Efficacy and Cost-Effectiveness of Portable Small-Bore Chest Tube (Thoracic Egg Catheter) in Spontaneous Pneumothorax

  • Background: Primary spontaneous pneumothorax is commonly treated with chest tube insertion, which requires hospitalization. In this study, we evaluated the efficacy, costs, and benefits of a portable small-bore chest tube (Thoracic Egg; Sumitomo Bakelite Co. Ltd., Tokyo, Japan) compared with a conventional chest tube. Methods: We retrospectively analyzed all primary spontaneous pneumothorax patients who underwent treatment at Gangnam Severance Hospital between August 2014 and May 2018. Results: A total of 279 patients were divided into 2 groups: the conventional group (n=236) and the Thoracic Egg group (n=43). Of the 236 patients in the conventional group, 100 were excluded because they underwent surgery during the study period. The efficacy and cost were compared between the 2 groups. There was no statistically significant difference between the groups regarding recurrence (conventional group, 36 patients [26.5%]; Thoracic Egg group, 15 patients [29.4%]; p=0.287). However, the Egg group had statistically significantly lower mean medical expenses than the conventional group (433,413 Korean won and 522,146 Korean won, respectively; p<0.001). Conclusion: Although portable small-bore chest tubes may not be significantly more efficacious than conventional chest tubes, their use is significantly less expensive. We believe that the Thoracic Egg catheter could be a less costly alternative to conventional chest tube insertion.

  • Purpose: This study investigates patterns of small group interaction and examines the influence among graduate nursing students of online collaborative learning strategies on small group interaction patterns, task performance and learning attitude in web-based team learning environments. Method: To analyze patterns of small group interaction, group discussion dialogues were reviewed by two instructors. Groups were divided into two categories depending on the type of feedback given (passive or active). For task performance, evaluation of learning processes and numbers of postings were examined. Learning attitude toward group study and coursework were measured via scales. Results: Explorative interactions were still low among graduate nursing students. Among the students given active feedback, considerable individual variability in interaction frequency was revealed and some students did not show any specific type of interaction pattern. Whether given active or passive feedback, groups exhibited no significant differences in terms of task performance and learning attitude. Also, frequent group interaction was significantly related to greater task performance. Conclusion: Active feedback strategies should be modified to improve task performance and learning attitude among graduate nursing students.

  • The Purpose of this study is to analyze the financial statements of ocean-going shipping companies that have experienced financial difficulties since the global financial crisis. Specifically, the study conducts comparing major firm with small and medium-sized firm from fianancial point of view, analyzes the different trends of two groups. As a result, this paper finds the different characteristics between two groups. There were known many financial difficulties in ocean-going shipping companies, but this is not applied to small and medium-sized firm group. Small and medium-sized firm group grew soundly and slowly during research period. But major firm group experienced the deficit and their management condition has deteriorated considerably during that period. To cope with this difficulties, major firm group should take self-effort to improve fianacial structure and establish the risk management system.

  • [$SF_6$] 가스차단기를 개발하기 위해서는 차단기가 동작하는 동안에 차단기 내부에서의 유동 파라메터(압력, 밀도, 온도, 속도 등)의 값을 정확하게 분석하는 것이 매우 중요하며, 특히, 아크가 존재할 경우 열가스 이동속도에 대한 분석은 필수적이다. 본 연구에서는 small-gap 측정시스템을 이용하여 시험용차단기가 동작할 때 차단부에서 발생하는 열가스의 이동속도를 측정하기 위한 연구내용을 정리하였다.

고지방식이에 의한 흰쥐의 시상하부 Neuropeptide Y-면역반응 신경세포와 장내 Cajal 세포의 변화 (Changes in Neuropeptide Y-Immunoreactive Cells in the Hypothalamus and Cajal Interstitial Cells in the Small Intestine of Rats with High-Fat Diet)

  • 목 적: 고지방식이를 섭취한 흰쥐에서 식이 유발인자인 NPY와 장내 운동 조절인자인 Cajal 간질세포의 변화를 관찰하고자 하였다. 대상 및 방법: 동일 연령의 Sprague-Dawley계 수컷 흰쥐를 임의로 대조군(7마리)과 고지방식이군(7마리)으로 분리한 후, 대조군에는 일반 흰쥐 사료를 충분히 섭취시켰으며, 고지방식이군에게는 60% kcal 지방이 포함된 사료(중앙실험동물센터)를 6주 동안 물과 함께 충분히 섭취하도록 하였다. 6주 후 시상하부 조직 절편에 rabbit polyclonal NPY (1 : 1000, Abcam)항체를 반응시키고, 소장의 조직 절편에 rabbit polyclonal c-kit (1 : 400, Neuromics)항체를 반응시켜서 면역조직화학 검사를 시행하였다. 결 과: 고지방식이군의 시상하부에서 NPY의 면역 반응성이 강하게 나타났으며, 특히 PVN에서 NPY의 면역 반응 세포수의 증가를 관찰할 수 있었다. 고지방식이군에서 소장의 Auerbach's plexus 부위의 Cajal 간질 세포의 면역 반응성이 떨어졌으며, 환상근과 종주근 내 Cajal 간질 세포 수도 현저히 감소하였다. 결 론: 고지방식이를 섭취한 흰쥐에서 시상하부 NPY 면역반응 신경세포수와 발현의 증가는 NPY의 저항성과 관련되며, 소장에서 Cajal 간질 세포의 감소는 장운동의 이상을 초래할 수 있음을 나타낸다.

  • The paper shows that certain hierachical Bayes (HB) predictors for small domain data in repeated surveys "universally" or "stochastically" dominate all linear unbiased predictors. Comparison of postnatal catch-up growth according to definitions of small for gestational age infants

  • Purpose: Small for gestational age (SGA) is confusingly defined as birth weight (BW) either below 3rd percentile or 10th percentile for infants. This study aimed to compare postnatal catch-up growth between SGA groups according to different definitions. Methods: Data of 129 infants born with BW below the 10th percentile and admitted to Korea University Anam Hospital and Ansan Hospital were retrospectively reviewed. Height and weight were measured at 6, 12, and 24 months. Results were compared between group A (BW: <3rd percentile) and group B (BW: 3rd-10th percentile). Results: Group A included 66 infants and group B included 63. At age 6 months (n=122), 62.9% of group A and 71.7% (P=0.303) of group B showed catch-up growth in weight. At 6 months (n=69), 55.9% of group A and 80.0% of group B (P<0.05) showed catch-up growth in height. At 12 months (n=106), 58.5% of group A, and 75.5% (P=0.062) of group B showed catch-up growth in weight. At 12 months (n=75), 52.8% of group A and 64.1% of group B (P=0.320) showed catch-up growth in height. Up to age 24 months, 66.7%/80.0% in group A and 63.6%/80.0% in group B showed catch-up growth in weight/height. Conclusion: Despite different definitions, there were no significant differences between the two SGA groups in postnatal catch-up growth up to age 24 months, except for height at 6 months. Compared to infants with appropriate catch-up growth, low gestational age and BW were risk factors for failed catch-up growth at 6 months.
