Acute Hydrofluoric Acid Exposure: Our Clinical Experience at Emergency Centers in Two University Teaching Hospitals (2개 대학병원 응급센터에 내원한 급성 불화수소산 노출 환자에 대한 임상적 경험)

  • Purpose: We investigated the clinical characteristics and demographics of patients who suffered from hydrofluoric acid chemical injury and the mechanism of damage. Methods: We retrospectively reviewed the medical records of patients who were exposed to hydrofluoric acid from March 2004 to March 2009 and who were seen at the emergency centers in two university teaching hospitals. Results: Forty four patients out of 47 patients suffered from chemical burn, while the injuries of the remaining 3 could not be identified by the medical records. A total of 17 hydrofluoric acid chemical injury patients were enrolled during the study period, and their mean age was $29.6{\pm}7.0$. All the patients were accidentally injured by contact with the material and none of them inhaled or ingested the material. Only 6 patients wore appropriate protective equipments and 5 underwent the water irrigation for more than 10 minutes. The most common exposure area was the hand and forearm (70.5%). Less than 1% of all of the patients had their total body surface (TBS) exposed to hydrofluoric acid (mean=0.35%). The mean time interval from calcium gluconate administration to pain relief was $33.6{\pm}8.8$ hours. Conclusion: When exposed to hydrofluoric acid, it is important to wear protective equipment and undergo water irrigation for more than 10 minutes. Pain and skin damage were observed in all the patients. After treatment, we concluded that administration of calcium gluconate and pain killers was successful in relieving pain, and the prognosis was also positive for the admitted and followed up patients when less than 1% of the TBS was exposed.

  • OBJECTIVE Iontophoresis is a kind of medical treatment of penetration of several ion into a body through skin by dirrect current. In oriental medicine for iontophoresis, it has little standard and method to extract the efficient element from herbal medicine. So we studied the quantitative analyzation of extracted Corydalis Tuber. METHODS AND MATERIALS Materials are Corydaline standard and extracted Corydalis Tuber, and the measurement methods of variation by time and pH are HPLC/UV, CG/MS. RESULTS 1. The chromatography and spectrometer of HPLC/UV on extracted Corydalis Tuber has no corydaline. 2. The CG/MS on extracted Corydalis Tuber has no corydaline. CONCLUSION According to the above results, when we extract herbal medicine for iontophoresis, we should consider special nature of the object herbal medicine to take the efficient element. And it needs more syudy on possibility of another element effect with the exception of Corydaline.

Bioactive Compounds and Antioxidant Activity of Jeju Camellia Mistletoe (Korthalsella japonica Engl.) (제주 동백나무 겨우살이의 용매별 기능성 성분 및 항산화 작용)

  • Mistletoes are hemi-parasitic plant growing on different host tree and shrubs. They are traditionally used in folkloric medicine for the treatment of diarrhea, cough, diabetes, hypertension, cancer and skin infection. The purpose of this study was to determine the contents of phenolics and antioxidant activity of 70% ethanol, 100% methanol and hot water extracts of Jeju camellia mistletoe (Korthalsella japonica Engl.). Ethanol was most effective in extracting total phenols (7,427 mg gallic acid equivalent (GAE)/100 g) and flavonoid (1,777 mg rutin equivalent (RE)/100 g). The free radical scavenging activity, 1,1-diphenyl-2-picryl hydrazyl (DPPH) (EC50 = 7.8 mg/ml) and hydrogen peroxide (H2O2) (EC50 = 1.4 mg/ml), and the capacity for chelating metal ions (EC50 = 8.0 mg/ml) and reducing power (EC50 = 14.9 mg/ml) of the samples also higher in ethanolic extracts. The strong correlation (r2 = −0.996~−0.881) between antioxidant capacities and the phenolic contents implied that phenolic compounds are a major contributor to the antioxidant activity of the ethanolic extracts of Jeju camellia mistletoe. As conclusions, Jeju camellia mistletoe contains bioactive substances with a potential for reducing the physiological as well as oxidative stress and this could explain the suggested cancer preventive effect of these plants as well as their protective role on other major diseases.

Antioxidant and Protective Effects of Leonurus sibiricus L. Extract on Ultraviolet B (UVB)-induced Damage in Human Keratinocytes (익모초 추출물의 항산화 효과와 자외선으로 유도된 각질형성세포 손상에 대한 보호효과)

  • In this study, we investigated the anti oxidative potential and protective effects of water extract of Leonurus sibiricus L. leaf (LSLW) against ultraviolet B (UVB)-induced oxidative damage in human keratinocytes (HaCaT cells). To evaluate the anti oxidative activity of LSLW, we measured DPPH radical, hydroxyl radical, hydrogen peroxide, superoxide anion scavenging activities, lipid peroxidation inhibition, and reducing power of LSLW. For induction of oxidative stress in HaCaT cells, the cells were irradiated with UVB at 40 mJ/㎠. To investigate the protective effects of LSLW against UVB, we measured cell viability and apoptotic bodies using annexin V staining. LSLW showed anti oxidative activities by scavenging DPPH radical, hydroxyl radical, hydrogen peroxide, superoxide anion and by reducing lipid peroxidation. In addition, LSLW showed high reducing values. The UVB-induced oxidative conditions led to cell apoptosis. However, treatment with LSLW ameliorated oxidative stress conditions, including inhibition of cell death, apoptotic body. Taken together, LSLW exhibited anti oxidative and protective effects against UVB-induced damage in HaCaT cells. Thus, LSLW could be useful for the development of cosmetics for UVB-induced skin aging.

Determination of 3-phenoxybenzoic Acid in Urine and Exposure Assessment of Pyrethroid Insecticides to Human Being (요중 3-phenoxybenzoic acid 미량 분석 및 pyrethroid계 살포자 노출 평가)

  • Pyrethroid insecticide have widely been used for agricultural sector and residential environments. To assess the exposure of insecticide which is absorbed through skin the analysis of urinary metabolite is essential. At present, the urinary 3-PBA was analyzed using liquid-phase extraction. But LPE have many limitations, such as long pre-treatment time and low recovery. So, this study was conducted to determine the optimum conditions for analysing 3-PBA in urine using solid phase extraction. Furthermore, this study intend to investigate the relation of concentrations of pyrethroid, deltamethrin in air and 3-PBA in urine. The optimum condition for hydrolysis was found to be done with hydrochloric acid for one hour. The recovery rates of 3-PBA were $84.6%{\pm}1.2%$, $54.8{\pm}0.9%$, $99.8{\pm}1.2%$ with XAD-2, XAD-7, XAD-16 using as the aborbents and acetone as eluents respectively. But acetonitrle and methanol gave low recovery rate and methyl cellosolve could not elute the compound. The amount of acetone for elution were 6mL, 9mL, 3mL for XAD-2, XAD-7, XAD-16 as absorbents respectively. The non-absorbed rates was $0.8{\pm}0.5%$, and $0.7{\pm}0.3%$ under XAD-16, mesh size 140-200, amount of resin 1.4g and the flow rate of eluent was 0.1mL/min. In the concentration process, we obtained 11 times higher concentration of material. The amounts of urinary 3-PBA were. The LODs of 3-PBA and deltamethrin were 0.004 mg/L, 0.038 mg/L, respectively. The further research of minute monitoring which include spray pattern, environmental condition is needed And more research about the relation between total pyrethroid exposure and urinary various metabolite are also necessary.

Effect of Hwanggumgung, a Natural Product, on Hair Growth Promotion in C57BL6 Mice (C57BL6 마우스에서 복합생약제인 황금궁의 육모 효과)

  • Hwanggumgung (HGG) is a hair-care product which is composed of several plant extracts used in oriental medicine. This study was carried out to investigate effect of HGG on hair regrowth in a shaving model of C57BL6 mice. Five-week-old mice were acclimated for 1 week under 23$\pm$3$^{\circ}C$, 50$\pm10\%$ relative humidity and 12 h of a light/dark cycle before beginning experiment. There were four experimental groups including distilled water (D.W., control), 10$\%$ ethanol (EtOH, vehicle control), a positive control of 3$\%$ minoxidil (MXD), and HGG for female and male mice, respectively; Six-weeks old mice were trimmed by electric clippers so as not to damage the skin. The next day; mice without visible scraches were selected, randomized and separated in groups of 11 mice. The test compounds were topically treated with 0.15ml per mouse per day for 21 days. The hair regrowth was photographically and histologically determined during the experimental period of 21 days. Enzyme activities of $\gamma$-glutamyl transpeptidase and alkaline phosphatase were also determined using a rate assay method. There were no clinical signs in all experimental groups. The topical application of 3$\%$ MXD and HGG in female mice promoted hair regrowth earlier and faster than the control groups. In male mice, the topical application of 3$\%$ MXD and HGG also accelerated hair growth compared with the controls. Ten percent ethanol also promoted hair growth faster than D.W group. The histology of hair growth in experimental groups was strongly associated with the hair regrowth. 3$\%$ MXD and HGG promoted elongation of hair follicles compared with the controls in both female and male mice. Activities of alkaline phosphatase and $\gamma$-glutamyl transpeptidase, enzymes related to hair growth, significantly increased after treatments of 3$\%$ MXD and HGG for 2 weeks in both female and male mice (p < 0.05). These results suggest that HGG has hair growth promoting activities and it can be for treatment for alopecia.

Antioxidant and Tyrosinase Inhibitory Effects of Paeonia suffruticosa Water Extract (목단피 물 추출물의 항산화 및 Tyrosinase 억제효과)

  • Tyrosinase catalyzes melanin synthesis in skin melanocytes. The effects of Paeonia sufficinalis water extract (MDP) on antioxidant and tyrosinase activities have been studied in a cell-free system and mouse melanoma B16 cell. Radical scavenging activity of MDP was tested by DPPH assay and it showed high DPPH radical scavenging activity. The cellular tyrosinase activity was measured in mouse melanoma B16 cell by RT-PCR and enzyme activity. Treatment with MDP for 24 hr resulted in decreased tyrosinase mRNA level. Tyrosinase activity was decreased, compared with control, in cells exposed to MDP. Thus, Paeonia sufficinalis water extracts may be a candidate for cosmetic use.

Behavioral Problem, Self-Perceived Competence, Social Competence, and Parental Stress in Children with Atopic Dermatitis (아토피 피부염 아동의 문제행동, 자아 역량 인식, 사회적 능력과 양육 스트레스와의 관계)

  • Atopic dermatitis is the fastest growing skin disorder among children from infancy to adolescence in Korea. The side-effects of atopic dermatitis not only include physical discomfort, but also psychological trauma which ultimately affects the developmental growth of children. This study was conducted in order to investigate the behavioral characteristics of an atopic dermatitis sufferer. As part of this research, the relative influence of behavioral problems and, self-perceived competence were analyzed in relation to the social competence of an atopic child. In total, 301 atopic and non-atopic children, between 2 and 6 years of age, and their mothers and teachers participated in the study. From this number, 109 children had atopic dermatitis, while 192 children did not. Mothers were asked to complete a parent-report questionnaire that required information on parental stress, according to the scale parenting methodology of Abbdin(1990) and Cho(1999). Teachers were subjected to teacher-report questionnaires which included topics on social competence, socia-emotional assessment and behavioral problems of an atopic child. Furthermore, children also completed questionnaires on self-perceived competence. According to analysis on K-CBCL, ITSEA, social competence, self-perceived competence and parental stress, children with atopic dermatitis showed higher scores in depression/anxiety and depression/withdrawal, compared to children in the normal control group. In relation to parental stress, daily stress, parental role stress, and stress related disease, mothers with atopic dermatitis children exhibited higher scores. In establishing relationships among the related variables, atopic children who demonstrated more social competence were more likely to suffer less from withdrawal, attention problems, and depression/anxiety. Stress related disease in mothers with atopic children was positively related to attention problems of the child. In terms of relative influences, behavioral problems was the most significant variable, accounting for 23% of variance. Lower behavioral problems was positively related to more social competence. In summation, this study investigated the general characteristics of atopic children. In conclusion, atopic children and their mothers had difficulty in dealing with this disease. It is our belief that an atopic child would not only require physical treatment, but also need appropriate psychological care.

Skin Rejuvenation by Novel Synthetic Compound Containing Retinoidal Activity (레티노이드 효과를 가지는 합성물에 의한 피부재생 효과)

  • Several hydroxamic acid derivatives are synthesized to observe retinoidal effect and transactivation potential for $RAR{\alpha}/{\gamma]$ is screened. Among the synthesized compounds, N-(4-N-hydroxycarbamoyl )phenyl) [4-(tert-butyl )phenyl] carboxamide (2f) showed the best compatibility for potent $RAR{\alpha}/{\gamma].$ The acidic hydroxy of hydroxamic acid was easily deprotonated to form an enolate. A formed enolate has a similar role like that of all-trans-retinoic acid. This is the first example of the hydroxamic acid derivative with retinoidal activity. The retinoidal activity of 2f was further confirmed by enhancing activity for the expression of retinoid-responsive genes. To evaluate the possibility for anti.-aging agent, effect on the expression of MMP-1 was measured comparing with all-trans-retinoic acid and retinol. At 10 uM treatment, compound 2f inhibited the expression of MMP-1. These results suggest that new hydroxamic acid derivative 2f could be used as a promising anti-aging agent.

Chemical Composition and Biological Activities of Essential Oil from 'Setoka' Branches (세토카 가지 정유의 성분 분석 및 생리 활성)

  • This study was designed to analyze the chemical compositions of 'Setoka' branch essential oils (SEBO) and to test their biological activities. 'Setoka' is a Citrus species widely cultivated in Jeju Island. At the present, 'Setoka' branches produced by thinning process were mostly discarded as a waste. Therefore, utilization of this branch waste has received much attention. 'Setoka' branch essential oils (SBEO) were prepared by treatment of its ethanol extracts with jojoba oil. SBEO were chemically analyzed using gas chromatograph-mass spectrometry (GC-MS), and following components were identified; ethyl linoleate (64.14%), ethyl palmitate (16.50%), neophytadiene (11.06%) and beta-citronellol (5.09%). The anti-inflammatory activity in the SBEO was examined using RAW 264.7 murine macrophage cells stimulated with LPS. As a result, the SBEO inhibited nitric oxide (NO) productions with a dose-dependent manner. In addition, SBEO showed good anti-microbial activities against drug-susceptible and -resistant skin pathogens such as Staphylococcus aureus and Propionibacterium acnes, which are acne-causing bacteria. Based on these results, we suggest that SBEO has the possibility for use as an anti-inflammatory and anti-microbial agent in cosmetic applications.
