A Multicenter Noncomparative Clinical Study on Midface Rejuvenation Using a Nonabsorbable Polypropylene Mesh: Evaluation of Efficacy and Safety

  • Background Facial rejuvenation can be achieved using a variety of techniques. Since minimally invasive procedures for face lifting have become popular because of their convenience and short operating time, numerous minimally invasive surgical procedures have been developed. In this study, a nonabsorbable polypropylene mesh is introduced as a new face lifting instrument, with the nasolabial fold as the main target area. In this paper, we report the efficacy and safety of a polypropylene mesh in midface rejuvenation. Methods Thirty-three subjects with moderate-to-severe nasolabial folds were enrolled from two medical institutions for a noncomparative single-sample study. A mesh was inserted above the superficial muscular aponeurotic system layer, reaching the nasolabial folds through a temporal scalp incision. After 3 weeks, the temporal end of the mesh was pulled to provide a lifting effect. Then, the mesh was fixed to the deep temporal fascia using nonabsorbable sutures. To evaluate efficacy, we compared the scores on the Wrinkle Severity Rating Scale and a visual analog scale for patient satisfaction between the baseline and 7 weeks postoperatively. In addition, we evaluated safety based on the incidence of adverse events. Results The treatment was deemed effective at improving wrinkles in 23 of 28 cases, and patient satisfaction improved significantly during the study period. There were seven cases of skin or subcutaneous tissue complications, including edema and erythema, but there were no suspected serious adverse events. Conclusions Face lifting using a nonabsorbable mesh can improve nasolabial folds without serious adverse effects. Thus, this technique is safe and effective for midface rejuvenation.

  • To investigate the effects of facial blood flow rates(FBFR) and facial skin temperature(FST) generated by Gagam-Jawoonaek(GJ) application(appl.) after Miso Facial Rejuvenation Acupuncture(MFRA). Ten people in their twenties to fifties with no skin diseases were recruited. We randomly divided subjects two groups(A, B) and set the GJ appl. site(group A - right side, group B - left side). MFRA was performed on both sides of their face. Immediately after acupuncture treatment(AT treat.), GJ was applied only half of the face. We measured their FBFR using Laser Doppler Perfusion Imaging(LDPI) and FST using Digital Infrared Thermal Imaging(DITI) at pre-AT treat., immediately after AT treat., twenty and sixty minutes after GJ appl.. We analyzed data using Mann-Whitney test and Wilcoxon test(p < 0.05). After MFRA treat., FBFR on both sides increased. Twenty minutes after JW appl., the changes of FBFR on GJ appl. side($122.9{\pm}43.1PU$) were bigger than GJ non-appl. side($80.9{\pm}38.4PU$), a statistically significant decrease. Sixty minutes after application, FBFR on both sides were recovered almost at the same level as that of pre-AT treat. After MFRA treat., FST on both sides increased. Twenty minutes after GJ appl., the changes of FST on GJ appl. side($1.1{\pm}0.6^{\circ}C$) were comparable to that of GJ non-appl. side($1.2{\pm}0.5^{\circ}C$). Sixty minutes after application, FST on both sides were recovered almost at the same level as that of pre-AT treat.. Gagam-Jawoonaek could decrease facial blood flow rates.

랫드에서의 생분해성 매선요법에 대한 조직학적 분석 (Histological Evaluation of Bioresorbable Threads in Rats)

  • 매선요법은 다른 시술에 비해 비침습적이고, 인체 내에서 생분해가 일어나는 물질로 제작되기 때문에 성형수술에서 각광받고 있다. 상업적으로 이용가능한 매선의 종류로는 poly-L-lactic acid (PLLA), polycarprolactone (PCL), polydioxanone (PDO) 등이 있으나, 매선으로 사용되는 생분해성 물질들의 종류에 따른 차이점이나, 콜라겐을 형성하는데 있어서 매선의 어떤 부분이 결정적으로 작용하는지에 대한 연구는 잘 알려져 있지 않다. 따라서 이 연구에서는 현재 판매되고 있는 3가지 생분해성 물질(PLLA, PCL, PDO)로 만들어진 매선을 동물모델에서 그 효과를 비교하고, 매선의 콜라겐생성능에 미치는 영향 인자가 무엇인지를 분석하였다. 매선을 랫드의 등에 삽입하여 2주, 4주, 8주, 12주동안 경과를 관찰하고 특수염색을 통해 콜라겐의 생성 정도를 비교하였다. 매선에 의한 조직학적 변화를 관찰하기 위해서, 조직검체는 H&E, Masson's trichrome, Herovici's collagen stain이 진행되었다. 그 결과 3가지 모두 다 동일한 시기에 비슷한 양의 신생콜라겐을 형성하는 것을 관찰할 수 있었고, 3가지 매선 모두 type 3의 콜라겐을 형성하는 것을 확인할 수 있었다. 또한 생성되는 콜라겐의 양은 매선이 피부조직에 닿는 면적이 크면 클수록 증가하는 것을 관찰할 수 있어 한 번의 시술만으로도 많은 양의 매선이 들어갈 수 있는 PDO cavern타입의 매선이 가장 많은 콜라겐을 형성하는 것을 확인할 수 있었다.

A Pilot Study of Skin Resurfacing Using the 2,790-nm Erbium:YSGG Laser System

  • Background The erbium:yttrium scandium gallium garnet (Er:YSGG) laser differs from other laser techniques by having a faster and higher cure rate. Since the Er:YSGG laser causes an appropriate proportion of ablation and coagulation, it has advantages over the conventional carbon dioxide ($CO_2$) laser and the erbium-doped yttrium aluminum garnet (Er:YAG) laser, including heating tendencies and explosive vaporization. This research was conducted to explore the effects and safety of the Er:YSGG laser. Methods Twenty patients participated in the pilot study of a resurfacing system using a 2,790-nm Er:YSGG laser. All patients received facial treatment by the 2,790-nm Er:YSGG laser system (Cutera) twice with a 4-week interval. Wrinkle reduction, reduction in pigment inhomogeneity, and improvement in tone and texture were measured. Results Study subjects included 15 women and five men. Re-epithelization occurred in all subjects 3 to 4 days after treatment, and wrinkle reduction, reduction in pigment inhomogeneity, and improvement in tone and texture within 6 months of treatment. Conclusions The 2,790-nm YSGG laser technique had fewer complications and was effective in the improvement of scars, pores, wrinkles, and skin tone and color with one or two treatments. We expect this method to be effective for people with acne scars, pore scars, deep wrinkles, and uneven skin texture and color.

Clinical Applications of a Non-ablative Fractional Dual Laser (1550/1927 nm)

  • The non-ablative fractional dual laser is equipped with two types of lasers, 1550 nm and 1927 nm in one device, and was approved by the United States Food and Drug Administration in 2013. The advantages of the non-ablative fractional laser (NAFL) include fewer side effects such as erythema, edema, post-laser pigmentation, and scab formation. Thus, the NAFL is preferred by both practitioners and consumers because it is convenient and safe for use. The 1550 nm erbium glass and 1927 nm thulium lasers are representative NAFLs that have been developed separately and are often used as a single-wavelength laser with proven clinical efficacy in various indications. The 1550 nm wavelength laser penetrates the dermis layer and the 1927 nm wavelength laser is effective for epidermal lesions. Therefore, targeting the skin layer can be easily achieved with both the 1550 and 1927 nm lasers, respectively, or in combination. Clinically, the 1550 nm laser is effective in the treatment of mild to moderate sagging and wrinkles, scars, and resurfacing. The 1927 nm laser improves skin texture and treats skin pigmentation and wounds. It can also be used for drug delivery. The selection and utilization rate of NAFL has been increasing in recent times, due to changes in lifestyle patterns and the need for beauty treatments with fewer side effects and short downtime. In this study, we present a plan for safe and effective laser therapy through a review of literature. Clinical applications of the multifunctional NAFL are also described.

Application of 630-nm and 850-nm Light-emitting Diodes and Microcurrent to Accelerate Collagen and Elastin Deposition in Porcine Skin

  • Background and Objectives Skin aging is reportedly associated with regulation in collagen and elastin synthesis. This study investigated the potential of combining light-emitting diode (LED) treatments using a 630-nm and 850-nm LED with simultaneous microcurrent application. Materials and Methods The dorsal skin of female pigs was treated with a home-use device. We examined the treatment effects using photography, thermocamera, microscopic pathology, and histological examination to determine the mechanism of action, efficacy, and safety of the procedure. A histological observation was performed using hematoxylin and eosin, Masson's trichrome, Victoria blue, and immunohistochemical staining. We also used the Sircol soluble collagen and elastin assay kit to measure the amounts of collagen and elastin in the porcine back skin tissue after 2 and 6 weeks. Results Evaluation by visual inspection and devices showed no skin damage or heat-induced injury at the treatment site. Histological staining revealed that accurate treatment of the targeted dermis layer effectively enhanced collagen and elastin deposition. Collagen type I, a protein defined by immunohistochemical staining, was overexpressed in the early stages of weeks 2 and 6. Combined therapy findings showed the superior capability of the 630-nm and 850-nm LED procedures to induce collagen; in contrast, elastin induction was more pronounced after microcurrent treatments. Conclusion The home-use LED device, comprising a combination of 630-nm and 850-nm LEDs and microcurrent, is safe and can be used as an adjunctive treatment for self-administered facial rejuvenation.

Case of Prominent Periorbital Vein after Lower Eyelid Blepharoplasty Treated with a Long-pulse 1,064-nm Neodymium: Yttrium-aluminum-garnet Laser

  • Lower eyelid blepharoplasty is a popular facial rejuvenation surgery that restores the infraorbital region. The procedure is performed by removing excessive fatty tissues, muscles, and other unnecessary structures. The structure of the eyelid differs from person to person and is quite complex; therefore, numerous side effects are expected. Common lower eyelid blepharoplasty complications are asymmetrical faces, retrobulbar hemorrhage, blurred vision, and ectropion. This paper reports a case of a prominent periorbital vein after lower eyelid blepharoplasty. The prominent periorbital vein after surgery is a rare side effect that has not been reported to the best of the author's knowledge. This case was treated with a long-pulse 1,064-nm neodymium:yttrium-aluminum-garnet laser.

Unilateral Platysma Muscle Rupture as an Effect of Using a Hard Wooden Block for Facial Massage

  • Facial massages are frequently performed to achieve a feeling of freshness, rejuvenation, skin tightening, and delayed onset of wrinkles. However, vigorous massages can induce unexpected symptoms. Here, we present a case of a woman who complained of an asymmetric facial appearance and a mass-like lesion following a long-term facial massage intervention. A facelift incision was performed. Platysma muscle rupture was observed intraoperatively, which was then repaired. To our knowledge, this is the first report of a vigorous facial massage-induced ipsilateral platysma rupture.

Lateral Brow Lift: A Multi-Point Suture Fixation Technique

  • Background Descent of the lateral aspect of the brow is one of the earliest signs of aging. The purpose of this study was to describe an open surgical technique for lateral brow lifts, with the goal of achieving reliable, predictable, and long-lasting results. Methods An incision was made behind and parallel to the temporal hairline, and then extended deeper through the temporoparietal fascia to the level of the deep temporal fascia. Dissection was continued anteriorly on the surface of the deep temporal fascia and subperiosteally beyond the temporal crest, to the level of the superolateral orbital rim. Fixation of the lateral brow and tightening of the orbicularis oculi muscle was achieved with the placement of sutures that secured the tissue directly to the galea aponeurotica on the lateral aspect of the incision. An additional fixation was made between the temporoparietal fascia and the deep temporal fascia, as well as between the temporoparietal fascia and the galea aponeurotica. The excess skin in the temporal area was excised and the incision was closed. Results A total of 519 patients were included in the study. Satisfactory lateral brow elevation was obtained in most of the patients (94.41%). The following complications were observed: total relapse (n=8), partial relapse (n=21), neurapraxia of the frontal branch of the facial nerve (n=5), and limited alopecia in the temporal incision (n=9). Conclusions We consider this approach to be a safe and effective procedure, with long-lasting results.

Ginsenoside Rb2 suppresses cellular senescence of human dermal fibroblasts by inducing autophagy

  • Background: Ginsenoside Rb2, a major active component of Panax ginseng, has various physiological activities, including anticancer and anti-inflammatory effects. However, the mechanisms underlying the rejuvenation effect of Rb2 in human skin cells have not been elucidated. Methods: We performed a senescence-associated β-galactosidase staining assay to confirm cellular senescence in human dermal fibroblasts (HDFs). The regulatory effects of Rb2 on autophagy were evaluated by analyzing the expression of autophagy marker proteins, such as microtubule-associated protein 1A/1B-light chain (LC) 3 and p62, using immunoblotting. Autophagosome and autolysosome formation was monitored using transmission electron microscopy. Autophagic flux was analyzed using tandem-labeled GFP-RFP-LC3, and lysosomal function was assessed with Lysotracker. We performed RNA sequencing to identify potential target genes related to HDF rejuvenation mediated by Rb2. To verify the functions of the target genes, we silenced them using shRNAs. Results: Rb2 decreased β-galactosidase activity and altered the expression of cell cycle regulatory proteins in senescent HDFs. Rb2 markedly induced the conversion of LC3-I to LC3-II and LC3 puncta. Moreover, Rb2 increased lysosomal function and red puncta in tandem-labeled GFP-RFP-LC3, which indicate that Rb2 promoted autophagic flux. RNA sequencing data showed that the expression of DNA damage-regulated autophagy modulator 2 (DRAM2) was induced by Rb2. In autophagy signaling, Rb2 activated the AMPK-ULK1 pathway and inactivated mTOR. DRAM2 knockdown inhibited autophagy and Rb2-restored cellular senescence. Conclusion: Rb2 reverses cellular senescence by activating autophagy via the AMPK-mTOR pathway and induction of DRAM2, suggesting that Rb2 might have potential value as an antiaging agent.
