Well-Differentiated Squamous Cell Carcinoma Found on the Tail of a Captive Leopard (Panthera Pardus): The Importance of Diagnostic Tests

  • A 14-year-old intact male captive leopard (Panthera pardus) first presented with a focal alopecic lesion on the dorsal aspect of the distal one-third of its tail. Although itraconazole was administered, the lesion progressed deeper and became ulcerated due to self-trauma. Due to the wild nature of the leopard, daily dressing of the wound and replacing the bandages without anesthesia became too dangerous, and amputation became necessary. A postoperative tissue sample was submitted for histopathological examination, and tramadol, amoxicillin/clavulanate, and gabapentin were administered. In the days following the amputation, the leopard's appetite significantly decreased, and the patient passed away 18 days later. Histopathological examination of the specimen revealed well-differentiated squamous cell carcinoma (SCC). In hindsight, had more cells been collected by a fine-needle aspirate (FNA) biopsy or had additional FNA biopsies been performed, SCC might have been diagnosed based on its cytological features rather than delayed histopathological findings. The current study highlights the critical reasons why clinicians may often misdiagnose SCC and the importance of being more aware of potential tumors in ulcerative lesions, which are often incorrectly treated as infectious skin diseases. This is also the first report of cutaneous SCC in a leopard.

Granuloma Formation, a Rare Complication after PDO Threads Lifting, and Adjuvant Treatment Using Dual-Frequency Ultrasound (LDM®-MED)

  • Thread-lifting is a minimally invasive procedure that shows good results and fewer complications as compared with those results and complications of standard surgery. Many procedures and techniques have been developed to create a younger appearance of facial/neck skin for aging people, and the demand for an improved aesthetic appearance is increasing. Since the incidence of side effects is much less than that of non-absorbable threads, which can lead to complications such as foreign body reactions, polydioxanone (PDO) threads are predominantly used for face lift procedures. A 66-year-old woman presented to our clinic with inflamed palpable masses. She had undergone a face lift with absorbable threads in our clinic 5 months previously. Excisional biopsy was performed with the patient under local anesthesia. During the operation, any threads were not detected and there was both fibrotic scar tissue and granulomatous tissue. For effectively promoting healing and managing the scars, treatment with LDM®-MED was performed on the day after surgery. The treatment was performed according to the author's protocol. Although foreign body granuloma as a complication after using non-absorbable thread types have been previously reported, it is relatively rare to find this type of complication after using absorbable thread. In this report, we present a case in which a 66-year-old female with foreign body granuloma after undergoing a face lift using absorbable threads was treated with the application of dual-frequency ultrasound, which promoted wound healing.

Primary cutaneous mucinous carcinoma in a periorbital lesion: two case reports and literature review

  • Primary cutaneous mucinous carcinoma (PCMC) is a rare malignancy of the sweat glands that most commonly affects the periorbital area. It is characterized by slow growth over a prolonged period, and its morphology can be easily confused with a benign tumor, such as an epidermal cyst. Consequently, many patients experience recurrence after undergoing multiple resections. However, there are few reports concerning the surgical management of PCMC. We present two cases of PCMC originating in the periorbital area. The first case involved a 76-year-old man with a mass measuring 3.0×1.5 cm that had been increasing in size. The second case was a 61-year-old man with two masses, each measuring 1.0×1.0 cm, that were also growing. Both patients underwent wide excision with a 5-mm safety margin, which was determined based on the widest view of the cross-section of the mass on the magnetic resonance imaging. Subsequently, based on the intraoperative frozen biopsy results, both patients underwent additional excision with a 5-mm safety margin in only one direction. This report shows that, when determining the surgical margin of PCMC in periorbital area, employing imaging modalities and intraoperative frozen biopsies can be helpful for narrowing the surgical margin.

저분자화된 Polydeoxynucleotide (PDRN)의 흰쥐에 대한 외과적 창상 치유 효과 (Wound Healing Effect of Low Molecular PDRN on Experimental Surgical Excision Rat Model)

  • 본 연구에서는 피부재생 화장품 소재로 활용하고자 저분자화 시킨 Polydeoxynucleotide (PDRN)의 창상 치유 효과를 조사하였다. 이를 위하여 연어 정소 유래 PDRN 단백질 제거공정, 내독소 제거공정을 거쳐 순수분리 정제하였고 분자량 저감공정을 거쳐 기존 PDRN 보다 피부 침투율을 높인 고순도 PDRN을 제조하였다. 상처 치료 과정 중 PDRN 처리에 의한 효능을 평가하기 위해 sprague-dawley rats (SD)의 배부에 bioxy punch를 이용한 4개의 창상을 유발하고, 시료를 포함한 총 5종의 실험시료를 마리당 $500{\mu}L$씩 도포한 후 7일 간격으로 4주간 피부조직 변화를 관찰하였다. 상처에 PDRN을 도포한 후, 절개된 상처의 표피화와 수축이 더 빨라졌고, 창상면적에 있어서 PDRN의 도포는 양성대조군인 $Fucidin^{(R)}$ 도포군과 비교하여 유의하게 줄어들었다. 염색한 조직의 현미경 관찰 결과에서는 양성대조군이 가장 빠르게 재상피화가 이루어졌으며, 그 다음으로는 PH군, PD군, HA군으로 교원질 재합성 및 형성 수준을 보였다. 또한, 병변의 형질전환성장인자($TGF-{\beta}$) 및 혈관 내피성장인자(VEGF) 등의 성장인자에서도 염색 조직의 결과와 유사하게 나타났다. 이러한 결과를 종합하여 볼때, 저분자화된 PDRN은 창상에 치료효과가 있다고 판단되며, 화장품 및 의료산업 분야의 기능성 소재로 활용 가능할 것으로 판단되어 진다.

황기약침이 전층피부손상 마우스의 치유에 미치는 영향 (Effect of Astragali Radix Pharmacocupuncture for Wound Healing of Full-Thickness Skin Wound in Mouse)

  • Objectives : In this study, we investigated whether Astragali Radix Pharmacocupuncture (ARP) has an effective on the full thickness defect wound healing process of mouse. Methods : A total of 50 mice (ICR mouse, 7 week-old male) were divided into control group and ARP group. A single full thickness skin defect was made on the dorsal side of the each mouse using an 8mm diameter biopsy punch. Control group were treated with 0.2㎖ saline and ARP group were treat with 0.2ml ARP at 8 points around the wound every three days total 4 times during the experimental period. The change in wound size, contraction rate, healing rate, and epithelization rate was measured by digital images taken on days 3, 6, 9, and 13, and evaluated using a digital image analysis program. Tissues were collected for histological analysis, RT-PCR, and Western blot on days 3, 6, 9, and 15. Results : The results are as follows. ARP group accelerated the rate of wound contraction, wound healing and epithelization compared to the control group. ARP group showed the decrease of inflammatory cells in early inflammatory phase compared to the control group. ARP group upregulated PECAM-1 mRNA and protein expression compared to the control group. ARP group inhibited the scar width and area compared to the control group. Conclusions: ARP showed positive effects on wound healing through the inhibition of inflammatory reaction and increase of PECAM-1 expression related to the wound healing process.

신증후군을 동반한 Henoch-$Sch{\ddot{o}}nlein$ 신염 환아의 임상병리 적 고찰 (Clinicopathological evaluation of Childhood Henoch-$Sch{\ddot{o}}nlein$ Purpura(HSP) Nephritis with Nephrotic Syndrome)

  • 목 적 : Henoch-$Sch{\ddot{o}}nlein$ 자반증은 자반증, 위장관 증상, 관절통, 그리고 신염을 특징으로 하는 전신적인 질환으로, 신염이나 신증후군의 증상을 나타내거나 심한 단백뇨가 한 달 이상 지속되면 반드시 신생검을 해야 할 뿐 아니라 예후도 좋지 않은 것으로 알려져 있다. 저자들은 Henoch-$Sch{\ddot{o}}nlein$ 신염으로 진단받은 환아들 중 신증후군을 동반했던 예를 모아 임상적 특징 및 병리학적 소견과 치료에 대한 반을 양상에 대해 고찰하였다. 방 법 : 1990년 1월부터 1998년 12월까지 경희의대 소아가에서 Henoch-$Sch{\ddot{o}}nlein$ 자반증으로 진단받은 206명의 환아 중 신증후군을 동반한 34명의 환아를 대상으로 후향적으로 조사하였다. 결 과 : 1) 남녀비 1:3:1이었고, 발병당시 연령은 평균 8.3세였다. 2) 첫 조사의 시작으로부터 신생검을 시행할 때까지의 기간은 평균 10.5주였다. 3) 가장 처음 나타난 증상은 자반증(74$\%$)이 위장관 증상(26$\%$)보다 많았으며 발병당시의 동반증상으로는 위장간 증상이 68$\%$, 관절증상이 35$\%$에서 있었다. 4) 급성 신염을 동반한 신증후군이 32.4$\%$였고, 육안적 혈뇨를 동반한 경우가 17.6$\%$, 현미경적 혈뇨를 동반한 경우가 50$\%$였다. 5) 신생검 소견은 ISKDC의 분류에 따라 II도가 20례, III도가 11례, I도가 2례, IV도가 1례 순이었고, V, VI도를 보이는 증례가 없었으며, 신증상의 정도에 따라 병리 조직학적 소견이 심해지는 양상은 관찰되지 않았다. 6) 신조직의 면역형광 검사상 IgA와 C3의 침착이 가장 많았다. 7) 모든 례에서 Methylprednisolone pulse therapy 후 prednisolone을 투여하였고, 1례에서는 cyclophosphamide를 추가하였다. 8) 신생검상 grade I 의 환아는 모두 완전히 회복되었으며(states A) grade IV의 1례는 활동성 신질환(states C)을 보여주었다. grade II의 21$\%$, grade III의 27$\%$에서 states C를 나타냈으며 states D는 관찰되지 않았다. 결 론 : 신증후군을 동반한 Henoch-$Sch{\ddot{o}}nlein$ 자반증 환아에서급성 신염을 동반하거나 신생검상 crescents를 보일 경우 좀 더 적극적인 치료가 필요하며, 이 질환의 경우를 자세히 알기 위해 추적 기간을 늘려서 전향적인 연구가 필요하다.

양막과 콜라겐을 이용한 생체 적합 드레싱 소재 개발 및 백서 창상치유 실험 (DEVELOPMENT OF BIOCOMPATIBLE DRESSING MATERIAL MADE OF COLLAGEN AND AMNIOTIC MEMBRANE AND WOUND HEALING EXPERIMENT IN RAT)

  • Purpose of study: Partial thickness skin graft is the golden standard regimen for full-thickness skin defect caused by burn or trauma. However, in case of extensive burns of more than 50% of total body surface area, the donor site is not sufficient to cover all defects. As a second choice, allograft, xenograft and synthetic materials have been used to treat skin defect. Among them the amniotic membrane(AM) was used as a biological dressing for centuries because of its potential for wound healing. In this study, quantification of EGF in AM and effect of AM-collagen complex on full thickness skin defects was examined. Materials & Methods: The concentration of EGF in fresh, deep frozen and freeze-dried AM was evaluated by ELISA. EGF-R immunostaining was performed in freeze-dried AM. SD rats weighing 250${\sim}$300g was used for wound healing experiment. Three full thickness skin defects(28mm diameter) were made on dorsal surface of SD rat. The control group was covered by Vaselin gauze and AM-collagen complex and $Terudermis^{(R)}$. was grafted in two other defects. Healing area, Cinamon's score were evaluated before biopsy. Grafted sites were retrieved at 3 days, 1 week, 2 weeks and 4 weeks after operation. H & E and Factor VIII immunohistochemical stain was performed to evaluate the microscopic adhesion and structural integrity and microvessel formation. Results: 1. EGF concentration of fresh, deep frozen and freeze-dried AM showed similar level and EGF-R was stained in epithelial layer of freeze-dried AM. 2. At 4 weeks after grafting, the healing area of AM-collagen and Terudermis group was 99.29${\pm}$0.71% and 99.19${\pm}$0.77 of original size. However, that of control group was 24.88${\pm}$2.90. 3. The Cinamon's score of AM-Collagen and $Terudermis^{(R)}$. group at 4 weeks was 15.6${\pm}$1.26 and 14.6${\pm}$3.13 and that of control group was 3.7${\pm}$0.95. Significant difference was observed among control and experimental groups(p<0.05). 4. Histologic examination revealed that AM protected leukocyte infiltration and epithelial migration was nearly completed at 4 weeks. $Terudermis^{(R)}$. group showed mild neutrophil infiltration until 2 weeks and completion of epithelization at 4 weeks. Control group showed massive leukocyte infiltration until 4 weeks. 5. Microvessels were increased sharply at 1 week and control group at 1 and 4 week showed significant differences with $Terudermis^{(R)}$. group of same interval(p<0.05) but no differences were found with AM group(p<0.05). Conclusion: EGF and EGF-R were well preserved in freeze-dried AM. AM attached to collagen acted as excellent biologic dressing which had similar effect with $Terudermis^{(R)}$. AM showed anti-inflammatory action and healing was completed at 4 weeks after full-thickness skin defect.

건선의 조직소견과 비만세포수의 변화 (Study of Relationship between Histologic Findings and Variation of Number of Mast Cell in Psoriasis)

  • 건선병변의 조직소견과 비만세포의 수적 변화를 비교하여 건선의 병리조직 형성 비만세포가 관여하는지를 알고자 본 연구를 시행하였다. 임상적인 특징으로 진단된 건선 환자중 일주일 이상 전신치료를 받지 않은 60례를 대상으로 하였다. 병변의 성숙도에 따른 병리조직적인 변화 및 유두상진피와 표피직하부 진피내의 비만세포의 수를 측정하여 다음과 같은 결과를 얻었다. 1. 건선병변에서 표피의 변화중 이상각화증과 극세포증이 초기병변과 성숙병변에서 차이를 보였다. 초기병변에서는 이상각화증이 부분적으로(mounds)나타났으나 성숙병변에서는 융합되어(confluent)나타났다. 극세포증은 초기병변에서는 경도내지 중등도의 분포를 보였고 성숙병변에서는 중등도내지 고도의 분포를 보였다. 진피의 변화중 유두종증이 초기병변과 성숙병변에서 차이가 있었으며 초기병변에서는 정상범위에서 중등도의 분포를 보였고 성숙병변에서는 중등도내지 고도의 분포를 보였다. 2. 표피의 극세포증이 심할수록 진피의 유두종증도 심하였다. 극세포증, 유두종증, 이상각화증이 심할수록 유두부내의 비만세포의 수는 증가하였다(p<0.01). 3. 유두상 진피내의 비만세포의 수는 성숙병변에서 초기병변보다 증가하였다. 이상의 소견으로 미루어 비만세포는 건선병변 형성에 직접 관여할 것으로 사료된다.

백지 에탄올추출물의 미백효능 연구 (Whitening Effects of Angelica dahurica Radix Ethanol Extract)

  • 본 논문은 백지 에탄올추출물의 미백효능을 조사하기 위하여 melan-a cell, 브라운 기니피그 및 HMB-45 염색을 사용하였다. melan-a 세포에 시료를 6.25, 12.5, 25, 50, ${\mu}g/m{\ell}$ 농도로 처치하여 세포 생장율의 변화 관찰 및 멜라닌세포의 형태학적 변화를 관찰하였다. 또한 brown guinea pig (450~500 g)등 부위에 1500 mJ/$cm^2$ 광량의 ultraviolet B조사로 유발시킨 인공색소반 (${\Phi}$ 2 mm)에 1일 2회, 주 5일, 매회 30 ${\mu}{\ell}$씩 총 5주간 시료를 도포하여 주 1회 육안적 변화를 관찰 하였고, 실험 5주째 되는 날 실험동물은 염산 케타민으로 마취 후 시료를 도포한 인공색소반 부위를 biopsy punch로 절취하여 HMB-45 염색으로 멜라닌소체 내 glycoprotein(gp100) 단백질의 조직학적 변화를 관찰 하였다. 세포생장율 측정에서 시료처치 6.25~50 ${\mu}g/m{\ell}$에서의 세포생장율은 높았고, 멜라닌세포의 형태학적 변화에서는 시료처치 농도가 증가할수록 멜라닌합성 및 수지상 돌기가 적게 관찰 되었다. 동물 실험 육안적 관찰에서 시료 도포군이 용매대조군에 비해 멜라닌 색소침착 정도가 확연하게 밝아졌고, gp100 단백질의 조직학적 관찰에서 시료 도포군은 gp100 단백질 발현 정도가 현저하게 줄어들었으며, 영상분석에서도 시료 도포군이 용매대조군에 비해 유의하게(p<0.001) 감소하였는데 이러한 결과는 현미경 관찰 결과와 일치하였다. 이상의 결과를 종합하면 백지 에탄올추출물은 우수한 미백효과가 있는 것으로 판단된다.

개에서 손바닥 선인장의 피부 결손창 치료 효과 (The Wound Healing Effects of Opuntia Ficus-indica in Dogs)

  • The aim of this study is to investigate the wound healing effects of the Opuntia ficus-indica. Four full-thickness skin wounds(2.5 cm × 2.5 cm) were made in different areas on the back in each of five dogs. Two wounds were used for wound size measurement and two for skin biopsy. The ten full-thickness wounds were treated with the extract of Opuntia ficus-indica, and the ten full-thickness wounds were treated with normal saline. The wound areas were measured every other day for three weeks after wounding. Statistical analysis was performed with the paired t-test. The wound healing rates were 32.9±4.0%, 28.2±7.1%, 42.2±4.1%, 55.0±4.3%, 76.6±2.8%, 86.8±2.3%, 94.0±1.l%, 97.3±0.8%, 99.3±0.3%, 99.8±0.2%, 100.0±0.0% in the experimental group, and 18.0± 8.2%, 13.S± 11.6%, 15.2±6.1 %, 36.9±8.8%, 62.4±4.1%, 80.0±2.3%, 86.0±1.4%, 94.0±0.9%, 96.9±0.8%, 97.8±0.3%, 99.7±0.1% in the control group on the 1/sup st/, 3/sup rd/, 5/sup th/, 7/sup th/, 9/sup th/, 11/sup th/, 13/sup th/, 15/sup th/, 17/sup th/, 19/sup th/, 21/sup st/ day. On the 17/sup th/ day, the wound closed above 99.9% in 1 dog of the experimental group. On the 19/sup th/ day, the wound closed above 99.9% in 3 dogs of the experimental group. On the 21/sup st/ day, the wound closed above 99.9% in 5 dogs of the experimental group, and 1 dog of the control group. The wound healing period was shortened by 2 to 4 days in the experimental group as compared with the control group. The tensile strength was 4,508±513 g/㎠ in the experimental group and 2,772±400 g/㎠ in the control group on the 21/sup st/ day after wounding. The tensile strength was significantly higher in the experimental group than in the control group(p< 0.05). Inflammation rapidly spread and disappeared in the experimental group but not in the control group. Fibroblast and vascularization were found on the 5/sup th/ day in both group and increased only in the experimental group on the 9/sup th/ day. The firs finding of epithelialization was on the 13/sup th/ day in the experimental group and the 21/sup st/ day in the control group. A significantly higher wound healing rate(p< 0.05), tensile strength(p< 0.05) and better histopathological findings were observed in the experimental group than in the control group throughout the experimental period. These results suggest that topical application of Opuntia ficus-indica extract can promote wound healing in dogs.
