Additive Anti-photoaging Effect of the ZnO Microsphere Soaked with Esculetin (Esculetin을 담지한 ZnO microsphere 외용제의 광노화 차단 상가 효능)

  • This study was aimed to examine the synergetic effects of photochemopreventive external agents composed of inorganic ZnO and esculetin. Zinc oxide (ZnO) is widely used in sunblocks because of its excellent biostability and little toxicity. Esculetin derived from Citrus Peel has an anti-oxidative effect. We made the hollow-shaped ZnO microsphere (MS), soaked it with esculetin (EZnO). We used SKH-1 mice to measure the photodamaging effects of UVB. The mice were divided into five groups as follows; UVB nontreated group (N), vehicle (C), esculetin (E), ZnO MS (ZnO), esculetin + ZnO MS(EZno) group. Each group of samples was topically applied to the dorsal skins before the UVB irradiation. The changes of collagen fibers in the skin tissues were observed by H & E staining and Van Gieson staining. The expression of mast cells in skin tissue was observed by immunohistochemical staining of tryptase present in the mast cell granules. Expression of inflammatory cytokines such as TNF-${\alpha}$, IL-$1{\beta}$, and MMP-9, which plays an essential role in wrinkle formation, was measured by RT-PCR. Interestingly, the composition of collagen fibers was better in the EZnO applied group than in the E or ZnO group. Moreover, mast cell expression and the expression of TNF-${\alpha}$, IL-$1{\beta}$, and MMP-9 mRNA were markedly suppressed in the EZnO group, indicating that the synergetic effects of esculetin and ZnO were excellent.

The superior lateral genicular artery flap for reconstruction of knee and proximal leg defects

  • Reconstruction of defects around the knee region requires thin and pliable skin. The superior lateral genicular artery (SLGA) flap provides an excellent alternative to muscle-based flaps. The anatomy and the surgical techniques of the SLGA flap were reviewed and the results of cases using the SLGA flap for coverage of knee and proximal leg defects were analyzed. SLGA flaps were performed in two cases and followed up for at least 6 months. Twelve articles on the use of the SLGA flap were also identified. A review of 39 cases showed that the mean diameter of the perforator supplying the skin of the flap was 1.04 mm, while the mean diameter of the SLGA at its origin was 1.78 mm. The mean length of the pedicle measured from the origin of the popliteal artery was 7.44 cm. The average dimensions of the flap were 14.8×6.6 cm with primary closure of the donor site in 61.5% of cases. Of these cases, 38.5% were due to trauma, 23.1% were post-burn complications, 12.8% were defects after resection of tumors, and 10.3% were for ulcers post-bursectomy. The most common complication was flap tip necrosis. All studies reported favorable outcomes with complete wound healing.

Anti-inflammatory Effect of Sabaek-san Extract through ECS Control in Atopic Dermatitis (아토피피부염에서 ECS 조절을 통한 사백산추출물의 염증 완화 효과)

  • Objective The purpose of this study was to confirm the effect of Sabaek-san extract on skin damage recovery and inflammation relief in atopic dermatitis-induced mice through Endocannabinoid system (ECS) control. Methods In this study, we used 6-week-old NC/Nga mice were divided into 4 group: control group (Ctrl), lipid barrier elimination group (LBEG), palmitoylethanolamide (PEA) treated group after lipid barrier elimination (PEAG), and Sabaek-san extract treatment group after lipid barrier elimination (SBSG). Each group was assigned 10 animals. After drug administration of three weeks duration following lipid barrier elimination, cannabinoid receptor (CB) 1, CB2, CD (Cluster of Differentiation) 68, phosphorylated inhibitor kappa B (p-IκB), inducible nitric oxide synthase (iNOS), Fc ε receptor, substance P and serotonin were observed to confirm the regulation of the ECS, macrophage activity and mast cell activity. Results We found that 8-hydroxydeoxyguanosine (8-OXdG) positive reaction was significantly lower in the SBST group than in LBET and PEAT groups. Both CB1 and CB2 showed higher positive reactions in the SBST group than in the LBET and PEAT. CD68, p-IκB, iNOS, Fc ε receptor, Substance P and serotonin showed lower positive reaction in the SBST compared to the LBET and PEAT. Conclusion It was confirmed that the Sabaek-san extract can reduce the inflammation of atopic dermatitis by restoring the structural damage of the skin lipid barrier through ECS activity.

The Cytotoxicity and Antitumor Activity of Palsun Brewing Water (팔성 양조용수의 세포독성 및 항암활성)

  • In the present study, we have evaluated cytotoxic effects, antitumor activities and metastasis inhibitory effects of Palsun brewing water in NIH 3T3 cells, human epitheloid carcinoma cells, and human skin melanoma cells. The light microscopic study showed morphological changes, AG-NOR (argyrophylic nucleolar organizer region) by silver chloride stain, and glycoprotein by PAS (periodic acid stain) reaction of the treated cells. Disruptions in cell organelles were determined by colorimetric methods; MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-2H-tetrazoliumbromide) and SRB (sulforhodamine B protein) assays. These results suggest that Palsun Brewing Water retains no cytotoxic effects in NIH 3T3 cells and a growth-inhibitory activity in cancer cell lines.

Approach to the Children with Recurrent Infections (재발성 감염 질환의 접근 방법)

  • The major function of immune system is to protect infections. The immune systems are composed of innate and adaptive immunity. In adaptive immunity, the cellular and humoral components interact each other. Neonates and infants are infected frequently, because immune systems are naive and easy to expose to infectious agents. The complete history and physical examination is essential to evaluate the child with recurrent infections. The environmental risk factors of recurrent infections are day care center, cigarette smoke, and air pollution. The underlying diseases such as immunodeficiency, autoimmune diseases, allergy, and disorders of anatomy or physiology increase the susceptibility to infections. In immunodeficiency, infections are characterized by severe, chronic, recurrent, and unusual microbial agents infection. The defects of antibody production are susceptible to sinopulmonary bacterial infections. T cells defects are vulerable to numerous organisms such as virus, fungi, bacteria and etc. The screening tests for immune functions are the quantitative and qualitative measurements of each immune components. A complete blood count with white blood cell, differential, and platelet provide quantitative informations of immune components. Total complement and immunoglobulin levels represent the humoral component. Antibody levels of previously injected vaccines also provide informations of the antigen specific antibody immune responses. T cell and subsets count is quantitative measurement of cell mediated immunity. Delayed hypersensitivity skin test is a crude measurement of T cell function. The long term outcome of children with recurrent infections is completely dependent on the underlying diseases, the initial time of diagnosis and therapy, continued management, and genetic counscelling.

Spinal c-fos Expression in a Rat Model of Incisional Pain (흰쥐의 발바닥 절개 통증모형에서 척수 후각의 c-fos 발현)

  • Background: The expression of the proto-oncogene c-fos in spinal cord neurons following various noxious stimuli has been demonstrated in numerous studies. However, the pattern of expression of c-fos after incisional stimulus has not been evaluated. This study was designed to examine c-fos expression in an incisional pain model of rats. Methods: A 1 cm longitudinal incision was made through the skin, fascia and muscle of the plantar aspect of the hindpaw in enflurane-anesthetized rats. Withdrawal responses were measured using von Frey filaments at areas around the wound before surgery and for the next 48 hours. The expression of c-fos protein in the lumbar spinal cord was examined by immunohistochemistry. Results: After incision, c-fos was strongly expressed within laminae I, II, III, IV, V and VI ipsilateral to the incision. C-fos positive neurons were detected in the controlateral site, as well. Conclusions: These studies suggest that spinal c-fos protein may not be used as a specific marker for spinal nociceptive processing in an incisional pain model.

Secondary Reconstruction of Frontal Sinus Fracture

  • Fractures of frontal sinus account for 5%-12% of all fractures of facial skeleton. Inadequately treated frontal sinus injuries may result in malposition of sinus structures, as well as subsequent distortion of the overlying soft tissue. Such inappropriate treatment can result in aesthetic complaints (contour deformity) as well as medical complications (recurrent sinusitis, mucocele or mucopyocele, osteomyelitis of the frontal bone, meningitis, encephalitis, brain abscess or thrombosis of the cavernous sinus) with potentially fatal outcomes. Frontal contour deformity warrants surgical intervention. Although deformities should be corrected by the deficiency in tissue type, skin and soft tissue correction is considered better choice than bone surgery because of minimal invasiveness. Development of infection in the postoperative period requires all secondary operations to be delayed, pending the resolution of infectious symptoms. The anterior cranial fossa must be isolated from the nasal cavity to prevent infectious complications. Because most of the complications are related to infection, frontal sinus fractures require extensive surgical debridement and adequate restructuring of the anatomy. The authors suggest surgeons to be familiar with various methods of treatment available in the prevention and management of complications following frontal sinus fractures, which is helpful in making the proper decision for secondary frontal sinus fracture surgery.

The Effects of Hangryunhaedocktang on Allergic Contact Dermatitis Based on the Morphological Changes in Epidemal Damage in Mice (황연해독탕(黃連解毒湯)이 알러지성 접촉피부염에 미치는 영향 -알러지성 접촉피부염 유발로 손상된 생쥐 상피세포의 완화를 중심으로)

  • This study was performed to investigate the effectiveness of Hangryunhaedocktang (HHT) on epidermal damages induced by allergic contact dermatitis (ACD). The sensitization were caused by one application of $25{\mu}l$ of 5% 2,4-dinitroch1orobenzene (DNCB) onto a back-lumbar skin of BALB/c mice. 2 weeks later, ACD was elicitated with $4{\mu}l$ of 2.5% DNCB and then mice were given HHT extract in doses of 3.3ml/kg/day, for 72 hours. The ACD induced epidermal damages in HHT treated ACD mice was more mitigated than non-treated ACD elicited mice. The features related with epidermal damage such as epidermal hyperplasia, infiltration of inflammatory cells, increase of nuclear shrinkages and vacuolation, and enlargement of intercellular space softened. And the distribution of soybean agglutinin (SBA) positive reaction in stratum spinosum (SS) and stratum basale (SB) were similarly maintained in a normal configuration. The numerical decrease of BrdU, TUNEL, and Fas positive cells observed were prominent in SB. Results suggest a benefit role for HHT in mitigating epidermal damages in mice with allergic contact dermatitis.

Effects of DHU001, a Mixed Herbal Formula on Acute Inflammation in Mice

  • The effects of DHU001, a mixed herbal formula consisted of 7 types aqueous extracts for treating respiratory disorders were observed on xylene-induced acute inflammation. The xylene was topically applied 60 min after administration of 500, 250 and 125 mg/kg of DHU001, and all animals were sacrificed 2 hrs after xylene application. The changes on ear weights, histolopathological analyses of ear were evaluated and compared to those of indomethacin and dexamethasone(15 mg/kg treated)-Both of drugs are well-known by anti-inflammatory agents. Xylene application resulted in marked increases in induced ear weights as compared with intact control ear. Severe vasodilation, edematous changes of ear skin and increase in the thickness of the ear tissues, neutrophil infiltration as acute inflammation were detected in xylene-treated control ears at histopathological observation. However, these xylene-induced acute inflammatory changes were dose-dependently decreased by oral treatment of DHU001. Therefore, it is concluded that DHU001 has favorable anti-inflammatory effects on xylene-applicated acute ear inflamed mice.

  • 12-month-old Buldig and 3-year-old German Shepherd, treated with corticosteroids duringthe late pregnancy, were referred to the Veterinary Teaching Hospital, Chonnam National University, with chief complaints of vaginal discharge. Radiographic, ultrasonographic and surgical findings that obtained from each cases were as follows; case I: Radiographic findings were identified 2 fetuses that were normal skeleton, however, fetal viability could not be determined. Ultrasonographic findings were identified absence of fetal heartbeat and fetal movement, abnormal echogenicity of the abdominal visera, and echogenic materials remaining within the fetal fluid. After hysterotomy, macroscopic findings of uterus were liquefied materials remaining within the uterine lumen. Fetuses were excessively autolysis (falling off the hair, depression of the eye, and necrosis of the abdormal visera and skin). case II: Radiographic findings were identified irregular fetal structures and fetal movement, loss of recognizable fetal anatomy, dislocation of fetal veterbrae and echogenic materials remaining within the uterine lumen. After ovariohysterectomy, macroscopic finding of uterus and fetus were thickened uterine wall, yellowish-brown colored administration of corticosteroids during gestation should be contraindicated because fetal death is inducced. Ultrasonography has an advantage over radiography in that it permits evaluation of fetal viability.
