중소규모 병원의 리모델링 계획 특성에 관한 연구 - 300병상 종합병원 계획사례를 중심으로 (A Study on the Characteristics of Remodeling Planning in Small-mid sized Hospital - Focused on the case study of 300beds general hospital)

  • Purpose: This study is a case study of a small-mid sized hospital to promote a full-scale remodeling to ensure the quality and competitiveness of the medical services and trying to arrange the remodeling characteristics of small-mid sized hospital which is suffering from aging facilities as well as chronic congestion and lack of spaces. Methods: Research was conducted by consultation with hospital executives and each department operators, and on-site investigation, Results: The result of this study can be summarized into two points. The first one is that existing statistical value like area per bed, in the remodeling of the small-mid hospitals may not mean much. planners have to verify the necessary room space areas in close consultation with the department operators. And after confirming layout possibilities, they have to create the program. The second one is that remodeling planning can be a realistic plan with structural and installation diagnosis at the same time. If so, it is advisable to proceed architectural design from the beginning for cost and time savings. Implications: This results can be applied to small-mid hospitals to apply to the medical law revision and others.

소형종합병원 간호사의 직무스트레스와 조직몰입의 관계에서 직무만족의 역할 (The Role of Job Satisfaction in the Relationship between Job Stress and Organizational Commitment among Nurses in Small-sized General Hospitals)

  • Objectives : The purpose of this study was to analyze the effects of job stress on organizational commitment among nurse working in small sized general hospitals and determine moderating effects of job satisfaction. Methods : A self-report questionnaire survey was completed by 292 nurses working at five small sized hospitals in P and K. Data were collected during March 2015 and analyzed with the SPSS 21.0 program. Results : Job satisfaction was a positive factor in organizational commitment. Job satisfaction had significant moderating effects on the relationship between job stress and organizational commitment of nurses working in small sized general hospitals. Conclusions : The results of this study indicate that various job stresses and job satisfaction have a significant effect on the organizational commitment of nurses working in small sized general hospitals. Therefore, to decrease the job stress and increase the job satisfaction of nurses working in small sized general hospitals, consideration must be given to improving the financial and psychological rewards as well as providing differentiated job training. To reduce the psychological burden of nurses, it is necessary to develop adaptation programs of nurses.

병원규모에 따른 간호사의 조직문화유형과 직무만족 비교연구 (A Comparative Study on Nurses' Organizational Culture and Job Satisfaction according to the Hospital Size Differences)

  • Purpose: This study was conducted to compare the characteristics of organizational structure and occupational satisfaction among nurses in general hospital and small to medium-sized hospital and to investigate the affecting factors on their occupational satisfaction. Methods: The study was based on the cross-sectional descriptive survey. A self-report questionnaire was used to collect data from 343 nurses between June and July, 2010. Data were analyzed by ${\chi}^2$-test, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient, and multiple regression using the SPSS/WIN 14.0 program. Results: Hierarchical structure was dominant in general hospital whereas relationship was highly valued in small to medium-sized hospital. Occupational satisfaction was positively correlated with work environment built on relationship, innovation and tasks. Factors significantly influencing on occupational satisfaction in general hospital included innovative work environment, nurses' income and their health status ($R^2$=40.3%). For the small to medium-sized hospital, they included innovative work environment, satisfaction in life, tasks and professionalism ($R^2$=40.4%). Conclusion: Organizational structure, especially innovative work environment and relationship-oriented attitude had a significant influence on nurses' occupational satisfaction. Therefore, nursing administrators have to develop and consider organizational structure to improve occupational satisfaction.

소비가치 이론을 이용한 의료소비자의 의료기관 선택 요인 분석 : 중소병원, 종합병원, 대형종합병원 비교 중심으로 (A study on healthcare institution selection of healthcare consumers using theory of consumption values : Focusing on relations among clinics or small sized hospitals, general hospitals, and large-sized hospitals)

  • The healthcare environment today is changing rapidly with factors of healthcare consumers in selecting medical institutions also altering at a fast pace under the circumstances. In this study, the theory of consumption values established by Sheth in 1991 is adopted in order to examine particular value affecting consumer selection of healthcare institutions. For the purpose of this study, healthcare consumers were surveyed using questionnaires developed based on the five values of Sheth supplemented by value of effort to acquire hospital information and value in health. Consequently, 24 consumption values affecting selection process were confirmed through discriminant analysis. As a result of regression analysis on factors affecting consumer selection of healthcare institution, effort to acquire hospital information and age among demographic characteristics of respondents are determined important predictors for consumer selection of general hospitals over clinics or small-sized hospitals. Further, service, reputation scale of healthcare institution among functional values and importance of health and effort to acquire hospital information among value in health are identified as significant predictors for consumer selection of large-sized general hospitals over clinic or small-sized hospitals. This study suggests not only vital implications for marketing strategy of healthcare institutions, but also methods to promote positive image for healthcare providers. In addition, this study closely examines the cause of the leaning phenomenon of healthcare comsumers toward large-sized general hospitals.

중소병원 간호사의 경력개발시스템 기대감, 경력몰입, 이직의도와의 관계 (Relationship between Expectations of Clinical Ladder System, Career Commitment and Turnover Intention of Nurses in Small and Medium-Sized Hospitals)

  • Purpose: This study was done to explore the relationship between expectations of the clinical ladder system (CLS), career commitment and turnover intention in nurses employed in small-medium sized hospitals. Methods: Participants were 154 nurses from 3 small-medium sized hospitals in Gyeonggi Province and Gwangju City. From February, 8 to April, 14, 2017, self-report questionnaires were collected and analyzed using frequency, t-test, ANOVA, $Scheff{\acute{e}}$ test, and Person correlation coefficients. Results: Of the participant nurses, 61% perceived the CLS as needed. The mean score for expectation of CLS, career commitment, and turnover intention on 5-point scale were 3.38, 3.17, 3.21, respectively. There were no significant differences in expectation of CLS according to general characteristics, but career commitment and turnover intention did show significant differences depending on age, position, type of work shift. Expectation of CLS correlated positively with career commitment (r=.23, p=.005), and career commitment correlated negatively with turnover intention (r=-.49, p<.001). Conclusion: The results of this study show that there is a need to adopt the clinical ladder system to improve career commitment.

중소병원 응급실 간호사 업무에 대한 영향요인 (Factors Affecting Emergency Room Nurse Job in Small and Medium Sized Hospitals)

  • Purpose: The purpose of this study was to identify the job of emergency room (ER) nurses working in small and medium sized hospitals and to explore factors affecting their job. Methods: The survey data were collected between January 2014 and May 2014 and participants were 159 nurses working in Seoul, Kyunggi, Incheon, and Chungnam in hospitals of less than 500 beds. Results: The score for nurses job was 2.12/4.0, and emergency treatment (1.87/4.0) followed basic nursing (3.51/4.0) and counseling (2.32/4.0). The nursing job was significantly different depending on the age, education level, position, resident doctor(emergency medicine specialty or other) and grade of ER (regional ER or local ER). In the multiple regression, education level (${\beta}=.18$), position (${\beta}=.24$), hospital size (${\beta}=.20$), and grade of ER (${\beta}=.21$) explained 17.0% of variance in ER nurses' job in small and medium sized hospitals. Conclusion: The findings indicate that ER nurses in vulnerable areas do more nursing practice including emergency treatment as well as the usual independent nursing practice. Accordingly, a systematic assignment of nursing professionals is needed to reduce loading of ER nurses in small and medium sized hospitals.

중소병원 입원환자의 낙상발생 시 현황, 낙상위험요인 및 낙상발생 예측요인 (Circumstances, Risk Factors, and the Predictors of Falls among Patients in the Small and Medium-sized Hospitals)

  • Purpose: This study examined the circumstances, risk factors, and the predictors of fall incidents among patients in the small and medium-sized hospitals. Methods: Fifty patients with any fall experiences were matched by gender, age, and medical departments with 100 patients without fall incident at the same hospital. Data were collected from 5 small and medium-sized hospitals. Data were analyzed using descriptive statistics, a Chi-square test, a Fisher's exact test, and a logistic regression with the SPSS/WIN 21.0 program. Results: In the patients with falls, the largest number of falls occurred during the day shift, in the patients' rooms, and while they were walking. Further 74.0% of the patients had physical injuries, and 34.0% had to take further medical diagnostic tests. Significant differences were found between the patients with falls and the others on 14 variables (cardiovascular disease, anemia, sedative-hypnotics, vasodilators, narcotic analgesics, dizziness, general weakness, unstable gait, walking aids, anger, anxiety, depression, orientation, and fear of fall). Narcotic analgesic use, dizziness, walking aids, and cardiovascular disease were identified as the predictors of fall incidents. Conclusion: These findings are hoped to be used in developing a fall risk assessment tool and fall prevention nursing programs for small and medium-sized hospitals.

중소병원 간호사의 성인애착유형에 따른 직무스트레스 및 조직몰입 (Job Stress and Organizational Commitment with Regard to Adult Attachment Style of Small and Medium-sized Hospital Nurses)

  • This study was designed to identify the adult attachment style among hospital nurses, and to examine its effect on nurses' job stress and organizational commitment. Self-administered survey questionnaires with structured instruments were developed, and a total of 281 responses from five small to medium-sized hospitals were used for statistical analyses with SPSS Windows version 24.0 program. Major findings of the study are three fold. First, among the four styles of adult attachment, found in 30.6% of survey respondents, stability style was the most popular, followed by fear style(28.5%), disregard style(24.2%), and devotion style(17.8%). Second, according to the general characteristics of study participants, adult attachment style showed a significant difference in age($x^2=23.29$, p=.025). Third, multiple regression analyses showed that hospital nurses with the fear style among other adult attachment styles had a significantly higher job stress compared to those with the stability style. The fear style also affected the nurses' organizational commitment level in a negative way, but the effect was only marginal. Results from this study suggest that customized program based on the nurses' adult attachment style should be developed to enhance their organizational commitment while reducing job stress in small to medium-sized hospitals.

중소 종합병원 입원환자와 간호사의 간호서비스에 대한 기대와 지각, 질 평가와 만족도 차이 (The Differences in Quality Perceptions, Expectations, Evaluation, and Satisfaction for Nursing Service between Patients and Nurses: Small-medium Sized General Hospitals)

  • Purpose: This study was performed to give direction to quality improvement strategies of nursing services by comparing the differences in quality perceptions and satisfaction for nursing services between patients and nurses in small-medium sized general hospitals with 200 beds. Method: The subjects, who were 150 inpatients and 162 nurses of 4 general hospitals in a community, answered a self-report questionnaire with a SERVQUAL scale. Result: There were differences between patients' and nurses' expectations and perceptions of nursing service and satisfaction. In the service expectation, the highest factor was 'the responsiveness', and in the perceived performance, the highest was the 'assurance'. In addition, overall patients' perceptions on nursing services showed higher than nurses'. There were positive correlations among the expectations and perceptions on nursing service, and satisfaction. The correlation between perception and satisfaction was higher than the correlation between expectations and satisfaction. Conclusion: To improve the nursing service quality at small-medium hospitals, strengthening the 'assurance' factor and improving the nursing service support system is needed. Also, this study on nurses' perceived nursing service at small-medium sized hospitals should be duplicated.

중소병원 간호사의 사건보고태도와 환자안전간호활동의 관계에서 환자안전관리 중요성 인식의 매개효과 (Mediating Effects of Perceptions Regarding the Importance of Patient Safety Management on the Relationship between Incident Reporting Attitudes and Patient Safety Care Activities for Nurses in Small- and Medium-sized General Hospitals)

  • Purpose : The purpose of this study was to examine the mediating effect of perceptions regarding the importance of patient safety management in the relationship between incident reporting attitudes and patient safety care activities for nurses in small-and medium-sized general hospitals. The objective was to provide a basis for planning tailored training programs aimed at improving patient safety care activities. Methods : This study was conducted with 187 participants in small- and medium-sized general hospitals in K city in South Korea from March 15 to March 31, 2019. The data collected from participants were analyzed using descriptive statistics, a t-test, ANOVA, Pearson's correlation coefficients, and a multiple regression using IBM SPSS/WIN 21.0 software. Results : Patient safety care activities were found to be correlated with incident reporting attitudes (r=.27, p < .001) and perceptions of the importance of patient safety management (r=.59, p < .001). Further, perceptions of the importance of patient safety management had a complete mediating effect (${\beta}=.409$, p < .001) on the relationship between incident reporting attitudes and patient safety care activities. Conclusion : Based on the findings of this study, tailored training programs regarding patient safety care activities focused on boosting perceptions of the importance of patient safety management are highly recommended to improve nurses' patient safety care activities in small- and medium-sized general hospitals.
