두경부 환자의 VMAT 시 체형변화와 환자 정렬과의 상관관계 고찰 (Evaluate the Change of Body Shape and the Patient Alignment State During Image-Guided Volumetric Modulated Arc Therapy in Head and Neck Cancer Patients)

  • 목 적: 두경부 환자가 입체적 세기변조 회전 방사선치료 시 변화하는 체형과 환자 정열 상태를 조사하고 그에 따라 치료 시 발생하는 선량분포의 변화와 적절한 재 CT 모의치료의 필요성을 평가하고자 한다. 대상 및 방법: 근치적 목적으로 방사선 치료를 시행한 두경부암 89명에 대해 입체적 세기변조 방사선치료계획을 수립하고 치료 기간 동안의 체중을 측정하였다. 그 중 10명의 환자에 대하여 체형변화를 평가하였다. ARIA software(Offline review)를 이용하여 ramus, chin, posterior neck, clavicle 등의 landmark를 지정한 후 skin까지의 거리를 측정하여 차이를 분석하였다. 결 과: 남성 60명을 대상으로 치료시행 $17{\pm}4$회에서 re-CT 모의치료를 시행한 결과 체중 감소율이 $-6.47{\pm}3.5%$로 나타났으며, 여성 29명은 경우는 $17{\pm}5$회에서 체중 감소율이 $-5.73{\pm}2.7%$로 나타났다. 피부위축(Skin shrinkage) 변화를 관찰한 결과 대표 landmark에서의 표준편차는 C1(${\pm}0.44cm$), C3(${\pm}0.83cm$), C5(${\pm}1.35cm$)로서 0.5 kg 감소 당 약 1 mm shrinkage 발생함을 알 수 있었다. 치료 횟수에 따른 피부위축은 1~4 fx (변화 없음), 5~13 fx (-2 mm), 14~22 fx(-4 mm), 23~30 fx(-6 mm)로 나타났다. 결 론: 체형이 약 5 mm 변화하게 되면 중심부 선량은 약 3 % 이상 차이나기 시작하게 되므로 적응증치료에 필요한 CT 모의치료를 추가로 진행하여야만 한다. 또한 적극적으로 lower neck의 CT 모의치료 방법 및 set up 방법에 관하여 연구하고 새로운 고정용구(Immobilization device)의 사용도 검토할 필요가 있다.

황련염색 피혁물의 가지처리에 관한 연구: 우피의 염색성과 가지처리에 의한 물성 변화에 관하여 (A Study on Fatliquoring Cow Skin Dyed with Natural Coptis Chinensis Franch: Dyeability and Changes in the Physical Properties by Fatliquoring of Cow Skin)

  • This was a study examining the fatliquoring of natural dyed cow skin with Coptis chinensis Franch. The K/S according to optimal dyeing condition, mordants and mordanting method was measured. And the surface, sections, tensile strength, elongation, stiffness and colorfastness according to post-dyeing fatliquoring were observed. The optimal conditions include 300%(o.w.f.), $50^{\circ}C$, 30 minites. Fe-simmordanting showed the highest dyeability. Coptis chinensis Franch showed a yellow color regardless of mordants and mordanting methods. Tensile strength and elongation increased in the case of simmordanting compared with untreated and the process of fatliquoring further increased. The softness significantly increased when fatliquored. Shrinkage was slightly reduced when dyed, but showed almost no change with fatliquoring. Lastly, color fastness increased overall with mordanting and partly increased with fatliquoring. The rubbing and light fastness were excellent Cu- mordanting, and drycleaning fastness also improved with Cr-mordanting.

  • Interlace texturing is very useful method to make compound yarns for new synthetic fabrics. In this study, we make the compound ems for peach skin fabric by interlace texturing method. This study surveys relationship between physical properties of interlace textured yarns and process conditions such as air pressure, yarn tension and take-up speed. Nip density, tensile properties and multi-step shrinkage of the various specimens were discussed with process conditions.

Full thickness skin grafts from the groin: donor site morbidity and graft survival rate from 50 cases

  • Objectives: Full thickness skin grafts (FTSG) offer several advantages; they are esthetically superb, have less postoperative shrinkage, and offer minimal postoperative pain and scar formation at the donor site. As a donor site of FTSG, the groin offers a relatively large area of skin with high elasticity. The aim of this study was to evaluate FTSG from the groin for reconstruction in oral and maxillofacial surgery. Materials and Methods: In a retrospective study, 50 patients (27 males, 23 females) who received FTSG from the groin were evaluated for their operation records, clinical photography, and medical records. Results: The width of skin from the groin was distributed from 2-8 cm (mean: 5.1 cm) at the donor site, while the long axis length was distributed from 3-13 cm (mean: 7.4 cm). A high number of patients, 47 patients (94%) out of 50, showed good healing at the donor site. Wound impairment was seen in 3 patients (6%), minor wound dehiscence in 2 patients, and severe wound dehiscence in 1 patient. In the recipient site, delayed healing was observed in 2 patients (4%). Conclusion: FTSG from the groin to repair soft tissue defects in reconstruction surgery is a good method due to the relatively big size of the graft, decreasing morbidity at the donor site, and higher graft survival rates.

가호조건에 따른 호부사의 물성변화 (The Physical Properties of Sizing Yarn According to Sizing Condition)

  • Polyester fabrics, which is a major product in Daegu and Kyungbuk district, recently have a frequent warp streak phenomenon of sizing textiles for peach skin. So, this paper intensively focuses on the analysis of finding causes of poor products which may be produced in the process of sizing. Depending on the changes of sizing yarn's physical characteristic varying on sizing conditions, results of the research are as followed. 1. Sizing add-on rate, following tension changes of originally supplied yarn in sizing, does not fluctuate until 30g sizing tension, but marks high on the increase of sizing speed. 2. Sizing add-on amount increases approximately by 2.5% when interlacing pressure is raised from 1.5kg/$cm^2$ to 3.5kg/$cm^2$ at $130^{\circ}C$ sizing temperature. 3. Following the increase of sizing tension from 10g to 50g, boiling shrinkage rate of desizing yarn changes approximately from 7% to 11%. But the more sizing temperature and speed increases, the bigger the change rate becomes. 4. The thermal stress of desizing yarn is sensitively influenced by sizing tension in the sizing degree of above $130^{\circ}C$. 5. The deviation rate of sizing yarn's initial modulus value, which is influenced by air pressure in interlacing raw yarn, marks the highest at 40g and 50g strength.

지반 변형 대응형 말뚝 기초의 거동 특성 분석 (Analysis of the Behavior Characteristics of Pile Foundations Responding to Ground Deformation)

  • 세계적으로 대규모 인프라 구조 건설 시장이 확대됨에 따라 극한지 및 극서지와 같은 극한 환경에서의 토목 구조물 시공이 계획 혹은 시공 중에 있다. 이에 따라 구조물의 지지력 확보를 위한 말뚝 기초의 시공이 필수적이나 극한지 및 극서지의 지반 변형 가능성으로 인해 말뚝 기초의 안정성 및 기능 상실이 우려된다. 따라서 본 연구에서는 새로운 형식의 말뚝 기초를 개발함으로써 지반 변형에 대응하고자 하며, 극한지 및 극서지에서 발생 가능한 지반 변형을 크게 융기 및 침하로 구분하였다. 지반 변형 대응형 말뚝은 강관 말뚝 내부에 수축 및 팽창이 가능한 실린더가 삽입된 형태로 융기 및 침하 과정에서 실린더의 거동에 따른 말뚝 영향을 수치해석적으로 분석하였다. 수치해석 결과 지반 융기는 말뚝의 과도한 인장응력을 발생시켰으며, 실린더의 팽창 조건은 말뚝에 작용하는 인장 응력을 분담해 주어 전체적으로 말뚝에 작용하는 축 응력을 감소시켰다. 지반 침하는 부주면 마찰력 발생에 따른 말뚝의 압축응력을 증가시켜 주었는데, 실린더는 중립점 이하에 위치하여 수축 거동 시 최적의 효율을 보여주었다. 하지만 지반 변형 대응형 말뚝 시공 시 수축 및 팽창량은 상부 구조체의 허용 변위 범위를 준수하여야 하며, 설계 시 이에 따른 고려가 필요할 것으로 판단된다.

  • Vestibuloplasty are following categories : Mucosal advancement(submucous), secondary epithlization(reepithelization) and grafting vestibuloplasty. Although certain procedures are indicated for alveolar bone loss and sulcus shortening, relapse can occur. Every efforts to minimize or compensate for it is controversy. O'Steen(1970) reported the mucous graft methods that none of vestibular shrinkage and graft contracture. 15patients in mucous graft vestibuloplasty with fibrin adhesive system(Beriplast) were taken in cases of alveolar bone resorption and mucosal shortening due to traumatized alveolar bone defects, senile atrophic alveolar bone, postoperative cyst or tumor resection, edentulous alveolar bone loss, and others. A technique in the use of small piece of palatal mucosa$(1{\times}20mm)$ from the lateral aspect of the palate with adhesive system provided to secure the skin grafts, avoid stent fixation, postoperative patient's comfort and less time-consuming than the standard technique, especially excellent bleeding control.

  • Authors observed 12 cases among the 46 corps which are died from 'charring', caused by traffic accident, for the purpose of individual identification. Concerning to the method of identification, authors took the dental hard tissue which is high in resitance to heat, using the method of Forensic Odontology and got a good results from it. In this inspection, we observed 'pugilistic posture' which results from heat coagulation & shrinkage of muscular bundle with predominance of the stronger flexor muscles. On the other hand, there appeared the fact that hair is more resistant to heat than skin or muscle.

Changes in Breast-tumor Blood Flow in Response to Hypercapnia during Chemotherapy with Laser Speckle Flowmetry

  • Development of a biomarker for predicting tumor-treatment efficacy is a matter of great concern, to reduce time, medical expense, and effort in oncology therapy. In a preclinical study, we hypothesized that the blood-flow parameter based on laser speckle flowmetry (LSF) could be a potential indicator to estimate the efficacy of breast-cancer treatment. To verify this hypothesis, a 13762-MAT-B-III rat breast tumor was grown in a dorsal skinfold window chamber applied to a nude mouse, and the change in blood flow rate (BFR) - or the speckle flow index (SFI) is used together as the same meaning in this manuscript - was longitudinally monitored during tumor growth and metronomic cyclophosphamide treatment. Based on the daily LSF angiogram, several BFR parameters (baseline SFI, normalized SFI, and △rBFR) were compared to tumor size in the normal, treated, and untreated tumor groups. Despite the incomplete tumor treatment, we found that the daily changes in all BFR parameters tended to have partially positive correlation with tumor size. Moreover, we observed that the changes in baseline SFI and normalized SFI responded one day earlier than the tumor shrinkage during chemotherapy. However, daily variations in the hypercapnia-induced △rBFR lagged tumor shrinkage by one day. This study would contribute not only to evaluating tumor vascular response to treatment, but also to monitoring blood-flow-mediated diseases (in brain, skin, and retina) by using LSF in preclinical settings.
