간호사의 피부소독에 대한 지식, 태도 및 수행과의 관계 (Nurse's Knowledge, Attitude and Practice of Skin Disinfection)

  • Purpose: This study was to provide baseline data about knowledge, attitude and practice of skin disinfection and to identify the influencing factors among nurses related to skin disinfection. Methods: The subjects were 174 nurses who worked at one Hospital in A city. Data were collected by self-reported questionnaires during August, 2010. The collected data were analyzed with use of SPSS/WIN 18.0. Results: The knowledge of skin disinfection varied significantly according to unit, position and job satisfaction. Nurses' attitude towards skin disinfection differed significantly according to age, unit, career, position and job satisfaction. Nurses' practice of skin disinfection varied according to unit and position. Significant correlations were found between knowledge, attitude and practice. The attitude and knowledge were influencing factors of practice (55.6%). Conclusion: An educational program focusing on changing nurses' knowledge and attitude can be effective for the practice of skin disinfection.

신생아에서 알코올 솜 및 포비돈 솜의 피부 소독효과 비교 (Effects of Cotton Balls of Alcohol and Povidone for Disinfectoin of Skin in Newborns)

  • 목 적 : 신생아 중환자에 대한 치료법이 급속도로 발전함에도 불구하고 신생아 감염은 여전히 큰 문제이다. 특히 정맥천자나 여러 침습적 처치가 많은 신생아 중환자에서 이를 통해 피부 상주균에 의한 패혈증이 증가하고 있다. 이는 시술상의 문제로 야기될 수 있지만 처치 전 소독제의 소독력에 문제가 있을 수 있다. 특히 병원에서 희석하여 사용하고 있는 알코올 솜은 공기중에 자주 노출되어 알코올 성분이 증발되기 쉬워 소독력에 문제가 있을 수 있다. 이에 포비돈 소독제가 알코올 소독제보다 소독력이 좋은 것이라는 가정하에 이 연구를 시행 하였다. 대상 및 방법 : 신생아 중환자실에 입원한 미숙아 및 신생아중 정맥 수액요법이 필요하였던 신생아들을 대상으로 정맥 천자가 주로 시행되는 부위의 피부에서 멸균된 면봉으로 멸균 생리 식염수를 묻혀 피부를 문질러 배양 검사를 시행하였고 피부가 마른 후 무작위로 알코올군은 병원에서 희석하여 만든 70% 알코올 솜으로 닦고 마른 후 같은 방법으로 피부를 닦아 배양검사를 하였고(20례), 포비돈군은 10% 포비돈(1% iodine) 솜으로 닦고 마른 후 70% 알코올 솜으로 다시 닦아 마른 후 피부 배양검사를 하였다(20례). 대조군은 생리 식염수 솜으로 닦은 후 배양검사를 하였다(10례). 결 과 : 소독 전 알코올 군과 포비돈군 각각 20례중 9례에서 균이 자랐고, 대조군은 10례중 8례에서 균이 검출되었다. 소독후 알코올 군은 3례, 포비돈군 2례, 대조군은 8례에서 균이 계속 자라 세군간에 유의한 차이가 있었다(p=0.0387). 포비돈군의 소독효과는 알코올군과 비슷하였고, 대조군보다는 높았지만(p=0.0156), 알코올군은 대조군과 비교하여 유의한 차이가 없었다. 결 론 : 포비돈 솜의 소독효과는 알코올 솜과 비슷하였지만, 생리식염수 솜보다는 높았다. 그러나 알코올 솜의 소독효과는 생리식염수 솜과 유의한 차이가 없었다. 병원에서 사용되는 알코올의 순도, 알코올 솜의 정도관리, 소독방법 등에 대한 재평가가 필요함을 시사한다.

Ultraviolet-C 조사의 살균 효과 (The Effect of Ultraviolet-C Radiation on Disinfection)

  • Traditionally, ultraviolet (UV) has been used for treating the pressure sore and skin wound. The effects of UVA and UVB radiation on disinfection have been reported. The purpose of this study was to examine the effectiveness of UVC radiation on disinfection of Escherichia coli, Staphylococcus aureus, Salmonella typhimurium in vitro. Three bacterium were radiated by UVC (250 nm, 20 seconds) and incubated at $37^{\circ}C$ for 24 hours at the agar culture medium. Kill rates of all three bacterium were 99.9%. UVC radiated on three kinds of bacterium for 30 or 60 seconds. Kill rates were 99.9% both 30 and 60 seconds. This data suggests that UV light at 250 nm could be a useful method to minimize infection and shorten healing time in pressure sore and skin wound condition.

대전 일부 지역 치과종사자의 화학소독제에 관한 지식도 연구 (Knowledge on the chemical disinfectants among dental workers in some Daejeon region)

  • The usage of appropriate disinfectants is essential for infection control in dental hospitals, dental clinics. Inadequate use of disinfectants is the cause of human or environmental toxicity and is a waste of cost. This study was aimed to assess the level of knowledge on the disinfection and chemical disinfectants among dental workers in dental hospitals, dental clinics. It's ultimately intended to serve as a basis for the preparation of more effective, appropriate educational programs on the disinfection and manuals on the use of disinfectants for dental workers. This study enforced a questionnaire with 184 dental workers employed in dental hospitals, dental clinics from 15 Aug. to 15 Sep. 2007, in the area of Daejeon. The obtained result were as follows; 1. The population sociological feature of dental hospital and dental clinic showed that significant differences of dental service career(pM0.039). Dental service career of dental workers shows; below 3 years 42.9%, 4~6 years 34.7%, more than 7 years 22.4% in dental hospitals, below 3 years 30.4%, 4~6 years 26.7%, more than 7 years 43.0% in dental clinics(pM0.039). 2. The average score of dental workers knowledge in 'Critical item soaks in high-level disinfectants for 20minutes was 2.73V0.49 point, got from knowledge of dental instrument is appropriate to immerse before sterilization in the dental device disinfection(pM0.002). 3. In the general disinfection which it follows in education experience of chemical disinfectants direction for use, 'Direction for use by Spaulding process classification' responded that the correct answer was the education experience dental workers 60.0%, the education non-experience dental workers 39.5%(p=0.026). 4. In the dental device disinfection which it follows in education experience of chemical disinfectants direction for use. 'High level disinfection is not applied for the non-critical items and equipment' responded that the education experience dental workers 49.2%, the education non-experience dental workers 31.9%(pM0.045), 'Semi-critical items is applied same method in presence of the infection disease which it acts responded that the education experience dental workers 44.6% answer back, the education non-experience dental workers 24.4%(pM0.017). 5. 'A hand disinfectants of anticeptics have effect' the education experience dental workers 78.5% answer back, the education non-experience dental workers 52.9%(pM0.003). 1t uses with hand disinfectants when the instruments which be imbrued and patient contact', the education experience dental workers 78.5% answer back, the education non-experience dental workers 62.2%(pM0.026), 'Boric acid solution uses for the skin disinfectants the education experience dental workers 52.3% answer back, the education non-experience dental workers is 37.0%(pM0.016), 'Gluconate have effective difference which it follows in chemical disinfectant consistency and the solution type' education experience dental workers 72.3% answer back, education non-experience dental workers 47.9%(pM0.004). 6. The education experience dental workers were appeared higher than the education non-experience dental workers in knowledge of the disinfection and chemical disinfectants. Consequently system and the specialty education which is standardized continuously must be provided to all dental workers.

양식장 넙치 폐사어를 이용한 단백질 소재의 개발에 관한 연구(2) -산업화공정 연구- (A Study on Development of Protein Materials using Dead Flatfish from Fish Farms(2) -Industrial Process-)

  • In manufacturing of flatfish skin collagen peptide (FSCP) and flatfish protein hydrolysate (FPH) by reuse of dead flatfish from fish farm in Jeju island, the industrial process was optimized with the laboratory scale research and the on-field process. Segmented unit processes from raw material incoming to shipment were established to produce commercial product of FSCP and FPH. Total plate counts of FSCP were twenty five times of FPH, but food poisoning bacteria were not detected in two samples. FSCP and FPH were safe from heavy metal such as Pb(II), Cd(II) and Hg(II). The residual contents of antibiotics and disinfection matter in FSCP and FPH were not detected. The optimized process for mass production made the one-third of the running time and two times of the yield. From economic analysis, the production cost was estimated to 22,000 and 12,000 won/kg for FSCP and FPH, respectively. Therefore the product from the reuse of dead flatfish was expected to have a considerable competitive price and high added-value functional food material compared with other commercially available fish products.

실내 클라이밍 짐 홀드의 관리방법에 따른 미생물 오염에 관한 연구 (A Study on Microbial Contamination according to Effective Management Strategies of Indoor Climbing Gym Holds)

  • Background: Despite the rise in the number of domestic indoor climbing gyms, there is a lack of specific hygiene standards and research on the holds installed in them. Holds can act as vectors for microbial transmission through the hands, posing a risk of infectious diseases, especially with damaged skin. Objectives: The aim of this study is to investigate the contamination level and species of microorganisms on holds according to the management methods practiced in indoor climbing gyms and identify effective strategies for reducing microbial contamination. Methods: We investigated factors that may influence microbial contamination of holds, including hold management methods, user information, and hygiene management at three climbing gyms in Seoul. A total of 72 holds were sampled, 18 for each management method of brushing, high-pressure washing, and ethanol disinfection. Samples were cultured on LB and blood agar at 37℃ for 48 hours to calculate CFUs. PCR assay targeting 16S rRNA was carried out to identify microorganisms. Dunn-Bonferroni was employed to see the microbial reduction effect of the management method and the difference in microbial contamination by management method and climbing gym. Results: As a result of microbial identification, microorganisms such as Bacillus, Staphylococcus, and Micrococcus, which were derived from various environments such as skin and soil, were discovered on the surface of the climbing hold. Among the discovered microorganisms, some species had potential pathogenic properties that could cause food poisoning, gastrointestinal disease, bacteremia, and sepsis. All hold management methods were effective in reducing microorganisms (p<0.05), with ethanol disinfection being the most effective (p<0.001). Conclusions: Our results indicate that there are potential pathogens on holds that demand thorough management for microbial prevention. Proposed methods include regular brushing and ethanol disinfection in addition to high-pressure washing with long cycles, which are the existing forms of hold management. Further studies on shoe management are advised to curb soil-derived microorganisms.

투관침 부위에 적용하는 포비돈 아이오딘과 클로르헥시딘-에탄올의 소독제 효과 연구 (Clinical Study Comparing the Skin Antiseptics of Povidone-iodine and Chlorhexidine-ethanol on Port Site)

  • 본 연구의 목적은 투관침 부위를 중심으로 복강경하 담낭절제술에 적용하는 소독제인 포비돈 아이오딘과 클로르헥시딘-에탄올에 따른 소독효과의 차이를 비교하기 위한 비동등성 대조군 사전사후설계 실험연구이다. 연구대상자는 서울 소재 1개의 대학병원 외과에 입원한 46명의 환자를 대상으로 난수표를 이용하여 무작위 할당하였으며, 균의 배양과 동정, 균 수(CFU/ml) 측정, 수술부위감염 발생 유무를 통해 소독제 효과의 차이를 비교하였다. 연구결과, 포비돈 아이오딘과 클로르헥시딘-에탄올은 소독 3분 후와 투관침 제거 전의 균 발생률, 균수 변화의 차이와 수술부위감염발생률은 통계적으로 유의한 차이가 없어 소독효과에 대한 차이는 없는 것으로 나타났다. 따라서, 적절한 소독제 선택시 소독효과 뿐만 아니라 비용적 측면과 사용의 편의성 등을 고려하는 것이 필요하다. 본 연구는 소독제의 효능과 사용방법에 대한 기초자료 및 교육 자료로 활용될 수 있으며, 소독제에 대한 이해를 높이는데 의의가 있다.

Geranial, Cineol, Menthol과 편백의 유효성분을 조성물로 하는 손소독제 연구 (Study of a Hand Sanitizer Composed of Active Ingredients Geranial, Cineol, Menthol, and Chamaecyparis obtusa)

  • 수장고 및 서고에서 곰팡이는 문화재와 기록물의 생물학적 열화를 일으키는 주요 원인이다. 또한, 독소를 생성하고 피부알레르기를 유발하여 유물과 기록물 및 도서를 다루는 근무자의 호흡기질환과 피부질환 등을 일으키기도 한다. 특히 근무자의 피부 및 손에 접촉되어 2차 감염을 일으킬 수 있다. 이를 예방하기 위하여 시중에서 쉽게 구입할 수 있는 손소독제는 일반세균에 대하여 소독효과를 보이지만, 곰팡이에는 소독효과가 떨어져 이에 대한 대체 연구가 필요한 실정이다. 본 연구에서는 손소독제 조성물로 친환경적인 Geranial, Cineol, Menthol과 편백(Chamaecyparis obtusa)의 유효성분으로 하는 손소독제를 연구 개발하여 박물관 및 기록관의 서고에서 조사된 곰팡이 10종(Alternaria citrimacularis, Alternaria consortialis, Aspergillus niger, Aspergillus versicolor, Cladosporium sphaerospermum, Coniothyrium aleuritis, Penicillium brevicompactum, Penicillium corylophilum, Penicillium paneum, Penicillium polonicum)에 대하여 소독효과를 시험한 결과, 우수한 소독효과를 나타내는 것을 확인하였다. 이에 Geranial, Cineol, Menthol과 편백의 유효성분을 조성물로 하는 손소독제의 개발로 문화재와 기록물 및 도서를 다루는 근무자의 피부질환과 2차 감염을 예방할 수 있을 것으로 기대한다.

모발 및 피부 개선의 오존화 올리브 오일 생산에 대한 안전성에 관한 연구 (The Study On Safety of Olive Oil Production Ozonized of Hair and Skin Improvement)

  • 본 연구는 부작용 없고 문제성 두피 및 피부개선 등에 효과가 좋지만 온도와 습도에 민감한 오존크림을 일정한 과산화물가와 방전관 내구성 안정적인 생산을 위한시스템을 개발하고자 한다. 더 나아가 올리브 오일과 오존과 반응시켜 제조하는 기술에 적용하고자 한다. 오존화 올리브 오일의 과산화물가가 1200 meg/kg 넘을 경우 미생물 살균에는 좋으나 두피 및 피부에 과민반응을 보이기 때문에 위험을 최소화하여 생산되도록 하는 것이 본 연구 및 실험의 목적이다. 그리고 안정된 생산을 위하여 제조 장치중에 방전관의 내구성도 실험하였다. 그 실험 결과 오존화 올리브오일의 과산화물가가 1300 meg/kg 로 적정하게 측정되었고, 방전관도 안정된 결과를 보였다.

급식시설의 손 세척을 위한 70% 알콜 소독제 사용 및 세척방법의 적용효과 분석 (Effectiveness of 70% Alcohol Solution and Hand Washing Methods on Removing Transient Skin Bacteria in Foodservice Operation)

  • Hand washing is an important component of hygiene program for food handlers. Hands can be a source of direct or indirect contamination of foods with pathogenic microorganisms. In this study, the effectiveness of hand washing methods and the use of 70% alcohol solution against transient skin bacteria was tested in an university foodservice facility. 70% alcohol solution is sprayed for 5 seconds automatically when hands are placed in the dispenser. Samples were taken using swab technique in meat cutting area, vegetable trimming area, and vegetable cutting area: before and after washing hands according to planned methods, and after being sprayed with 70% alcohol solution after washing hands. The bacteriological analysis of total plate counts, coliform, fecal coliform of food handlers' hands was done. Statistical data analysis was completed with Mann-Whitney U test and Kruskal-Wallis model using the SPSS program. The levels of initial contamination of workers' hand were significantly different by the work areas($x^2$=9.156, p<0.01). Workers in the vegetable trimming area had more heavily soiled hands than in the other work areas. Mean of TPC counts and coliform was 8.97×$10^5$ CFU/12.4$cm^2$, 2.93×$10^2$ MPN/12.4$cm^2$ respectively, but fecal coliform was not detected. Transient bacteria were removed from hands after washing and using 70% alcohol solution but were not removed completely. Mean reduction percentage in TPC varied among work areas and ranged from 93.19% to 94.99%, and in coliforms from 97.31% to 100%. A significant difference in TPC was found between before and after hand disinfection (Z=-2.714, p<.01) and between standardized hand washing procedures and un-standardized hand washing procedures(z=-2.301, p<.01). Subjects using the hand sanitizer showed a great elimination of TPC(99.45% reduction), but this effect was valid only after following proper washing procedures. Based on the results, the most effective hand washing method was recommended as the combination of the standardized hand washing procedure with warm-water and use of the 70% alcohol solution. The results can be used to develop hand hygiene programs and training strategies for enhancing hand hygiene practices for food handlers in foodservice operations.
