion: PS-1000 showed a flow accuracy to be within the infusion pump standard, which is ± 5% of flow accuracy. Occlusion alarm of PS-1000 was quickly transmitted, resulting in better safety for patients receiving IV infusion of opioids. PS-1000 is sufficient for a portable smart PCA infusion pump.

  • 본 논문은 스마트 글래스 기반 응급원격의료지도 시스템을 제안한다. 여기서, 원격의료지도 시스템이란 병원의 전문의사가 현장의 해양경찰이나 응급구조사에게 원격의료지도를 수행할 때 사용하는 시스템을 의미한다. 시스템 개발에 필요한 요구사항을 파악하기 위해 관련 기술 동향 및 사례를 분석하였다. 분석을 바탕으로 3가지 시스템 요구사항(1. 환자 수송의 필요성을 결정할 수 있어야 함, 2. 병원으로 수송 중 응급 의료 제공에 도움을 주어야 함, 3. 의료 시설로 환자 정보 전달이 가능해야 함)을 정의하였다. 그리고, 요구사항을 충족하는 프로토타입을 개발하고 사용성 평가를 수행하였다.
